Abstract
Description
ROLUPERIDONE FOR THE TREATMENT OF LYSOSOMAL STORAGE DISORDERS AND THEIR SYMPTOMSRELATED APPLICATIONS
[0001] This application claims priority to, and the benefit of, U.S. Provisional Application No. 63 / 592,675 filed on October 24, 2023. The contents of the aforementioned patent application are incorporated herein by reference in their entirety.FIELD OF THE DISCLOSURE
[0002] The present disclosure relates generally to the treatment of lysosomal storage disorders (LSDs), including Niemann-Pick disease.BACKGROUND
[0003] Lysosomal storage disorders (LSDs) are inherited metabolic disorders where a genetic variation disrupts the normal activity of lysosomes in human cells causing abnormal build-up of various toxic materials in the body’s cells due to enzyme deficiencies. It is estimated that LSDs affect about 1 in every 5,000 live births. Although these disorders are considered rare, they are known to affect individuals globally. At present, there are no cures for lysosomal storage disorders and there are not yet specified treatment options for many of the diseases (https: / / rarediseases.org / rare-diseases / lysosomal-storage-disorders / ).
[0004] Niemann-Pick Disease Type Cl is an autosomal recessive, rare, and ultimately fatal lysosomal storage disorder characterized by progressive neurovisceral manifestations due to mutations in the Npcl gene. The defect causes the dramatic decrease of NPC1 protein content and the lysosomal accumulation of cholesterol which induces the degeneration of specific neurons. Like other LSDs, Niemann-Pick Disease Type C does not have a cure. Thus, treatment options are focused on the specific symptoms of each individual. As such, improved methods to treat LSDs and their symptoms, including Niemann-Pick Disease, are needed.SUMMARY

[0109] The foregoing description has been presented only for the purposes of illustration and is not intended to limit the disclosure to the precise form disclosed, but by the claims appended hereto.EXAMPLESExample 1:
[0110] Cell culture
[0111] Healthy and I1061T fibroblasts (Coriell Institute Camden USA, cod. GM05659 (Healthy) and GM18453 (1106 IT)) were grown in air containing 5% CO2 in Dulbecco's Modified Eagle Medium (DMEM) medium, containing 10% v / v fetal calf serum, 2 mM L- glutamine, 0.1 mg / mL gentamicin, and 100 U / mL penicillin. Cells were passaged every 3 days.
[0112] Roluperidone effect on NPC1 protein levels
[0113] To evaluate the effect of Roluperidone onNPCl protein levels, I1061T cells were treated for 24 hours (FIG. IB), 48 hours (FIG. 1C), and 72 hours (FIG. ID) with Roluperidone (CA) at three different concentrations (0.3 pM, 1 pM, and 3 pM), a vehicle control (Ctr), or a positive control, 2.5 pM Rimcazole (Rim) (Tocris #1497). 1.5 mg of each of the compounds were weighed, dissolved in the proper solvent to reach a 3 mM concentration, kept at 4°C, and used within 15 days. The dilutions for the treatment were carried out in order to administer a 1 : 1000 volume. Specifically, 2 pL in 2 mL medium was administered. Ctr was made with DMSO.
[0114] Western Blot Analysis
[0115] Cells were washed twice in phosphate buffer saline (PBS), harvested with 1% v / v trypsin, and pelleted by centrifugation. Pellets were re-suspended in PBS containing 0.1 M sucrose, 0.05 M KC1, 0.04 M KH2PO4, 30 mM EDTA, pH 7.4, and sonicated. The protein concentration was determined by the Lowry method.
[0116] Samples were solubilized in 0.25 mM Tris-HCl, (pH 6.8) containing 20% (w / v) SDS, protease inhibitor cocktail (Merck #PIC0002), then boiled for 2 minutes. 10 pg of solubilized proteins were resolved by SDS-PAGE at 100 V for 1 hour and then electrophoretically transferred to nitrocellulose with Turboblot (Biorad) for 10 min at 1.3 A and 25 V. The nitrocellulose was treated with 3% (w / v) bovine serum albumin (BSA) in Tris Buffered Saline (20 mM Trizma base, 137 mM NaCl, 0.1%, v / v, Tween-20, pH 7.6) and then probed overnight with the anti-NPCl antibodies (1 :5000 Novus Biological #400148). The nitrocellulose was successively incubated for 1 hour at room temperature with HRP- conjugated secondary anti-rabbit antibodies (Bio-Rad Laboratories, Milan, Italy). Proteinbound antibodies were visualized by clarity Enhanced Chemiluminescence (ECL) Western blotting (Bio-Rad Laboratories, Milan, Italy, #1705061), and chemiluminescence was registered through the ChemiDoc MP system (Bio-Rad Laboratories, Milan, Italy).Densitometric analysis derived from Western blots was then carried out using ImageJ version 1.52t (National Institutes of Health, Bethesda, MD, USA) software for Windows. Vinculin was used as a housekeeping protein, which served as an internal control for protein loading. Densitometric calculations were obtained as arbitrary units, derived from the ratio between the intensity of protein band and the housekeeping protein.
[0117] Without wishing to be bound by theory, FIG. 1A demonstrates that there was a strong difference between healthy and I1061T fibroblasts in terms ofNPCl protein content demonstrating that the above method is sensitive in detecting it. FIGS. 1B-1D demonstrate that Roluperidone did not have a statistically significant effect on NPC1 protein expression within the time period and conditions described in Example 1 indicating that physiological changes that occur after treatment with Roluperidone may be independent of the function of NPC1.Example 2:
[0118] Filipin staining
[0119] Healthy and H061T fibroblasts were cultured as in Example 1. To evaluate the effect of Roluperidone on cholesterol accumulation, cells were treated with either 3 pM Roluperidone, 300 pM beta-cyclodextrin, or 2.5 pM Rim for 72 hours. Following treatment, the cells were stained with filipin. A 10 mg / mL filipin stock solution (in PBS) was prepared immediately before use. Cells were fixed in paraformaldehyde (4% solution in PBS) for 10 minutes and washed with PBS. Fibroblasts were then stained with 1 mL filipin workingsolution (0.05 mg / mL in PBS) for 1 hour in the dark, at room temperature. Subsequently, cells were washed three times with PBS, and coverslips were mounted with Fluoroshield™ mounting medium and immediately analyzed through confocal microscopy using a UV filter set (340-380 nm excitation). Images were acquired at 40X magnification (FIG. 2A). Filipin quantification was calculated as mean fluorescence intensity per cell area by using Image! software for Windows (FIG. 2B).
[0120] Without wishing to be bound by theory, Example 2 demonstrates that II 06 IT cells had increased cholesterol accumulation compared to healthy cells (FIG. 2B). Treatment with Roluperidone was able to reduce unesterified cholesterol accumulation in 1106 IT cells demonstrating its potential to be used in subjects with LSDs like Niemann-Pick disease. Additionally, since Example 1 indicates that Roluperidone may function independently of NPC1, the results of Example 2 demonstrate that Roluperidone has the potential to be used in the treatment of high cholesterol broadly, such as, for example, in the treatment of aberrant cholesterol accumulation observed in diseases including cancer and Alzheimer’s disease (see Yang et al., T. “Sigma-2 Receptor — A Potential Target for Cancer / Alzheimer’s Disease Treatment via Its Regulation of Cholesterol Homeostasis.” Molecules 2020, 25, 5439, the relevant disclosures of which are incorporated by reference for the purpose and subject matter referenced herein).Example 3:
[0121] LysoTracker™ Analysis
[0122] Healthy and 1106 IT fibroblasts were cultured as in Example 1 and treated as in Example 2. To evaluate the effect of Roluperidone on lysosomal size, cytofluorimetric assays using Lysotracker™ (Life Technology #L7528) fluorescence as a tool was used and performed according to the manufacturer’s instructions. LysoTracker™ provides fluorescence detection for live-cell staining of acidic environments and can be used for labeling and tracing acidic organelles such as lysosomes. Stained and unstained cells were analyzed by Cytofluorimeter CytoFLEX (Beckman Coulter). Data provided are the ratio of geometric (geo) mean of LysoTracker™ levels in stained divided by unstained samples, collected at 24 hours (FIGS. 3-4, left), 48 hours (FIGS. 3-4, middle), and 72 hours (FIGS. 3-4, right). Example 3 was repeated in duplicate, and the results are illustrated in FIGS. 3-4.
[0123] Without wishing to be bound by theory, Example 3 demonstrates that there was a statistically significant increase in the lysosomal size of I1061T fibroblasts when compared tohealthy cells (FIG. 3). Treatment with Roluperidone showed a trend towards reduced fluorescence of the lysosomes in the I1061T fibroblasts after 48 hours, particularly at a concentration of 0.3 pM, representing a decrease in lysosomal size (FIGs. 3-4). In contrast, rimcazole (Rim) showed a statically significant increase of the signal at 72 hours, representing an increase in lysosomal size (FIGs. 3-4). Although Rim was able to reduce unesterified cholesterol accumulation in the 1106 IT fibroblasts, it was unable to decrease the lysosomal size of I1061T fibroblasts. In contrast, treatment with Roluperidone resulted in a reduction in both unesterified cholesterol accumulation and a reduction in lysosomal size further demonstrating its potential for use in patients with LSDs like Niemann-Pick disease.Example 4:
[0124] NPC1 and Lamp2 co-localization
[0125] NPC1 protein localization at the lysosome is important to perform its function as a cholesterol transporter. Although the NPC1 protein is mutated in I1061T fibroblasts, it is still functional. Once produced, it is retained in the endoplasmic reticulum, before being degraded by cell quality control system. The ability to successfully relocate the residual NPC1 protein to lysosomes could be a strategy to rescue, at least in part, NPC1 physiologic function in I106IT fibroblasts. Example 4 evaluates the effect of Roluperidone on the localization ofNPCl protein at the lysosome using co-immunolocalization experiments with Lamp2 as a lysosomal marker.
[0126] Healthy and I1061T fibroblasts were cultured as in Example 1. To evaluate the effect of Roluperidone on NPC1 localization, Healthy and I1061T fibroblasts were treated with either vehicle (Ctr), 2.5 pM rimcazole (Rim), or 3 pM Roluperidone for 72 hours. Following treatment, the cells were fixed in 4% paraformaldehyde for 5 minutes before adding methanol (-20°C) for 2 minutes. Permeabilization was then performed using 0.1% Triton for 5 min, then the reaction was blocked with 3% BSA in PBS containing 0.1% Triton. NPC1 antibody (1 TOO, Novus Biological #400148) and LAMP2 (1 :50, Santa Cruz abl 8822) were added and maintained overnight at 4 °C. Staining with 4',6-diamidino-2-phenylindole was used as a control of cell nuclei. After washing with PBS, fixed cells were incubated with Alexa-Fluor 543 anti-mouse and 488 anti-rabbit secondary antibodies (Invitrogen, Carlsbad, CA, USA) for 1 hour at room temperature. Images were captured with a confocal microscope (LCS Leica Microsystems, Wetzlar, Germany) (data not shown).
[0127] Data analysis was performed by calculating Pearson’s correlating coefficient using ImageJ version 1.52t (National Institutes of Health, Bethesda, MD, USA) software forWindows (FIG. 5). The experiments were carried out three times. The statistical analysis was performed with one-way ANOVA followed by Dunnett's multiple comparisons test where ****=p<0.0001 vs Healthy.
[0128] Without wishing to be bound by theory, Example 4 demonstrates that co- immuno localization between NPC1 and Lamp2 (as a lysosomal marker) was seen in healthy fibroblasts, however, Rim and Roluperidone did not have a significant effect on NPC1 localization (as evidenced by enhanced signal compared to control).Equivalents
