Abstract
Description
Technical Field
[0001] The present invention relates to a device for examining a patient's cavity, in particular an internal cavity of a patient, where the internal cavity should be understood as a natural cavity, such as an anal and / or vaginal cavity, or an artificial cavity created by an operator. Background Art
[0002] In the following, explicit reference will be made to the field of colorectal medicine, ie to devices such as an anoscope, proctoscope or proctoscope, although the invention may have other diagnostic and / or surgical applications.
[0003] The closest known technology to the present invention is the inspection device described in patent application EP3329834 by the same applicant.
[0004] According to the disclosure, a dilator body is described that can be coupled at the rear with an adapter for a camera and special lateral windows that enable the operation of surgical instruments inside the dilator body to act on the patient's mucosa while the camera captures the operation images for viewing on a monitor. This allows the operator to have an excellent field of view and high surgical precision.
[0005] The Applicant has noticed that the device described in the above patent application, although it functions very well, can be further improved in some aspects.
[0006] In particular, for example when using a laser, the high precision required requires that there be a resting point for the laser, otherwise the laser would have to be fully supported by the operator, making it difficult to achieve the necessary precision. However, having a resting seat for fixing the laser head may be inconvenient or interfere with the operator's field of view.
[0007] Although there are supports applied inside the dilator body to support surgical instruments in the prior art, they are not suitable for this application because their use would reduce the field of view and thereby complicate the surgical procedure, or alternatively require sacrificing the steering accuracy of the laser head. Summary of the invention
[0008] It is therefore an object of the present invention to provide a device for examining a patient's cavity which has a high flexibility of use and in particular enables its use with a wide variety of surgical instruments including laser tools without obstructing the operator's field of vision.
[0009] Another object of the present invention is to provide a device for inspecting a patient's cavity which can be stably placed in an operating position to allow higher surgical precision, particularly when using laser tools, and which can be placed in a rest position when the laser tool is not needed to ensure that the operator has as much maneuvering space as possible.
[0010] This object is substantially achieved by a device for examining a cavity of a patient having the features stated in one or more of the appended claims.
[0011] In particular, the inspection device according to the present invention comprises: a dilator body extending between a distal end and a proximal end along a longitudinal axis; and a handle applied to or fixed to the proximal end of the dilator body to allow an operator to manipulate the dilator body according to insertion and / or rotational movement around the longitudinal axis.
[0012] The terms "distal" and "proximal" refer to areas further away from an operator (and thus closer to the patient) and closer to the operator, respectively, under conditions where the device is used.
[0013] The device also includes a distal portion having a substantially tubular shape to define an expansion region of the patient's internal cavity and adapted to internally define an interior volume including a front opening to enable access to the patient's mucosa or tissue from the interior volume.
[0014] The device also includes a proximal portion extending divergently from the distal portion and including a rear access opening formed in the proximal portion, the rear access opening being in communication with the interior volume.
[0015] The device also includes at least one arm mounted on the proximal portion and including at least one rest seat configured to receive a front surface of a laser head resting thereon and configured to enable the laser head to move to emit a linear laser beam through the front opening.
[0016] At least one arm is rotatable relative to the proximal portion according to a main movement direction and is movable between an operating position, in which the arm externally surrounds the proximal portion, and a rest position, in which the arm does not surround the proximal portion.
[0017] The device also includes a first securing member operatively interposed between the at least one arm and the proximal portion.
[0018] The first fixing member is configured to be actuated when the at least one arm reaches the operating position to prevent the at least one arm from moving forward and backward relative to the proximal portion along the main movement direction.
[0019] The first securing member is configured to be deactivated to enable the at least one arm to move from the operating position to the rest position.
[0020] Other features and advantages of the invention will become more apparent from the illustrative and therefore non-limiting description of an embodiment of a device for examining a patient's cavity formed in accordance with the present claims. BRIEF DESCRIPTION OF THE DRAWINGS

[0027] According to the drawings, an inspection device according to the present invention is generally designated by 1 .
[0028] In the illustrated embodiment, the device 1 includes a dilator body 100 and a handle 200 applied to or secured to the dilator body 100 .
[0029] The dilator body 100 extends along a longitudinal axis “X” between a distal end 101 and a proximal end 102 .
[0030] Preferably, the dilator body 100 and the handle 200 are made of plastic, more preferably transparent or translucent.
[0031] In use, the handle 200 can assume different orientations, which will be defined as "downward" in this description, so that the definitions for "low" and "high" should be understood as "in the extension area of the handle 200" and "in the direction opposite to the handle 200". The same applies to the definition for "lateral", which means that the handle 200 is generally facing downward.
[0032] The handle 200 is applied to or fixed to the proximal portion 120. This connection between the proximal portion 120 and the handle 200 may be formed integrally or by a reversible or irreversible stabilizing application.
[0033] The handle 200 allows an operator to manipulate the dilator body 100 according to insertion and / or rotational movement about the longitudinal axis "X".
[0034] The device 1 comprises a distal portion 110 having a substantially tubular shape to define an expansion area of the patient's internal cavity and adapted to internally delimit an internal volume "V" comprising a front opening 111 to enable access to the patient's mucosa or tissue from the volume "V". Thus, such mucosa or tissue may protrude into the interior of the volume "V" or simply remain exposed, depending on the case.
[0035] The limitation of "tubular" does not exclude the possibility that the outer shape of the distal portion 110 has a slightly tapered shape, or is provided with a segment that is not a perfect circle (it can also be an ellipse, an oval, or other shape), because the specific shape of the distal portion 110 can be selected as needed without departing from the same inventive concept of the present invention.
[0036] Thus, the distal portion 110 extends along a longitudinal axis "X" and is shaped to define an interior volume "V" in the form of a longitudinal tube.
[0037] The device 1 includes a proximal portion 120 extending in a divergent shape from the distal portion 110 and including a rear access opening 130 formed in the proximal portion 120. The rear access opening 130 is in communication with the internal volume "V".
[0038] In particular, the proximal portion 120 opens into the rear access opening 130 facing the operator and arranged on the longitudinal axis "X". Thus, the rear access opening 130 allows viewing of the internal volume "V" and entry of instruments into the internal volume "V", and thus access to the mucosa accessible through the front opening 111 of the distal portion 110.
[0039] The device 1 also includes at least one arm 410, which is mounted on the proximal portion 120 and includes at least one rest seat 500, which is configured to receive the front surface of the laser head resting thereon and is configured to enable the laser head to move to emit a linear laser beam through the front opening 111.
[0040] A "shelf" is understood to be a universal support portion for the front of a laser head (or other instrument suitable for holding against) to define support at least in the longitudinal position of the laser head. To this end, the shelf 500 is arranged at least partially transverse to the longitudinal axis "X".
[0041] At least one arm 410 is rotatable relative to the proximal portion 120 according to a primary direction of movement "R" and can be in an operational position (e.g. Figure 1 As shown, at least one arm 410 externally surrounds the proximal portion 120) and the rest position (e.g. Figure 2 As shown, at least one arm 410 does not surround the proximal portion 120 and moves between them.
[0042] The device 1 further comprises a first fixing member 430 operatively interposed between the above-mentioned arm 410 and the proximal portion 120 .
[0043] In particular, the first fixing member 430 is configured to be actuated when at least one arm 410 reaches the operating position to prevent the arm 410 from moving forward and backward relative to the proximal portion 120 along the main movement direction "R", and the first fixing member is configured to be deactivated to enable the arm to move from the operating position to the rest position.
[0044] Movement "R" is intended to represent rotation of arm 410 along a plane of alignment.
[0045] "Forward and rearward movement" is intended to refer to the two primary directions of movement "R" of the arm 410, and not all other movements (such as, for example, lateral or sideways movement relative to the plane in which the primary movement "R" occurs).
[0046] Specifically, “forward” refers to the moving direction when the arm 410 approaches the operating position, and “rearward” refers to the moving direction when the arm 410 moves away from the operating position.
[0047] In particular, the arm 410 includes a first end 411 and a second end 412 , the arm being rotationally connected to the proximal portion 120 near the first end, and the first fixing member 430 being disposed near the second end.
[0048] The operational position may be envisaged with the second end 412 being connected to the proximal portion 120 by means of the first fixing member 430 .
[0049] In contrast, the rest position may be envisioned as the second end 412 being disconnected from the proximal portion 120 and moved away from the proximal portion 120 .
[0050] The first fixing member 430 includes a first part 431 located on the proximal end 102 and a second part 432 located on the arm 410 .
[0051] The first component 431 includes a first stopper configured to prevent the second component 432 from moving forward along the main moving direction “R” and a second stopper configured to prevent the second component 432 from moving backward along the main moving direction “R”.
[0052] The first fixing member 430 defines a reversible latching system which can be activated when at least one arm 410 is rotated towards the operating position and can be manually deactivated to enable the arm 410 to pass towards the rest position.
[0053] In particular, the first stop is a front stop 433 that blocks forward movement relative to the main movement direction "R", and the second stop is a rear stop 434 that blocks rearward movement relative to the main movement direction "R". The front stop 433 is rigid. On the other hand, the reversible locking system includes a second component 432 and a rear stop 434 that is configured to elastically deform relative to the second component 432 and the first front portion 433. This deformation occurs during the passage of the arm 410 toward the operating position and then returns to the initial undeformed position, thereby preventing the arm 410 from moving forward and backward relative to the proximal portion 120 along the main movement direction "R".
[0054] In particular, the rear stop 434 is shaped like a step that is at least partially flexible relative to the proximal portion 120 .
[0055] Even more particularly, the rear stop 434 extends in height relative to the proximal portion to a greater extent than the front stop 433 extends in height relative to the proximal portion.
[0056] The device 1 also includes a second fixing member 440 operatively interposed between the arm 410 and the proximal portion 120 .
[0057] The second securing member 440 is configured to prevent lateral movement of the arm 410 relative to the proximal portion in a direction perpendicular to the primary direction of movement "R".
[0058] In particular, the second fixing member 440 includes a channel 441, which is suitable for receiving a guide member 442 when at least one arm 410 is rotated toward an operating position, wherein the guide member 442 includes a first side wall 443 and a second side wall 444 that are parallel and opposite to each other, and the first side wall 443 and the second side wall 444 are configured to laterally abut the inner surface of the channel 441, thereby preventing the arm 410 from moving laterally relative to the proximal portion 120 in a direction perpendicular to the main movement direction "R".
[0059] Preferably, the channel 441 is formed in a substantially hourglass shape in the longitudinal direction, including a widened end portion adapted to assist the guide member 442 in entering and exiting relative to the channel 441 .
[0060] In particular, in the embodiment shown in the present figures, the channel 441 is "U"-shaped in the transverse direction and oriented along the main movement direction "R", while the guide 442 is cantilever-shaped relative to the proximal portion 120 and is linear in the longitudinal direction and oriented along the main movement direction "R", and the guide is compatible in shape with the channel 441 and slides internally along the channel.
[0061] Preferably, the above-mentioned front stop 433 and rear stop 434 are arranged in the front and rear relative to the guide 442, respectively, all three being arranged successively along the main movement direction "R".
[0062] The second part 432 further comprises a front surface 435 and a rear surface 436 arranged transversely with respect to the main movement direction "R". The front surface 435 and the rear surface 436 are configured to abut against the front stop 433 and the rear stop 434, respectively, when the arm 410 is in the operating position.
[0063] Advantageously, in this embodiment, the first fixing member 430 and the second fixing member 440 simultaneously and cooperatively allow a stable connection, while the first component 431 effectively allows the arm 410 to slide and be relatively fixed in the operating position, thereby ensuring high stability during and after arranging the arm 410 in the operating position.
[0064] The first and second fixation members 430 , 440 are preferably disposed laterally on the proximal portion 120 .
[0065] Even more preferably, the first fixing member 430 and the second fixing member 440 are preferably arranged high in the proximal portion 120 .
[0066] In this embodiment, at least one arm 410 is connected to the proximal portion 120 by means of an at least partially spherical hinge 450 .
[0067] In particular, the hinge 450 includes a recess 451 that is at least hemispherical and arranged on the arm 410 and an engagement head 452 that is at least hemispherical and arranged to protrude from the proximal portion 120. The engagement head 452 is reversibly connected to the recess 451 and is configured to allow at least partially spherical movement relative to the recess 451.
[0068] Preferably, the hinge 450 is disposed below the proximal portion 120 , preferably laterally proximal to the junction between the dilator body 100 and the handle 200 .
[0069] Advantageously, the arm allows reversible connection and disconnection with the proximal portion 120 of the dilator body 100 by means of the recess 451, which can be added or removed as needed. In addition, the arm 410 can be easily moved between the operating position and the rest position due to the movement with at least partial spherical mobility.
[0070] Despite this movement with at least partial spherical mobility, the arm 410 substantially follows the main movement direction "R" to arrange itself in the operating position, rotating substantially along a plane by a section given by the length of the guide 442 when it engages inside the channel 441 .
[0071] In particular, according to the first embodiment, the main movement direction "R" lies in a plane parallel to the longitudinal axis "X". That is, the arm 410 rotates longitudinally relative to the longitudinal axis "X".
[0072] Alternatively, according to a second embodiment, the main movement direction "R" lies in a plane perpendicular to the longitudinal axis "X". That is, the arm 410 rotates vertically relative to the longitudinal axis "X".
[0073] An alternative embodiment may envisage a main direction of movement "R" along an inclined plane which is an intermediate plane relative to the first and second embodiments.
[0074] Basically, the arm 410 is reversibly switched between the operating position and the rest position by coupling and decoupling the first part 431 and the second part 432 of the first fixing member 430. However, preferably, after being in the rest position, the arm 410 can be moved and arranged in various intermediate rest positions, which are selected according to the convenience of the operator to observe the exposed tissue and perform surgery by means of the rear opening 130 and through the volume part "V" and the front opening 111.
[0075] To further stabilize the arm 410 in the operative position, the arm 410 includes an extension 461 near the first end 411 , and the proximal portion 120 includes a receiver 462 configured to receive the extension 461 when the arm 410 is in the operative position.
[0076] In particular, the receiving portion 462 is shaped like a protruding "C" relative to the proximal portion 120, which is configured to receive the extension 461 and help prevent any displacement of the arm 410 when in the operative position.
[0077] The extension 461 is substantially shaped like an elongated portion of the arm 410 .
[0078] Preferably, the receiving portion 462 is disposed substantially near the junction between the dilator body 100 and the handle 200 .
[0079] The arm 410 preferably has at least one curved portion 411 defining a concave surface 413 , and the rest seat 500 is arranged on the corresponding curved portion 411 .
[0080] Preferably, the arms 410 externally surround the proximal portion 120 of the dilator body 100 along the concave surface 413 .
[0081] According to the embodiment shown in the present figure, the rest 500 is arranged inside the concave surface 413 and outside the proximal portion 120 .
[0082] An alternative embodiment can be envisaged in which the shelf 500 is arranged outside the concave surface 413 in a manner opposite to the concave surface 413 relative to the arm 410 .
[0083] Preferably, the proximal portion 120 has at least one lateral window 140 at which the rest 500 is arranged such that when the arm 410 is in the operating position, the rest 500, the front opening 111 and the lateral window 140 are connected by a linear segment.
[0084] Preferably, the rest 500 is arranged to overlap with a corresponding lateral window 140 of the adapter body 300 in a radially outer position relative to this lateral window 140 .
[0085] However, variants of embodiment not shown are possible in which the rest 500 is arranged on the respective arm 410 such that the arm is situated in a radially inner position of the respective lateral window 140 , or straddles the lateral window 140 itself.
[0086] The rear opening, the volume "V" and the front opening are preferably aligned on the axis "X". Advantageously, it follows that the seat 500 is positioned so as not to obstruct the operator's field of vision when manipulating tissue.
[0087] The window 140 preferably extends along the entire proximal portion, thereby extending from the rear opening 130 of the proximal portion 120 all the way to the distal portion 110 .
[0088] Alternatively, the lateral window 140 may extend less or more, in which case also affecting a portion of the distal portion 110 .
[0089] The lateral window 140 is preferably defined by a rear recess that interrupts the profile of the rear edge of the proximal portion 120 and then extends from the rear opening of the proximal portion 120 toward the distal portion 110 .
[0090] The lateral windows 140 define visual and instrument access to other areas of the interior volume "V".
[0091] According to this embodiment, reference Figure 1 In the operating position, at least one arm 410 is proximate to the front opening 111 and / or the side opening 140, and the shelf 500 can be used by an operator to insert and manipulate the laser head.
[0092] In the shelving position, refer to Figure 2 , at least one arm 410 is flipped over, preferably close to or resting on the handle 200, so as to place the rest seat 500 in a position away from the expander body 100, in particular away from the front opening 111 and the lateral opening 140. In this latter position, the operator can use the free front opening 111 so that other surgical instruments can be inserted by means of the lateral window 140.
[0093] According to the embodiment shown in the present figures, the device 1 comprises an adapter body 300 which is stably applied to the dilator body 100 in the vicinity of the rear opening 130 .
[0094] The arm 410 is connected to the adapter body 300 .
[0095] The adapter body 300 can be stably applied to the dilator body 100, in particular by means of a first connection part 320, preferably by means of a snap connection around the handle 200, and by means of a second connection part 321 preferably including means for interlocking connection with the proximal part 120.
[0096] That is, the adapter body 300 is configured for axial sliding (parallel to the longitudinal axis "X") of the reversible applicator over the proximal portion 120 of the dilator body 100, preferably with a snap lock (with a stop) at one or more possible preset positions.
[0097] The adapter body 300 comprises a seat for a camera 310 arranged to be aligned with the axis “X”, or in other words, capable of displaying the tissue exposed from the front opening 111 by means of the rear opening 130 .
[0098] In particular, the adapter body 300 has a first front end 301 and a second rear end 302, wherein the first front end can be reversibly connected to the dilator body 100, in particular to the proximal part 120, and the second rear end is opposite to the first front end and is preferably provided with a connection device (known as a snap-on or threaded connection, for example) for connecting to an operating accessory, in particular a camera.
[0099] Preferably, the adapter body 300 has a tapered shape in the direction of the second end 302, wherein the tapered shape may only affect the end portion of the adapter body 300 facing the second end 302, in particular the area in the first connecting portion 320, which is concave and / or shaped substantially opposite to the corresponding portion of the handle 200.
[0100] According to the embodiment shown in the present figures, the device 1 preferably comprises a pair of arms 410 , at least one of the two arms 410 (preferably both) having a corresponding rest 500 , and the two arms 410 are configured to surround two opposite symmetrical sides of the proximal portion 120 .
[0101] Preferably, the proximal portion 120 has two opposite lateral windows 140 which preferably extend substantially along the entire axial extension of the proximal portion 120 .
[0102] Thus, in the illustrated embodiment, the proximal portion 120 is substantially defined by two opposing petals 121 (one upper petal and one lower petal) that are connected to one another at a desired radius of curvature at a junction region between the proximal portion 120 and the distal portion 110 .
[0103] The handle 200 is applied to or fixed at one of the petals 121 .
[0104] When envisioned, the adapter body 300 is stably applied to the dilator body 100 at the two petals 121 in a reversible manner.
[0105] Preferably, the adapter body 300 also has a pair of opposing lateral windows 310 which are intended to engage with the above-mentioned lateral windows 140 of the proximal portion 120 when in use (ie after connection with the dilator body 100).
[0106] The resting seat 500 is preferably defined by a stop wall 510 in the shape of a spherical dome, thereby having a closed extension, and having a central opening 520. The central opening 520 is intended for the passage of the laser beam and is preferably circular.
[0107] Referring to the laser head, the laser head is configured to emit a laser beam and has means for connecting to a power source or laser source. The front surface of the laser head has a spherical shape that can engage with each shelf 500 in a resting relationship.
[0108] In use, after the dilator body 100 is inserted into the appropriate position in the patient's cavity, the operator can intervene in the mucosa with the help of a surgical instrument at least through the lateral window 140 and / or the rear opening 130, and / or operate with the help of a laser by arranging the laser head to be rotatably resting inside one of the rest seats 500 and rotating the laser head as needed to move the laser beam and direct the laser beam to the desired location of the mucosa.
[0109] The invention thus makes it possible to achieve the proposed objects, allowing a high flexibility of use with high precision thanks to the multipurpose support of the arm 410, which can be flexibly but firmly arranged in an operating position or can be used in an operating position in which the operator can accurately place and direct the laser beam emitted by the laser head.
