Abstract
Description
[0001] The invention relates to a handpiece of a reusable biopsy gun, into which a biopsy needle is placed by means of the receptacle attached proximally to the inner needle and cutting cannula onto the clamping slides provided for this purpose, which are arranged inside the handpiece and can be operated from the outside, and onto which round pins on the clamping slide side are placed, and the clamping slides can be brought from a starting position into a clamping position by means of a hand-wound clamping device, in which a locking device holds the clamping slides until release by the triggering device.
[0002] Biopsy guns typically consist of a reusable handpiece and a biopsy needle. In these types of guns, the biopsy needle is usually placed onto round pins on the clamping slide, which have corresponding receptacles for the needle. A manually operated clamping mechanism, using a double stroke, moves the clamping slide into the clamping position and locks it in place. After the biopsy needle is attached and the cap is closed, the biopsy gun is ready to insert the needle into the tissue being examined. The insertion is achieved by releasing the clamping slide with a trigger mechanism.Unlocking the mechanism triggers the proximal clamping slide with the inner needle. Upon impact, the proximal clamping slide releases the distal clamping slide with the attached cutting cannula. After the inner needle penetrates the tissue to be examined, the tissue sample is then excised using the cutting cannula surrounding the inner needle. The sample collection process is fully automated.
[0003] A disadvantage of this automatic process is that it is not possible, for example, to monitor the fill level of the sample in the sampling chamber of the inner needle before cutting it out and, if necessary, to improve it by changing the needle.
[0004] German patent application DE 10 2008 038 414 A1 discloses a manually tensionable biopsy gun in which the biopsy needle and the coaxially surrounding cutting cannula are arranged in the longitudinal center axis of the handpiece. The biopsy needle and the cutting cannula in the handpiece are moved from their rest position into a tensioned position by means of a tensioning slide, whereby the cannula slide is moved into the tensioned position with a first stroke and the needle slide with a subsequent second stroke. The sequence for releasing the slides—first the needle slide, then the cannula slide—is ensured by the design of the release device and a tongue plate. The release is displacement-dependent. A comparable biopsy gun is also disclosed in German patent application DE 697 07 721 T2.
[0005] The object of the present invention is to eliminate this disadvantage and to give the operator the opportunity to selectively influence the sampling process.
[0006] The biopsy gun has a handpiece with two independently operable trigger devices. One trigger device, when a first trigger button is pressed, releases only the proximal clamping slide of the inner needle. The other trigger device, when a second trigger button is pressed, serves to release the distal clamping slide with the attached cutting cannula at a time interval determined by the operator in order to cut out the tissue sample. Alternatively, the second trigger button can unlock the inner needle and cutting cannula together, so that the cutting cannula always follows the inner needle with a time delay and cuts out the tissue sample.
[0007] By arranging two independently operable release buttons, where the first release button activates only the proximal tensioning slide of the inner needle and simultaneously ensures that the distal tensioning slide with the cutting cannula cannot be activated, the operator can, for example, use ultrasound to check the position of the inner needle in the tissue under examination and, if necessary, correct and intervene before releasing the distal tensioning slide carrying the cutting cannula via the second release button. However, the operator also has the option of activating both tensioning slides with a single actuation of the second release button in the usual manner.
[0008] Because the first release button and its attachment can only press on the distal part of the two-armed inner lever, it is prevented from accidentally releasing the distal clamping slide. This therefore provides an additional safety feature for the operator.
[0009] When the two clamping slides are cocked, a sliding sleeve is pushed under the inner lever during the second cocking action, preventing the distal clamping slide from being released. Only when the proximal clamping slide contacts the sliding sleeve with its ring is the distal clamping slide released. This safety mechanism ensures that when the clamping slides are simultaneously released by pressing the second release button, the clamping slide carrying the cutting cannula releases slightly after the clamping slide carrying the inner needle; that is, the cutting cannula can never advance ahead of the inner needle.
[0010] The invention is explained in more detail below with reference to drawings. These show: Fig. 1 Handpiece of a biopsy gun with closed housing and inserted biopsy needle (side view) Fig. 2a Handpiece with open lid and removed biopsy needle (top view) Fig. 2b Exploded view of biopsy needle and clamping slide Fig. 3 Cross-section GG through the handpiece Fig. 4 Cut AA through Fig. 1 after clamping the two clamping slides Fig. 5. [0011] The handpiece 1 of a reusable or multi-use biopsy gun consists of a rectangular housing 5 with a hinged lid 2 on the top, allowing the biopsy needle 3 to be inserted from above with its corresponding plate-shaped receptacles. Two clamping slides 4 and 8, each with a receptacle (round pin 9), are arranged in the housing 5. The proximal clamping slide 4 is connected to the inner needle via the plate-shaped, needle-side receptacle 6 and the round pin 9. The distal clamping slide 8 is connected to the proximal receptacle 6 of the cutting cannula via another round pin 9. Both clamping slides are guided along the longitudinal axis of the housing on both the right and left sides. The clamping slides 4 and 8 are moved from a rest position to a clamped position by means of a clamping device against the action of coil springs 7, in which they are locked. The clamping slides 4, 8 are guided on both longitudinal sides so that, after unlocking, they move with minimal play from the proximal rear side to the distal inner side of the housing at high speed. The receptacle provided on each clamping slide, which is connected to the needle-side receptacles 6, consists of an upwardly pointing round pin 9. The round pins serve to receive the plate-shaped receptacle 6 with a hole (see in particular . ) arranged on the respective proximal side of the inner needle 21 and the surrounding cutting cannula 22. Fig. 2b) In the exemplary embodiment, the needle-side receptacles have round holes in the center of the receiving plate into which the respective round pin 9 is inserted. The biopsy needle itself consists of an inner needle 21 with a distally located tip and a sampling chamber behind it, and a cutting cannula 22 surrounding the inner needle, which serves as a cutting edge distally for excising the tissue sample. Proximally, the end of the inner needle, like the end of the cutting cannula, is connected to the respective plate-shaped receiving device and the respective clamping slide. After the clamping slides have been brought into the clamping position by means of a manual winding mechanism, the biopsy needle is inserted by connecting the respective round pin to the respective hole in the receptacle of the inner needle and cutting cannula.Locking devices, which can be released via manually operated release devices, serve to lock the clamping slides 4, 8 in the clamping position.
[0012] How Fig. Figure 5 shows that the locking device for the proximal clamping slide 4 of the inner needle 21 consists of a two-armed lever 11, which is mounted on the inside of the longitudinal side of the housing 12 below the clamping slides 4, 8. Another two-armed inner lever 13 is rotatably mounted, also below the clamping slides, on an inner longitudinal side of the housing opposite the longitudinal side 12. On the same side, a sliding bushing 14 is mounted on the rotary tube 15. The rotary tube 15 serves to adjust the penetration depth of the needle to 8 mm or 16 mm. The sliding bushing 14 with ring 10 mounted on it serves as a locking device for the cutting cannula 22 when the proximal clamping slide 4 of the inner needle 21 is released. The rotary tube 15 is located laterally to the inner lever 13 (see Figure 5). Fig. 4- Fig. 7)
[0013] The two-armed lever 11 has a retainer 16 on its proximal side, into which the downwardly projecting part of the round pin 9 engages, thus locking the proximal clamping slide 4 in the clamping position. The proximal clamping slide 4 with round pin 9, which serves to receive the receiving plate located on the inner needle side, is held in the clamping position as long as the retainer 16 of the two-armed lever 11 blocks it. The round pin 9 located on the proximal clamping slide 8 serves to receive the receiving plate located on the cutting cannula side. Both round pins 9 penetrate the clamping slides 4 and 8 and protrude from the underside (see in particular the figure). Fig. 2b), so that the locking levers 11, 13 arranged below the clamping slides 4, 8 hold the two clamping slides 4, 8 in the clamping position by engaging the round pins 9 until the connection round pin 9 and lever part is released.
[0014] How Fig. As shown in Figure 4, a hook 18, located on the distal part of the double-armed inner lever 13, holds the round pin 9, which projects from the underside of the slide, in a clamped position. The distal lever part 23 of the lever 11, which interacts with the release buttons A and S, has two projections at its distal end for interaction with the release buttons A, S, and lever 11. When release button A is pressed, it presses on the proximal projection; when release button S is pressed, it presses on the distal projection of the lever part 23 (see in particular Figure 4). Fig. 4- Fig. 7)
[0015] The internal lever 13 is also designed with two arms. The distal arm locks the through round pin 9 of the distal clamping slide on its underside by means of a hook 18, thus holding it in the clamping position. The round pin 9 is chamfered on the side penetrating the clamping slide 8, on the side facing outwards (i.e., towards the housing wall), to improve the lever's contact with the pin and facilitate the release of the clamping slide. The other proximal arm of the internal lever 13 interacts with the distal arm of the two-armed lever 11.
[0016] During loading the biopsy gun, the sliding bushing 14 with the ring 10 mounted on it is moved proximally along the rotating tube 15 during the loading process, usually the second stroke, until the ring is positioned under the hook 24 of the inner lever 13 located on the proximal arm. If, as in Fig. As shown in Figure 5, when the release button S is pressed, the lever 11 pivots about the axis of rotation 17 and the clamping slide 4, which carries the inner needle, is released. The inner needle 21 can then penetrate the tissue to be examined. The sliding bushing 14 or the ring 10 prevents the release of the distal clamping slide 8, which carries the cutting cannula 22, by means of the release button S during the triggering process. Since the release button S also has an appendage 25 pointing towards the center of the housing, which could come into contact with the distal arm of the inner lever, but which, due to its design,Since the position could potentially act on the distal part of the inner lever 13, the clamping slide of the cutting cannula 8 cannot be triggered by the S button, even if the sliding bushing 14 is moved by the inner needle-side clamping slide 4 into a position (namely towards the distal side) that releases the proximal arm of the inner lever 13 for triggering (see in particular . Fig. 5). Releases only by pressing the trigger button A (see especially). Fig. 6 and Fig. 7) the clamping slide 8 can be unlocked, since the sliding bushing with ring 10 has been moved into a position that no longer prevents release.
[0017] However, both clamping slides can be activated using the release button A (see below). Fig.7) are unlocked. By pivoting the lever 11 about the axis of rotation 17, the proximal clamping slide 4 is first released, and then the distal clamping slide 8 is released by pivoting the inner lever 13 about the pivot axis 20. The hook 18 leaves the flattened part of the round pin 9, and the proximally located partial arm of the inner lever 13, with its molded-on hook 24, can pivot into a recess in the sliding bushing 14. This is made possible by the displacement of the sliding bushing 14. The clamping slide 4 moves the sliding bushing 14 out of its path towards the distal side, placing it in the release position.
[0018] By separately triggering the inner needle and cutting cannula with the trigger buttons A and S, it is possible, after insertion of the inner needle, to check, for example, the fill level in the sampling section of the inner needle, and possibly improve it by means of suitable measures or, if necessary, to make changes to its position by means of ultrasound control before the tissue sample is cut out of the tissue to be examined using the cutting cannula.
[0019] However, the trigger button A can also be used to initiate the insertion of the internal needle and the subsequent excision of the tissue sample in a single action. Due to the arrangement of two trigger buttons, A and S, which interact with the locking units, the operator can, depending on the type of tissue being examined, either perform the sample extraction as usual or check the tissue sample before excision and then excise it.
