Abstract
Description
There is a growing need for innovative hygienic solutions with the intention to improve patient well being and aid in the growing pressure of the NHS. More recently it has become a priority to manufacture as many cost effective, eco friendly products as possible. There's a huge amount of plastic waste and as a reflection of social awareness and call to market, more and more eco friendly sources and packaging are replacing the day to day products on the shelves. This tends to be found in retail for consumers but there is very little improvement for professionals in the healthcare industry due to the sterile nature of products required. The NHS has made a commitment in October 2020 to become the first health service in the world to reach carbon net zero in response to climate change*. Currently there is a huge amount of non recyclable waste throughout the NHS - Based on 2017 data supplied by NHS Supply Chain, in the NHS, over 630 million individual wipes* delivered into the NHS. These are usually plastic based and used for cleaning the body with no real solution to clean the hair without costly hours of heated water, washing bed sheets and other single use disposable plastic based wash caps. Caps are ordinarily layers of non woven absorbent plastic based material glued to a sheet of plastic that once used, remains non-recyclable and extremely wasteful. Another alternate is for aerosols foaming / dry cleaners with implications that include flammable substances control and increased insurance rates with the Control of Major Accident Hazards Regulations (COMAH)*. It became quite clear that there is a call for cost effective, eco friendly personal hygiene alleviating staff hours and providing an eco friendly wash. The concept of the Shampoo Wrap came to fruition and development started straight away. A biodegradable wrap saturated in a finely balanced foaming formula for the use of cleaning hair with no additional water required. This is a product that is completely unique in operation and method. Simply spiral the material around the head of the patient and massage (see Fig 4). When the formula begins to activate a foaming solution is formed lifting dirt, grease and odours out of the hair and into the foam (see Fig 5). Finish by towel drying (see Fig 6) which absorbs the solution and consequently any dirt and grease. The towel removes any build up leaving clean, conditioned hair. This is a solution to all of the problems faced for those who struggle with any mobility ailment, cancer, suffer with Alzheimer and dementia or convalescing after operation. There were many issues found during the development process that had to be considered during the trials of the formulation and a substantial amount of work providing the best wash combining with cellulose material. 19 02 25 A major issue to face with cellulose became apparent - When any water based substance is combined with biodegradable material it will degrade the fibres as they are natural and one of the most nurturing hosts for the development of pathogens is water. I therefore had to develop a finely balanced water based shampoo with an attuned preservative base and effective pH level. The base will stay effective on biodegradable material such as viscose and other forms of cellulose and minimise the degrading of any natural fibres. Not only will it remain effective on these materials it is also unlikely to cause any irritation to skin, it is vegan friendly and remains biodegradable. As part of the development of the formula it had to conform with the UK Cosmetics Regulation 2009 / 1223 namely passing a challenge test - Preservative Efficacy Test Report - ISO 11930:2019, Stability test, Compatibility test and a Cosmetics Product Safety Report (CPSR). Part of the CPSR is to review the formula for it's potential to irritate the skin, sensitisers or photos sensitisers. Check for available systemic or sub chronic toxicity and a review of the structure of the chemistry for each ingredient with the likely hood of genotoxicity, reproductive effects and carcinogenicity. No animal testing was conducted on the formulation. The CSPR confirms this formula is above standards and is unlikely to degrade deeming the final product SAFE. When developing the formula it took a number of iterations to ensure consistent quality. It's optional for the Wraps to be microwaved for a warm wash, this however can affect the odour. A number of different fragrance oils had to be trialled before finding the perfect fragrance and acidity to aid in the pH preservation of the formula. The fragrance oil volume must not overpower the user and provide a pleasant wash. This would also have a like for like relationship with the thickness and quality of the material, being disposable it would need to be thick and absorbent enough to interact and hold enough formula but remain cost effective and accessible for use within the NHS. A test of volume saturation and gsm per wipe was trialled to affect the hair in the same way. Too little formula and the wrap would provide an insufficient wash, too much formula leaving product build up and the user feeling unclean with a sticky residue. The next issue to resolve was the condition of the hair after operation of the Wrap. Namely the level of surfactant within the formula and it's foaming activity once combined with the biodegradable material. Too low and the formula would not foam sufficiently enough to exfoliate the hair lifting dirt and grease to the surface. Too much and the formula would leave the hair heavy and unclean. After several iterations I found the formula that worked well but needed a nourishing aspect so finalised by adding Vitamin E (tocopherol). Tocopherol reduces lipid per-oxidation and oxidative stress, strengthening the skin and hair for a well balanced effective formula. What was left as per the Abstract is the perfect functioning formula that is biodegradable material compatible with enough foaming action and fragrance that can be easily removed once used by towel drying. Finally following a compatibility test it was found a PE material for packaging was slightly porous and therefore absorbs some of the moisture from the Wrap. A combination of PET and PE materials had to be used to encase the wrap reducing any loss in moisture without any effect on the cellulose material. Propylene Glycol is used as an organic solvent and diluent ion pharmaceuticals and many other industrial applications. NCI thesaurus (NCIt) 19 02 25 Propylene glycol (1,2-propanediol is an organic compound (a diol alcohol), usually a tasteless, odorless and colorless clear oily liquid that is hygroscopic and miscible with water, acetone and chloroform. It is manufactured by the hydration of propylene oxide. Propylene glycol is used as a solvent for intravenous, oral and topical pharmaceutical preparations. It is generally considered safe. However in large doses it can be toxic, especially if given over a short period of time. Intravenous lorazepam contains the largest amount of propylene glycol of commonly used drugs. In adults with normal liver and kidney function, the terminal half-life of propylene glycol ranges from 1.4 to 3.3 hours. Propylene glycol is metabolized byu the liver to form lactate and acetate and pyruvate. The nonmetabolized drug is excreted in the urine mainly as the glucuronide conjugate, approximately 12 to 45 percent is excreted unchanged in urine. Renal clearance decreases as the dose administered increase (390ml / minute / 173m2 at a dose of 5g / day but only 144ml / minute / 173m2 at a dose of 21 g / day). These data suggest that renal clearance decline at higher propylene glycol dose because of saturation of proximal tubular secretion of the drug. As an acceptable level of propylene glycol has not been defined, the clinical implication of propylene glycol level is unclear. The World Health Organisation (WHO) recommends a maximum consumption of 25 mg / kg / day (1.8 / day for a 75kg male) of propylene glycol when used as a food additive, but this limit does not address its use as a drug solvent. No maximum dose is recommended in the literature for intravenous therapy with propylene glycol intoxication occurs at much higher dose than WHO dose limit and is exclusive to pharmacological exposure. Propylene glycol toxicity includes development of serum hyperosmolality, lactic acidosis, and kidney failure. It has been suggested that proximal tubular necrosis is the cause of acute kidney injury from propylene glycol. Along these lines proximal tubular cell injury occurs in cultured human cells exposed to propylene glycol. Acute tubular necrosis was described with propylene glycol toxicity in a case of concomitant administration of intravenous lorazepam and trimethoprim sulfamethoxazole. Propylene glycol induced intoxication can also mimic sepsis or systemic inflammatory response syndrome (SIRS). Patients suspected of having sepsis with negative cultures should be a valuated for propylene glycol toxicity if they have been exposed to high dose lorazapam or other medications containing solvent (A7897) Toxin and Toxin Target Database (T3DB) Propylene glycol is a metabolite found in or produced by saccharomyces cervisiae Yeast metabolome Database (YMDB) 19 02 25 Benzyl Alcohol Molecular Formula: C7H8O (C6H2CH2OH) Synonyms: Benzyl Alcohol, 100-51-6, Phenylmethanol, Benzenemethanol, phenylcarbinol Molecular Weight: 108.14g / mol Computed by PubChem 2.1 (PubChem release 2021.05.07) Description: Benzyl Alcohol appears as a clear colourless liquid with a pleasesant odour.
