Abstract
Description
[Technical field]
[0001] The present invention is directed to a system for the recognition of biological changes in tissue, and in particular to a system using electromagnetic waves having an operating frequency comprised between 2.0 and 3.0 GHz. [Background technology]
[0002] The use of radio frequencies for the recognition of biological changes in tissues has been disclosed in several patent applications. WO 01 / 07909 (Vedruccio) discloses an apparatus comprising a coherent transceiver probe generating radio frequencies at three different wavelengths (436, 872 and 1308 MHz) and a spectrum analyzer. The probe is manually swept along the patient's body and the drop or disappearance of one of the three peaks is considered an indication of a certain type of change. For this, three different wavelengths are considered essential. A device commercially known as Trimprob has been used by many physicians but was considered to be of little reliability as the results are highly operator dependent.
[0003] EP 2364647 (Centro Studi e Ricerche Sant' Angela srl) discloses an apparatus for detecting anomalies in biological tissue, which attempts to overcome the limitations of the prior art. In particular, the apparatus addresses the problem of unreliability by keeping the distance between the transmitter and the antenna constant. The '647 apparatus also attempts to overcome operator dependency by automatically moving the antenna. The preferred frequency range is between 400 and 1000 MHz.
[0004] EP2465428 (Medielma srl) also addresses the same problem and tries to solve it by increasing the number of receiving antennas so that one of them is always connected to the transmitter, so that the device can detect anomalies. The device operates at frequencies lower than 700 MHz. Indeed, this patent considers it very important to irradiate the tissue in "near-field conditions". According to the patent, such conditions can only be achieved if the wavelength is shorter than 700 MHz.
[0005] EP3257439 (Medielma srl) discloses a development of the '428 device in which the antenna radiating element is rotated by a servo mechanism through a predetermined angle in both clockwise and counterclockwise directions, again attempting to reduce human action and to define a standard method for carrying out the analysis.
[0006] The results of the prior art devices, although useful, are considered preliminary screening tests since the final determination regarding the presence of cancer is always left to other tests, e.g., NMR analysis, colonoscopy, biopsy, etc. However, the above tests are expensive and invasive.
[0007] U.S. Patent Application No. 2017 / 007149 discloses an imaging system for detecting abnormalities within the human body through the use of microwaves in the range of 10 to 26 GHz. The system is intended as an alternative to X-ray imaging and does not suggest the use of microwaves in the frequency range of 2 to 3 GHz.
[0008] It would therefore be highly desirable to be able to reduce the number of patients who undergo the above-mentioned invasive examinations. The present invention represents a very important step in this direction, providing a reliable analysis tool comparable to more expensive imaging examinations.
[0009] The present invention is directed to a system for the recognition of biological changes in human tissue, and more particularly for the detection of cancer in certain organs of the human body, without the need to undergo an imaging examination, leading to a diagnosis that is accurate not only in terms of a correct assessment of whether cancer is present, but also with regard to its location. [0011] The present invention is directed to a system for the recognition of biological changes in human tissue, comprising a microwave transmitter, a microwave receiver and a data processing unit, the operating frequency of said microwave transmitter and receiver being comprised between 2.0 and 3.0 GHz. The use of frequencies within this range allows a very reliable diagnosis and localization of cancer. [Brief description of the drawings]
[0012] [Figure 1] 1 is a block diagram of an embodiment of an apparatus according to the present invention; [Diagram 2] FIG. 2 is a block diagram of another preferred embodiment of the present invention. [Diagram 3] 3 is a photograph of a system according to the block diagram of FIG. 2. [Figure 4] The transmitter screen of the system in FIG. 3 is shown. DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS
[0013] The present invention is directed to a system for detecting tissue anomalies, comprising a transmitter device having at least one transmitting antenna, a transmitter, and a power source, a receiving device having at least one receiving antenna, a receiver, a pre-processing module, and a power source, and a data processing device having a microprocessor, a display, and a power source, wherein said transmitter device and said receiving device are configured to operate at frequencies comprised between 2.0 GHz and 3.0 GHz.
[0014] The block diagram of Figure 1 shows a transmitter unit 100 with an antenna 101, a transmitter 102 and a power supply 103, a receiver unit 200 with four receiving antennas 201, a receiver 202, a pre-processing module 204 and a power supply 203, and a data processing unit 300 with a microprocessor 301, a display 302 and a power supply 303.
[0015] 2 shows another preferred embodiment of the present invention, in which a data processing device 300 is integrated into the transmitter device 100 together with a Bluetooth transmitter / receiver 106. In this embodiment, the transmitter 100 comprises an antenna 101, a transmitter 102, a power source 103, a Bluetooth transmitter / receiver 106, a microprocessor 104 and a display 105. The receiver device 200 also comprises a Bluetooth transmitter 206 and sends data to the microprocessor 104 through the Bluetooth transmitter / receiver 106. In this embodiment, the operator can view the results of the analysis directly on the transmitter 100.
[0016] Figure 3 is a photograph of a prototype of the system according to the block diagram of Figure 2. The transmitter device is equipped with a screen and receives data from the receiver device through a Bluetooth connection. This prototype is very light and can be easily carried in a bag. The receiver is about 0.25m x 0.20m in size and contains four antennas arranged in a square configuration.
[0017] Figure 4 shows the display of the transmitter of Figure 3. From the display, the operator can follow the analysis response.
[0018] In one embodiment, the transmitter device includes a single antenna, optionally a resonant cavity dipole antenna or a directional antenna, which may be used in the case of small organs, and a crystal oscillator for locking a specific emitted radio frequency. A preferred type of directional antenna is a Yagi antenna. Preferably, the directional antenna used in the transmitter of the present invention irradiates at an angle comprised between 10° and 30°.
[0019] The transmitter is preferably structured to emit a narrowband incident microwave frequency signal to irradiate the patient tissue. For the purposes of this application, a narrowband signal means a signal having a bandwidth Bw small enough that the assumption can be used that the response as a result of the interaction with the body is considered constant within the bandwidth Bw, provided that 1 / Bw (bandwidth) is shorter than the relaxation time of the irradiated patient biological tissue (typically Bw>1KHz). The frequency emitted by the transmitter device is comprised in the range of 2.0 GHz to 3.0 GHz, for example 2.2 GHz to 2.7 GHz, more preferably 2.3 GHz to 2.5 GHz, most preferably around 2.4 GHz, meaning 2.35 GHz to 2.45 GHz.
[0020] The receiver device comprises at least one receiving antenna, a pre-processing module and a power source.
[0021] The power source preferably comprises a rechargeable battery. The receiving antenna is tuned to the frequency of the transmitting antenna, i.e. around 2.4 GHz, meaning 2.0 GHz to 3.0 GHz, preferably 2.2 GHz to 2.7 GHz, more preferably 2.3 GHz to 2.5 GHz, most preferably 2.35 GHz to 2.45 GHz. The receiving antenna may be either directional or omnidirectional, preferably the receiving antenna has 180° coverage.
[0022] The pre-processing module preferably comprises a filter used to clean the signal excluding all other frequencies different from the emitted frequency to eliminate environmental electromagnetic noise. The signal from the system of receiving antennas is digitized through a receiver and sent to the pre-processing module. From the pre-processing unit, the signal is sent to a processing unit, preferably located either in the transmitting device or in another device, e.g. a laptop computer or equivalent console.
[0023] Through processing algorithms, the data processing unit provides parameters that represent a measure of the backscattered wave field in terms of different characteristics such as attenuation, polarization, resonance and interference. Most preferably, the data processing unit is configured to execute algorithms that determine the presence of nulls or minima (i.e. values below a threshold) in the received signal that are associated with tissue anomalies, more particularly with the presence of cancer. Examples of these algorithms are disclosed in EP2465428, which is incorporated herein by reference in its entirety. The data processing unit may also comprise a display and a power source, which preferably comprises a rechargeable battery. Alternatively, the receiving device comprises a Bluetooth transmitter and transmits the data to a transmitting device that comprises a data processing unit and visualizes the results of the diagnosis on a screen.
[0024] The size of the receiver is small. It is important to note that the receiver for the 2.4 GHz frequency is small because the size of the antenna is related to the wavelength λ of the radiation (2.4 GHz, λ=12 cm). If two or more receiving antennas are used, the length of the receiver is preferably less than 0.80 m, more preferably less than 0.50 m, and even more preferably less than 0.30 m.
[0025] Although there can be multiple antennas, it is also possible to have a single antenna in the receiving device. The use of a single antenna allows for a further reduction in the size of the receiver device. A receiver with a single receiving antenna preferably has a length that is less than 0.40 m, more preferably less than 0.30 m, and even more preferably less than 0.25 m.
[0026] In either case, the receiver preferably has a width of less than 0.30 m and a thickness of less than 0.030 m. The small size and thickness of the receiver make it possible to place it on the couch beneath the patient in a position corresponding to the organ to be examined.
[0027] In view of the small size of both the transmitter and the receiver, the device according to the invention can be easily transported to the patient bed or another location without any problems. In fact, the whole device can be placed in a bag that facilitates transportation. The total weight of the device according to the invention is very limited, for example less than 10 kg, preferably less than 5 kg, more preferably less than 2 kg.
[0028] The device according to the invention can be used in any room and on any available treatment table and does not require dedicated treatment tables and rooms.
[0029] To this end, the device of the present invention is preferably provided with a rechargeable battery that allows operation of the device without the need for an external power source.
[0030] Another strength of the device of the invention is its ability to detect the location of the cancer. In the case of prostate cancer, the device according to the invention can determine whether the cancer is located in the left or right lobe. To obtain this information, until now, it was necessary to carry out an NMR analysis. Similarly, with regard to colon cancer, the device according to the invention allows the diagnosis of the intestinal tract in which the cancer is located.
[0031] The method of use of the device according to the invention is simple: the receiver is placed under the patient corresponding to the organ to be checked. The transmitter is then switched on and placed in contact with the body as close as possible to the organ to be examined.
[0032] In a preferred embodiment, the screens of the system of Figures 3 and 4 will provide the operator with screenshots requesting the selection of the organ to be examined. A possible procedure of prostate and colon examination is presented here as an example. However, it is clear that other displays of information are possible. Similarly, other organs can be introduced into the menu of the system. If the organ is the prostate, the operator will select prostate and the screen will request examination of the left lobe of the prostate. The operator will place a transmitter device corresponding to the left lobe and will press a button on the screen to save the data. The screen will then request examination of the right lobe, the operator will place a transmitter device on the right lobe and press the button again.
[0033] After selecting the large intestine as the organ, the screen will request analysis of six different locations corresponding to the relevant points to be examined: 1) right iliac (cecum and ascending colon), 2) right lumbar (ascending colon and right flexure), 3) mid-abdominal (transverse colon), 4) left lumbar (left flexure and descending colon), 5) left iliac (descending colon and sigmoid colon), and 6) lower abdominal (sigmoid and rectal colon). The operator will place a transmitter at each location in turn and press a button to acquire the data.
[0034] Thus, in a preferred embodiment of the invention, the system comprises software adapted to the examination of at least one of the following organs: prostate, colon, breast and thyroid. Preferably, the display of the system guides the examination and by pressing a button the operator stores the data measured by the system and indicates whether the examination is positive or negative. Once all of the areas to be examined have been completed, the system provides the result of the examination (positive or negative) and the area in which the cancer is probably located (left or right lobe for the prostate, area of the colon for the colon, etc.).
[0035] Preferably, the processor is adapted to store data of a number of tests corresponding to at least one working day. For example, the processor can store at least 100 results of tests. In this way, the operator can perform several tests and at the end of the day can connect the system to a processor, such as a computer, and the data can be downloaded. Thus, the system of the invention is preferably adapted to transfer the stored data to an external processor. The transfer is preferably performed by a Bluetooth connection (106, 206).
[0036] Experimental part Prostate exam 155 urological patients were examined by multiparameter NMR and by the system of Figures 3 and 4 according to the present invention (2.4 GHz). The results are compared with those of transrectal echo-guided prostate biopsies, which are considered the gold standard in prostate cancer diagnosis.
[0037] For each method, two different analyses were performed, the first on the entire prostate (Table 1) and the second to assess laterality of possible prostate cancer (Table 2). [Table 1] [Table 2]
[0038] The results in Table 1 show that the device according to the invention does not produce any false negatives and the number of false positives is comparable to that determined by NMR. It is important to note that false negatives are the most dangerous cases, since when they occur, a person diagnosed as healthy is actually affected by cancer. Tables 3 and 4 report the statistical evaluation of the data in Tables 1 and 2, respectively, by calculating the sensitivity, specificity, negative predictive value (NPV), positive predictive value (PPV) and accuracy, where:
[0039] Sensitivity=TP / (TP+FN)·100
[0040] Specificity=TN / (TN+FP)·100
[0041] PPV = TP / (TP + FP) · 100
[0042] NPV=TN / (TN+FN)·100
[0043] Accuracy=(TP+TN) / (TP+FP+TN+FN)·100 [Table 3] [Table 4]
[0044] From the data in Tables 3 and 4, it can be seen that the device of the invention allows the screening of patients to detect the presence of prostate cancer in a reliable, easy and economical way. The device is also suitable for the recognition of alterations in other organs, for example for the screening of colon, breast and thyroid cancer.
[0045] Colon Examination A prospective single-center blinded study of consecutive adults undergoing colonoscopy was performed. Before the patients underwent colonoscopy, they were screened with the system according to the present invention.
[0046] During the procedure, the subjects were clothed. A handheld device was moved over the abdomen and the electromagnetic response of the tissue signals was recorded (2.4 GHz). A single examiner performed the tests using the system. Abnormal signals were identified and recorded as malignant or benign (adenoma, hyperplastic polyp or diverticulum). Findings were compared with those from colonoscopy. Statistical analysis was then performed.
[0047] result A total of 107 consecutive patients who met the inclusion criteria were enrolled over a 5-month period. The most frequent symptoms related to colonoscopy were constipation, diarrhea, abdominal pain, or blood in the stool. The system according to the present invention detected and characterized all 32 adenocarcinomas and polyps.
[0048] The method identified cancers and polyps with a sensitivity of 96.97%, specificity of 78.38%, and diagnostic accuracy of 84.11% compared to colonoscopy. The positive predictive value was 66.67% and the negative predictive value was 98.31%. Among the 107 subjects, there were 16 false positive results (14.95%) and 1 false negative (0.93%) result. The results are summarized in Table 5. [Table 5]
[0049] Colonoscopy is a highly invasive examination that requires extensive preparation, whereas an examination with the system according to the invention is easy to perform and does not require any preparation.
