Abstract

[Problem] In common surgical procedures, a plurality of instruments are required for a plurality of operations including the crushing of an affected part (e.g., a tumor, a liver parenchyma), the subsequent irradiation of the affected part (i.e., the crushed affected part) with microwaves, and the subsequent excision, of the coagulated affected part that has been subjected to the irradiation with the microwaves, with scissors, a scalpel or the like. In addition, there is a problem that, when the affected part is cut before being coagulated, blood gushes due to the damage of blood vessels or the like, and therefore surgery becomes difficult. [Solution] A medical treatment tool according to the present disclosure has a mechanism (a blade protrusion mechanism) that prevents a blade from coming into contact with an affected part during the irradiation with microwaves and allows the blade to protrude from a cutting board after the irradiation. As a result, it has been confirmed that the above-mentioned problems are solved by the medical treatment tool, and the present invention is accomplished.

Medical treatment tools

[0001] This application claims priority from Japanese Patent Application No. 2023-145692, which is incorporated herein by reference.

[0002] (Devices using microwaves) It is known that microwaves can coagulate (fix) biological tissues at low temperatures, such as those of the digestive organs, liver, bladder, prostate, uterus, blood vessels, and intestinal tract. Various devices using microwaves to assist surgery have been developed.

[0003] (Prior Documents) The following several devices using microwaves have been reported. Patent Document 1 discloses "a microwave device capable of irradiating microwaves, comprising a central conductor that transmits the microwaves, an insulator covering the central conductor, and an outer conductor covering the insulator, wherein the central conductor is first exposed to the outside from the most distal end of the microwave device, and the exposed portion of the central conductor is bent toward the base end of the microwave device so as to follow the outer conductor." This patent document does not disclose the configuration of the blade protruding mechanism of the medical treatment tool disclosed herein.

[0004] Patent Document 2 discloses an "organ resection tool including a brush-like structure capable of irradiating microwaves and / or a brush-like structure capable of receiving microwaves." This patent document does not disclose the configuration of the blade protruding mechanism of the medical treatment tool disclosed herein.

[0005] Patent Document 3 discloses "a medical treatment tool comprising: a first electrode including a microwave receiving antenna; a second electrode including a microwave applying antenna including an insulator, wherein the insulator is disposed on a surface facing the microwave receiving antenna; a coaxial cable or semi-rigid cable including a center conductor and an outer conductor, wherein the tip of the center conductor and the tip of the outer conductor are connected directly or indirectly to the end of the microwave applying antenna and the end of the microwave receiving antenna, respectively; a microwave transmitting coaxial cable; a holder cover, wherein the holder cover houses the microwave applying antenna and a portion of the end side of the microwave receiving antenna; and an inner protrusion of the microwave applying antenna, wherein the protrusion extends toward the inside of the holder cover." This patent document does not disclose the configuration of the blade protruding mechanism of the medical treatment tool disclosed herein.

[0006] For the reasons stated above, the prior patent documents do not disclose or suggest a medical treatment tool capable of irradiating microwaves that has the blade protruding mechanism of the medical treatment tool of the present disclosure.

[0007] WO2023 / 013239 Re-tabled 2013 / 172361WO2022 / 185488

[0008] In general surgery, multiple instruments are required for multiple operations, such as crushing the affected area (e.g., tumor or liver parenchyma), irradiating the crushed area with microwaves, and then cutting out the coagulated area after microwave irradiation with scissors, a scalpel, etc. Furthermore, if the affected area is cut before coagulation, blood may spurt out due to damage to blood vessels, making the surgery difficult.

[0009] As a result of extensive research into solving the above-mentioned problems, the inventors have confirmed that the medical treatment tool of the present disclosure solves the above-mentioned problems by having a mechanism (blade protrusion mechanism) in which the blade does not substantially come into contact with the affected area during microwave irradiation and the blade protrudes from the cutting board after irradiation, and have thus completed the medical treatment tool of the present disclosure.

[0013] The medical treatment tool of the present disclosure will be described below with reference to the drawings, but the medical treatment tool of the present disclosure is not limited to the medical instruments depicted in the drawings. Fig. 1 or Fig. 6 will be described as a left side view of the medical treatment tool (1) of the present disclosure, viewed from the front. When viewed from the front in Fig. 6, the lower side is the lower side of the second tip portion (20), and the upper side is the upper side of the first tip portion (10). Additionally, the first tip portion (10) and the second tip portion (20) in Fig. 2 are the distal side of the medical treatment tool (1), and the microwave transmission coaxial cable (30) side is the proximal side of the medical treatment tool (1). The direction extending from the proximal side to the distal side (arrow 1 (60) in Fig. 1) is the extension direction of the medical treatment tool (1). The shapes of the "holes" and "protrusions" in the medical treatment tool of the present disclosure are not particularly limited, as long as the protrusions can fit into the holes. For example, the protrusion may be a protrusion, a convex portion, or the like, and the hole may be a hole, a concave portion, a slit, or the like. "Engagement" in the medical treatment tool of the present disclosure includes fitting, and is not particularly limited as long as each engaging portion directly or indirectly contacts, connects, or the like. "Fixing tool" in the medical treatment tool of the present disclosure refers to a part for fixing A and B, and includes, for example, a pin or a screw. "Concave" in the medical treatment tool of the present disclosure refers to a state in which the surface is concave inward, and the concave shape is not particularly limited, but includes, for example, a groove. "Convex" in the medical treatment tool of the present disclosure refers to a state in which the surface bulges outward, and the bulging shape is not particularly limited, but includes, for example, a protrusion shape.

[0014] The basic configuration of the medical treatment tool (1) of the present disclosure is not particularly limited as long as microwaves can be irradiated from the microwave-applying antenna (11), but has the following configuration (see: Figures 1 to 14). First tip portion (10): The first tip portion (10) includes the microwave-applying antenna (11), an insulator or dielectric portion (12), and an external conductor portion (13). The insulator or dielectric portion (12) covers a portion of the microwave-applying antenna (11). The external conductor portion (13) covers a portion of the insulator or dielectric portion (12) (see: Figures 1 to 4). Second tip portion (20): The second tip portion (20) is disposed opposite the first tip portion (10) (see: Figures 1, 2, and 6). The term "facing" is not particularly limited as long as the first tip portion (10) and the second tip portion (20) are arranged facing each other and can grasp, crush, irradiate microwaves, or separate the affected area. The second tip portion (20) includes a second support (21), a blade (22), an elastic portion (23), and a cutting board (24). The blade (22) is attached to the second support (21). As a result, microwaves received by the blade (22) flow to the second support (21). The material of the second support (21) is not particularly limited as long as it is a material that can receive microwaves. The material of the blade is preferably stainless steel. The elastic portion (23) is attached so as to surround the outside of the blade (22). The material of the elastic portion is not particularly limited, but known rubber or the like is preferred. The cutting board (24) is attached to the surface of the elastic portion (23) facing the first tip portion (10) (see: the extension direction of arrow 2 (61) in Figure 13). The cutting board (24) is preferably made of a heat-resistant material, such as a high-engineering plastic material such as PEKK. A coaxial cable or semi-rigid cable (30) including a central conductor (31) and an outer conductor (32): The tip of the central conductor (31) and the outer conductor (32) are connected directly or indirectly to the end of the microwave application antenna (11) and the outer conductor portion (13), respectively. The outer conductor portion (13) serves as a microwave receiver.

[0015] The medical treatment tool (1) of the present disclosure may further include a handle 1 (51), a handle 2 (52), a finger ring 1 (53), and a finger ring 2 (54) as necessary (see FIG. 1). The medical treatment tool (1) of the present disclosure is not particularly limited as long as it is movable so as to be able to grasp tissue (subject to microwave irradiation) in the gap between the first tip portion (10) and the second tip portion (20), and examples thereof include forceps and tweezers. Examples of forceps of the present disclosure include, but are not limited to, forceps known per se, such as Kelly forceps, Kocher forceps, Pean forceps, and Allis forceps.

[0016] (Irradiated Microwaves) The microwaves transmitted through the microwave transmission coaxial cable of the medical treatment device (1) of the present disclosure are not particularly limited, but are 300 MHz to 300 GHz (wavelength: 1 m to 1 mm), preferably 0.9 GHz to 30 GHz. The transmission method can be easily achieved by a method known per se, for example, by connecting the medical treatment device (1) to a microwave oscillator that generates microwaves known per se, or by incorporating the oscillator into the medical treatment device (1). The power used in the present disclosure is 0.1 W to 200 W, preferably 1.0 W to 80 W.

[0017] (Coaxial Cable) The coaxial cable used in the medical treatment device of the present disclosure comprises, for example, a central conductor made of copper, a shield tube made of an insulator or dielectric (made of, for example, Teflon (registered trademark), polyethylene, etc.) covering the central conductor, and an earth pipe or braided copper wire as an outer conductor (conductor) made of copper, stainless steel, brass, etc. The coaxial cable may also be a publicly known semi-rigid coaxial cable.

[0018] (Blade Protrusion Mechanism) The blade protrusion mechanism of the medical treatment tool (1) of the present disclosure will be described with reference to Figures 13, 15, and 16, but is not limited to the examples shown in Figures 13 and 15. When the first tip portion (10) and the second tip portion (20) are not in contact (Figures 13 and 15A: Ring 1 and Ring 2 are separated), the tip of the blade (22) does not extend beyond the surface of the cutting board (24) facing the first tip portion (10) (the horizontal line (62) of the facing surface) (the blade is recessed inside the cutting board). When the first tip portion (10) and the second tip portion (20) are in contact with each other via the microwave irradiation target (40) (Figure 15B: Ring 1 and Ring 2 are approaching each other), the tip of the blade (22) does not extend beyond the surface of the cutting board (24) facing the first tip portion (10) (the horizontal line (62) of the facing surface). In this state, the microwave irradiation target (40) between the cutting board (24) and the microwave-applying antenna (11) and / or the external conductor (13) can be grasped and / or crushed. Next, microwaves are applied to the microwave irradiation target (40) (the microwave irradiation target after crushing), causing the target to solidify. Note that contact does not only mean direct contact between A and B, but also includes contact between A and B via C. By increasing the gripping force from the state shown in FIG. 15B (by bringing finger ring 1 and finger ring 2 even closer together), the surgeon causes the externally exposed surface (121) and / or the external conductor (13) to push the cutting board (24) in a direction substantially opposite to the first tip. The pushing force presses the first tip (10) and the second tip (20) together, and the elastic portion (23) contracts in a direction generally opposite to the first tip (see arrow 2 (61)). This causes the tip of the blade (22) to extend beyond the surface of the cutting board (24) facing the first tip (10) (the horizontal line (62) of the surface (62) of the cutting board; see FIG. 15C). Furthermore, the protruding blade alone or in combination with the microwave application antenna (11) cuts and separates the coagulated target. The blade protrusion mechanism of the medical treatment tool (1) of the present disclosure allows the surgeon to smoothly grasp, crush, microwave irradiate, and / or separate the target (40) for microwave irradiation.More specifically, the medical treatment tool (1) of the present disclosure can grasp, crush, irradiate with microwaves, and separate a microwave irradiation target (40) with a single treatment tool. The microwave application antenna (11) may have a recess (112) that accommodates the protruding blade (22) (see FIGS. 15 and 16). The recess (112) holds the microwave irradiation target (40) when the blade (22) protrudes, assisting in the separation. Furthermore, the recess (112) may be groove-shaped (see FIG. 15). The groove shape allows the tip of the blade (22) to fit into the groove, which applies a shearing force, making it easier to separate the microwave irradiation target (40).

[0019] (Configuration Example of Second Tip Portion and Blade Protruding Mechanism) Configuration examples of the second tip portion (20) and blade protruding mechanism of the medical treatment tool (1) of the present disclosure will be described with reference to FIGS. 10-13, but are not limited to the descriptions of FIGS. 10-13. The cutting board (24) has a protrusion (241) extending downward from the second tip portion (extending substantially in the opposite direction to the first tip portion) and a slit portion (242) that houses the blade (22). It is preferable that two protrusions (241) are provided near both ends of the cutting board (24). It is also preferable that the slit portion (242) is formed near the center of the cutting board (24). The elastic body portion (23) has a socket portion (231) that passes through the protrusion (241) of the cutting board (24) and a slit portion (232) that houses the blade (22). Preferably, two receiving openings (231) are provided near both ends of the elastic body (23) to correspond to the protrusions (241) of the cutting board (24). Preferably, the slits (232) are formed near the center of the elastic body (23) to correspond to the slits (242) of the cutting board (24). Furthermore, the elastic body (23) may include two elastic bodies of the same shape. The blade (22) is housed in the slits (242) of the cutting board and the slits (232) of the elastic body (see Figures 11 and 12). Preferably, the tip of the blade (22) is exposed, as shown in Figure 11A. This allows the blade (22) to more easily cut tissue, etc. In the stored state, it is preferable that the tip of the blade (22) does not extend beyond the surface (horizontal line (62) of the surface) facing the first tip portion (10) of the cutting board (24) (the blade is recessed inside the cutting board: see Figure 13).

[0020] The second support (21) has at least two or more holes (211) for passing through the fastener (213) as needed, preferably four holes (211). The blade (22) has a hole (221) for passing through the fastener (213), preferably two holes (221). The protrusion (241) of the cutting board has a hole (2411) for passing through the fastener (213). The protrusion (241) preferably has two holes (2411). An example of the assembly method is the method shown in FIG. 10. An elastic body portion (23) composed of two elastic bodies is placed on the second support (21). Since the two elastic bodies have the same shape, an elastic body socket portion (231) and an elastic body slit portion (232) are formed. Next, the cutting board (24) is attached to the elastic body (23) by passing the protrusion (241) of the cutting board (24) having the slit (242) through the receiving opening (231) of the elastic body. The blade (22) is housed in the slit (242) of the cutting board and the slit (232) of the elastic body. Furthermore, the elastic body (23), cutting board (24), and blade (22) are fixed with a fixture to prevent them from coming out. Specifically, one hole (211) in the second support (21) and one hole (2411) in the cutting board (24) are penetrated by the same fastener, one hole (211) in the second support (21) and one hole (221) in the blade (22) are penetrated by the same fastener, one hole (211) in the second support (21) and another hole (221) in the blade (22) are penetrated by the same fastener, and one hole (211) in the second support (21) and the other hole (2411) in the cutting board (24) are penetrated by the same fastener. As explained above, the hole (2411) in the protrusion of the cutting board is preferably shaped to allow movement of the cutting board (24) and the elastic body part (23) (a shape with play).

[0021] (Recesses in tip portion) As illustrated in Fig. 6, the first tip portion (10) and the second tip portion (20) of the medical treatment tool (1) of the present disclosure have recesses (14) and recesses (25), respectively. These recesses (14, 25) can prevent damage to tissues and organs due to pinching of tissues that are not the target of irradiation.

[0022] (Exposed External Portion) The first distal end portion (10) of the medical treatment tool (1) of the present disclosure preferably includes an exposed external portion (111), as illustrated in FIG. 7 . The exposed external portion (111) is exposed to the outside from the distal end of the first distal end portion, as shown in FIG. 7A . Furthermore, as shown in FIG. 7D , the exposed external portion (111) extends below the first distal end portion toward the proximal end of the medical treatment tool. More specifically, it extends toward the proximal end substantially parallel to the external conductor portion (13). It is more preferable that the exposed external portion (111) is partially or entirely coated with a material that resists adhesion of coagulated tissue. The coating can be made of gold, a Teflon-based material (especially PTFE), or the like. The exposed external portion (111) of the microwave-applying antenna has the effect of suppressing attenuation of microwaves (heat generation) at the antenna tip.

[0023] (Configuration Example of First Tip Portion) Configuration examples (sandwich-type configuration examples) of the first tip portion of the medical treatment tool (1) of the present disclosure will be described with reference to FIGS. 3 to 8, but are not limited to the description in these drawings. The outer conductor portion (13) includes a first support (131) and an outer conductor cover (132). The insulator or dielectric portion (12) includes two insulators or dielectrics. The two insulators or dielectrics are arranged to sandwich the tip of the coaxial cable or semi-rigid cable (30). It is more preferable that the outer conductor portion (13) is partially or entirely coated with a material that is resistant to adhesion of coagulated tissue. The coating can be made of gold, a Teflon-based material (particularly PTFE), or the like. The first support (131) and the outer conductor cover (132) are each arranged to sandwich two insulators or dielectrics. As shown in FIG. 7D , the insulating or dielectric portion (12) has an externally exposed surface (121) that is exposed to the outside on the side that contacts the second tip portion. The externally exposed surface (121) is preferably configured to sandwich the externally exposed portion (111) of the microwave-applying antenna. The first tip portion is assembled by fixing the first support (131) and the external conductor cover (132) with a fixture. After this fixing, the first tip portion is welded using a laser or the like. Specifically, one hole in the first support (131) and one hole in the external conductor cover (132) are penetrated by the same fixture, and another hole in the first support (131) and another hole in the external conductor cover (132) are penetrated by the same fixture. The sandwich-type configuration described above is preferred for a processing tool having a microwave-applying antenna (11) with an externally exposed portion (111). In detail, a processing tool having a microwave application antenna with an externally exposed portion (111) can have an externally exposed surface (121) of an insulating portion or a dielectric portion (12) by the sandwich-type configuration described above.

[0024] It is preferable that the exposed outer surface (121) has an uneven shape, and the surface of the cutting board (24) also has an uneven shape. Specifically, it is preferable that the convex shape of the uneven shape of the exposed outer surface (121) is in contact with the convex shape of the uneven shape of the surface of the cutting board (24). This combination of shapes has the effect of making the microwave irradiation target (40) less slippery when gripping it.

[0025] (Microwave Application Antenna) The microwave application antenna (11) of the present disclosure is not particularly limited as long as it is made of a material capable of supplying microwaves. For example, a wide variety of conductive materials can be used, such as silver, copper, gold, iron, titanium, stainless steel, phosphor bronze, or brass. Suitable examples include SUS304, silver, copper, gold, stainless steel, and brass. The shape of the antenna is not particularly limited, and examples include a cone, a triangular pyramid, a square pyramid, a cylinder, a square prism, a triangular prism, a sphere, a cube, and a rectangular parallelepiped. Furthermore, the inner surface of the antenna (particularly the surface that comes into contact with tissue) can be widely applicable in shapes such as a blade shape, a flat shape, a round shape, a rod shape, a concave-convex shape, and a sawtooth shape.

[0026] (First Tip Portion and Second Tip Portion) The first tip portion (10) and the second tip portion (20) of the medical treatment tool of the present disclosure are preferably configured to be movable (particularly forceps) so that tissue can be grasped in the gap between the two tip portions, and further have coagulation and cutting functions as necessary. It is more preferable that both tip portions are partially or entirely coated with a material that makes it difficult for coagulated tissue to adhere when they come into contact with tissue. The coating is made of gold, a Teflon-based material, or the like. This allows for continuous coagulation and cutting processes without the adhesion of coagulated tissue.

[0027] (Prototype Results) A prototype was created based on the configurations shown in the figures. A simulated organ was cut using this prototype (see Figure 17). Two types of conventional surgical instruments were used as comparative examples. One was able to grasp the organ (tissue and blood vessels), crush it, and then coagulate it, but was unable to cut it. The other coagulated while crushing it when cutting the organ, but was unable to grasp it. On the other hand, with this prototype, grasping, crushing, coagulation, and cutting could all be achieved with a single action (particularly, a single-directional action).

[0028] The present disclosure can provide a medical treatment tool that can perform multiple operations of grasping, crushing, irradiating with microwaves, and cutting an affected area using the same instrument.
