[0001] The present invention belongs to the field of medicine, and relates to compounds containing lactam, preparation methods thereof, pharmaceutical compositions comprising the compounds, and medical uses thereof.

[0002] BACKGROUD OF THE INVENTION

[0003] Hypertrophic cardiomyopathy (HCM) is a genetic disorder prevalent in approximately 1 in 500 individuals. Patients with HCM typically exhibit an enlarged left ventricle that cannot be attributed to other underlying conditions. Microscopic examination reveals an abnormal enlargement and disorganization of heart muscle cells, accompanied by an increase in myocardial fibrosis. HCM is characterized by forceful contractions and impaired relaxation of the heart muscle.

[0004] Patients with HCM, whether due to familial or somatic mutations, may experience symptoms such as chest pain, shortness of breath, fatigue, palpitations, and, in severe cases, sudden cardiac death. Despite the prevalence and severity of HCM, targeted therapies that address the root cause of the disease and alter its progression are limited. Current off-label use of medications like beta-blockers or calcium channel blockers can reduce heart muscle contractility, providing some symptom relief, but they do not prevent disease progression. There is a critical need for pharmaceutical agents that can inhibit the development of ventricular hypertrophy, cardiomyocyte disarray, and myocardial fibrosis.

[0005] Modulation of the hypercontractility of cardiac sarcomere is a promising targeted approach for HCM. The new mechanisms of action may offer therapeutical advantages in terms of relief of symptoms, improved therapeutical window, and reduction of patient mortality. The present disclosure provides such agents and methods for their use.SUMMARY OF THE INVENTION

[0466] Myosin ADP-Glo ATPase Assay

[0467] To assess the inhibition properties of the invented compounds on ATPase activity of myosin, ATPase assays using ADP-Glo assay kit (Promega, Madison, WI) were performed. In the ATPase assay, the full length bovine cardiac muscle myosin purchased from Cytoskeleton (Denver, CO) was used. Initial experiments were conducted to establish the optimal enzymatic assay conditions (including buffer conditions, reaction time and concentrations of protein and ATP) through time course studies. In a standard reaction, 100nM myosin and 50μM ATP are combined in an assay buffer containing 12 mM Pipes, pH 6.8, 2 mM MgCl2 and 1 mM DTT. Serial dilutions of the invented compounds were prepared with DMSO, and then 2 μl of each concentration were added to 38 μl of assay buffer for further dilution. 1 μl of each diluted compound solution was transferred to a 384-well assay plate and pre-incubated with 2 μl of myosin at room temperature for 30 minutes. Subsequently, 2 μl of assay buffer containing ATP were added to each well and incubated for 60 minutes. All the test wells were set as triplicate, protein-free control (maximal inhibition) and no-compound control (no inhibition) were included in the test. The reaction was stopped and excess ATP was depleted by adding 5 μl of the ADP-GloTM Reagent from the assay kit, followed by a 40-minute incubation. Afterwards, 10 μl of the ATP detection reagent was added and allowed to incubate for an additional 30 minutes prior to measurement. The luminescence signal was measured using Victor Nivo multimode plate reader (Perkin Elmer, Waltham, MA) . Dose-response curves, spanning IC50s, were generated by GraphPad Prism 9 (GraphPad Software, San Diego, CA) .
