INCRETIN ANALOGS AS GLP-1, GIP AND GLUCAGON RECEPTOR TRIPLE AGONISTS AND USES THEREOFTECHNICAL FIELD

[0001] The present invention relates to novel incretin analog compounds that are agonists of the glucagon-like peptide 1 receptor (GLP-1R) , the glucose-dependent insulinotropic polypeptide receptors (GIPR) , and the glucagon receptor (GcgR) with a protracted profile of action.BACKGROUND ART

[0002] Glucagon-like peptide-1 (GLP-1)  / glucose-dependent insulinotropic polypeptide (GIP)  / glucagon (Gcg) receptor triple agonists and their potential medical uses are described in several patent applications. Triple agonists with protracted effect appropriate for once-weekly administration in humans are described in WO 2019 / 125929, WO2019 / 125938, and WO 2015 / 067716. Co-administration of protracted glucagon analogs with a protracted GLP-1 / GIP co-agonist is described in WO 2017 / 009236. Triple agonists designed for extended duration of action via conjugation to an Fc fragment are described in US 2019 / 0218269 and US 2019 / 0153060. Exendin-4 derivatives with triple agonistic activity and protracted effect appropriate for once-daily administration in humans are described in WO2017009236, WO2015 / 155141, WO2014 / 096145, WO2014 / 096148, and WO 2014 / 096150. However, no triple agonistic products have so far obtained market approval.

[0003] Nevertheless, a need remains for treatments, especially for T2DM and obese patients, that are capable of providing effective glucose control, with weight loss benefits and a favorable side effect profile. There also is a need for therapeutic agents available for use with sufficiently extended duration of action to allow for dosing as infrequently as once a week or once two weeks, or oral dosing.SUMMARY OF THE INVENTION

[0004] The incretin analogs described herein seek to meet the needs above. Accordingly, this disclosure provides incretin analogs with activity at each of the GIP, GLP-1 and glucagon receptors. Advantageously, the incretin analogs described herein have balanced activity allowing for administration of doses that provide sufficient activity at each receptor to provide the benefits of agonism of that receptor while avoiding unwanted side effects associated with too much activity. Moreover, the incretin analogs described herein have extended duration of action at each of the GIP, GLP-1 and glucagon receptors allowing for dosing as infrequently as once-a-week, thrice-monthly, or twice monthly. In this manner, the incretin analogs result in enhanced glucose control, metabolic benefits such as body weight lowering and / or improved body composition, lipid benefits such as proprotein convertase subtilisin / kexin type 9 (PCSK9) lowering, and / or other benefits such as an increase in bone mass or bone formation or a decrease in bone resorption. This disclosure also provides effective treatments for other disorders or conditions, including obesity, MAFLD, MASH, dyslipidemia, and / or metabolic disorder. In one embodiment, an incretin analog is provided that includes the formula:

[0239] Below is a depiction of the structure of the current incretin analogs using the standard single letter or the three letters amino acid codes with the exception of residues Aib2, αMeL13, K17, αMeK20 and Aib29, where the structures of these amino acid residues have been expanded. The peptide backbone of the current incretin analogs is synthesized using Fluorenyimethyloxycarbonyl (Fmoc)  / tert-Buty (t-Bu) chemistry on Fmoc-Rink Amide AM-Resin (Sub=0.3-0.4mmol / g) beads manually.

[0240] The resin consists of 1%DVB cross-linked polystyrene (Fmoc-Rink-MBHA Low Loading Resin, 100-200 mesh, at a substitution of Sub=0.3-0.4mmol / g, Motif Biotech, Suzhou China) . Standard side-chain protecting groups are used. Synthesis of the side chain at position 17 are conducted by two methods: 1) . Standard side-chain protecting groups are used. Fmoc-Lys (Mtt) -OH) , Fmoc-oxyLys (Mtt) -OH) , Fmoc-Orn (Mtt) -OH) , or Fmoc-oxyOrn (Mtt) -OH) , are used for the lysine, ornithine, orxyOrnithine or oxylysine at position 17; 2) . Fully protected side chain attached Fmoc-Lys, Fmoc-Orn, Fmoc-OxyOrn, Fmoc-OxyLys mentioed above are used as fully protected amino acids for lysine, ornithine, oxy ornithine or oxylysine at position 17; and Boc-Tyr (tBu) -OH) is used for the tyrosine at position 1. Fmoc groups are removed prior to each coupling step (2x7 minutes) using 20%piperidine in DMF. All standard amino acid couplings are performed for 1 hour to a primary amine and 3 hour to a secondary amine, using an equal molar ratio of Fmoc amino acid (0.3 mM) , diisopropylcarbodiimide (0.9 mM) and HOBt (0.9 mM) , at a 9-fold molar excess over the theoretical peptide loading. Exceptions are couplings to Ca-methylated amino acids, which are coupled for 3 hours. After completion of the synthesis of the peptide backbone, the resin is thoroughly washed with DCM for 6 times to remove residual DMF. When using method 1) the Mtt protecting group on the lysine, ornithine, oxy ornithine or oxylysine at position 17 is selectively removed from the peptide resin using two treatments of 30%hexafluoroisopropanol in DCM (2×40-minute treatment) . Subsequent attachment of the fatty acid-linker moiety or phosphonic acid-linker moiety is accomplished by coupling of the corresponding side chain acids mentioed above. 3-fold excess of reagents (AA: PyAOP: DIPEA=1: 1: 1 mol / mol) are used for each coupling that lasted for 1 hour.

[0681] Radioligand competition binding assays are run to determine the equilibrium dissociation constant for exemplary compounds and comparator molecules. Such assay use scintillation proximity assay (SPA) methods and membranes prepared from transfected HEK293 or CHO-K1 cells overexpressing the human GIP receptor (GIPR) , GLP-1 receptor (GLP-1R) or human glucagon receptor (GcgR) .

[0682] Specifically, membrane were extracted from GLP-1R / HEK293, GIPR / CHO-K1 and GCGR / HEK293 cells. The reference compounds and screening compounds were 4-fold serially diluted in 100%Assay Buffer for 8 points. Transfer 25 μL of serial diluted references and screening compounds to the assay plates. Then add 25 μL / well of membrane and 100 μL / well of radio ligand 125I-GLP-1 (7-36)  / 125I-GIPR / 125I-Glucagon. Incubate at room temperature for 1 hour. Filter the reaction mixture through GF / C plate using PerkinElmer Filtermate Harvester and wash the plates. Dry the filter plate for 1 hour at 50 ℃. Seal the bottom of the filter plate using Perkin Elmer Unifilter-96 backing seal tape. Add 50 μL of Perkin Elmer Microscint 20 cocktail to each well of assay plate and count 3H trapped on filter plate using Perkin Elmer MicroBeta2 Reader.

[0688] The cAMP functional assays are conducted in HEK293 cells expressing the human GLP-1, GIP and glucagon receptors. Using homogeneous time resolved fluorescence methods, assays are conducted to determine the intrinsic potency of exemplary analogs and comparator molecules performed in the presence of casein (instead of serum albumin) as a nonspecific blocker, which does not interact with the fatty acid moieties of the analyzed molecules.

[0689] Speciafically, Suspension cells were harvested and resuspended in assay buffer (5 mM HEPES, 500 μM IBMX, 0.1%casein) according to the following procedure: 5 μL of 2X cell suspension were added to each well, where the optimal cell density was 1, 000 cells per well (2, 000 cells per well for GIPR) in a low volume 384-well plate. Agonist serial dilutions in a separate 384 well dilution plate in a 10-point series of 4X dilutions of agonist in assay buffer were performed as follows. 40 μL of the highest concentration of Agonist / DMSO was added to well No. 1.10 μL was removed from well No. 1 and added it to well No. 2, followed by gentle mixing. 10 μL was removed from well No. 2 and added it to well No. 3, followed by gentle mixing. This process was repeated until well No. 10. Additional serial dilutions for additional agonists were set up in a similar manner. 5 μL of each 2X agonist serial dilution was added in duplicate to the designated agonist rows of the assay plate using the Bravo (Agilent Technologies, BravoV11) . Assay plate was incubated for 30 minutes at room temperature. Following agonist incubation, a stock of cAMP Working Detection Solution in a separate 15 ml polypropylene tube was prepared by mixing 38 parts of cAMP Lysis Buffer, 1 parts of cAMP-D2, 1 part anti-cAMP cryptate reagent (Cisbio #62AM4PEJ) ) . 10 μL of cAMP Working Detection Solution was added to all wells of the assay plate. Assay plate was incubated for 1 hour at room temperature in the dark for the immunocompetition reaction to occur. Samples were read on Envision (PerkinElmer, Envision2015) .

[0722] GLP-1R / GIPR / GCGR receptor binding assay and GLP-1R / GIPR / GCGR cAMP functional assay were performed as described in Example 2 &3 in the presence and absence of 2%HSA to evaluate the incretin analog’s plasma protein binding ability. A large shift of IC50 / EC50 in the presence vs. absence of 2%HAS indicates a tight albumin binding and thus ensures a long half-life of the incretin analogs in animal or human bodies (Journal of Medicinal Chemistry, 2015, page 7370) . Data are provided below in Table 5.

[0724] As seen in Table 5, exemplary incretin analogs showed dramatically reduced IC50 / EC50 shift compared to Retatrutide, yet they have comparable PK profiles as illustrated in Table 4A &4B. These results clearly demonstrate that the incretin analogs of the present invention are superior to Retatrutide. They have much less plasma protein binding, like the native ligand GLP-1, GIP or Glucogon, yet still have prolonged half-life in vivo like Retatrutide. The combined results could yield a potent and long acting incretin analogs when used in treating disease in human.

[0725] Example 6: Oral Dosing Pharmacokinetic study in Cynomolgus Monkeys of Compound 110 and Retatrutide

[0726] Preparation of tablets for oral administration: tablets containing the compounds of the present invention and reference compound Retatrutide used for the dosing described herein were immediate release SNAC-based tablets. Equal amount of the test compound and Retatrutide were freeze-dried as neutral sodium salt (pH 7-8) together. The proper amount of the compound of the present disclosure and Retatrutide, SNAC, magnesium stearate and other excipients were weighed, mixed on a rotor mixture over night. The dry mixture was then pressed into tablets containing 4mg of the compound of the present invention and 4mg of Retatrutide in each tablet. The tablets are dosed orally to the same group of monkeys. The blood samples were collected, analyzed and PK parameters were calculated according to the procedures described in Example 4. The results are illustrated in Table 6.
