[0004] Potassium-sodium activated channel subfamily T member 1 ("KCNT1") is one of a family of genes responsible for providing instructions for making potassium channels. KCNT1 encodes a protein called Slack (e.g., calcium-activated K + These channels are present in neurons throughout the brain and can mediate the sodium-activated potassium current I KNa This delayed outward current can regulate neuronal excitability and the rate of adaptation in response to sustained stimulation. Aberrant Slack activity has been associated with the development of early-onset epilepsy and intellectual disability. Therefore, selective regulation of sodium-activated potassium channels (e.g., aberrant KCNT1 or aberrant I KNa ) can be used to treat neurological diseases or disorders or diseases or conditions associated with excessive neuronal excitability and / or KCNT1 gain-of-function mutations. (2018) Neurotherapeutics. 15(4):1112-1126.These mutations can be gain-of-function missense mutations that are dominant (i.e., present on only one allele) and result in a functional change in the encoded potassium channel that results in a significant increase in whole-cell current when tested in Xenopus oocytes or mammalian expression systems (see, e.g., Milligan et al., (2015) Annals Neurosci 75(4):581-590; Barcia et al., (2012) Nature Genetics 44(11):1255-1259; and Mikati et al., (2015) Annals Neurosci 78(6):995-999).

[0203] The onset of ADNFLE is later than that of EIMFS, usually in mid-childhood, and is usually a less severe symptom. It is characterized by nocturnal frontal lobe attacks, and may cause mental, behavioral and cognitive disorders in patients with the symptom. Although ADNFLE is related to genes encoding several neuronal nicotinic acetylcholine receptor subunits, mutations in the KCNT1 gene have been implicated in more severe cases of the disease (Heron et al., (2012) Nature Genetics 44: 1188-1190). Functional studies of the mutant KCNT1 gene associated with ADNFLE show that basic mutations (M896I, R398Q, Y796H and R928C) are dominant gain-of-function mutations (Milligan et al., Annals of Neurology 75 (4): 581-590; Mikati et al., (2015); Mikati et al., (2015) Annals of Neurology 78 (6): 995-999).

[0204] West syndrome is a severe form of epilepsy consisting of a triad of infantile spasms, an interictal electroencephalogram (EEG) pattern known as hypsarrhythmia, and mental retardation, but a diagnosis of one of these elements missing can be made. Mutations in KCNT1, including G652V and R474H, have been associated with West syndrome (Fukuoka et al. (2017) Brain and Development 39:80-83 and Ohba et al. (2015) Epilepsy 56:el21-el28). Treatment targeting the KCNT1 channel suggests that these mutations are gain-of-function mutations (Fukuoka et al., (2017) Brain and Development 39:80-83).

[0205] In one aspect, disclosed herein is a method for treating a disorder associated with excessive neuronal excitability or a disorder associated with a gain-of-function mutation in a gene such as KCNT1 (e.g., epilepsy and other encephalopathy (e.g., MMFSI or EIMFS), ADNFLE, West syndrome, infantile spasms, epileptic encephalopathy, focal epilepsy, Ohtahara syndrome, DEE, Lennox-Gastaut syndrome, seizures, leukodystrophy, leukoencephalopathy, intellectual disability, multifocal epilepsy, generalized tonic-clonic seizures, drug-resistant epilepsy, temporal lobe epilepsy, cerebellar ataxia, asymmetric tonic seizures), cardiac dysfunction ( For example, arrhythmias, Brugada syndrome, myocardial infarction), pain and related conditions (e.g., neuropathic pain, acute / chronic pain, migraine, etc.), muscle disorders (e.g., myotonia, neuromyotonia, cramping muscle spasms, spasticity), pruritus and pruritus, ataxia and cerebellar ataxia, psychiatric disorders (e.g., major depressive disorder, anxiety, bipolar disorder, schizophrenia), learning disabilities, fragile X, neuronal plasticity and autism spectrum disorders), the method comprising administering to a subject in need thereof a compound disclosed herein or a pharmaceutically acceptable salt thereof, or a pharmaceutical composition disclosed herein.

[0206] In some examples, prior to administering a compound disclosed herein or a pharmaceutically acceptable salt or composition thereof, a subject presenting with a condition that may be associated with a gain-of-function mutation in KCNT1 is genotyped to confirm the presence of a known gain-of-function mutation in KCNT1. For example, whole exome sequencing can be performed on the subject. The gain-of-function mutations associated with EIMFS may include, but are not limited to, V271F, G288S, R428Q, R474Q, R474H, R474C, I760M, A934T, P924L, G243S, H257D, A259D, R262Q, Q270E, L274I, F346L, C377S, R398Q, P409S, A477T, F502V, M516V, Q550del, K629E, K629N, I760F, E893K, M896K, R933G, R950Q, and K1154Q. The gain-of-function mutations associated with ADNFLE may include, but are not limited to, M896I, R398Q, Y796H, R928C, and G288S. The gain-of-function mutations associated with West syndrome may include, but are not limited to, G652V and R474H. The gain-of-function mutations associated with temporal lobe epilepsy may include, but are not limited to, R133H and R565H. The gain-of-function mutations associated with Lennox-Gastaut syndrome may include, but are not limited to, R209C. The gain-of-function mutations associated with seizures may include, but are not limited to, A259D, G288S, R474C and R474H. The gain-of-function mutations associated with leukodystrophy may include, but are not limited to, G288S and Q906H. The gain-of-function mutations associated with multifocal epilepsy may include, but are not limited to, V340M. The gain-of-function mutations associated with early-onset epilepsy (EOE) may include, but are not limited to, F346L and A934T. The gain-of-function mutations associated with early-onset epileptic encephalopathy (EOEE) may include, but are not limited to, R428Q. The gain-of-function mutations associated with developmental and epileptic encephalopathy may include, but are not limited to, F346L, R474H and A934T. Gain-of-function mutations associated with epileptic encephalopathy may include, but are not limited to, L437F, Y796H, P924L, and R961H. Gain-of-function mutations associated with early infantile epileptic encephalopathy (EIEE) may include, but are not limited to, M896K. Gain-of-function mutations associated with drug-resistant epilepsy and generalized tonic-clonic seizures may include, but are not limited to, F346L. Gain-of-function mutations associated with infantile wandering partial seizures may include, but are not limited to, R428Q. Gain-of-function mutations associated with leukoencephalopathy may include, but are not limited to, F932I. Gain-of-function mutations associated with NFLE may include, but are not limited to, A934T and R950Q. Gain-of-function mutations associated with Ohtahara syndrome may include, but are not limited to, A966T.Gain-of-function mutations associated with infantile spasms may include, but are not limited to, P924L. Gain-of-function mutations associated with Brugada syndrome may include, but are not limited to, R1106Q. Gain-of-function mutations associated with Brugada syndrome may include, but are not limited to, R474H.

[0207] In other examples, subjects are first genotyped to identify the presence of a mutation in KCNT1, and then this mutation is confirmed as a gain-of-function mutation using a standard in vitro assay such as the assay described in Milligan et al. (2015) Annals Neurology 75(4):581-590. Typically, the presence of a gain-of-function mutation is confirmed when the whole-cell current produced by expression of the mutant KCNT1 allele is increased compared to the whole-cell current produced by expression of wild-type KCNT1, as can be assessed using whole-cell electrophysiology (as described in: Milligan et al. (2015) Annals of Neurology 75(4):581-590; Barcia et al. (2012) Nature Genetics 44(11):1255-1259; Mikati et al. (2015) Annals of Neurology 78(6):995-999; or Rizzo et al. Mol Cell Neurosci. (2016) 72:54-63). Such an increase in whole-cell current can be, for example, an increase of at least or about 50%, 100%, 150%, 200%, 250%, 300%, 350%, 400%, or more. The subject can then be confirmed to have a disease or condition associated with a gain-of-function mutation in KCNT1.

[0208] In specific examples, the subject is confirmed to have a KCNT1 allele containing a gain-of-function mutation (e.g., V271F, G288S, R398Q, R428Q, R474Q, R474H, R474C, G652V, I760M, Y796H, M896I, P924L, R928C, or A934T).

[0209] The compounds disclosed herein or their pharmaceutically acceptable salts or pharmaceutical compositions disclosed herein (e.g., pharmaceutical compositions comprising compounds disclosed herein or their pharmaceutically acceptable salts and pharmaceutically acceptable excipients) can also be used therapeutically for conditions associated with excessive neuronal excitability, wherein the excessive neuronal excitability is not necessarily the result of a gain-of-function mutation in KCNT1. Even in cases where the disease is not the result of increased KCNT1 expression and / or activity, inhibition of KCNT1 expression and / or activity still results in reduced neuronal excitability, thereby providing a therapeutic effect. Therefore, the compounds disclosed herein or their pharmaceutically acceptable salts or pharmaceutical compositions disclosed herein can be used to treat subjects with conditions associated with excessive neuronal excitability (e.g., epilepsy and other encephalopathy (e.g., EIMFS, ADNFLE, West syndrome, infantile spasms, epileptic encephalopathy, focal epilepsy, Ohtahara syndrome, developmental and epileptic encephalopathy, Lennox-Gastaut syndrome, seizures) or cardiac dysfunction (e.g., arrhythmia, Brugada syndrome, myocardial infarction)), regardless of whether the condition is associated with a gain-of-function mutation in KCNT1.

[0210] In some of the foregoing variations, the "subject" contemplated for administration includes, but is not limited to, humans (i.e., males or females of any age group, e.g., pediatric subjects (e.g., infants, children, adolescents) or adult subjects (e.g., young adults, middle-aged adults, or elderly adults)) and / or non-human animals, e.g., mammals, such as primates (e.g., cynomolgus monkeys, rhesus monkeys), cattle, pigs, horses, sheep, goats, rodents, cats, and / or dogs. In certain embodiments, the subject is a human. In certain embodiments, the subject is a non-human animal.

[0211] As used herein, some of the foregoing variations "treating" or "treatment" contemplate actions that occur when a subject develops a specified disease, disorder, or condition that reduce the severity of the disease, disorder, or condition, or that delay or slow the progression of the disease, disorder, or condition (also referred to as "therapeutic treatment"). In some variations, "treating" or "treatment" refers to a method or procedure for obtaining a beneficial or desired result - e.g., a clinical result. Beneficial or desired results can include (1) alleviating one or more symptoms caused by or associated with the disease, disorder, or condition; (2) reducing the extent of the disease, disorder, or condition; (3) slowing or stopping the development or progression of one or more symptoms caused by or associated with the disease, disorder, or condition (e.g., stabilizing the disease, disorder, or condition); and (4) ameliorating the disease (e.g., improving the disease state, enhancing the effect of another drug, delaying or stopping the progression of the disease, improving the quality of life, and / or prolonging survival), such as by causing regression of one or more clinical symptoms.

[0212] In some of the aforementioned variations, an "effective amount" of a compound or a pharmaceutically acceptable salt thereof refers to an amount sufficient to elicit a desired biological response. As will be appreciated by one of ordinary skill in the art, the effective amount of a compound or a pharmaceutically acceptable salt thereof may vary depending on factors such as the desired biological endpoint, the pharmacokinetics of the compound or a pharmaceutically acceptable salt thereof, the disease being treated, the mode of administration, and the age, weight, health, and condition of the subject.

[0213] In some embodiments, a therapeutically effective amount of a compound disclosed herein or a pharmaceutically acceptable salt thereof is administered to a subject (e.g., a human). In some of the aforementioned variations, a "therapeutically effective amount" of a compound or a pharmaceutically acceptable salt thereof is an amount sufficient to provide a therapeutic benefit in the treatment of a disease, disorder, or condition or sufficient to delay one or more symptoms associated with a disease, disorder, or condition or to minimize the symptoms. A "therapeutically effective amount" of a compound or a pharmaceutically acceptable salt thereof means an amount of a therapeutic agent alone or in combination with other therapies that provides a therapeutic benefit in the treatment of a disease, disorder, or condition. The term "therapeutically effective amount" may encompass an amount that improves overall therapy, reduces or avoids symptoms or causes of a disease or condition, or enhances the therapeutic efficacy of another therapeutic agent.

[0214] In some embodiments, the methods provided are directed to treating a disease associated with a gain-of-function mutation of KCNT1. In some variations, a "disease associated with a gain-of-function mutation in KCNT1" refers to a disease associated with a mutation in KCNT1, partially or completely caused by the mutation, or having one or more symptoms partially or completely caused by the mutation, the mutation leading to a gain-of-function phenotype, i.e., an increase in the activity of a potassium channel encoded by KCNT1, thereby leading to an increase in whole-cell current. In some variations, a "gain-of-function mutation of KCNT1" is a mutation in KCNT1 that causes an increase in the activity of a potassium channel encoded by KCNT1. Activity can be assessed, for example, by ion flux assays or electrophysiology (e.g., using whole-cell patch clamp techniques). Typically, a gain-of-function mutation results in an increase of at least or about 20%, 30%, 40%, 50%, 60%, 70%, 80%, 90%, 100%, 125%, 150%, 175%, 200%, 225%, 250%, 275%, 300%, 325%, 350%, 375%, 400% or more compared to the activity of the potassium channel encoded by wild-type KCNT1.

[0215] IV. Pharmaceutical Compositions and Routes of Administration

[0216] The compound or its pharmaceutically acceptable salt provided according to the present invention is usually applied in the form of a pharmaceutical composition. Therefore, disclosed herein is a pharmaceutical composition containing the following as an active ingredient: the compound or its pharmaceutically acceptable salt or ester and one or more pharmaceutically acceptable excipients, carriers (including inert solid diluents and fillers), diluents (including sterile aqueous solutions and various organic solvents), penetration enhancers, solubilizers and adjuvants. The pharmaceutical composition can be applied alone or in combination with other therapeutic agents. Such compositions can be prepared in a manner disclosed in the pharmaceutical field, for example, including "Remington's Pharmaceutical Sciences", Mace Publishing Co., Philadelphia, Pa., 17th edition (1985); and "Modern Pharmaceutics", Marcel Dekker, 3rd edition (GS Banker and CT Rhodes, ed.).

[0217] The pharmaceutical composition can be administered in a single dose or in multiple doses by any of the accepted modes of administration for pharmaceutical agents with similar utilities (e.g., those described in the patents and patent applications incorporated by reference), including rectal, buccal, intranasal, and transdermal routes, intraarterial injection, intravenous, intraperitoneal, parenteral, intramuscular, subcutaneous, oral, topical, as an inhalant, or, for example, by impregnated or coated devices such as stents or arterial inserted cylindrical polymers.

[0218] A mode of administration is parenteral, specifically by injection. The novel composition disclosed herein can be incorporated to include aqueous or oil suspensions or emulsions with sesame oil, corn oil, cottonseed oil or peanut oil and elixirs, mannitol, dextrose or sterile aqueous solutions and similar pharmaceutical vehicles by injection. Routinely, aqueous solutions in saline are also used for injection. Ethanol, glycerol, propylene glycol, liquid polyethylene glycols, etc. (and suitable mixtures thereof), cyclodextrin derivatives and vegetable oils can also be used. Suitable fluidity can be maintained, for example, by using coatings such as lecithin, by maintaining the required particle size in the case of dispersions, and by using surfactants. Prevention of the effects of microorganisms can be achieved by a variety of antibacterial and antifungal agents (such as parabens, chlorobutanol, phenol, sorbic acid, thimerosal, etc.).

[0219] Sterile injectable solutions are prepared by incorporating the compounds disclosed herein or their pharmaceutically acceptable salts in the desired amount in a suitable solvent together with the various other ingredients listed above, followed by filtration sterilization. Typically, dispersions are prepared by incorporating the various sterilized active ingredients into a sterile vehicle containing a basic dispersion medium and other desired ingredients from the ingredients listed above. In the case of sterile powders for the preparation of sterile injectable solutions, exemplary preparation methods include vacuum drying and freeze drying techniques that produce a powder consisting of the active ingredient plus any additional desired ingredients from a previously sterile filtered solution thereof.

[0220] Oral administration is another route for administering a compound as disclosed herein or a pharmaceutically acceptable salt thereof. Administration can be carried out through capsules or enteric coated tablets, etc. In the preparation of a pharmaceutical composition comprising at least one compound as described herein or a pharmaceutically acceptable salt thereof, the active ingredient can be diluted by an excipient and / or encapsulated in such a carrier that can be in the form of a capsule, a medicine bag, paper or other container. When the excipient acts as a diluent, it can be in the form of a solid, semisolid or liquid material (as described above), which acts as a vehicle, carrier or medium for the active ingredient. Therefore, the composition can be in the form of tablets, pills, powders, lozenges, medicine bags, cachets, elixirs, suspensions, emulsions, solutions, syrups, aerosols (solid or in a liquid medium), ointments containing, for example, up to 10% by weight of an active compound, soft gelatin capsules and hard gelatin capsules, sterile injectable solutions, and sterile packaged powders.

[0221] Some examples of suitable excipients include lactose, dextrose, sucrose, sorbitol, mannitol, starch, gum arabic, calcium phosphate, alginate, tragacanth, gelatin, calcium silicate, microcrystalline cellulose, polyvinylpyrrolidone, cellulose, sterile water, syrup and methylcellulose. In certain embodiments, the compositions disclosed herein may additionally include lubricants such as talc, magnesium stearate and mineral oil; wetting agents; emulsifying and suspending agents; preservatives such as methyl benzoate and hydroxypropyl benzoate; sweeteners; and flavoring agents.

[0222] The compositions disclosed herein can be formulated so that after being applied to a patient by using procedures known in the art, a rapid, sustained or delayed release of the active ingredient is provided. Controlled release drug delivery systems for oral administration include osmotic pump systems and dissolution systems containing polymer-coated reservoirs or drug-polymer matrix formulations. Examples of controlled release systems are given in U.S. Patents No. 3,845,770; No. 4,326,525; No. 4,902,514; and No. 5,616,345. Additional embodiments for the methods disclosed herein can employ transdermal delivery devices ("films"). Such transdermal films can be used to provide continuous or intermittent infusions of compounds disclosed herein or pharmaceutically acceptable salts thereof in controlled amounts. The construction and use of transdermal films for delivering medicaments are described in, for example, U.S. Patents Nos. 5,023,252, 4,992,445, and 5,001,139. Such membranes may be configured for continuous, pulsatile, or on-demand delivery of pharmaceutical agents.

[0223] The compositions disclosed herein can be formulated in unit dosage forms. The term "unit dosage form" refers to a physically discrete unit suitable as a unit dose for human subjects and other mammals, each unit containing a predetermined amount of active material calculated to produce the desired therapeutic effect and a suitable pharmaceutical excipient (e.g., tablets, capsules, ampoules). The compound is generally administered in a pharmaceutically effective amount. Preferably, for oral administration, each dosage unit contains about 1 mg to about 2 g of a compound described herein or a pharmaceutically acceptable salt thereof, and for parenteral administration, preferably about 0.1 mg to about 700 mg of a compound described herein or a pharmaceutically acceptable salt thereof. However, it should be understood that the amount of the compound or its pharmaceutically acceptable salt actually administered will generally be determined by the physician according to the relevant circumstances, including the condition to be treated; the route of administration selected; the actual compound or its pharmaceutically acceptable salt administered and its relative activity; the age, weight and response of the individual patient; the severity of the patient's symptoms, etc.

[0224] For the preparation of solid compositions such as tablets, the main active ingredient can be mixed with a pharmaceutical excipient to form a solid preformulated composition containing a homogeneous mixture of a compound disclosed herein or a pharmaceutically acceptable salt thereof. When these preformulated compositions are referred to as homogeneous, it is meant that the active ingredient is dispersed evenly throughout the composition so that the composition can be readily subdivided into equally effective unit dosage forms such as tablets, pills and capsules.

[0225] Tablets disclosed herein or pills can be coated or otherwise mixed, to provide a dosage form with an advantage of prolonged action time or protection from the acidic conditions of the stomach. For example, tablets or pills can comprise an inner dosage component and an outer dosage component, the latter being in the form of an envelope on the former. Two components can be separated by an enteric layer, which is used to resist the disintegration in the stomach and allows the inner component to be transmitted intactly into the duodenum or to be delayed release. Various materials can be used for such enteric layers or coatings, and such materials include many polymeric acids and polymeric acids and mixtures of materials such as shellac, hexadecanol and cellulose acetate.

[0226] Compositions for inhalation or insufflation include solutions and suspensions in pharmaceutically acceptable aqueous or organic solvents or mixtures thereof, and powders. Liquid or solid compositions may contain suitable pharmaceutically acceptable excipients as described above. In certain embodiments, the composition is administered by oral or nasal respiratory routes to produce local or systemic effects. Compositions in pharmaceutically acceptable solvents may be atomized using inert gases. Atomized solutions may be inhaled directly from a nebulizer, or the nebulizer may be attached to a mask holder or an intermittent positive pressure breathing machine. Solutions, suspensions or powder compositions may be administered in an appropriate manner by a device for delivering the preparation, such as orally or nasally.

[0227] In some embodiments, a pharmaceutical composition is provided, comprising a compound disclosed herein or a pharmaceutically acceptable salt thereof and at least one pharmaceutically acceptable excipient and / or carrier.

[0257] Example 1

[0258] Synthesis and characterization of exemplary compounds based on synthetic scheme A

[0259] Step 1

[0260] At about 0 ° C, cesium carbonate (2 equivalents) and alkyl halide (1 equivalent) were added to a stirred solution (0.6 M) of compound 1 (1 equivalent) in DMF, and then stirring was continued for about 12 hours at about 60 ° C. After the reaction was completed, the reaction mixture was diluted with water and extracted with ethyl acetate. The combined organic layer was dried over anhydrous sodium sulfate and concentrated under reduced pressure. Then, the crude residue obtained was purified by flash column chromatography, eluted with ethyl acetate / heptane mixture, to obtain compound 2.

[0261] Step 2

[0262] In a sealed tube, alkyl halide (1.5 eq.) and potassium carbonate (1.5 eq.) were added to a stirred solution (0.64 M) of compound 3 (1 eq.) in DMF, and stirring was then continued for about 4 hours at about 100 ° C. After the reaction was complete, the reaction mixture was diluted with water and extracted with ethyl acetate. The organic layer was washed with a saline solution, dried over anhydrous sodium sulfate, and concentrated under reduced pressure to give compound 2, which was used in the following step 3 without further purification.

[0263] Step 3

[0264] At about 0 ° C, lithium hydroxide hydrate (2 equivalents) was added to a stirred solution (0.35M) of compound 2 (1 equivalent) in a 1:3 water:THF mixture, and then stirring was continued at room temperature for about 16 hours. After the reaction was completed, the organic solvent was distilled off under reduced pressure. Water was added to the crude residue, which was acidified with 2N aqueous hydrochloric acid. The precipitate was filtered to obtain compound 4, which was used in the following step 4 without further purification.

[0265] Step 4

[0266] At about 0 ° C, HATU (1.5 eq.) and DIPEA (3 eq.) were added to a stirred solution (0.8 M) of compound 4 (1 eq.) and N, O-dimethylhydroxylamine hydrochloride (2 eq.) in DCM, and then stirring was continued for about 4 hours at the same temperature. After the reaction was completed, the reaction mixture was quenched with water and extracted with DCM. The combined organic layer was dried over anhydrous sodium sulfate and concentrated under reduced pressure. Then, the obtained crude residue was purified by column chromatography, eluted with ethyl acetate / hexane mixture to obtain compound 5.

[0267] Step 5

[0395] II: Efficacy of exemplary compounds in inhibiting KCNT1

[0396] Example B1

[0397] KCNT1-patch clamp assay

[0398] The inhibition of KCNT1 (KNa1.1, Slack) was evaluated using a tetracycline-inducible cell line (HEK-TREX). Current was recorded using a SyncroPatch 384PE automated patch clamp system. Pulse generation and data collection were performed using PatchController384 V1.3.0 and DataController384 V1.2.1 (Nanion Technologies). Access resistance and apparent membrane capacitance were estimated using built-in protocols. Current was recorded in a perforated patch mode (10 μM aescin) from a cell population. Cells were lifted, wet-ground and resuspended at 800,000 cells / ml. Cells were allowed to recover in a cell bank before the experiment. Current was recorded at room temperature. The external solution contained the following (in mM): NaCl 105, NMDG 40, KCl 4, MgCl2 1, CaCl2 5 and HEPES10 (pH=7.4, osmotic pressure of about 300 mOsm). The extracellular solution was used as a washing solution, a reference solution, and a compound delivery solution. The internal solution contained the following (in mM): NaCl 70, KF 70, KCl 10, EGTA 5, HEPES 5, and aescin 0.01 (pH=7.2, osmotic pressure of about 295mOsm). Aescin was prepared in a 5mM stock aqueous solution, divided into equal parts, and stored at -20°C. Compound plates were created in an extracellular solution in a 2x concentrated manner. When added to the recording well, the compound was diluted to 1:2. The amount of DMSO in the extracellular solution was kept constant at a level for the highest test concentration. A clamping potential from -80mV to 0mV was used with a 100ms step. The average current was measured during the step to 0mV. 100 μM Bepridil was used to completely inhibit the KCNT1 current to allow the non-KCNT1 current to be subtracted offline. The average mean current from the 3 sweeps was calculated and the percent inhibition was calculated for each compound. The percent inhibition as a function of compound concentration was fit to the Hill equation to yield IC 50, slope, minimum parameter, and maximum parameter. If KCNT1 inhibition was less than 50% at the highest concentration tested, or if the IC 50 , the percentage of inhibition is reported instead of IC 50 .

[0399] The results from this example are summarized in Table 6 below. In this table, "A" means IC 50 is less than or equal to 1 μM; "B" indicates inhibition between 1 μM and 20 μM; and "C" indicates inhibition greater than or equal to 20 μM.

[0402]

[0403] Example B2: Pharmacokinetics

[0404] Pharmacokinetic data were obtained for compounds 1063 and 1064 as detailed herein.

[0405] Kinetic solubility assay: Kinetic solubility assays were performed using a shake flask method followed by HPLC-UV analysis. The following step-by-step procedure was used:

[0406] 1) Samples were weighed and dissolved in 100% DMSO as a 10 mM stock solution. Approximately 10 μL (compound / media) of stock solution was required in this assay.

[0407] 2) Test compounds and controls (10 mM in DMSO, 10 μL / vial) were added to 50 mM pH 7.4 phosphate buffer (490 μL / well) placed in the Mini-Uniprep filter.

[0408] 3) Vortex the kinetic solubility samples for 2 minutes.

[0409] 4) The solubility solution was incubated and shaken at room temperature on an orbital shaker at 800 rpm for 24 hours.

[0410] 5) Centrifuge at 4000 rpm, 20°C for 10 minutes.

[0411] 6) 400 μL (with or without dilution) of each solubility supernatant was transferred to a 96-deep well for analysis, after which the samples were directly filtered through a syringe-free filtration device.

[0412] 7) Determine the test compound concentration of the filtrate using HPLC-UV.

[0413] 8) At least 5 UV standard solutions were then injected into the HPLC from low to high concentrations, and the kinetic solubility supernatants were then tested in duplicate.

[0414] 9) Use QC samples to monitor the progress of the kinetic solubility determination.

[0415] Log D: The Log D assay is a miniaturized 1-octanol / buffer shake flask method followed by LC / MS / MS analysis. It is typically measured by determining the partitioning of a compound between an organic solvent (1-octanol) and an aqueous buffer (0.1 M phosphate buffer, pH 7.4; a variable buffer pH can be set). Since log D is pH-dependent, the pH value of the aqueous phase is always specified and is typically measured at pH 7.4 (i.e., the physiological pH of body fluids). The following Log D method was used to calculate the Log D values in Table 8 below:

[0416] 1) Dissolve the appropriate test compound in 100% DMSO to make a 10 mM solution.

[0417] 2) Transfer the test compounds (10 mM in DMSO; 2 μL / well) and QC samples (10 mM in DMSO; 2 μL / well) from the storage tubes to 96-well polypropylene cluster tubes.

[0418] 3) Buffer-saturated 1-octanol (149 μL / well) and 1-octanol-saturated buffer (149 μL / well) were added to the wells, respectively.

[0419] 4) Each tube was mixed vigorously on its side for 3 minutes and then shaken at 880 rpm for 1 hour at room temperature.

[0420] 5) Centrifuge the tube at 4000 rpm for 5 minutes.

[0421] 6) Dilute the buffer layer sample 20 times and the 1-octanol layer sample 200 times with the internal standard (IS) solution. Note: The dilution factor is mainly based on the characteristics of the compound.

[0422] 7) Analyze the samples using a triple quadrupole mass spectrometer. Correct the peak areas by the dilution factor and the embedded internal standard, and calculate the results (Log D values) using the ratio of the corrected peak areas.

[0423] 8) Use QC samples to monitor process Log D determination.

[0424] 9) Data analysis: The Log D value of each compound was calculated by the following formula:

[0425]

[0426] For the treated samples, different dilution factors will be used to process the different dilution values in the formula.

[0427] MW, XLogP and TPSA: These data points are calculated using Dotmatics.

[0428] Liver microsomal metabolic stability assay (NADPH):

[0429] 1) Test compounds were incubated at 37°C with liver microsomes (pooled from multiple donors) at 1.0 μM in the presence of NADPH (approximately 1.0 mM) at 0.5 mg / ml microsomal protein.

[0430] 2) Positive controls included testosterone (3A4 substrate), propafenone (2D6) and diclofenac (2C9). The positive controls were also incubated with microsomes in the presence of NADPH.

[0431] 3) Time samples (0, 5, 15, 30, 45 and 60 minutes) were taken and immediately mixed with cold acetonitrile containing internal standard (IS). Test compounds were also included that were incubated with the microsomes for 60 minutes without NADPH.

[0432] 4) Replicate points for each test condition (n=2).

[0433] 5) Samples were analyzed by LC / MS / MS; disappearance of test compounds was assessed based on the peak area ratio of analyte / IS (without standard curve).

[0434] 6) Excel data summary, calculated intrinsic clearance and T1 / 2 values are provided.

[0435] 7) Calculate microsomal clearance using the formula:

[0436]

[0437] For the five species, mg microsomal protein / g liver weight is 45. For rat, monkey, dog, human and mouse, liver weight values will be 40 g / kg, 30 g / kg, 32 g / kg, 20 g / kg and 88 g / kg, respectively. Hepatic clearance will be CL 固有(微粒体) Calculated using the following formula:

[0438]

[0439] The results from this example are summarized in Table 7 below.

21. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (eg, KCNT1) is epilepsy, an epileptic syndrome, or an encephalopathy.

22. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is hereditary or pediatric epilepsy or a hereditary or pediatric epilepsy syndrome.

23. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (eg, KCNT1) is a cardiac dysfunction.

24. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is selected from epilepsy and other encephalopathy (e.g., malignant migratory focal seizures of infancy (MMFSI) or infantile epilepsy with migratory focal seizures (EIMFS)), autosomal dominant nocturnal frontal lobe epilepsy (ADNFLE), West syndrome, infantile spasms, epileptic encephalopathy, focal epilepsy, Ohtahara syndrome, developmental and epileptic encephalopathy, and Lennox-Gastaut syndrome), seizures (e.g., generalized tonic-clonic seizures, asymmetric tonic seizures), leukodystrophy, leukoencephalopathy, intellectual disability, multifocal epilepsy, drug-resistant epilepsy, temporal lobe epilepsy, or cerebellar ataxia.

25. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is selected from the group consisting of arrhythmia, Brugada syndrome, and myocardial infarction.

26. The method of claim 19 or 20, wherein the nervous system disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is selected from pain and related conditions (e.g., neuropathic pain, acute / chronic pain, migraine).

27. The method of claim 19 or 20, wherein the nervous system disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is a muscle disorder (e.g., myotonia, neuromyotonia, cramp muscle spasm, spasticity).

28. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is selected from pruritus and pruritus, ataxia, or cerebellar ataxia.

29. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is a psychiatric disorder (e.g., major depressive disorder, anxiety disorder, bipolar disorder, schizophrenia).

30. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is selected from a learning disability, fragile X, neuronal plasticity, or autism spectrum disorder.

31. The method of claim 19 or 20, wherein the neurological disorder, the disorder associated with excessive neuronal excitability, or the disorder associated with a gain-of-function mutation in a gene (e.g., KCNT1) is selected from epileptic encephalopathy with SCN1A mutation, SCN2A mutation, and / or SCN8A mutation, early infantile epileptic encephalopathy, Dravet syndrome, Dravet syndrome with SCN1A mutation, generalized epilepsy with febrile seizures, intractable childhood epilepsy with generalized tonic-clonic seizures, infantile spasms, benign familial neonatal-infantile seizures, SCN2A epileptic encephalopathy, focal epilepsy with SCN3A mutation, cryptogenic partial epilepsy in children with SCN3A mutation, SCN8A epileptic encephalopathy, Rasmussen encephalitis, encephalitis, malignant migrating partial seizures of infancy, autosomal dominant nocturnal frontal lobe epilepsy, KCNQ2 epileptic encephalopathy, or KCNT1 epileptic encephalopathy.
