Abstract
Description
TECHNICAL FIELD
[0001] The present application belongs to the technical field of medical devices, and particularly relates to a valve leaflet clamping device. BACKGROUND
[0002] During a heart valve repair procedure, an operator needs to clamp a native valve leaflet with a valve leaflet clamping device, and then anchor a prosthetic valve to the native valve leaflet, so as to achieve the purpose of enhancing the function of the valve leaflet. [0024] like Figures 1 to 6 , Figures 12 to 14 As shown, the leaflet clamping device provided in the embodiments of this application includes a catheter component 10, a drive mechanism 20, a first clamping arm 30, a second clamping arm 40, and an auxiliary clamping component 50. The drive mechanism 20 includes a handle assembly 21 and a drive assembly. The handle assembly 21 is connected to the proximal end of the catheter component 10, and the drive assembly is connected to the handle assembly 21 and passes through the catheter component 10. The first clamping arm 30 is located at the distal end of the catheter component 10 and has a first clamping surface 31. The first end of the second clamping arm 40 is rotatably connected to the first clamping arm 30 via a first pin 81, and the second end of the second clamping arm 40... A second clamping surface 41 is provided. A drive assembly is driven to the second clamping arm 40 to drive the second clamping arm 40 to open or close relative to the first clamping arm 30. When the second clamping arm 40 is closed, the second clamping surface 41 is adapted to contact the first clamping surface 31. An auxiliary clamping member 50 is assembled on the first clamping arm 30. The auxiliary clamping member 50 is provided with an auxiliary clamping part 51. The drive assembly is driven to the auxiliary clamping member 50. When the second clamping arm 40 is open, the drive assembly drives the auxiliary clamping member 50 to move toward the second clamping arm 40 so that the auxiliary clamping part 51 can abut against the second clamping surface 41.
[0025] The operator uses the leaflet clamping device provided in the embodiments of this application to clamp the original leaflet 71 to be reinforced during cardiac leaflet repair surgery. In the initial state, the second clamping arm 40 and the auxiliary clamping member 50 of the leaflet clamping device are both closed relative to the first clamping arm 30; that is, the first clamping arm 30, the second clamping arm 40, and the auxiliary clamping member 50 are closed and clustered together at the distal end of the catheter member 10, as shown below. Figure 1 , Figure 3 , Figure 5 , Figure 12 andFigure 13 As shown. Then, the first clamping arm 30, the second clamping arm 40, and the auxiliary clamping member 50, which are joined together, are pushed distally along with the catheter member 10 to the original leaflet 71 of the heart to be reinforced. Then, the operator drives the drive assembly via the operating handle assembly 21. The drive assembly first drives the second clamping arm 40 to open relative to the first clamping arm 30, as shown. Figure 6 As shown, the first clamping surface 31 and the second clamping surface 41 are positioned far apart to form a space for accommodating the native leaflet 71 to be reinforced. Next, the operator holds the handle assembly 21 and adjusts the relative positions of the first clamping arm 30, the second clamping arm 40, and the native leaflet 71 to be reinforced, so that the native leaflet 71 to be reinforced is positioned between the first clamping surface 31 and the second clamping surface 41, and that the native leaflet 71 to be reinforced is abutted against the second clamping surface 41, as shown. Figure 7 and Figure 15 As shown. Then, the operator continues to operate the handle assembly 21 to drive the drive assembly to move the auxiliary clamping member 50 toward the second clamping surface 41 of the second clamping arm 40, until the auxiliary clamping part 51 of the auxiliary clamping member 50 contacts the original leaflet 71 to be reinforced and clamps it on the second clamping surface 41, as shown. Figure 8 and Figure 15 As shown, at this time, the second clamping surface 41 supports the original leaflet 71, while the auxiliary clamping member 50 applies force to the original leaflet 71 to clamp it onto the second clamping surface 41. Then, the operator continues to operate the handle assembly 21 to drive the drive assembly to simultaneously drive the second clamping arm 40 and the auxiliary clamping member 50 towards the first clamping arm 30 to close. During this process, the second clamping arm 40 and the auxiliary clamping member 50 maintain a clamping state on the original leaflet 71 to be reinforced until the original leaflet 71 is moved onto the first clamping surface 31. Then, the first clamping surface 31 and the second clamping surface 41 stably clamp the original leaflet 71 to be reinforced, as shown. Figure 9 As shown. Thus, during the process of pushing the original leaflet 71 towards the first clamping surface 31, the second clamping surface 41 and the auxiliary clamping part 51 are always in a clamping state on the original leaflet 71 to be reinforced, that is, the original leaflet 71 is initially clamped and stabilized, reducing the possibility that the original leaflet 71 may slip off the second clamping surface 41 due to blood flow impact and tissue elasticity during the process of being pushed towards the first clamping surface 31. This is beneficial to improving the clamping success rate of the original leaflet 71 to be reinforced, reducing the number of repeated clamping operations, thereby reducing the operation time and thus reducing the surgical risk to a certain extent.
[0026] In the leaflet clamping device of this application, a head end member 36 is installed at the distal end of the first clamping arm 30. Furthermore, the distal end of the head end member 36 is configured with a smooth, rounded head. This allows the head end member 36 to first contact the human tissue during the process of pushing the first clamping arm 30, the second clamping arm 40, and the auxiliary clamping member 50 together to the original leaflet 71 to be reinforced in the heart. This opens up the human tissue, allowing the first clamping arm 30, the second clamping arm 40, and the auxiliary clamping member 50 to be smoothly advanced. Moreover, the smooth, rounded head of the head end member 36 reduces the possibility of scratching or injuring the human tissue during advancement, protecting the safety of the patient. In some feasible embodiments, the distal end of the first clamping arm 30 can be directly configured with a smooth, rounded head to replace the head end member 36.
[0027] Furthermore, in the leaflet clamping device of this application, the proximal end of the first clamping arm 30 can be fixedly connected to the distal end of the conduit member 10, or it can be detachably connected to the distal end of the conduit member 10, or the first clamping arm 30 and the conduit member 10 can be integrally formed. As an example, the proximal end of the first clamping arm 30 can be provided with a first thread, and the distal end of the conduit member 10 can be provided with a second thread adapted to the first thread, so that the proximal end of the first clamping arm 30 and the distal end of the conduit member 10 are screwed together by the first thread and the second thread. Alternatively, as an example, the proximal end of the first clamping arm 30 can be provided with a first locking portion, and the distal end of the conduit member 10 can be provided with a second locking portion adapted to the first locking portion, so that the proximal end of the first clamping arm 30 and the distal end of the conduit member 10 are locked together by the first locking portion and the second locking portion. Or, as an example, the proximal end of the first clamping arm 30 and the distal end of the conduit member 10 can be fused together. Therefore, the proximal end of the first clamping arm 30 and the distal end of the catheter component 10 can be connected in any way, without any limitation, but the connection stability between the first clamping arm 30 and the catheter component 10 must be ensured.
[0028] In some embodiments, a first leaflet clamping device is provided, such as Figures 1 to 6 , Figure 10 , Figure 11 As shown.
[0029] In the first type of leaflet clamping device, the auxiliary clamping member 50 includes an auxiliary clamping arm 52, which is rotatably connected to the first clamping arm 30. Specifically, as shown... Figure 10 and Figure 11As shown, the auxiliary clamping arm 52 has two opposing rotating posts 522, which are rotatably mounted on the first clamping arm 30. A drive assembly is driven to the first end of the auxiliary clamping arm 52, allowing the operator to open or close the auxiliary clamping arm 52 relative to the first clamping arm 30 via the operating handle assembly 21. Furthermore, an auxiliary clamping part 51 is located at the second end of the auxiliary clamping arm 52. When the drive assembly drives the auxiliary clamping arm 52 to move towards the second clamping arm 40 to open, the second end of the auxiliary clamping arm 52 (i.e., the auxiliary clamping part 51) can abut against the second clamping surface 41, thereby clamping the original leaflet 71 to be reinforced onto the second clamping surface 41. This is beneficial to improving the success rate of clamping the original leaflet 71 to be reinforced, reducing the number of repeated clamping operations, thereby reducing the operation time and thus reducing the surgical risk to a certain extent.
[0030] like Figure 4 and Figure 8 As shown, in the first leaflet clamping device, the first clamping arm 30 is provided with a receiving groove 32. When the auxiliary clamping arm 52 is closed, the auxiliary clamping arm 52 is accommodated in the receiving groove 32. Thus, when the first clamping arm 30, the second clamping arm 40, and the auxiliary clamping arm 52 are closed and hugged together, the auxiliary clamping arm 52 sinks into the receiving groove 32, making the volume of the first clamping arm 30, the second clamping arm 40, and the auxiliary clamping arm 52 as compact as possible, with a small volume. Furthermore, the distal end of the head member 36 is designed with a rounded and smooth head, which can reduce the propulsion resistance in the body and facilitate smooth propulsion in the body, thereby smoothly reaching the original leaflet 71 of the heart to be reinforced.
[0031] like Figure 10 and Figure 11 As shown, in the first leaflet clamping device, the auxiliary clamping arm 52 is provided with a clearance channel 521 for avoiding the first clamping surface 31, and the receiving groove 32 surrounds the first clamping surface 31. On the one hand, by avoiding the first clamping surface 31 through the clearance channel 521, the auxiliary clamping arm 52 can smoothly open or close between the first clamping arm 30 and the open second clamping arm 40, avoiding motion interference between the first clamping surface 31 and the auxiliary clamping arm 52. On the other hand, by providing the clearance channel 521, the size and weight of the auxiliary clamping arm 52 are reduced, thereby facilitating the overall lightweight optimization design of the leaflet clamping device.
[0032] like Figure 3 , Figure 4, Figures 6 to 9 As shown, in the first type of leaflet clamping device, the driving assembly includes a first driving rod 221, which passes through the catheter component 10. One end of the first driving rod 221 is connected to the handle assembly 21, and the other end is rotatably connected to the first end of the auxiliary clamping arm 52. In this embodiment, the rotating column is located between the first and second ends of the auxiliary clamping arm 52. Thus, when the operator operates the handle assembly 21, the first driving rod 221 is driven, which in turn drives the first end of the auxiliary clamping arm 52, allowing the auxiliary clamping arm 52 to open or close relative to the first clamping arm 30. When the auxiliary clamping arm 52 is open relative to the first clamping arm 30, the second end of the auxiliary clamping arm 52 (i.e., the auxiliary clamping part 51) can abut against the second clamping surface 41, thereby initially clamping and stabilizing the native leaflet 71.
[0033] like Figure 11 , Figure 13 and Figure 14 As shown, in the first type of leaflet clamping device, at least the auxiliary clamping part 51 of the auxiliary clamping arm 52 is provided with a first anti-slip structure 54, and at least a portion of the first anti-slip structure 54 can abut against the second clamping surface 41. Thus, when the auxiliary clamping part 51 of the auxiliary clamping arm 52 moves toward the second clamping surface 41 and clamps the original leaflet 71 to be reinforced onto the second clamping surface 41, the first anti-slip structure 54 is in direct contact with the original leaflet 71, effectively reducing the possibility of the original leaflet 71 slipping out, thereby achieving initial clamping and stabilization of the original leaflet 71 on the second clamping surface 41. Then, the second clamping arm 40 and the auxiliary clamping arm 52 move toward the first clamping arm 30 in a unified manner while clamping the original leaflet 71, until the second clamping surface 41 of the second clamping arm 40 and the first clamping surface 31 of the first clamping arm 30 clamp the original leaflet 71 (at this time, the auxiliary clamping arm 52 sinks into the receiving groove 32), improving the success rate and clamping stability of clamping the original leaflet 71.
[0034] In some feasible implementations, the auxiliary clamping arm 52, from the connection point between the auxiliary clamping arm 52 and the first clamping arm 30 to the auxiliary clamping portion 51, may be provided with a first anti-slip structure 54 throughout, or most of this portion may have a continuous first anti-slip structure 54 covering the entire auxiliary clamping portion 51. Thus, during the opening of the auxiliary clamping arm 52 relative to the first clamping arm 30, the original leaflet 71 is always in contact with the first anti-slip structure 54 to prevent it from slipping out of the space between the auxiliary clamping arm 52 and the second clamping arm 40, until the auxiliary clamping portion 51 clamps the original leaflet 71, thereby increasing the success rate of initially clamping and stabilizing the original leaflet 71 on the second clamping surface 41.
[0035] Furthermore, such as Figure 3 , Figure 6 ,Figure 7 As shown, the first clamping surface 31 is provided with a second anti-slip structure 34, which can abut against the second clamping surface 41 when the auxiliary clamping member 50 is closed. That is, when the second clamping surface 41 of the second clamping arm 40 and the first clamping surface 31 of the first clamping arm 30 clamp the original leaflet 71, although the auxiliary clamping arm 52 is sunk into the receiving groove 32, the first anti-slip structure 54 still abuts against the original leaflet 71, and the second anti-slip structure 34 also abuts against the original leaflet 71, thereby further stabilizing the clamping of the original leaflet 71 and reducing the possibility of the original leaflet 71 slipping off the first clamping surface.
[0036] like Figure 3 , Figure 4 , Figures 6 to 9 As shown, the drive assembly includes a second drive rod 224, which passes through the conduit member 10. One end of the second drive rod 224 is connected to the handle assembly 21, and the other end is rotatably connected to the first end of the second clamping arm 40 via a second pin 82. The axis of the second pin 82 is parallel to and spaced apart from the axis of the first pin 81. When the operator operates the handle assembly 21 to drive the second drive rod 224, the second drive rod 224 drives the first end of the second clamping arm 40 to rotate around the axis of the first pin 81, thus opening or closing the second clamping arm 40 relative to the first clamping arm 30.
[0037] In embodiments of this application, the leaflet clamping device further includes a puncture assembly 60, a release rod 227, and an artificial leaflet 72. The puncture assembly 60 includes an implantation anchor 61 and a fastening member 62. The implantation anchor 61 is fixedly connected to the artificial leaflet 72 and is disposed on the second clamping surface 41. Specifically, the implantation anchor 61 has a fixing hole 611. The distal end of the release rod 227 passes through the fixing hole 611 and is adapted to be inserted into the insertion hole of the second clamping arm 40, thereby fixing the implantation anchor 61 on the second clamping arm 40. When the second clamping arm 40 is closed relative to the first clamping arm 30, the artificial leaflet 72 is clamped between the first clamping arm 30 and the second clamping arm 40. When the second clamping arm 40 is opened relative to the first clamping arm 30, the artificial leaflet 72 opens along with the second clamping arm 40 relative to the first clamping arm 30, that is, a space for accommodating the original leaflet 71 to be reinforced is formed between the artificial leaflet 72 and the first clamping surface 31. The fastener 62 is disposed on the first clamping arm 30. Specifically, the first clamping arm 30 is provided with a guide groove 35, one end of which extends through to the first clamping surface 31. The fastener 62 is slidably installed in the guide groove 35. Further, the drive assembly includes a third drive rod 226 and a drive block 225. The distal end of the third drive rod 226 is fixedly connected to the drive block 225, and the proximal end of the third drive rod 226 is connected to the handle assembly 21. The drive block 225 is slidably installed on the first clamping arm 30 along the extension direction of the first clamping arm 30, and the drive block 225 is provided with a drive ramp 2251. The other end of the guide groove 35 extends through to the space where the drive block 225 is located, and the fastener 62 abuts against the drive ramp 2251.
[0038] When the end of the original leaflet 71 to be reinforced covers the implantation anchor 61 and overlaps with part of the artificial leaflet 72, and the original leaflet 71 and the artificial leaflet 72 are stably clamped by the first clamping surface 31 and the second clamping surface 41, the operator will continue to operate the handle assembly 21 to drive the third drive rod 226 of the drive assembly to move towards the proximal end. The third drive rod 226 pulls the drive block 225 to slide towards the proximal end. Since the fastening member 62 abuts against the drive slope 2251 of the drive block 225, the drive slope 2251 will drive the fastening member 62 to move towards the second clamping surface 41. The fastening member 62 includes a fastening plate 621 and a puncture nail 622. The puncture nail 622 includes a nail rod 624 connected to the fastening plate 621 at one end and a nail head 623 connected to the other end of the nail rod 624. The implantation anchor 61 is provided with an anchor hole 612 that can be elastically deformed and opened and closed. When the fastener 62 is driven by the inclined plane 2251 toward the second clamping surface 41, the nail head 623 pierces the original leaflet 71 and the artificial leaflet 72. Then, the nail head 623 expands and passes through the anchor hole 612, which elastically contracts to prevent the nail head 623 from dislodging. This completes the piercing operation of the original leaflet 71 and the artificial leaflet 72. Next, the operator continues to operate the handle assembly 21 to pull the release rod 227, causing the distal end of the release rod 227 to disengage from the second clamping arm 40 and from the fixing hole 611 of the implanted anchor 61, allowing the implanted anchor 61 to detach from the second clamping arm 40. Then, the operator again operates the handle assembly 21 to drive the second drive rod 224 to open the second clamping arm 40 relative to the first clamping arm 30, thereby releasing the original leaflet 71 and the artificial leaflet 72. Figure 16 As shown. When the original leaflet 71, the artificial leaflet 72 and the puncture assembly 60 are disengaged from the clamping space between the first clamping arm 30 and the second clamping arm 40, the second clamping arm 40 is closed again, and the leaflet clamping device can be extracted.
[0039] In other embodiments, a second leaflet clamping device is provided, such as Figures 12 to 15 As shown. Compared with the first type of leaflet clamping device described above, the second type of leaflet clamping device has the following differences.
[0040] In the second type of leaflet clamping device, such as Figures 12 to 15As shown, the auxiliary clamping member 50 includes a telescopic arm 53. The first clamping arm 30 is provided with a mounting hole 33, which passes through the first clamping surface 31. The telescopic arm 53 can be slidably installed in the mounting hole 33. The auxiliary clamping part 51 is the end face of the telescopic arm 53 facing the second clamping arm 40. That is, when the second clamping arm 40 is closed relative to the first clamping arm 30, the telescopic arm 53 retracts into the mounting hole 33; when the second clamping arm 40 is open relative to the first clamping arm 30 and performs initial clamping and stabilization on the original leaflet 71 to be reinforced, the telescopic arm 53 extends out of the mounting hole 33 towards the second clamping surface 41 until the auxiliary clamping part 51 of the telescopic arm 53 abuts against the second clamping surface 41. This is beneficial to improving the success rate of clamping the original leaflet 71 to be reinforced, reducing the number of repeated clamping operations, thereby reducing the operation time and thus reducing the surgical risk to a certain extent.
[0041] like Figures 13 to 15 As shown, in the second type of leaflet clamping device, the drive assembly includes a pull rope 222 and an elastic element 223. The pull rope 222 passes through the guide tube component 10, with one end connected to the handle assembly 21 and the other end connected to the telescopic arm 53. The elastic element 223 is installed in the mounting hole 33, with both ends of the elastic element 223 abutting against the telescopic arm 53 and the first clamping arm 30, respectively. By tightening the pull rope 222, the elastic element 223 is compressed, thereby allowing the telescopic arm 53 to retract into the mounting hole 33. When the pull rope 222 is released, the compressed elastic element 223 applies a spring force to the telescopic arm 53 to drive the telescopic arm 53 to extend out of the mounting hole 33, and the auxiliary clamping part 51 of the telescopic arm 53 abuts against the second clamping surface 41.
[0042] like Figure 15As shown, when the first clamping arm 30 and the second clamping arm 40 are pushed to the original leaflet 71 of the heart to be reinforced, the second drive rod 224 drives the second clamping arm 40 to open relative to the first clamping arm 30, allowing the original leaflet 71 to be reinforced to enter between the first clamping arm 30 and the second clamping arm 40. Next, the operator operates the handle assembly 21 to release the pull rope 222, causing the telescopic arm 53 to extend out of the mounting hole 33 under the elastic force of the elastic element 223, thus initially and stably clamping the original leaflet 71 onto the second clamping surface 41. Then, the operator operates the handle assembly 21 simultaneously to drive the second drive rod 224 and the pull rope 222, causing the second clamping arm 40 and the telescopic arm 53 to move together toward the first clamping arm 30 while still clamping the original leaflet 71, until the second clamping surface 41 clamps the original leaflet 71 onto the first clamping surface 31.
[0043] Furthermore, in the second type of leaflet clamping device, the mounting hole 33 and the guide groove 35 are spaced apart along the proximal to distal direction. Additionally, the drive block 225 is provided with a passage groove for avoiding the telescopic arm 53. Thus, during the process of the third drive rod 226 pulling the drive block 225 towards the proximal end, the telescopic arm 53 is located in the passage groove, thereby avoiding motion interference between the drive block 225 and the telescopic arm 53.
[0044] The second type of leaflet clamping device is identical to the first type of leaflet clamping device except for the structural differences mentioned above, and will not be described in detail here.
[0045] The above description is merely a preferred embodiment of this application and is not intended to limit this application. Any modifications, equivalent substitutions, and improvements made within the spirit and principles of this application should be included within the protection scope of this application.
