AI Technical Summary
Existing methods for alveolar bone regeneration and dental implantation in patients with atrophic alveolar bone require lengthy rehabilitation times, and dental implants are prone to complications like peri-implantitis, especially in cases of vertical bone loss.
A deflection device, known as the implantodistractor, comprising a deflectable dental implant and a distraction activator, allows simultaneous bone distraction and implant insertion, utilizing biocompatible materials to create a gap for new bone formation around the implant, thereby shortening treatment time and addressing peri-implant bone augmentation.
The implantodistractor device facilitates faster rehabilitation by integrating dental implants with augmented bone, reducing the risk of complications such as peri-implantitis and enabling treatment of atrophic alveolar bone and peri-implantitis through vertical bone distraction.

Abstract
Description
Field of the invention
[0001] The present invention relates to the field of bone distraction of alveolar bone and dental implants. Background of the invention
[0002] When a tooth is lost due to caries or trauma, the lack of natural stimulation during chewing reduces bone formation and leads to alveolar bone atrophy. If a patient lacks sufficient alveolar bone to receive dental implants, a procedure known as alveolar bone regeneration is usually required. Several techniques exist to achieve this goal, including the use of autologous bone, bone replacement biomaterials, and distraction histiogenesis. In all cases, however, the patient requires a rehabilitation period of 4 to 6 months until the alveolar bone is ready to receive a dental implant.
[0003] Distraction histiogenesis is a biological treatment option commonly used for bone defects in maxillofacial and orthopedic surgery. During distraction histiogenesis of the alveolar bone, the bone is cut, creating a moving segment known as the "transport" segment. A distraction histiogenesis device is then attached to the transport segment, and as the bone segment is distracted from the remaining jawbone, bone callus formation and soft tissue expansion occur in the created space. Once the desired distraction is achieved, the bone callus can heal and solidify into new bone. After the healing period, the distraction device is removed. There are two main parts to such a distractor.A superior part with the ability to move up and down to the sides using so-called "wings" (titanium plates) that are attached to the transport segment with titanium screws. And a lower part that is attached to the jawbone with titanium plates and screws, but does not move. The direction in which the lower part is fixed determines the deflection vector. After the placement of dental implants, they can be integrated into the bone (osseointegration) for 3 to 6 months, depending on the bone quality. Only when proper stability is achieved can a denture unit be attached to a dental implant. In general, the complete rehabilitation of a patient with alveolar bone atrophy takes between 7 and 12 months. Nevertheless, dental implants are widely used due to their high success rate. However, they are prone to complications such as bone infections (periimplantitis) after some time, which continue to pose a challenge. This is especially true in cases of vertical bone loss leading to implant loosening. Bone augmentation using distraction osteogenesis for the treatment of alveolar bone atrophy and periimplantitis has not been observed. Subjects of invention
[0004] The present invention intends to solve two (2) problems: 1- Reducing the rehabilitation time of patients with alveolar bone atrophy by performing bone distraction and implant placement simultaneously. 2- Bone augmentation of dental implants with peri-implantitis by allowing distraction of the peri-implant bone. This avoids the need for dental implant removal. Summary of the invention
[0005] The present invention solves the above-mentioned problems by providing a distraction device, hereinafter referred to as an "implant distraction device," consisting of two elements: Element A, or a deflectable dental implant, and Element B, or a distraction activator for distracting peri-implant bone segments. The implant distraction device can be made entirely of biocompatible materials, and its application methods are particularly useful in the fields of plastic surgery, orthopedic surgery, and implantology.
[0006] The deflected implant should be placed according to standard protocols for dental implant placement. After insertion into the bone, the implant head is attached to the stabilizing arm of the distraction activator. The upper part of the deflectable implant can then be deflected vertically, separating the lower and upper parts of the implant along with the moving transport segment. This creates a gap in which new bone forms around the implant. The amount of bone harvested can vary and is determined by the size of the deflectable dental implant, which incorporates a locking system. In the event of a bone infection surrounding placed deflected dental implants, the dental implant can be rehabilitated using the distraction activator. The stabilizing arm of the distraction activator is attached to the already placed deflected implant with a healing screw. The deflectable implant must be placed at the level of the alveolar bone and deflected to harvest the bone vertically.
[0007] The distraction activator consists of a stabilizing arm that can be attached to one or more distracted implants with a screw. This allows patients with atrophied alveolar bone to be treated simultaneously with distraction histiogenesis and dental implants, thus reducing treatment time. The same situation can be applied to jaw reconstructions with femoral bone grafts. The distraction activator consists of a central rod and a superior central island with wings. Place the stabilizing arm on the central island.
[0008] The stabilizing arm surrounds the central rod but is not wrapped around it. This allows the stabilizing arm to be moved in a vertical axis. The arm can also be extended horizontally to adjust the distance from the distracted dental implant. The wings allow the distractor to be attached to the transport segment and to the stable bone. The wing is designed to be triangular, as the bone area available for fixation is variable. This shape allows for the placement of a smaller screw near the apex of the triangle and a larger screw higher in the triangle, thus avoiding the need for larger plates. The above summary is for illustrative purposes only and is not intended to be limiting in any way.In addition to the illustrative aspects, in addition to the features described above, further aspects and features will become apparent with reference to the drawings, the following detailed description and the following claims. Short description of the drawing Fig. is an isometric rear view showing the implant distractor device with its 2 elements assembled. Fig. is a right isometric view of the implant distractor device with the deflectable dental implant and distraction activator assembled. Fig. is a left isometric view of the implant distractor device with the deflectable dental implant and the assembled distraction activator. Fig. is a side view of the implant distractor device with the deflectable dental implant and distraction activator assembled. Fig. is a half-sectional view of the deflectable dental implant. Specific description
[0009] Fig. Shows the distractor activator, which consists of a sleeve (1) to cover the central threaded rod (2). There are three grooves in the sleeve. Two side rails (3) on the sides allow the movement of the upper horizontal wings (4). The third groove is the posterior rail (5), located at the front of the distractor and facing the distracted dental implant, or any other type of denture, to allow the up and down movement of the stabilizing arm (6). Fig. shows the stabilizing arm resting on the deflected island (7) and around the central threaded rod (2), but not wrapped around it. In front of the stabilizing arm is a ring (8) that allows the stabilizing arm to be attached to the head of the deflectable dental implant or other type of dental prosthesis with a screw / healing screw. The stabilizing arm can be extended to adjust the distance from the ring to the deflectable dental implant (9). The upper part of the ring holds a bearing surface (10) for a screw / healing screw.
[0010] Fig. shows the hexagonal washer (11) immediately below the sleeve, which prevents the stabilizing arm from rotating. The central threaded rod (2) passes through the deflected island (7) and is inserted into the base of the device (12). The deflected island is smooth on the outer surface and threaded in the center to interlock with the central threaded rod (2). On the deflected island, there are horizontal wings on both sides. The upper horizontal wings (4) and the lower horizontal wings (13) consist of slots that can have a triangular or circular shape (14). The apex of the triangles points towards the base of the device. Screws of different sizes can be placed through the triangle or the circular slots and drilled into the bone to secure the device to the bone. As shown in Fig. As shown, the base of the device (12) is fixed and provides support for the lower horizontal wings (13). The device can be of various sizes. After the device is fixed, the deflectable dental implant can be stabilized, osteotomies can be performed, and the bone segments can be deflected vertically.
[0011] The deflectable dental implant can be attached to the distractor activator with a screw on the stabilizer. The deflectable implant consists of a lower (15) and an upper part (16).
[0012] ( Fig. ). Fig.shows that the lower part is a compromised cylinder on the outside (17). In the upper part of the lower part there is a locking system with hooks and self-locking threads (18). The purpose of the locking system is to control vertical deflection and telescoping. The upper part is a compromised cylinder (19). In the lower part of the upper part there are hooks (20). When the upper part is pulled up, the hooks slide over the threads and the gear with the hooks of the lower part of the implant (21) to prevent further vertical deflection. In a half-section view of a deflectable dental implant, a cylinder with an internal thread (22) can be seen in the center of the upper part of the deflectable dental implant, into which the healing screw can be threaded after the implant is inserted. Around the internally threaded cylinder there is a sacred space (23). Inside, the lower part of the implant has the sacred peripheral shape (24).The internally threaded cone of the upper part, i.e., the healing screw or abutment, rests within the sacred peripheral shape of the lower part when inserted. The lower segment of the lower part of the implant is a fixed part (25). The distractor device and the deflectable dental implant can be manufactured in various sizes and in all types of biocompatible materials.
[0013] The description of the embodiments of the device focuses on the jaws to understand the principles of the device. The methods of the invention are applicable to other parts of the body.
[0014] According to the features of the present invention, one embodiment is the distraction of the atrophied alveolar bone (Problem 1). The method comprises the following steps: (a) cutting the bone to form the moving segment; (b) inserting the distracted implant; (c) securing the distractor device using the wings and screws; (d) attaching the stabilizing arm and the distracted implant; (e) applying torque to the central rod to mobilize the central island and the bone segment.
[0015] According to the features of the present invention, another embodiment is the implementation of the deflectable implant into healthy alveolar bone with sufficient height (Problem 2). The intention of using the invention in patients with sufficient bone levels is to enable bone deflection in the event of complications following implant placement, i.e., peri-implantitis or loss of alveolar bone levels due to overloading. In such a case, the distractor activator would be placed, and distraction can occur.
[0016] According to the features of the present invention, another embodiment is the treatment of an already placed deflectable dental implant with peri-implantitis (Problem 2). The method comprises the following steps: (a) removing the infected bone surrounding the deflectable dental implant; (b) cutting the healthy peri-implant bone to form the moving segment; (c) telescoping the deflectable dental implant until it corresponds to the alveolar bone level; (d) securing the distractor device using the wings and screws; (e) attaching the stabilizing arm to the deflectable dental implant; (f) distracting the deflectable dental implant and the transport segment to the desired alveolar bone level.
[0017] According to another aspect of the present invention, an embodiment is for treating a multiple-placed deflectable dental implant with peri-implantitis (Problem 2). The stabilizing arm can have multiple rings, allowing multiple deflectable dental implants to be deflected simultaneously. This method can also be applied to reconstruct critical jaw bone defects using fibula or other types of grafts.
