Abstract
Description
Title of the invention: Post-operative compression device Technical field
[0001] The invention relates to a post-operative compression device, intended more particularly to be used following an operation to implant a cardiovascular implantable electronic device. Technical background
[0002] After a surgical operation, in particular an operation to implant a cardiovascular implantable electronic device, such as a permanently implantable cardiac pacemaker, an implantable automatic defibrillator, or a cardiac resynchronization therapy device, it is recommended to exert compression on the part of the body where the device has been implanted within 24 to 48 hours following the operation in order to limit the risk of hematomas or bleeding.
[0003] For this purpose, it is known to manufacture a compression bandage using an assembly of compresses forming a compression block which is fixed to the patient's body using elastic adhesive strips. Thus, an operator must first prepare the compression block using compresses generally held together using, for example, an envelope formed by elastic adhesive strips, and, secondly, position this compression block on the person to be treated and place the elastic adhesive strips over it while attempting to apply a desired compression force. However, such a solution has several drawbacks. First of all, removing the elastic adhesive strips can be painful for the patient, or even cause skin injuries. Secondly, it does not allow the practitioner to examine the bandage or to change the compression force applied throughout the duration of the compression hold. Then, the quality of this compression bandage depends on the expertise of the operator who applies it, which is generally not desirable. Finally, depending on the movements made by the patient once the compression bandage is applied, there is a risk that one or more of the elastic adhesive strips will come loose, leading to a change in the compression force applied and a risk of displacement, or even falling, of the compression block. Summary of the invention
[0004] The invention aims in particular to provide a postoperative compression device making it possible to overcome the drawbacks mentioned below. More particularly, the invention aims to provide a postoperative compression device operating procedure which is simple, fast and reliable to use, and for which management of the applied compression force is facilitated.
[0005] To this end, the invention relates to a post-operative compression device, comprising: - a compression member comprising a foam compression block and at least three textile strip fixing means distributed angularly around a central portion of the compression block with an angle of between 90° and 150° between two consecutive fixing means, and - at least three textile strips, each textile strip being fixed by one of its longitudinal ends to one of the textile strip fixing means.
[0006] The use of textile bands advantageously makes it possible to best control the pressure applied by the compression member to the area of the patient's body where post-operative compression is required. It is understood in particular that it is easy to adjust the compression force applied by modulating the tension of the textile bands. In addition, the use of textile bands whose length can be adjusted reduces the patient's discomfort because they adapt best to the patient's morphology and the movements that the patient makes. It is understood in particular that it is possible to adapt to the different chest sizes of patients.
[0007] Since the textile strips are each fixed to one of the fixing means, these fixing means being angularly offset by 90° to 150° between two successive fixing means, it is understood that the textile strips can extend from these fixing means towards the outside while being themselves angularly offset by approximately 90° to 150° between two successive textile strips. Thus, the compression force is applied in the most uniform manner possible, which makes it possible to obtain good results in reducing the risk of hematoma and bleeding and improves the comfort of the person wearing the compression device.
[0008] Optionally, it may be provided that the compression member comprises a support plate, the textile strip attachment means preferably being attached to or formed by the support plate. The use of a solid support plate and a foam compression block makes it possible to obtain a semi-rigid compression member which advantageously makes it possible to facilitate the application of the pressure force by applying tension at the solid support plate while having a foam surface which presses directly on the patient's body, which further reduces the discomfort that the patient may feel.
[0009] The invention may also include one or more of the following optional features, taken alone or in combination.
[0010] Preferably, at least one of the textile strips is made of non-elastic textile, preferably all the textile strips are made of non-elastic textile. It is thus easier to control the tension that is applied to the compression block by the textile bands, and therefore to control the compression force exerted by the compression block on the body of the person wearing the postoperative compression device. It is understood that it is possible to easily select the desired textile properties, such as tension, dimensions, or density, depending on the desired use. The textile fibers used for the textile bands are for example chosen from natural textile fibers such as cotton, linen, silk, or wool, or from synthetic textile fibers such as polyester or polyamide.
[0011] Preferably, at least one of the textile strip fastening means comprises a fastening loop attached to the compression block, preferably each textile strip fastening means comprising a fastening loop attached to the compression block. The use of a fastening loop to make the fastening of the textile strips to the compression block simple and economical to implement.
[0012] Preferably, at least one of the textile strip fastening means is formed by a fastening orifice, preferably each textile strip fastening means is formed by a fastening orifice. This is a simple and economical way of fastening the textile strips to the compression block. Preferably, at least one of the textile strip fastening means comprises a fastening loop fixed to the compression block, preferably each textile strip fastening means comprising a fastening loop fixed to the compression block. It is understood that each fastening loop defines a fastening orifice. According to one embodiment, it is provided that the textile strips are fixed directly in their respective fastening orifices. According to another embodiment, it is provided that the textile strips are fixed indirectly in their respective fastening orifices, for example by means of a connecting member.
[0013] Preferably, the angle between two consecutive textile band fastening means is 120°. Such an angular offset allows the most uniform application possible of the compression force, which improves the effectiveness of the compression and the comfort of the person wearing the compression device.
[0014] According to one embodiment, at least one of the textile bands forms an elastic band. Patient comfort is thus improved.
[0015] According to one embodiment, at least one of the textile strips is made of fabric. The textile strips are thus inexpensive and easy to implement. It is understood in particular that it is possible to easily select the desired fabric properties, such as elasticity, dimensions, or density, depending on the desired use. The fabric strips are preferably non-elastic in order to best control the tension applied to the compression block and therefore the compression force exerted by the compression block on the patient.
[0016] According to one embodiment, at least one of the textile strips is made of knitting. Knitting advantageously makes it possible to easily obtain a very elastic textile. In addition, the elastic strips are thus inexpensive and easy to implement. It is understood in particular that it is possible to easily select the desired properties of the knitting, such as elasticity, dimensions, or density, depending on the desired use.
[0017] Advantageously, the post-operative compression device further comprises adjustment means capable of increasing or decreasing the apparent length of the textile bands. The post-operative compression device can thus be best adapted to the different morphologies of the people on whom the compression device is used in order to exert the desired compression force. For example, when a person has a larger than average build, provision may be made to lengthen the apparent length of the textile bands. Conversely, when a person has a smaller than average build, provision may be made to shorten the textile bands. By "apparent length" is meant the useful length of the textile band, from its end fixed to one of the textile band fixing means to its other end.
[0018] Advantageously, the adjustment means comprise self-gripping fastening means formed on the one hand by a fastening strip fixed to one of the textile strips and on the other by a longitudinal end of the textile strip to which the fastening strip is fixed. A self-gripping type fastening is also known as a Velcro fastening. Such a fastening is typically made up of two components: a first component comprising hooks and a second component comprising loops. Such a fastening allows a temporary fastening to be carried out simply when the two components are pressed against each other. A self-gripping fastening allows a reversible, rapid, simple and economical fastening of two elements together. In the present case, it is understood that the fixing strip comprises hooks or loops intended to cooperate respectively with loops or hooks present on a longitudinal end of the textile strip to achieve the fixing. It is understood that it is easy to modulate the place of the textile strip on which the fixing strip is pressed in order to increase or decrease the apparent length of the textile strip.
[0019] Preferably, the compression member and the textile bands are configured so that the compression member is capable of applying a force of 7 to 8 N when the compression device is worn by a person. Such a compression force makes it possible to obtain the best results in terms of reducing the risk of hematoma and bleeding. It is understood that a person skilled in the art can select the characteristics of the device, in particular the nature and length of the textile strips on the one hand, and the characteristics of the foam compression block on the other hand, to obtain a compression force of 7 N to 8 N.
[0020] Advantageously, the post-operative compression device further comprises a member for holding the textile strips, each textile strip being intended to be fixed to the member for holding the textile strips by its longitudinal end which is not fixed to the compression member. This is a simple means of holding in position the longitudinal ends of the textile strips which are not fixed to the compression member and thus keeping the textile strips in tension.
[0021] Advantageously, the band holding member is made of fabric, preferably of non-elastic fabric. In this way, the holding member is simple to produce, economical, compact and comfortable to wear for the user of the compression device. Provision may be made for the textile bands to be removably or non-removably attached to the textile band holding member.
[0022] Preferably, when a support plate is present, it can be provided that the support plate and the compression block are formed in a single piece. The manufacture and use of the compression device are thus facilitated. According to one embodiment, the support plate and the compression block are made in a single piece by assembling sewn and edged fabric, which allows for quick and simple manufacture. [0032] Figures 1 to 5 show a post-operative compression device 1 according to a first embodiment of the invention. According to the first embodiment, the compression device 1 is intended to be used following an operation to implant an implantable electronic cardiac device. By implantable electronic cardiac device is meant any permanently implantable cardiac pacemaker, any implantable automatic defibrillator, as well as any cardiac resynchronization therapy device. Of course, according to other embodiments, it is provided that the post-operative compression device 1 according to the invention can be used after an operation of another type, as soon as compression is desired following the operation.
[0033] The compression device 1 is shown worn by a person 2 in Figures 2, 3 and 5. In this case, as a result of an operation to implant an implantable cardiac electronic device, the compression device 1 is configured so as to apply a compression force to the rib cage, substantially in the area where the operation took place, i.e. in the left (Figures 2 and 3) or right ([Fig.5]) portion of the rib cage of the person 2.
[0034] The compression device 1 comprises a compression member 3, three textile strips 4 and a member 9 for holding the textile strips 4.
[0035] According to the first embodiment, the compression member 3 comprises a support plate 6 and a compression block 7 made of foam.
[0036] In the present case, the support plate 6 has a generally circular shape and has three fixing orifices 8 distributed angularly around a central portion of the support plate 6 with an angle of 120° between two consecutive fixing orifices 8. To measure the angle a between two consecutive fixing orifices 8, the angle a formed between the central / respective points of the fixing orifices 8 and the central point p of the support plate 6. Preferably, the length of the fixing openings 8 is slightly greater than the width of the textile strips 4 so as not to apply too much tension to the lateral edges of the textile strips 4.
[0037] According to the first embodiment, the foam compression block 7 is formed by thermocompression of a foam, for example a high-density polyethylene foam or a polyurethane foam. Preferably, the support plate 6 and the compression block 7 are formed in a single piece by assembling sewn and edged fabric. The thermocompression method is known per se and will not be detailed here.
[0038] In the present case, the textile strips 4 are made of non-elastic fabric. Alternatively, it is possible to provide that the textile strips 4 are made of knitted fabric. Alternatively, it is possible to provide that the textile strips 4 are made of elastic fabric. Each textile strip 4 comprises two longitudinal ends, a first of these longitudinal ends being fixed to one of the fixing orifices 8 of the support plate 6, the other longitudinal end being fixed to the member 9 for holding the textile strips 4. Thus, each textile strip 4 is fixed on the one hand to the support plate 6 and on the other hand to the member 9 for holding the textile strips 4.
[0039] Since the textile strips 4 are each fixed in one of the fixing holes 8, it is understood that they extend from their respective fixing holes 8 outwards while being angularly offset by approximately 120° between two consecutive textile strips 4. Thus, the compression force is applied in the most uniform manner possible, which makes it possible to obtain good results in reducing the risk of hematoma and bleeding and improves the comfort of the person 2 wearing the compression device 1.
[0040] At the end of the textile strips 4 which is fixed to the support plate 6, each textile strip 4 forms a closed loop passing through the fixing orifice 8 associated with it, thus ensuring the fixing of the corresponding elastic strip 4 to the support plate 6 ([Fig.l]). Thus, in the present case, the textile strips 4 are directly fixed to the support plate 6. According to other embodiments, it can be provided that at least one of the textile strips 4 is fixed indirectly to the support plate 6, for example by means of a connecting member.
[0041] At the longitudinal end of the textile strips 4 fixed to the member 9 for holding the textile strips 4, each textile strip 4 is sewn to the member 9 for holding the textile strips 4. According to other embodiments, the fixing of the textile strips 4 to the member 9 for holding the textile strips 4 is carried out differently. In particular, it is possible to provide means for removable fixing of the textile strips 4 on the member 9 for holding the textile strips 4. For example, it is possible to use a self-gripping fixing of the Velcro type.
[0042] The compression device 1 further comprises adjustment means 10 capable of modifying the apparent length of the textile strips 4. The apparent length corresponds to the useful length of the textile strip 4, that is to say the length from its longitudinal end fixed to the support plate 6 to its longitudinal end fixed to the member 9 for holding the textile strips 4. In the present case, the adjustment means 10 are formed by an adjustment loop, i.e. a loop of the type used to adjust the length of a strap. More particularly, in the present case, each textile strip 4 is provided with an adjustment loop in order to be able to modify the respective apparent lengths of the textile strips 4 independently of each other. Thus, depending on the morphology of the person 2 wearing the compression device 1 and depending on the compression force to be applied, it is possible to easily adjust the apparent length of the textile bands 4 using the adjustment buckles. According to other embodiments, other means for adjusting the apparent length of the textile bands 4 are used.
[0043] According to the present embodiment, the member 9 for holding the textile strips 4 is advantageously made of fabric. According to other embodiments, it is provided that the member 9 for holding the textile strips 4 is made of any other material making it possible to hold the textile strips 4 in position.
[0044] The member 9 for holding the textile bands 4 has a generally substantially triangular shape, each vertex of the triangle corresponding to a zone in which one of the textile bands 4 is fixed respectively. In front view, as shown in Figures 1 and 2, according to a first configuration, the upper point of the triangle is offset to the right so as to facilitate the passage of the textile band 4 over the left shoulder of the person 2 wearing the compression device 1. Such a configuration facilitates the positioning of the compression device 1 when pressure must be applied substantially to the left portion of the rib cage. In addition, the comfort of the person 2 wearing the compression device 1 is improved by preventing a lateral edge of the textile band 4 from rubbing against the neck of the person 2 wearing the compression device 1.
[0045] The member 9 for holding the textile bands 4 makes it possible to simply modify the area of the rib cage to which the compression is applied. Indeed, depending on the area of implantation of the implantable cardiac electronic device, it may be desirable to apply pressure in the right portion of the rib cage rather than in the left portion. In this case, before fixing the textile bands to the support plate 6, it is possible to simply turn over the member 9 for holding the textile bands 4 according to a second configuration shown in FIGS. 4 and 5, so that, in front view, the upper point of the triangle is offset to the left so as to facilitate the passage of the textile band 4 over the shoulder. right of person 2 wearing the compression device 1. Such a configuration thus facilitates the placement of the compression device 1 when pressure must be applied substantially to the right portion of the rib cage.
[0046] Thus, the member 9 for holding the textile bands 4 is reversible and can adopt two different configurations, depending on whether compression is desired on the left portion (figures 1 to 3) or on the right portion (figures 4 and 5) of the rib cage.
[0047] In the present case, the compression member 3 and the textile strips 4 are configured in such a way that the compression member 3 applies a force of 7 to 8 N to the part of the body of the person 2 below which the compression block 7 is positioned. Such a force makes it possible to obtain the best results in terms of reducing the risks of hematoma and bleeding.
[0048] Figures 6 to 8 show a post-operative compression device 1 according to a second preferred embodiment of the invention.
[0049] Below we describe the compression device 1 according to the second embodiment of the invention only with regard to its differences from the first embodiment. Thus, the elements not described must be considered as identical to those described for the first embodiment.
[0050] According to the second embodiment, the compression member 3 does not comprise a support plate. Thus, the compression member 3 comprises a compression block 7 and three textile strip fixing means 4.
[0051] According to the second embodiment, each textile strip fastening means 4 comprises a fastening loop 11 which is fixed to the compression block 7 by means of a fastening strip 12. Each fastening strip 12 is, for example, made of non-elastic fabric and is fixed on the one hand to the compression block 7 and on the other hand to one of the fastening loops 11. Provision may be made for the fastening strips 12 to be fixed to the compression block 7 by any known means, for example by sewing, by gluing or by means of a self-gripping fastener of the Velcro type. At its end by which it is fixed to the fastening loop 11, each fastening strip 12 forms a loop which passes through the fastening orifice 8 of the fastening loop 11 in order to achieve the fastening of the fastening strip 12 to the fastening loop 11 ([Fig.8]).
[0052] In the present case, the fixing loops 11 are angularly distributed around a central portion of the compression block 7 with an angle of 120° between two fixing orifices 8 of consecutive fixing loops 11. To measure the angle a between two consecutive fixing orifices 8, the angle a formed between the central / respective points of the fixing orifices 8 and the central point b of the compression block 7 is measured. Preferably, the length of the fixing openings 8 is slightly greater than the width of the textile strips 4 so as not to apply too much tension to the lateral edges of the textile strips 4.
[0053] According to the second embodiment, the adjustment means 10 capable of increasing or decreasing the apparent length of the textile strips comprise self-gripping fastening means formed on the one hand by a fastening strip 13 which is fixed to one of the textile strips 4 and on the other hand by a longitudinal end of the textile strip 4 on which the fastening strip 13 is fixed ([Fig.8]). According to the second embodiment, each textile strip 4 is provided with a fastening strip 13 capable of forming a self-gripping fastening with the longitudinal end of the textile strip 4 to which it is fixed. For this, the fastening strip 13 comprises hooks (not shown) intended to cooperate with loops present on the textile strip 4 to achieve the self-gripping fastening. It is understood that it is then easy to modulate the apparent length of the textile strip 4 by modifying the area of the textile strip 4 on which the fixing strip 13 grips.
[0054] According to the second embodiment, each fixing strip 13 is preferably divided into two fixing sub-strips 13a, 13b which sandwich the longitudinal end of the textile strip 4 in order to reinforce the fixing ([Fig.8]).
[0055] Each textile strip 4 is therefore fixed respectively to one of the fixing loops 11 by passing through the fixing orifice 8 formed by the fixing loop 11, and by itself forming a loop by closing on itself and being held in position by the self-gripping fixing means. It is therefore understood that the self-gripping fixing means have, according to the present embodiment, both a function of adjusting the apparent length of the textile strips 4 and a function of fixing the textile strips 4 to the textile strip fixing means.
[0056] As for the first embodiment, the compression device 1 can have two configurations.
[0057] According to a first configuration of the second embodiment shown in [Fig. 6], the compression device 1 is intended to apply pressure to a left portion of the rib cage of the person 2 wearing the compression device 1. As for the first embodiment, it is noted that the member 9 for holding the textile bands 4 has a triangle shape whose upper point is offset to the right in front view ([Fig. 6]) in order to facilitate the passage of the textile band 4 over the left shoulder of the person 2 wearing the compression device 1 in order to apply pressure to the left portion of his rib cage.
[0058] According to a second configuration of the second embodiment shown in [Fig.7], the compression device 1 is intended to apply pressure to a right portion of the rib cage of the person 2 wearing the compression device 1. compression. As with the first embodiment, the transition from the first configuration to the second configuration is carried out simply by turning over the member 9 for holding the textile bands 4 so that the upper point of the triangle is offset to the left in front view ([Fig.7]). This configuration facilitates the passage of the textile band 4 over the right shoulder of the person 2 wearing the compression device 1 in order to apply pressure to the right portion of their rib cage.
[0059] Thus, the member 9 for holding the textile bands 4 is reversible and can adopt two different configurations, depending on whether compression is desired on the left portion ([Fig.6]) or on the right portion ([Fig.7]) of the rib cage.
[0060] The invention is not limited to the embodiments presented and other embodiments will become clear to those skilled in the art. List of references
