[Technical field]

[0001] CROSS-REFERENCE TO RELATED APPLICATIONS This application claims the benefit of U.S. Provisional Application No. 62 / 725,725, filed Aug. 31, 2018, the entirety of which is incorporated herein by reference.

[0002] Field of the Disclosure The present disclosure relates to methods of making and using pharmaceutical compositions that include cyclic guanylate monophosphate (cGMP)-specific phosphodiesterase type 9 inhibitors (hereinafter referred to as PDE9 inhibitors). [Background technology]

[0003] Sickle cell disease (SCD, also known as sickle cell anemia (SCA)) is an inherited disease that results in a vaso-occlusive process that is responsible for the majority of deaths in SCD patients. SCD disease is caused by a point mutation in the hemoglobin (HBB) gene that produces abnormal sickle hemoglobin (HBS or HbSS), which polymerizes to create rigid, sticky sickle-shaped red blood cells. Sickle cells lead to chronic inflammation, abnormal cell adhesion, oxidative stress, and endothelial dysfunction that ultimately causes a vaso-occlusive process.

[0004] There is currently no cure for SCD. Treatment options include blood transfusions and treatment with the anticancer drug hydroxyurea. Transfusions increase the number of normal non-sickle red blood cells in the circulation and correct the anemia. Regular transfusion therapy may help prevent recurrent strokes in high-risk children. Hydroxyurea (HU) is approved for the treatment of SCD and has been shown to reduce the frequency of painful attacks and hospitalizations. Unfortunately, HU is often poorly tolerated, and its widespread use is limited by concerns about its possible effects on fertility and reproduction, difficulties in achieving and maintaining effective doses due to hematological toxicity, and the need for monthly monitoring (Heeney et al., Pediatr Clin North Am., 2008, 55(2): 483). In fact, it is estimated that only one in four adult patients, and perhaps even fewer, are treated with this drug (Stettler et al., JAMA, 2015, 313: 1671). In addition, these challenges result in many patients receiving sub-effective doses of HU.Therefore, there is an urgent need for novel, safe, and effective treatments that can be safely adopted worldwide to prevent the pathological complications of SCD in patients of all ages.

[0005] There remains a need for treatments for SCD. Summary of the Invention

DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS

[0011] Phosphodiesterases (PDEs) are a family of enzymes that degrade cyclic nucleotides, thereby regulating cellular levels of second messengers throughout the body. PDEs represent drug targets that have proven attractive with many compounds introduced into clinical trials and on the market, respectively. PDEs are encoded by 21 genes that are functionally segregated into 11 families that differ in terms of kinetic properties, substrate selectivity, expression, localization patterns, activation, regulators, and inhibitor sensitivity. The function of PDEs is the degradation of the cyclic nucleotide monophosphates cyclic adenosine monophosphate (cAMP) and / or guanosine monophosphate (cGMP), which are key intracellular mediators involved in numerous vital processes, including the regulation of neurotransmission and smooth muscle contraction and relaxation.

[0012] PDE9 is cGMP-specific and (K m cAMP is 1000 times more abundant than cGMP), and the K m PDE9 is hypothesized to be an important player in regulating cGMP levels because it has the lowest cGMP levels in the brain. PDE9 is expressed at low levels throughout the brain and may regulate basal cGMP levels.

[0013] In the periphery, PDE9 expression is highest in the prostate, intestine, kidney, and hematopoietic cells, making it a potential treatment for a variety of non-CNS indications.

[0043] In some embodiments, the pharmaceutical composition further comprises an enteric coating. An enteric coating is a polymeric barrier applied to oral medications that prevents dissolution or disintegration in the stomach environment. Tablets, mini-tablets, pellets, and granules (usually filled into a capsule shell) are the most common enteric coated dosage forms. Most enteric coatings work by presenting a surface that is stable at the highly acidic pH found in the stomach, but rapidly degrades at higher pHs (alkaline pH). For example, an enteric coating will not degrade in gastric acid (pH-3), but will degrade in the alkaline environment of the small intestine (pH 7-9). By preventing the drug from degrading in the stomach, the enteric coating can protect the gastric mucosa from the irritating effects of the drug itself. Once the drug reaches the neutral or alkaline environment of the intestine, its active ingredients dissolve and become absorbed into the bloodstream. Materials used for enteric coatings include, but are not limited to, fatty acids, waxes, shellac, plastics, and plant fibers. A conventionally used material is a film resin solution.

[0071] As used herein, the term "administered simultaneously" is not particularly limited and means that a compound of the present disclosure and an additional active agent are administered at about the same time, e.g., as a mixture, or in immediate sequential order.

[0072] In the present specification, the term "administered sequentially" is not particularly limited and means that the compound of the present disclosure and the additional active agent are not administered simultaneously, but are administered alternately or in a set with a certain time interval between administrations. The time interval may be the same or different between administrations of the compound of the present disclosure and the additional active agent, and may be selected from the ranges of, for example, 2 minutes to 96 hours, 1 to 7 days, or 1 week, 2 weeks, or 3 weeks. Typically, the time interval between administrations may be from a few minutes to a few hours, such as 2 minutes to 72 hours, 30 minutes to 24 hours, 1 to 12 hours, etc. Further examples include time intervals of 24 to 96 hours, 12 to 36 hours, 8 to 24 hours, and 6 to 12 hours.

[0073] The molar ratio of the compound of the present disclosure and the additional active agent is not particularly limited. For example, when the compound of the present disclosure and one additional active agent are combined in a composition, their molar ratio may be in the range of 1:500-500:1, 1:100-100:1, 1:50-50:1, 1:20-20:1, 1:5-5:1, or 1:1.When the compound of the present disclosure and two or more other active agents are combined in a composition, the same molar ratio is also applied. The compounds of the present disclosure may comprise a given molar weight percentage of the composition, between about 1% and 10%, between about 10% and about 20%, between about 20% and about 30%, between about 30% and 40%, between about 40% and 50%, between about 50% and 60%, between about 60% and 70%, between about 70% and 80%, between about 80% and 90%, or between about 90% and 99%.

[0074] Methods of Use of the Disclosed Compounds PDE9 is specifically expressed in human hematopoietic cells, including neutrophils, reticulocytes, and erythroleukocyte cells. Moreover, PDE9 expression in reticulocytes and neutrophils in SCD patients is markedly and significantly elevated compared to healthy controls (Almeida et al.,Br J Haematol.2008 Sep;142(5),836). In addition, pharmacological PDE9 inhibition ameliorates the abnormal adhesive properties of SCD neutrophils, thus demonstrating a link between PDE9 and cell adhesion (Miguel et al.,Inflamm Res.2011 Jul;60(7),633). The mechanism by which PDE9 inhibition reduces cell adhesion has been shown to be mediated by an increase in cGMP and a decrease in endothelial adhesion molecule expression. Importantly, in animal models of SCD, PDE9 inhibitor-mediated reduction in cell adhesion had the functional effect of increasing cell survival. In addition to demonstrating less cell adhesion compared to HU, PDE9 inhibition results in increased fetal non-sickle hemoglobin (HbF) production, which reduces the cellular concentration of hemoglobin (HbS) in red blood cells (Rbc), resulting in less polymerization of hemoglobin and its associated sequelae. The importance of increasing HbF in the treatment of SCD is supported by the results of large studies such as the Cooperative Study of Sickle Cell Disease, as well as studies on various patient cohorts outside the United States, with data indicating that HbF is the most important modifier of the disease (Alsultan et al., Am J Hematol. 2013, 88(6), 531), and that modifiers of HbF improve other hematological parameters (Akinsheye, Blood, 2011, 118(1): 19). Finally, Almeida et al. demonstrated that combined treatment of SCD mouse models with HU and PDE9 inhibition resulted in an additive and beneficial amplification of the cGMP-elevating effect of HU (Almeida et al., Blood. 2012 Oct; 120(14), 2879). In conclusion, PDE9 inhibition can regulate the expression of fetal hemoglobin production as well as reduce cell adhesion, both of which are important mechanisms for the treatment of SCD.

[0075] The PDE9 inhibitors of the present disclosure and hydroxyurea (HU) act via different mechanisms. HU increases nitric oxide (NO) levels, which activates soluble guanylyl cyclase (sgc) to produce cGMP. The PDE9 inhibitors of the present disclosure block the degradation of cGMP by inhibiting PDE9 enzyme activity, thereby increasing cGMP levels. In the erythroid lineage, cGMP binds to protein kinase G (PKG) and signals for the synthesis of fetal gamma globin and ultimately the production of HbF. In hematopoietic cells with high PDE9 expression, direct inhibition of PDE9 activity increases cGMP levels, which promotes reduced leukocyte adhesion.

[0099] definition As used herein, the articles "a" and "an" should be understood to mean "at least one," unless specifically indicated to the contrary.

[0100] As used herein, the phrase "and / or" should be understood to mean "either or both" of conjoint elements, i.e., elements that are sometimes conjointly present and other times separately present. Other elements may optionally be present separately from the elements specifically identified by the "and / or" clause, whether or not associated with the elements specifically identified, unless expressly indicated to the contrary. Thus, as a non-limiting example, a reference to "A and / or B," when used with open-ended language such as "comprising," may, in one embodiment, refer to A without B (optionally including elements other than B), in another embodiment, refer to B without A (optionally including elements other than A), and in another embodiment, refer to both A and B (optionally including other elements).

[0101] As used herein, "or" should be understood to have the same meaning as "and / or," as defined above. For example, when separating listed items, "or" or "and / or" should be interpreted as inclusive, i.e., including not only at least one, but more than one of a number or list of elements, and possibly additional unlisted items. Only terms clearly indicating to the contrary, such as "only one of," "exactly one of," and when used in the claims, "consisting of," refer to the inclusion of exactly one element of a number or list of elements.

[0102] Generally, as used herein, the term "or" when preceded by exclusive terms such as "either," "one of," "only one of," "exactly one of," or, when used in the claims, "consisting of," shall be construed only as an exclusive alternative (i.e., "one or the other, but not both") and shall have its ordinary meaning as used in the field of patent law.

[0103] As used herein, the phrase "at least one," in reference to a list of one or more elements, is understood to mean at least one element selected from one or more of the listed elements, but not necessarily including at least one of every element specifically listed within the list of elements and excluding any combination of elements in the list. This definition further allows for elements to optionally exist apart from the elements specifically identified within the list of elements to which the phrase "at least one" refers, whether or not the specifically identified elements are related.

[0104] Thus, as a non-limiting example, "at least one of A and B" (or, equivalently, "at least one of A or B," or "at least one of A and / or B") can refer in one embodiment to more than one, i.e., including A but not including B (optionally including elements other than B), in another embodiment to more than one, i.e., including B but not including A (optionally including elements other than A), in yet another embodiment to at least one of more than one, i.e., including A, or more than one, i.e., including B (optionally including other elements), etc.

[0105] As used herein, all transitional phrases, such as "comprising," "including," "carrying," "having," "containing," "involving," "holding," and the like, should be understood to be open-ended, i.e., to mean including, but not limited to.

[0106] Only the transitional phrases "consisting of" and "consisting essentially of," respectively, are closed or semi-closed transitional phrases as set forth in the United States Patent Examining Guidelines.

[0107] As used herein, a "subject" or "patient" refers to any mammal (e.g., a human), including a mammal that may be susceptible to a disease or disorder, such as tumor formation or cancer. In various embodiments, a subject refers to a subject that has been or will be the subject of a treatment, observation, or experiment. For example, a subject may refer to a subject who has been diagnosed with cancer or is otherwise found to have cancer, or who is selected for a treatment, observation, or experiment based on the subject's knowledge of the cancer.

[0108] As used herein, "treatment" or "treating" refers to the amelioration of a disease or disorder, or at least one sign or symptom thereof. "Treatment" or "treating" can refer to a reduction in the progression of a disease or disorder, as determined, for example, by stabilization of at least one sign or symptom, or a slowing of the rate of progression, as determined, for example, by a slowing of the rate of progression of at least one sign or symptom. In another embodiment, "treatment" or "treating" refers to delaying the onset of a disease or disorder.

[0109] As used herein, "prevention" or "preventing" refers to a reduction in the risk of acquiring or suffering from a sign or symptom of a given disease or disorder, ie, prophylactic treatment.

[0110] As used herein, the phrase "therapeutically effective amount" refers to an amount of a compound, material, or composition containing a compound of the present disclosure that is effective to produce a desired therapeutic effect. Thus, a therapeutically effective amount treats or prevents a disease or disorder, or improves at least one sign or symptom of the disorder. In various embodiments, the disease or disorder is cancer.

[0111] A dash (-), when not between two letters or symbols, is used to indicate the point of attachment of a substituent, for example, -CONH2 is attached through a carbon atom (C).

[0112] "Optional" or "optionally" means that the subsequently described event or circumstance may or may not occur, and the description includes examples in which said event or circumstance occurs and examples in which said event or circumstance does not occur. For example, "optionally substituted aryl" encompasses both "aryl" and "substituted aryl" as defined herein. With respect to any group that contains one or more substituents, those of skill in the art will understand that such groups are not intended to introduce any substitutions or substitution patterns that are sterically impractical, synthetically impractical, and / or inherently unstable.

[0113] All numerical ranges herein include all numerical values and ranges of numerical values within the recited numerical range. As a non-limiting example, (C1-C6) alkyl includes any one of C1, C2, C3, C4, C5, C6, (C1-C2), (C1-C3), (C1-C4), (C1-C5), (C2-C3), (C2-C4), (C2-C5), (C2-C6), (C3-C4), (C3-C5), (C3-C6), (C4-C5), (C4-C6), and (C5-C6).

[0114] Further, while the numerical ranges and parameters setting forth the broad scope of the disclosure are approximations as set forth above, the numerical values set forth in the Examples section are reported as precisely as possible, with the understanding that such numerical values inherently contain errors resulting from any measuring device and / or measuring technique.

[0119] The Compound 1 drug product to be used in ongoing clinical development is an immediate release tablet, which may be available in a variety of tablet strengths and a coating to ensure uniformity of appearance for the placebo.

[0120] Early clinical trials were conducted with compound 1 drug substance directly packed into opaque white gelatin capsules (Powder in Capsule, PIC) containing no excipients or processing aids. Excipient-blended tablets of the oral drug product were developed, which allowed for scale-up of the manufacturing process and ensured content uniformity. These tablets were tested for purity, potency, dissolution, total aerobic microbial count, and prescribed limits for total yeast and viable counts. Additionally, tests were performed against specific microorganisms.

[0121] Each tablet contains 20 mg, 50 mg, 100 mg, or 200 mg of Compound 1 drug substance (monohydrate of API), or placebo. Representative tablet compositions are shown in Table 1 below.

[0122] [Table 1]

[0123] All tablets were formulated to have a target weight of 400 mg for the core tablet and 440 mg for the coated tablet. To achieve this, the amounts of Compound 1 and microcrystalline cellulose were adjusted accordingly. The amounts of all other excipients remained constant.

[0124] Description of manufacturing process and process control The manufacture of coated tablets follows standard procedures for compounding, tablet compression, and coating.

[0125] Roller compaction of API: Compound 1 drug substance was first processed by roller compaction to achieve a more uniform intermediate suitable for further compounding and processing. A defined amount of API was passed through a roller compactor at 200-300 PSI and a roller speed of 4.0 RPM. The roller compacted material was passed through a 20 mesh screen to obtain a uniform particle size. The compressed and screened API was stored in double-lined polyethylene bags until use in tablet manufacturing.

[0126] Roller compacted drug substance and excipient blend: The specified amount of roller compacted API was mixed with the specified excipients (excluding the coating fluid) using a 16 qt. V-shell Blender. The blending was carried out in such a way that the blender rotated at least 300 revolutions within the specified time period. Samples were taken from multiple locations within the V-Shell Blender to verify the homogeneity of the blend.

[0127] The compounded material was stored in a suitable HDPE container within a double lined polyethylene bag until further processing.

[0128] Tablet Formulation: Tablets were produced on a gravity fed tablet press with 11 mm upper and lower plain-faced tablet punches. The tablet press was adjusted to meet predetermined specifications for tablet weight, hardness, thickness, friability, and disintegration, as defined below. All core tablets were screened with a metal detector and de-dusted prior to film coating.

[0129] Film Coating: A uniform coating mixture was prepared by mixing the specified amount of Opadry II White in purified water and mixing the suspension for at least 45 minutes. The tablets were loaded into a pan coating apparatus and preheated to 43°C before starting the coating operation. The preheated tablets were then coated in a 15 inch coating pan while rotating at 10-15 RPM. The spray rate of the coating suspension was adjusted to 10 g / min and the inlet air temperature was maintained at 40-60°C.

[0130] Tablets were sampled and weighed at regular intervals to determine if the desired target weight gain (10%) had been achieved. After coating was complete, the coated tablets were cooled to below 30°C.

[0131] Bulk Packaging: The coated tablets were sampled for release testing and subsequently transferred into HDPE containers double lined with double lined polyethylene bags.

[0132] Clinical packaging: The tablets were placed into clinical trial packaging, which included inkjet labeling of each container with a batch number to ensure identification. Thirty-three tablets were hand-counted into each 60cc round-bottom white HDPE bottle. A second person verified the count before filling each bottle. Once filled, each bottle was hand-sealed with a 33mm white polypropylene child-resistant cap and induction sealed.

[0133] Example 2. Development of 300mg tablets In the 300 mg tablet development activity, it was observed that the addition of hydroxypropyl cellulose (HPC-SSL-SFP) and increasing the percentage level of microcrystalline cellulose (Avicel PH200) gave acceptable results in increased tablet hardness and reduced loss during friability testing. However, the addition of HPC increased the average disintegration time from less than 1 minute to approximately 37 minutes. Multiple HPC-SSL-SPF percentage levels were evaluated in the 300 mg tablet formulation. The formulations evaluated are shown in Table 2 below.

[0134] [Table 2]

[0135] Multiple formulations of hydroxypropyl cellulose (HPC-SSL-SFP) at various percentage levels were evaluated to identify a formulation that produced acceptable results. The formulations utilized in the development of the film coated tablets 300 mg are shown in Table 3 below.

[0136] [Table 3]

[0137] Compounding / granulation: Excipients used in development were recorded for a batch size of 2,380 tablets. Compound 1 drug substance was roller compacted. Compound 1 and all excipients were screened through a 20 mesh hand screen in the following order: 1) Compound 1 drug substance, 2) alpha starch (Starch 1500), 3) colloidal silicon dioxide (Cab-O-Sil), 4) microcrystalline cellulose (Avicel PH200).

[0138] After the target weight was achieved, a hardness profile (tablet hardness vs. disintegration time) was established and samples were taken. Each sample was used to test the tablet physical properties including weight, hardness, thickness, disintegration, and friability.

[0139] In-process guidelines: Tablet thickness range: 4.65-4.85mm.

[0140] Tablet hardness target: 10kP.

[0141] Friability (USP Friability): 0.3% or less, no capping.

[0142] Disintegration (per USP): All tablets disintegrate completely within 15 minutes.

[0143] Coverage: Tablets from the development batch were coated under target conditions to evaluate the core tablet's ability to withstand pan coating. Before the tablets were placed in the coating pan, the pan was sprayed with Opadry II, 85F18422, White and allowed to dry. The operator monitored the spray and drying process to ensure there was no coating delamination. This resulted in the formation of a thin layer of coating that prevented tablets from developing coating pan induced veins such as scratches. Tablets coated with Opadry II, 85F18422, White gained 4.8% weight.

[0144] Results and Discussion: The goal of the development activity was to develop a tablet formulation of Compound 1 with an optimal percent level of hydroxypropyl cellulose (HPC-SSL-SFP) that would result in acceptable results. This formulation was utilized to produce tablets with acceptable hardness, thickness, friability, and disintegration results. Tablets from each formulation were tested for hardness, thickness, friability, and disintegration. Target results were: (1) thickness: 4.50-4.80 mm, (2) friability: 0.3% loss or less, (3) disintegration: 15 minutes or less, and (4) hardness 10 kPa. Friability, disintegration, and hardness were tested against USP compounding standards (the United States Pharmacopeia and National Formulary are the accepted test standards).

[0145] Feasibility batches at 0.5% HPC: The first formulation evaluated contained 0.5% hydroxypropyl cellulose (HPC-SSL-SFP) and is shown in Table 4 below.

[0146] [Table 4]

[0147] Tablets were to be prepared with a target fill weight of 420.0 mg and a thickness range of 4.50 mm to 4.80 mm.

[0148] To test the disintegration time, tablets were submerged in water in succession until six tablets were completely disintegrated, the results of which are shown in Table 5 below.

[0149] [Table 5]

[0150] The goal was for all tablets to completely disintegrate within 15 minutes. According to the data in Table 5, the target time was achieved as all tablets completely disintegrated in under 15 minutes. The remaining tablet formulation was compressed using a target fill weight of 420.0 mg.

[0151] Additionally, physical testing for friability was performed. The friability test examines tablets for their tendency to pulverize, crumble, or shatter after compression. The acceptance criteria for a batch was a percent loss of 0.3% or less. The data collected during the friability test is shown below in Table 6.

[0152] [Table 6]

[0153] Achieving a percent level of 0.5% hydroxypropyl cellulose (HPC-SSL-SFP) met the acceptance criteria provided in the batch record.

[0154] Friability batch at 1.5% HPC Tablets using a formulation containing 1.5% hydroxypropyl cellulose (HPC-SSL-SFP) were also compressed and the physical properties of these tablets were tested to determine whether acceptable results were obtained. The formulations are shown in Table 7 below.

[0155] [Table 7]

[0156] The tablets were successively submerged in water until three tablets were completely disintegrated, and the results are shown in Table 8.

[0157] [Table 8]

[0158] All disintegration times exceed the 15 minute pass criteria, indicating a fail disintegration result. Increasing the level of hydroxypropyl cellulose (HPC-SSL-SPF) increases the disintegration time. Note that the loss after crushing is less than 0.3%.

[0159] After physical testing was completed, the tablets were film coated to increase their weight by approximately 4.8% w / w. After comparing the results of the physical testing performed on each formulation, it was determined that Compound 1 tablets 300 mg (0.5% HPC) provided acceptable results for the tablets.

[0160] Example 3. Compound 1 reduces leukocyte adhesion and activation Polymorphonuclear cells (PMNs), specifically neutrophils, play an important role in the pathogenesis of sickle cell disease (SCD), and activated neutrophils have been shown to increase platelet adhesion to the vascular endothelium. Recently, various drugs targeting leukocyte binding to endothelial cells have been advanced in clinical studies in patients. Compound 1 can increase fetal hemoglobin expression in patient-derived cells and in SCD mouse models, and reduce vascular occlusion in SCD mouse models. In this example, we tested the ability of compound 1 to reduce the adhesive properties of neutrophils from SCD patients as well as to reduce sE-selectin (sE-Sel) and PMN activation markers in a mouse SCD model.

[0161] Endothelial E-selectin (E-Sel) retards the rolling motion of leukocytes, which is followed by fixed adhesion and transmigration of activated leukocytes. Plasma levels of se-Sel, which result from enzymatic cleavage of the extracellular domain of E-Sel, are elevated in SCD patients, and this increase may be mediated by interaction with leukocytes. Plasma sE-Sel levels in the Townes mouse model were 144% higher (139 mg / ml) than in control mice (57 mg / ml). This level was significantly reduced in Townes mice treated with compound 1, where plasma sE-Sel levels were only 61% higher than in control mice (92 mg / ml).

[0162] Compound 1 was found to reduce circulating levels of PMN in SCD models, but not in long-term studies in healthy animals. This may be concomitant with a compound 1-mediated reduction in disease-specific cell activation, including a 67% reduction in bone marrow-derived myeloperoxidase (MPO) and a 26% reduction in neutrophil-derived arginase levels in the lungs (Figure 1). In a previously described in vitro adhesion assay that mimics blood flow, activated endothelial cells HMEC-1 line the inner surface of a microchannel. Using this assay, neutrophils are observed to aggregate and bind to the endothelial monolayer in perfused whole blood samples taken from patients with SS (the most common form of sickle cell disease). This was quantified by real-time monitoring of green fluorescent patches in the microchannels, as neutrophils are labeled with a specific Alexa Fluor® 488-conjugated antibody prior to the perfusion step. A significant amount of adhesion to activated HMEC-1 was observed compared to neutrophils from untreated patients. When added to blood samples before the perfusion step, compound 1 significantly and dose-dependently reduced adhesion. The inhibitory effect began as early as 15 min of incubation, with the strongest adhesion inhibition observed after 30 min of incubation with 30 μM compound 1. Under these conditions, adhesion was reduced by an average of 54% (p=0.03) (Figure 2). Mechanistically, without wishing to be bound by any theory, compound 1 may target the neutrophil fixative adhesion step, as it reduces the expression levels of major neutrophil integrins, including CD11a [23% reduction (p=0.002)], CD11b [39% reduction (ns)], and CD18 [47% reduction (p=0.03)] (Figure 3).

[0163] Together, these data support a role for Compound 1 in reducing PBMC-mediated symptoms of SCD by targeting aberrant neutrophil adhesion independent of circulating cell numbers.

[0164] Example 4. Phase 2a Randomized, Double-Blind, Placebo-Controlled Study of Compound 1 in Adult Patients with Sickle Cell Anemia (SCA) the purpose: The primary objective is to identify patients with homozygous sickle cell anemia (HbSS) or sickle beta 0 The objective of this study was to evaluate the safety and tolerability of Compound 1 in adult patients with sickle cell anemia (SCA), defined as thalassemia, who are not receiving hydroxyurea (HU) and in adult patients with SCA who are receiving a stable dose of HU.

[0165] Secondary objectives are to characterize the pharmacokinetic (PK) profile of compound 1 in adult patients with SCA with and without a stable dose of HU to determine whether clinically relevant PK interactions are observed, and to characterize the PK profile of HU in adult patients with SCA before and after administration of compound 1.

[0166] The exploratory objectives are to evaluate the pharmacodynamic (PD) effects of Compound 1 in adult patients with SCA on and off stable HU, and to evaluate the possible efficacy of Compound 1 on SCA-related clinical outcome measures in adult patients with SCA on and off stable HU.

[0167] Methodology: The study is a randomized, double-blind, placebo-controlled trial that will evaluate the safety, tolerability, PK, exploratory PD, and clinical outcomes of Compound 1, a phosphodiesterase 9 (PDE9) inhibitor, administered once daily over 16-24 weeks in two populations of SCA patients: one not receiving HU (Population A) and one receiving a stable dose of HU per standard of care (Population B). Up to approximately 36 patients will be enrolled in Population A and 18 patients in Population B.

[0168] In Cohort A, after a screening period of up to 4 weeks, eligible patients in Cohort A (i.e., patients not receiving HU) will be administered Compound 1 or placebo for a total of 24 weeks. On day 1, patients were randomized 1:1:1 to receive 30 mg or 50 mg of Compound 1, 100 mg of Compound 1, or placebo daily for the first 12 weeks, and then over the next 12 weeks (weeks 13-24), each patient's dose may be doubled (i.e., doubling from 50 mg to 100 mg, doubling from 100 mg to 200 mg, or doubling placebo) (dose escalation does not affect blinding of study drug, since all dose levels of placebo and Compound 1 are seemingly the same). Throughout the study, all available clinical data will be reviewed approximately every 2 weeks, and dose escalation will occur on an individual patient basis on day 85 only if approved based on review of each patient's individual clinical safety data.

[0169] For Cohort B, after a screening period of up to 4 weeks, eligible patients in Cohort B (i.e., patients receiving stable HU) will enter a lead-in period during which blood will be drawn to characterize the PK profile of the patient's prescribed dose of HU in the absence of Compound 1 (i.e., to characterize the patient's baseline HU PK profile). Two complete baseline HU PK profiles (from blood samples drawn over 10 hours, at least 48 hours apart) will be determined.

[0170] Compound 1 was initiated in Cohort B after at least 4 weeks of safety data had been reviewed from 6 patients in Cohort A and it was determined that initiation of dosing in Cohort B was safe and appropriate. Once approval for initiation of dosing in Cohort B was obtained and baseline HU PK blood draws were completed, patients were randomized 2:1 on Day 1 to receive 30 mg or 50 mg of Compound 1 or placebo for 16 weeks. For the first 4 weeks (Weeks 1-4), patients received study drug according to their randomized treatment assignment. For the next 12 weeks (Weeks 5-16), each patient's dose may be doubled (e.g., doubling from 50 mg to 100 mg, or doubling placebo). As with Cohort A, dose escalation occurs on Day 29 only if approved based on review of each patient's individual clinical safety data.

[0171] Study design rationale: This is the first study in this patient population (patients with SCA) and is therefore designed to investigate the safety, tolerability, and PK of compound 1 across a range of doses in adult patients with SCA, as well as potential PD effects and clinical efficacy. Given that compound 1, if approved, may be administered as a single agent or in conjunction with HU, the effects of compound 1 will be evaluated in SCA patients with moderate HbF levels who are not receiving HU or other treatments (Population A), as well as in patients currently receiving a stable dose of HU (Population B).

[0172] Available nonclinical data and clinical data in healthy volunteers support that compound 1 is safe and well tolerated at doses of 30, 50, 100, and 200 mg once daily, and that possible clinically beneficial PD effects may be observed when doses of at least 100 mg are administered for at least 24 weeks. Therefore, Cohort A is designed to examine the PD dose response in patients as well as the tolerability of the 200 mg dose in sickle cell patients who tolerated the 100 mg dose well.

[0173] Results for Cohort B are intended to provide information on Compound 1 when administered simultaneously with HU, both of which increase HbF levels via an alternative biochemical pathway that increases intracellular cGMP. Because there are no clinical data supporting the coadministration of Compound 1 with HU, patients in Cohort B will begin receiving Compound 1 at the lower doses used in Cohort A (30 mg or 50 mg) and will be titrated to a dose of 100 mg only if the 50 mg dose is safe and tolerated for 4 weeks. In addition, although available nonclinical data do not support the coadministration of HU with Compound 1 to increase Compound 1 exposure, dosing for Cohort B will begin once 4 weeks of safety data are available from the two patients in Cohort A who will receive 30 mg or 50 mg (starting dose for Cohort B), 100 mg (2x starting dose), and placebo, respectively.
