Abstract
Description
[0001] A CATHETER GUIDED CIRCUMFERENTIAL MICRO ANASTOMOTIC VASCULAR STAPLING DEVICE
[0002] FIELD OF INVENTION
[0003] The present invention relates generally to a surgical system for minimally invasive surgery, particularly to a stapling device. More particularly, the present invention relates to a catheter guided circumferential micro anastomotic vascular stapling device.
[0004] BACKGROUND OF INVENTION
[0005] Surgical environment is becoming increasingly challenging for the cardiac surgeon since off- pump coronary arteries bypass grafts and minimally invasive approach came up. Different suture techniques have been used for anastomosis in coronary artery bypass graft surgery. In general, coronary artery bypass grafting is a surgical procedure for revascularization of obstructed coronary arteries and anastomosis is the surgical joining of tissues of tubular structures having a lumen, such as blood vessels, to create communication between the lumina of the structures. Proximal and distal anastomotic devices are ideally suited to complement the ever-growing interest in minimal-access and non-stemotomy surgical revascularization. The suture technique for coronary anastomosis construction is becoming inadequate to meet new surgeons' demand while performing Robotic surgeries. Therefore, there is an increasing need for alternative ways to perform coronary bypasses. Suture less coronary anastomotic devices are intended to facilitate minimally invasive coronary artery bypass grafting by easing and eventually standardizing the anastomotic technique. However, the anastomotic techniques require a specially designed implanting device, and therefore require device-related expertise.
[0006] Robotically assisted surgical systems have been adopted worldwide for minimally invasive surgery. Robotic totally endoscopic coronary artery bypass grafting represents the most advanced form of coronary surgery. Experiments were being carried out to study the feasibility of catheter-based endoscopic bypass grafting. The combination of robotic technology and simple technique for anastomotic construction may facilitate beating heart total endoscopic coronary artery bypass. Also, many types of surgical stapling instruments have been devised for different surgical procedures for making tissue attachments faster and more reliable. Various surgical stapling instruments have been developed for end-to-end, side-to-side, and end-to-side anastomoses of hollow or tubular organs. However, there has been increasing number of adverse events associated with the surgical staplers. US20130274772A1 discloses apparatuses and methods for performing anastomosis of distal and proximal vessels. An exemplary apparatus includes a distal anastomosis includes a distal hub and a distal engagement segment. The distal hub is configured to detachably connect to a distal catheter. The distal engagement segment is connected to the distal hub and includes one or more distal arm segments that are configured to engage an inner wall of the distal vessel.
[0007] US5843169A disclosed an apparatus and method for stapling graft material to a vessel wall utilizing the same entry point into the vessel as used for initial deployment of the graft while providing substantially unobstructed blood flow during the stapling procedure and preserving the patency of orifices of branches of the vessel. In particular, the apparatus is comprised of a tube, a stapling device, a balloon catheter, and a sheath.
[0008] Accordingly, there exists a need for a catheter guided circumferential micro anastomotic vascular stapling device which deploys and retracts with the help of a manual trigger.

[0011] It is the primary object of the present invention to provide a catheter guided circumferential micro anastomotic vascular stapling device.
[0012] It is another object of the present invention, wherein the stapling device includes male circumferential micro anastomotic vascular stapling device and female circumferential micro anastomotic vascular stapling device.
[0013] It is another object of the present invention, wherein the male and female micro anastomotic vascular stapling devices are attached to distal ends of guide wires, which passes through the catheter.
[0014] It is another object of the present invention, wherein the catheter guided minimally invasive coronary anastomotic circumferential micro stapling device penetrates, deploys, staples, cut the vessel wall tissue and retracts with the help of a manual trigger. It is another object of the present invention, wherein the deployment and retraction of the male and female circumferential micro anastomotic vascular stapling device involves cutting of the vessel wall tissue and instantly stapling the edge of the Internal Thoracic Artery (ITA) to the Coronary Artery (CA) by coupling at plurality of circumferential points.

[0054] For illustration purpose:
[0055] Working:
[0056] The Internal Thoracic Artery (ITA) is harvested by a Surgeon using a surgical robotic system. Referring to Figures 6 and 7, a 1 ,25mm to 4mm diameter angiogram catheter is introduced into the Internal Thoracic Artery (ITA) followed by the female circumferential micro anastomotic vascular stapling device [4] which is attached to distal end of the guide wire [2b] . Said female circumferential micro anastomotic vascular stapling device [4] is introduced under the X-ray guidance by the Surgeon to penetrate the Coronary Artery (CA) at the desired location for the perfect Anastomosis. The manoeuvring of the guidewire [2b] along with the female micro anastomotic vascular stapling device [4] is controlled at proximal end of the guide wire [2b] by the Surgeon through trigger knob [5b] . The female micro anastomotic vascular stapling device [4] penetrates the end of the Internal Thoracic Artery (ITA) and the Coronary Artery (CA) at a desired location with a trigger from the surgeon on the proximal end of the guide wire [2b] as depicted in Figure 7. The female micro anastomotic vascular stapling device [4] facilitates deployment process with appropriate trigger from the Surgeon to maneuver said stapling device [4] onto the tissues. Once triggered the female micro anastomotic vascular stapling device [4] deploys circumferentially on the vessel wall tissue and cuts said vessel wall tissue in a circular manner. The eight pin levers [4d] with sixteen individual staple pins [4e] facilitates the stapling process. The T - shaped distal ends [4g’] of the eight release wires [4g] passes through either side of each of the eight pin levers [4d] , where the couplet of staple pins [4e] are suspended to be released for stapling the tissues during anastomosis stapling process.
[0057] Referring to Figures 7 and 8, once the trigger knob [5a] at proximal end of the guide wire [2a] is triggered, the male circumferential micro anastomotic vascular stapling device [3] is maneuvered by the Surgeon to concentrically coincide with the female micro anastomotic vascular stapling device [4] to penetrate and facilitate deployment process wherein the eight pin levers [3d] with sixteen individual staple pins [3 e] deploys on the edges of the ITA and the CA.
[0058] When the trigger knobs [5a and 5b] are maneuvered by the Surgeon the sixteen individual staple pins [3e] of the male circumferential micro anastomotic vascular stapling device [3] are first brought in exact orientation with the sixteen individual staple pins [4e] of the female micro anastomotic vascular stapling device [4] . Said orientation process is achieved by a magnetic field of attraction induced between the sliding mount disc [3c] of the male circumferential micro anastomotic vascular stapling device [3] and the sliding mount disc [4a] of the female circumferential micro anastomotic vascular stapling device [4] . That is, the sliding mount disc [3c] with its one of the disc portion facing exactly the opposite disc portion of the sliding mount disc [4a]. Once the magnetic opposite field of attraction between the sliding mount discs [3 c and 4a] are achieved, subsequently, the sixteen individual staple pins [3e] of the male circumferential micro anastomotic vascular stapling device [3] and the sixteen individual staple pins [4e] of the female micro anastomotic vascular stapling device [4] are now oriented closer to each other. Further maneuvering of the respective trigger knobs [5a and 5b] move the sliding mount discs [3a and 4c] simultaneously towards each other which instantly initiates the stapling process anastomosing the tissues together.
[0059] Referring to Figures 9, 10 and 11, after coupling all the sixteen individual staple pins [3e and 4e], the trigger knobs [5a and 5b] are further maneuvered for the sliding mount discs [3a and 4c] to move away from each other in reverse direction which subsequently retracts the sixteen pin levers [3b, 3d and 4b, 4d] which further retracts the eight release wires [3g] attached to the pin levers [3b, 3d] and eight release wires [4g] attached to the pin levers [4b, 4d] and subsequently retracts the sieve [4h] which collects the cut tissue. Said retraction of the release wires [3g and 4g] attached to the pin levers [3, 3d and 4b, 4d] facilitate the release of all the sixteen individual staple pins [3e and 4e] circumferentially at the anastomosed tissue stapled intact in a circular pattern as depicted in Figure 12, said stapling is done without physical pull or push force. The staple pins are stapled intact in a circular pattern only on the tissue wall and not within the lumen.
[0060] Once the instant stapling process gets completed, the trigger knobs [5a and 5b] simultaneously activated by the Surgeon to retract both the male circumferential micro anastomotic vascular stapling device [3] and female circumferential micro anastomotic vascular stapling device [4] simultaneously and co-axially along with the cutout vessel wall tissue and retrieved back through the guide wires [2a and 2b] as depicted in Figure 11 and externalized outside the catheter [1], The entire procedure is performed within 3 minutes.
[0061] Thus the catheter guided circumferential micro anastomotic vascular stapling device according to the present invention performs anastomosis of the two blood vessels and provide streamline and steady blood flow.
