AI Technical Summary

Technical Problem

The steam vapor effect in tumor ablation procedures using RF or Microwave methods leads to a significant delay in restoring normal ultrasound imaging, as physicians must wait for 5-15 minutes for steam bubbles to dissipate, hindering quick evaluation of ablation efficacy and complicating the process of sealing the needle insertion entry point.

Method used

A method and system utilizing a special ultrasonic waveform sequence with extended half-period negative pressure pulses are applied to the steam cloud formed during tumor ablation, effectively breaking up steam bubbles and rapidly eliminating the steam vapor effect, thereby restoring normal ultrasound imaging in less than a couple of minutes.

Benefits of technology

This approach significantly reduces the time required to eliminate steam vapor, allowing for immediate evaluation of ablation efficacy and facilitating quicker patient turnover, while also potentially reducing the risk of tumor recurrence associated with prolonged steam bubble presence.

Abstract

This invention presented a method and an ultrasonic imaging system that is capable of rapidly eliminating steam cloud created by steam pops during RF or Microwave tumor ablation procedure. The present invention creates a special waveform sequence which is constructed by repeating a carefully designed basic waveform multiple times where the basic emission waveform has at least one extended half-period negative pressure pulse, then delivers the special waveform sequence into where the steam cloud is located to dissolve the steam bubbles during or after the tumor ablation. The special elongated negative rarefaction phase pulse makes the bubble crashing teasier. A user interface is also presented to allow the user to target the focal position of the special waveform sequence to where the steam cloud is centered to quickly eliminate the steam cloud effect and allow fast and accurate tumor ablation effect evaluation.

Description

A METHOD AND SYSTEM FOR RAPIDLY ELIMINATING STEAM VAPOR EFFECT IN TUMOR ABLATION

[0001] CROSS REFERENCE TO RELATED APPLICATION

[0002] This application claims the benefit of Chinese Patent Application No. 202311183622.8, filed on September 14, 2023, entitled “a method and system for rapidly eliminating steam vapor effect in tumor ablation” , which is hereby incorporated by reference in its entirety.BACKGROUND

[0003] Tumor ablations with RF (Radio Frequency) or Microwave methods are always associated with steam pop phenomenon, create vapor filled steam cloud that generates strong echo which results in blocking of ultrasound waves beneath the steam cloud. Further, the steam bubbles are so bright under ultrasound that local detail information is always missed. Physicians have to wait 5-15 minutes for the steam bubbles to dissolve before tissue at the ablation location and tissue underneath the steam bubbles can be observed again under ultrasound. This is a severe problem as physicians often want to quickly evaluate the efficacy of the ablation effect and move on to next patient. This situation is getting worse as the needle path is filled with steam bubbles when the physician tries to seal the needle insertion entry point with ablation at the end of the surgery. As described above, an echogenic steam cloud around the tumor and a strong echo needle path are normal in tumor RF or Microwave ablation. Echo shadows underneath them block all tissue information to be seen with ultrasound. To make it worse, studies suggest that steam popping may be related with tumor recurrence.

[0004] There is a strong need to quickly eliminate the steam bubbles at the tumor location, and whereas they have been spread to in order for physicians to quickly regain normal ultrasound imaging capability on ablated tissue. This can help avoid the shadowing underneath strong echoes and the strong echoes coverage of normal tissue echoes. Currently, there is no method except waiting for 5-15 minutes for the ablation created steam bubbles to be naturally dissolved. The 5-15 minutes is a significant time as most  of the liver tumors undergone ablation are under 3cm, thus the ablation process takes less than 5 minutes typically. Too much time wasted in after ablation waiting.SUMMARY

[0031] In another embodiment, a method for rapidly eliminating steam vapor effect in tumor ablation by using the above mentioned ultrasonic imaging system is provided, the method comprises:

[0032] creating an ultrasonic emission waveform sequence by the said ultrasonic imaging system with steam eliminating function, the ultrasonic emission waveform sequence comprises one or more basic emission waveforms, the one or more basic emission waveforms have at least one extended half-period negative pressure pulse and in a sinusoidal or rectangular waveform style; and

[0033] applying the ultrasonic emission waveform sequence to a steam cloud formed by steam bubbles that generated in tumor ablation for rapid dispersion of the steam cloud.

[0034] In a preferred embodiment, the method further comprises: allowing a user to turn on / off the ultrasonic emission waveform sequence by a user interface, and selecting a  region or a position at where the ultrasonic emission waveform sequence to be applied.

[0035] In a further preferred embodiment, the user is allowed to select a rectangular region or a point position to apply the ultrasonic emission waveform sequence, the rectangular region or the point position is centered at a center of the steam cloud.

[0036] In a more preferred embodiment, the method further comprises: identifying the center of the steam cloud and placing a rectangular region or a point position to apply the ultrasonic emission waveform sequence through artificial intelligence.

[0037] In a preferred embodiment, the method comprises after adjusting the rectangular region or the point position to fit the steam cloud, the user turns on steam vapor clear function to send the ultrasonic emission waveform sequence to kill the steam bubbles.

[0038] The present invention developed a new method to eliminate the steam bubbles generated during the RF or Microwave ablation procedure, thus to reduce the time needed for waiting the steam cloud to naturally dissipate after ablation. In this method, during and / or immediately after the ablation, special ultrasonic emission waveform sequences are created and sent to tissue ablation position to tear up the steam bubbles, and eliminate the steam cloud that blocks the normal ultrasound echoes from tissue at ablation location and underneath the steam could.

[0039] This technology can get rid of the steam cloud in less than a couple minutes. This is significant less than the 5-15 minutes waiting time for current RF or Microwave ablation procedures in which clinicians wait for the steam cloud to naturally dissipate into normal imaging level. Further, the steam elimination process can be applied in real time during the ablation process.

[0040] An ultrasound imaging system that incorporated this functionality is developed in this invention. A user interface is provided to allow the user to enter into this special steam clear mode. A user can select the location where steam cloud shall be removed and enable the system to start the steam clear operation. The user can then quit this special mode after the steam vapor effect is eliminated to back to normal imaging. This process can also be implemented automatically.

[0041] The ultrasound imaging system can find the steam cloud automatically, then start the steam elimination process till the steam cloud is cleared out then back to normal imaging. As a result, it can provide better monitoring of tumor ablation procedure, help  the clinician to understand the ablation effect on the fly. Great clinical benefit can be expected with this new technology.BRIEF DESCRIPTION OF THE DRAWINGS

[0060] The present invention is described in detail below with reference to accompanying figures.

[0061] Unless defined otherwise, technical and scientific terms used herein have the same meaning as is commonly understood by one of ordinary skill in the art to which this invention belongs. The terms “a” and “an” do not denote a limitation of quantity, but rather denote the presence of at least one of the referenced items.

[0062] This invention creates special waveforms and firing sequences to eliminate the steam bubbles generated by steam pop phenomenon during RF or Microwave ablation. The steam pop due to quick steam explosion during ablation procedure results in large amount of small steam bubbles that cover the ablation region. These steam bubbles are clustered together, forming a dense steam cloud which the imaging ultrasound waves can’t penetrate. This is the reason of the blocked ablation region and shadowed region underneath the ablation region. To get rid of these large amount small steam bubbles, the key is to tear up these steam bubbles, break them into much smaller pieces that can be quickly absorbed and dissolved by surrounding tissue. Since bubbles are much more fragile in rarefaction phase than in compression phase, the waveforms are created to have longer rarefaction time such that the steam bubbles can be easily teared up.

[0063] FIG. 1 demonstrates a special ultrasonic emission waveform sequence 100 designed for steam bubbles elimination.

[0064] In this special waveform sequence 100, a basic emission waveform 101 comprised by normal pulses 102 shown as dashed line and an extended half-period negative pulse 103 as solid line is repeated multiple times, for example 5 times. Notice here normal pulses 102 is a normal 1.5-cycle sine wave, and extended half-period negative pulse 103 is an extended negative half cycle pulse with the negative pulse has a length twice the negative half cycle of the normal waveform 102. The purpose of extended half-period negative pulse 103 is to form a long rarefaction phase such that the steam bubbles can’t hold on the surface tension and break into small pieces. Notice negative pulse 103 with elongated rarefaction phase happens every 2 normal cycles in this pulse sequence. This allows ultrasound wave to expand the bubbles more.

[0065] Fig. 2 (a) -2 (b) have compared the response of a normal ultrasonic pulse sequence and a special ultrasound wave sequence on steam bubbles. In Fig. 2 (a) , a normal ultrasonic pulse sequence 200, when applied to steam bubbles, its compression phase 201, shown as upper half cycles filled with oblique bars will compress the bubble to the minimal size illustrated in 203 corresponding to the highest pressure the pulse can reach. A rarefaction phase 202, identified as net filled lower half cycles, can expand the steam bubbles into a larger size, shown as 204. This is a normal case.

[0066] Fig. 2 (b) illustrates the case where another special waveform sequence 300, similar to the above mentioned waveform sequence 100 is applied to steam bubbles. It can be seen that in the compression phase 301 shown as wave pulse cycles filled with oblique bars, the bubble is compressed into smaller size 304. While in normal rarefaction phase 302, shown as wave pulse filled with net mesh in Figure 2 (b) , the bubble is expanded into a size 305 larger than the size 304. Notice here bubbles at sizes 304 and 305 are same to sizes 203 and 204 in Figure 2 (a) in normal case assume similar bubbles and waveforms. When the waveform is in elongated negative rarefaction phase 303 shown as vertical bar filled pulse regions, the bubble can reach a much larger expanded size 306. As the special waveform sequence repeats multiple times, the bubbles can easily be broken into much smaller ones or dissolved, then the steam cloud will disappear and ultrasound imaging will regain the capability for tissue imaging.

[0067] There are three ways in waveform sequence construction. One way is to adjust the basic waveform. E. g., the basic waveform may have different number of normal wave pulses. FIG. 3 (a) shows a basic waveform extracted from the basic emission waveform 101 of FIG. 1, where the number of normal pulses part, normal waveform 102 is 1.5 cycles. The number of normal waveform cycles can be different, as shown in FIG. 3 (b) waveform 400, where the normal waveform 401 shown as dashed lines has 2.5 cycles, and FIG. 3 (c) waveform 410, where the normal waveform 411 shown as dashed lines has 3.5 cycles. In FIG. 3 (a) -3 (c) , the elongated negative pulse 103, 403, and 413 are all just half cycle and the areas these negative half cycle pulses cover are twice the area the normal negative half cycle covers. Notice the basic waveform shown here are just examples, it can be just a normal waveform with 2 cycles, or 3 cycles, or can be other format that forms a waveform. For example, the basic emission waveforms are in a sinusoidal waveform or a rectangular waveform style.

[0068] The other way for waveform sequence construction is to adjust the number of repeat times of the basic waveform 101, 400, or 410. To enhance the steam vapor elimination capability, the basic emission waveform can be repeated multiple times to form a long pulse sequence, as illustrated in FIG. 1 and further illustrated in FIG. 4. In FIG. 4, the base waveform 410 in Figure 3 (b) is repeated 4 times to form a long pulse sequence 500.

[0069] The third way for special waveform sequence construction is to control the Pulse Repetition Frequency (PRF) of the waveform sequence in transmission at a certain location. E. g., the PRF can be ranged from 10Hz to 10KHz at a predefined location. Fig. 5 gives an example where special waveform sequence 600 is sent to tissue at a defined location in every 10ms, thus, at 100Hz.

[0070] A specially designed ultrasonic imaging system 700 is presented for generating and delivering the special waveform pulse sequences into human body. In this special imaging system 700, there is a new component, special waveform sequence generator 701, it generates special waveform pulse sequences like above mentioned sequences 100, 200, 300, 500, and 600, etc. . The generated special waveform sequence is then sent to transmit wave formation component 702 (a transmit waveform formation unit) , where the actual waveforms are created. In transmit beamformer 703, the generated  waveforms at each channel are delayed per selected focal position, then sent to transmit pulser 704 to form the real electronic waveforms. The waveform then passes the Transmit  / receive (T / R) switch 705, and is sent to transducer 710 to transfer into mechanical vibration and delivered into human body. The echo back from tissue received by the transducer and is transferred into electronic wave. It passes the T / R switch 705 again, and then is amplified by analog front end amplifier 706, converted into digital signal by A / D converter 707, followed by a dynamic receive beamforming functional unit 708 where signals received from all channels are beamformed into radio frequency (RF) data, then sent to receive mid processing unit 709 for processes like demodulation, filtering, thresholding, etc. . The processed data goes through receive post processing unit 710 where the mid processed data is further envelope detected, edge enhanced, or flow signal extracted before sent out for final display at display unit 711. However, it must be pointed out that the special waveform sequence created ultrasound echo may not need to be displayed at all due to its special pulse shape where no spatial resolution has left. A system control unit 712 controls all functional blocks, and a user interface functional block 718. The user interface allows the user to choose the steam cloud elimination functionality and the capability to adjust the waveforms, further, the capability to turn off this steam bubble elimination function.

[0071] This imaging system provides a user interface to enable steam bubble cleaning function, also named here as steam vapor clear functionality and the way to position the transmit focal position of the special waveform pulse sequence at where the steam cloud is located. As in detail implementation, FIG. 7 (a) gives an illustration of interventional page with steam vapor clear function. In FIG. 7 (a) , a user interface is illustrated. During tumor ablation, the user can enter into the intervention page 800. On the intervention page 800, the user can enable or disable the steam cloud elimination function by pushing VaporClear button 801, then select one of Easy button 802, Regular 803 and Hard 804 buttons. Each of Easy 802, Regular 803 and Hard 804 buttons correspond to a different waveform sequence. As an example, Easy 802 button corresponds to a special waveform sequence 812 with the basic waveform 101 repeats twice as shown in FIG. 7 (b) , Regular 803 button corresponds to a special waveform sequence 813 which is constructed by the basic waveform 101 repeats three times as shown in FIG. 7 (c) and Hard 804 button corresponds to a special waveform sequence  814 with the basic waveform 101 repeats four times as shown in FIG. 7 (d) . The longer the special waveform pulse sequence, the stronger capability it can break the steam bubbles and eliminate steam vapor effect. After the user selects the desired steam cloud elimination method, system will allow the user to use a cursor to position the central point of the steam bubble elimination region.

[0072] Notice here that even though Easy 802, Regular 803, and Hard 804 button are associated with the length of special waveform pulse sequence, they can also be associated with different transmit frequencies of the basic waveform, or the length of the basic waveform. Typically, the center frequency of the basic waveform shall fall inside the transducer bandwidth but toward the lower side.

[0073] As shown in FIG. 8, selection of steam vapor clear region in the imaging plane is demonstrated. The imaging plane 900 is a cross section of human liver tissue. At the upper left corner, there is a tumor 901 and an ablation needle 902 is inserted from upper right corner to the tumor 901. During ablation process, the therapy creates a steam cloud 903 around the tip region of the needle 902 which covers tumor 901 and the underneath tissue. When the user enables the steam vapor clear function, system allows the user to place a cursor 904 marked as a round circle at where the center of the steam cloud 903 is, and then the user can adjust the size of the rectangular frame 905 centered at cursor 904. This rectangular region 905 is where the special waveform sequence is applied.

[0074] FIG. 9 shows how the steam vapor clear special waveform is applied in the rectangular region 905. The invented imaging system will transmit multiple beams inside the rectangular region 905 for steam vapor elimination. For each beam, the transmit focus is designed in the way that the focal region, an ellipsoidal volume 907 covers most of the depth side of the rectangular region 905. The number of special waveform beams used to fill the horizontal side is calculated as: Number of Beams = Rectangular Width  / Beam Width,

[0075] Such that the fired special waveform beams can cover the whole rectangular region width.

[0076] As in FIG. 9, there are ten beams to fill the width of the rectangular region 905. The firing sequence can be as same as a normal CF beam firing sequence, with a predefined PRF, as shown in FIG. 10. In FIG. 10, 8 special waveform beams are  assigned to fill in the corresponding rectangular region the user has chosen. They are arranged and fired in an interleave way. In FIG. 10, firing 1, firing 1’ and firing 1” are fired at same spatial location and so do other beams with the same value, like firing 3, firing 3’ and firing 3”. The timing between firing 1 and firing 1’ is 1 / PRF, where PRF is the firing PRF defined by a user. The PRF can also have predefined values selected by functional buttons shown in FIG. 7 (a) like Easy 802, Regular 803, and Hard 804. The user can overwrite the default PRF value by manually selecting the available PRF values for each setting. The system fires the beams 1, 2, 3, …8 from left side to right side of the rectangular region 905, then return back to spatial location for firing 1 again, and repeat the firings sequence in space with firings 1’, 2’, …, 8’ then comes back again for firings 1”, 2”, …, 8”. This process repeats till the number of ensemble size for this Color Flow alike imaging mode has been reached. Same as in CF imaging, regular B mode imaging and this vapor eliminating mode can be interleaved as B / CF imaging mode has done.

[0077] After the user chooses the desired vapor clear mode, placed the eliminating firing rectangular box at where the steam cloud is, the ultrasonic imaging system will start the firing of the special waveform pulse sequences. The user can stop the process when the steam cloud pretty much disappears or it does not affect the normal monitoring of the tumor status any more.

[0078] FIG. 11 gives overall procedure 920 for steam cloud elimination through vapor clear function. After the imaging system starts imaging at 921, a user starts and finishes the tumor ablation procedure 922 and steam cloud is generated. The user then enters into the VaporClear mode in process 923 and moves a cursor to the center of the steam cloud in process 924. After adjusting the rectangular vapor clear region to fit the steam cloud in process 925, the user can turn on vapor clear function to send special waveform sequence to kill the steam bubbles in process 926. The user then can turn back to normal imaging mode after the steam cloud is dissipated in process 927.

[0079] Another way to implement the steam vapor clear function is to only apply the special waveform sequence to the beam located at where the cursor is placed shown as 904 in FIG. 8 with a predefined PRF or a PRF defined on the fly. The overall procedure is similar to the above procedure in FIG. 11, where a rectangular region is defined and  adjusted for steam vapor elimination. The difference is that after process 924 in which the user places the cursor at where the steam cloud center is, there is no rectangular region to adjust as in above process 925, instead, the user will directly enable the steam vapor elimination function to kill the steam bubbles as described in process 928 shown in Fig 12, where the whole process is defined as procedure 930. In Fig 12, after the steam bubbles are cleared at where the beam is located, user can then manually move the beam to another place to continue the steam vapor elimination in process 929. This operation continues until the user decides that the steam cloud is no longer a problem for tumor ablation efficacy evaluation. Then the user will turn off the steam vapor clear function and back to normal imaging mode in process 927, same as in Fig 12.

[0080] Notice in above illustrated designs, although the user is allowed to manually place the cursor to where steam cloud is to focus the special waveform sequence to eliminate the steam bubbles, this procedure can be done automatically through artificial intelligence (AI) . The steam cloud can be identified automatically through AI, then the imaging system applies the special waveform sequence focal positions to where the steam cloud located.

[0081] Further to be noticed is that although the invention describes a novel basic waveform with an elongated rarefaction phase to facilitate the bubble elimination process, a normal waveform can also be chosen as basic waveform as long as a longer pulse sequence is formed so enough energy can be delivered at where the steam bubbles are to kill the big steam bubbles.

[0082] Although the present invention and the implementations of the present invention have been schematically described above, such description is not limiting. The accompanying drawings shows only one of the implementations of the present invention, and the actual flow is not limited thereto. Therefore, any structure or embodiment similar to this technical solution designed without creative efforts by those of ordinary skill in the art based on the teaching of the present invention and without departing from the spirit of the present invention shall fall within the scope of protection of the present invention.
