THERAPEUTIC LY6K MRNA VACCINE FOR CANCERCROSS REFERENCE TO RELATED APPLICATIONS

[0002] The present disclosure relates to a vaccine comprising an RNA polynucleotide that comprises at least one open reading frame (ORF) encoding the LY6K protein, and lipid nanoparticle (LNP) composition thereof. The present disclosure also provides a method of treating cancer in the subject.BACKGROUND

[0003] Head and neck squamous cell carcinoma (HNSCC) is the sixth most common cancer worldwide (890,000 new cases and 450,000 deaths annually) and occurs in the mucosal surfaces of four major anatomical sites: oral cavity, sinonasal cavity, pharynx and larynx. The major cause of HNSCC are tobacco, alcohol and human papillomavirus (HPV) infection.

[0004] HNSCC of the oral cavity is generally treated with surgical resection, followed by adjuvant chemotherapy plus radiotherapy. Molecularly targeted agents, such as the epidermal growth factor receptor (EGFR) inhibitor cetuximab, have shown modest success in locally advanced disease. Finally, two immune-checkpoint inhibitors (ICIs) targeting PD1 (nivolumab and pembrolizumab) are approved for the treatment of recurrent and / or metastatic (R / M) HNSCC. Unfortunately, only 15–20%of patients with HNSCC achieve a durable response to these agents despite a twofold to threefold higher expression of PD1 and PDL1 within the tumor. Thus, there is an unmet need to develop new modalities and therapies that can improve therapeutic outcomes and prolong survival for patients with cancer.

[0005] Lung cancer is one of the most commonly diagnosed cancers, and the leading cause of cancer-related mortality worldwide (Siegel 2016) . In 2020, an estimated 228, 820 new cases of lung cancer were to be diagnosed in the United States, and 135, 720 people were estimated to die from the disease (American Cancer Society 2020) . Non-small cell lung cancer (NSCLC) accounts for 80-85%of all lung cancers, and is composed of several histopathological subtypes, the most common of which are adenocarcinoma (40-60%) and squamous cell carcinoma (30%) . The majority of patients with NSCLC are often found to have advanced cancer at the time of diagnosis.

[0006] Systemic therapy with platinum-based doublet regimens, with or without maintenance therapy was until recently the standard first-line treatment for patients with advanced NSCLC whose tumors do not have an epidermal growth factor receptor (EGFR) mutation, an anaplastic lymphoma kinase (ALK) mutation, or a c-ros oncogene 1 receptor tyrosine kinase (ROS1) fusion (Besse 2014, Ettinger 2016, Reck 2014) . This is still the standard of care for patients in countries where therapies with immune checkpoint inhibitors are not approved or available. Chemotherapy for metastatic NSCLC is not curative and despite optimal treatment, patients have a median overall survival (OS) of approximately 8 to 12 months, and a 5-year survival rate of approximately 18% (Siegel, 2016) . Therefore, newer therapeutic approaches are needed that will improve long-term survival and quality of life (QOL) in patients with lung cancer, particularly advanced NSCLC.

[0007] Messenger RNA (mRNA) has emerged as a new category of pharmaceutical agent for various therapeutic applications, including viral vaccines, protein replacements, immunotherapy, and gene editing. To function in vivo, mRNA requires safe, effective and stable delivery systems that protect the nucleic acid from degradation and that allow cellular uptake and mRNA release. To date, the most successful delivery system for mRNA in clinical trials are lipid-based carrier vehicles which are dissolved in ethanol and then mixed with an aqueous solution of the nucleic acid to form lipid nanoparticles (LNPs) . The LNP compositions of the present disclosure can induce an immune response and stimulate T effector cells in vivo and, accordingly are useful in treating cancer.SUMMARY

Example 4: mRNA-LNP preparation

[0119] A mix of lipids containing the ionizable lipid, phosphatidylcholine, cholesterol and PEG-conjugated lipid was dissolved in ethanol. The molar ratio of ionizable lipid (DLin-MC3-DMA) , phosphatidylcholine (DSPC) , cholesterol and Pegylated lipid (PEG2000-DMG) was 50: 10: 38.5: 1.5. And mRNA was dissolved in a sterile and RNase-free 10 mM sodium acetate buffer (pH 4.0) prior to preparation (the N: P ratio was 6: 1) . Lipid nanoparticles were prepared by a rapid mixing of lipids mixture containing ethanol phase with an aqueous phase containing the RNA using a microfluidic mixing device (AITESEN, model L2, one volume of lipid mixture in ethanol and three volumes of RNA in 10 mM sodium acetate buffer) at a total flow rate of 20 mL / min, and directly mixed and diluted with 10 volumes of 1x phosphate buffered saline (PBS) . The lipid nanoparticles containing RNA were subjected to diafiltration and ultrafiltration using Amicon Ultra-15 Centrifugal filters (Millipore) and / or Tangential Flow Filtration. The final lipid nanoparticle products were stored in 10%sucrose (w / v) in 5 mM Tris buffer (pH 8.0) and filtered through 0.22 μm filter in sterile condition. The mRNA-LNP was stored at -80℃ till use. Example 5: In vitro protein expression

Example 6: In vivo immunogenicity in mice

[0122] The target encapsulated mRNA-LNP (LY6K-1) was injected into female C57BL / 6 mice (Shanghai BK) via intramuscular (IM) injection at both sites of gastrocnemius muscle (0.1 mL / mouse) . The dose of mRNA is 10 μg / mouse, and the mRNA-LNP was injected on Day 0, Day3 and Day 7. On Day 7 and Day 14, mice were euthanized, and the spleen and inguinal and popliteal lymph nodes were collected for the IFN-γ Enzyme-linked immunospot (ELISPOT) assay. [0125] The full-length of human wild-type Ly6k gene (SEQ ID NO: 27) was cloned into the genomic DNA of murine colon cancer MC38 cell line through PiggyBAC transposase system. In brief, the full-length wild-type antigen Ly6k gene was inserted into the cloning vectors, which contains puromycin resistance gene. Then the PiggyBAC transposase plasmid (provided by Shanghai Model Organisms Center) and LY6K cloning vector plasmid (provided by Shanghai Model Organisms Center) were co-transfected into MC38 cells through Lipofectamine 3000 (Thermofisher, L3000015) . The Ly6k gene was randomly inserted into the genome DNA of MC38 cells. The transgenic MC38-Ly6K cells containing human Ly6K were enriched with puromycin treatment and then single cell clones were selected with limited dilution method. The LY6K transgenic MC38 single cell clones were confirmed through q-PCR sequencing and protein expression, and passed the mycoplasma test before in vivo study.

[0126] For in vivo efficacy study, 3x105 LY6K transgenic MC38 cancer cells were inoculated into the right flank of female C57BL / 6 mice (Shanghai Model Organisms Center, Inc. ) . The dose level of LY6K mRNA-LNP (LY6K-1) was 10 μg / mouse via intramuscular injection and the dosing volume was 0.1 mL / mouse. The LY6K mRNA-LNP was injected on Day 0, Day 3, Day 7, Day 14, Day 21, Day 28, and Day 35, and the MC38-hLy6k cancer cells were inoculated on Day 9. The mouse body weight and tumor volume were measure three times per week.
