[0051] Turning to FIG. 1, a surgical system 10 provided in accordance with the present disclosure includes a console 100, a footswitch 200, and one or more surgical cutting devices 300. Console 100 may include an outer housing 110 enclosing the internal operable components of console 100, a touch screen graphical user interface (GUI) 120 to receive user input and display information to the user, a plurality of device ports 130, one or more fluid pumps 140, and / or other suitable features. One or more processors and associated memory(s) (not shown) are disposed within outer housing 110 and function to provide power and control signals to devices connected to console 100; to process user inputs, feedback data, and other information received at console 100; and to control the one or more fluid pumps 140. Suitable hardware and drive mechanisms may be disposed within outer housing 110 to perform the various functions of console 100 and may include, for example, power-generating circuitry, sensor circuitry, motors, pump drivers, etc.

[0052] Footswitch 200 is configured to connect to one of the device ports 130 of console 100 and includes a base 210, a foot pedal 220 (including an underlying switch or switches (not explicitly shown), and one or more control buttons 230. A cord 240 is configured to connect footswitch 200 to one of the device ports 130 of console 100 such that user inputs provided to footswitch 200 are received at console 100, e.g., for controlling settings, actuating, and / or otherwise operating one or more devices associated with console 100.

[0053] The one or more surgical cutting devices 300 may define any suitable configurations for use in performing various different surgical tasks, for use in various different procedures, etc. One example of a suitable surgical cutting device, surgical cutting device 300, generally includes a handle 310, an outer shaft 320 extending distally from handle 310 (releasably or integrally connected thereto), a cutting tool 330 extending distally from outer shaft 320 (releasably or integrally connected thereto), a motor 340 disposed within handle 310 and operably coupled to cutting tool 330 to drive rotation of cutting tool 330 relative to outer shaft 320 to cut tissue, and aAttorney Docket: A0012486W001 cord 350 to connect motor 340 to console 100 to enable console 100 to power and control motor 340, thereby controlling cutting tool 330. Motor 340 may be an electric motor, pneumatic motor, ultrasonic transducer, or other suitable motor configured to drive cutting tool 330 to rotate for cutting tissue. In aspects, surgical cutting device 300 may further include additional features such as, for example, hand control(s), navigation, articulation, etc. In aspects, handle 310 is configured to be held and manipulated by a surgeon. In additional or alternative aspects, handle 310 is configured to mount to an arm of a robotic surgical system (not shown). Thus, the term “handle,” as utilized herein, is not limited to handheld surgical devices but also includes robotic surgical devices.

[0054] Cutting tool 330 may define any suitable configuration and may be integrated with surgical cutting device 300 or removable therefrom. More specifically, and with additional reference to FIG. 2, various different rotary cutting tools 332a, 332b, 332c may be configured for releasable attachment with surgical cutting device 300, thus defining cutting tool 330 thereof. In aspects, rotary cutting tools 332a, 332b, 332c are releasably engagable with outer shaft 320 (which, in turn, may be releasably or integrally connected to handle 310). Alternatively, rotational cutting tools 332a, 332b, 332c may be integral with corresponding outer shafts 320 that are, in turn, releasably engagable with handle 310. In either configuration, surgical cutting device 300 is thus capable of being interchangeably customized with a particular rotary tool 332a, 332b, 332c, depending upon a particular purpose.

[0055] Continuing with reference to FIG. 2, rotary cutting tools 332a, 332b, 332c each generally includes a stem 334 having a cutting head 336 disposed at a distal end of stem 334. At least a portion of each cutting head 336 defines an increased diameter compared to a distal end of the corresponding stem 334. In aspects, stems 334 define a body 335a having a first diameter and a neck 335b connecting the body 335a with the corresponding cutting head 336 and defining a diameter less than the diameter of body 335a along at least a portion of neck 335b. In aspects, neck 335b tapers in diameter in a proximal to distal direction. In other aspects, neck 335b is omitted and the body 335a of the stem 334 connects directly to the cutting head 336. In such aspects, the body 335a may define a substantially constant diameter along its length.

[0056] Cutting heads 336 may define any suitable shape such as, for example, a substantially spherical configuration 337a, a bullet configuration 337b, or any other suitable configuration including dome configurations, cylindrical configurations, polygonal configurations, etc.Attorney Docket: A0012486W001Further, cutting heads 336 may include one or more cutting features configured to facilitate cutting, e.g., bone, when disposed in contact with bone (or other suitable tissue to be cut) such as, for example, cutting flutes 338a, e.g., defining cutting edges, and / or abrasive material 338b, e.g., diamond.

[0057] With reference to FIGS. 3 and 4, surgical cutting device 300 may be utilized to cut cranial bone “C” (FIG. 3), to cut spinal bone “S” (FIG. 4), or for cutting any other bone or tissue. With respect to cutting cranial bone “C,” for example and as shown in FIG. 3, surgical cutting device 300 may be utilized for transverse cutting of cranial bone “C” in a desired pattern, e.g., to enable removal of a section of cranial bone “C” to provide suitable access to internal tissue, e.g., brain tissue. With respect to cutting spinal bone “S,” for example and as shown in FIG. 4, surgical cutting device 300 may be utilized for transversely cutting spinal bone “S,” e.g., to shave or otherwise shape the bone “S,” and / or to drill into tissue, e.g., spinal bone “S,” to a relatively greater depth.

[0058] Turning to FIGS. 5 and 6, another surgical cutting device provided in accordance with the present disclosure is shown generally identified by reference numeral 500. Surgical cutting device 500 is configured to inhibit and / or remove tissue wrap, as detailed below, and may include any of the features of surgical cutting device 300 (FIG. 1) as detailed above unless explicitly contradicted below. Accordingly, similarities between surgical cutting device 500 and surgical cutting device 300 (FIG. 1) are summarily described or omitted entirely.

[0059] Surgical cutting device 500 includes an outer shaft 520, a rotary cutting tool 530 extending through and distally from outer shaft 520, and a sleeve 550 extending through and distally from outer shaft 520 about rotary cutting tool 530. As an alternative to a separate sleeve 550, sleeve 550 may be defined at a distal portion of outer shaft 520. As another alternative, sleeve 550 may be disposed about outer shaft 520 rather than within outer shaft 520. Sleeve 550 may be permanently coupled to surgical cutting device 500 or may be removably coupled to surgical cutting device 500 to enable use of surgical cutting device 500 with or without sleeve 550.

[0060] Rotary cutting tool 530 includes a stem 534 and a cutting head 536 disposed at a distal end of stem 534. Stem 534 defines a body 535a and a neck 535b interconnecting body 535a with cutting head 536 at the distal end of stem 534. The proximal end of stem 534 is configured to connect to a drive source, e.g., a motor, to drive rotation of rotary cutting tool 530 relative to outer shaft 520. Cutting head 536 defines one or more cutting features, e.g., cutting flutes 538 definingAttorney Docket: A0012486W001 cutting edges 539, configured to facilitate cutting tissue in response to rotary driving of rotary cutting tool 530.

[0061] Sleeve 550 is configured to inhibit and / or remove tissue wrap and, as noted above, extends distally from outer shaft 520 about rotary cutting tool 530. Sleeve 550, more specifically, may include a proximal collar 552 received within an annular recess 512 defined within outer shaft 520 (or a portion of the handle (not shown, see FIG. 1) of surgical cutting device 500) to longitudinally fix sleeve 550 relative to relative to rotary cutting tool 530, while rotatably coupling sleeve 550 relative to rotary cutting tool 530, although sleeve 550 may be longitudinally fixed and rotatably coupled to rotary cutting tool 530 in any other suitable manner such as, for example, via a bushing, bearings, etc.

[0062] Sleeve 550 extends distally from outer shaft 520 about rotary cutting tool 530 to a distal end 556 of sleeve 550 wherein sleeve 550 defines a cutting feature such as, for example, a serrated annular distal edge 558. Distal end 556 of sleeve 550 terminates at a location disposed about neck 535b of stem 534 proximally spaced from head 536 of rotary cutting tool 530. Thus, angled cutting using head 536, even at steep angles wherein a side of head 536 contacts tissue, is not inhibited by sleeve 550. The positioning of distal end 556 of sleeve 550 about neck 535b of stem 534 together with the free rotation of sleeve 550 relative to rotary cutting tool 530 enables cutting of tissue disposed about neck 535b of stem 534, e.g., with serrated annular distal edge 558, to thereby inhibit tissue wrap around neck 535b of stem 534 and / or to remove tissue wrapped around neck 535b of stem 534 without compromising the rotary driving of rotary cutting tool 530 to cut tissue in contact with head 536. Thus, tissue wrap is inhibited and / or removed during use without the need to stop the procedure and manually remove the tissue wrap.

[0063] Referring to FIGS. 7 and 8, another surgical cutting device provided in accordance with the present disclosure is shown generally identified by reference numeral 700. Surgical cutting device 700 is configured to inhibit and / or remove tissue wrap and is similar to surgical cutting device 500 (FIGS. 5 and 6), except as detailed below.

[0064] Surgical cutting device 700 includes a sleeve 750 that, rather than being longitudinally fixed and rotatably coupled to the rotary cutting tool as with surgical cutting device 500 (FIGS. 5 and 6), is both longitudinally slidable and rotatable relative to rotary cutting tool 730. More specifically, sleeve 750 included a proximal collar 752 rotatably and slidably received within an elongate recess 712 defined within outer shaft 720 (or the handle (not shown, see FIG. 1) ofAttorney Docket: A0012486W001 surgical cutting device 700) to enable longitudinal translation of sleeve 750 relative to outer shaft 720 and rotary cutting tool 730, e.g., via movement of proximal collar 752 between the proximal and distal ends of elongate recess 712, and rotation of proximal collar 752 within elongate recess 712 and relative to outer shaft 720 and, thus, rotary cutting tool 730.

[0065] In aspects, an actuator 760 is coupled to sleeve 750 and accessible from a proximal end portion, e.g., a handle (not shown, see FIG. 1), of surgical cutting device 700 to enable selective translation, e.g., extension and retraction, of sleeve 750 relative to rotary cutting tool 730. Actuator 760, in aspects, includes a manipulation portion 762 disposed at a proximal end portion thereof and a collar 764 disposed at a distal end portion hereof. Collar 764 is rotatably coupled to sleeve 750 to enable selective translation of sleeve 750 in response to manipulation of manipulation portion 762 while allowing free rotation of sleeve 750 relative to actuator 760. Other configurations are also contemplated such as, for example, where actuator 760 is translatable and rotatable to respectively translate and rotate sleeve 750, or where actuator 760 is omitted and extension and retraction of sleeve 750 is accomplished by grasping sleeve 750 and moving sleeve 750 relative to rotary cutting tool 730.

[0066] The retracted position of sleeve 750 may correspond to a position wherein distal end 756 of sleeve 750 is proximally spaced from head 736 of rotary cutting tool 730 (exposing at portion of neck 734 of rotary cutting tool 730). The extended position of sleeve 750 may correspond to a position wherein distal end 756 of sleeve 750 at least partially overlaps head 736 of rotary cutting tool 730 or, in aspects, fully overlaps head 736 of rotary cutting tool 730 (see FIG. 10A, for example).

[0067] In use, sleeve 750 may be allowed to rotate relative to rotary cutting tool 730 or may be rotated relative to rotary cutting tool 730, and / or may be allowed to translate relative to rotary cutting tool 730 or may be translated relative to rotary cutting tool 730, to enable cutting of tissue disposed about stem 734, to thereby inhibit tissue wrap around stem 734 and / or to remove tissue wrapped around stem 734 without compromising the rotary driving of rotary cutting tool 730 to cut tissue in contact with head 736. Thus, tissue wrap is inhibited and / or removed during use without the need to stop the procedure and manually remove the tissue wrap.

[0068] With reference to FIGS. 9-10B, yet another surgical cutting device provided in accordance with the present disclosure is shown generally identified by reference numeral 900. Surgical cutting device 900 is configured to inhibit and / or remove tissue wrap and is similar toAttorney Docket: A0012486W001 surgical cutting device 700 (FIGS. 7 and 8) in that surgical cutting device 900 includes a sleeve 950 that is both longitudinally slidable and rotatable relative to outer shaft 920 and rotary cutting tool 930. However, rather than including an actuator or otherwise manually moving the sleeve between extended and retracted positions as with surgical cutting device 700 (FIGS. 7 and 8), surgical cutting device 900 includes a biasing spring 970 configured to bias sleeve 950 towards an extended position relative to outer shaft 920 and rotary cutting tool 930. Biasing spring 970 may be a coil spring disposed between or about a proximal end portion of sleeve 950 and an internal stop within outer shaft 920 (or a stop within the handle (not shown, see FIG. 1) of surgical cutting device 900), although other suitable biasing springs and / or positions thereof are also contemplated.

[0069] The at rest, extended position of sleeve 950 may correspond to a position wherein distal end 956 of sleeve 950 at least partially overlaps head 936 of rotary cutting tool 930 or, in aspects, fully overlaps head 936 of rotary cutting tool 930 (FIG. 10A). In use, when the distal end of surgical cutting device 900 is pressed against tissue, e.g., bone to be cut, sleeve 950 is urged proximally from the extended position towards the retracted position, e.g., by surrounding tissue, to expose or further expose head 936 of rotary cutting tool 930 (FIG. 10B) to enable tissue cutting with rotary cutting tool 930 in response to rotational driving thereof. During this tissue cutting, the bias of biasing spring 970 urges sleeve 950 distally such that distal end 956 of sleeve 950 is maintained in contact with tissue to enable cutting of tissue to inhibit tissue wrap and / or to cut tissue wrapped around stem 934 to remove tissue wrapped around stem 934. Free rotation of sleeve 950 relative to rotary cutting tool 930 further facilitates this tissue cutting to inhibit and / or remove tissue wrap during use.

[0070] Referring to FIGS. 11A-11C, still another surgical cutting device provided in accordance with the present disclosure is shown generally identified by reference numeral 1100. Surgical cutting device 1100 is configured to inhibit and / or remove tissue wrap. Surgical cutting device 1100, more specifically, includes a rotary cutting tool 1130 according to any of the configurations detailed herein or any other suitable configuration and a sleeve 1150 that may be configured similar to and include any of the features of sleeves 550, 750, 950 of surgical cutting devices 500, 700, 900 (FIGS. 5-6, 7-8, and 9-10B), respectively. That is, sleeve 1150 may be configured to rotate relative to rotary cutting tool 1130 of surgical cutting device 1100 while remaining translationally stationary relative thereto, sleeve 1150 may be configured to rotate and translate relative to rotary cutting tool 1130 of surgical cutting device 1100, and / or sleeve 1150Attorney Docket: A0012486W001 may be biased towards an extended position relative to rotary cutting tool 1130 of surgical cutting device 1100. Still further, in aspects, surgical cutting device 1100 is provided without a sleeve 1150.

[0071] Rotary cutting tool 1130 and / or sleeve 1150 (where provided) of surgical cutting device 1100 are selectively energizable, e.g., with Radio Frequency (RF) electrosurgical energy from an electrosurgical generator (not shown), to facilitate electrical cutting of tissue to inhibit and / or remove tissue wrap. More specifically, as shown in FIG. 11 A, in aspects, surgical cutting device 1100 defines a bipolar configuration wherein rotary cutting tool 1130 is energized to a first potential and sleeve 1150 is energized to second, different potential to conduct RF energy through tissue disposed therebetween to cut the tissue disposed therebetween to inhibit and / or remove tissue wrap. [0074] Turning to FIGS. 12A-13B, still yet another surgical cutting device provided in accordance with the present disclosure is shown generally identified by reference numeral 1200. Surgical cutting device 1200 is configured to inhibit and / or remove tissue wrap, as detailed below, and may include any of the features of surgical cutting device 300 (FIG. 1) as detailed above unless explicitly contradicted below. Accordingly, similarities between surgical cutting device 1200 and surgical cutting device 300 (FIG. 1) are summarily described or omitted entirely.

[0075] Surgical cutting device 1200 includes an outer shaft 1220, a rotary cutting tool 1230 extending through and distally from outer shaft 1220, and a sleeve 1250 extending through and distally from outer shaft 1220 about rotary cutting tool 1230. As an alternative to a separate sleeve 1250, sleeve 1250 may be defined at a distal portion of outer shaft 1220. Sleeve 1250 may be permanently coupled to surgical cutting device 1200 or may be removably coupled to surgical cutting device 1200 to enable use of surgical cutting device 1200 with or without sleeve 1250.

[0076] Rotary cutting tool 1230 includes a stem 1234 and a cutting head 1236 disposed at a distal end of stem 1234. The proximal end of stem 1234 is configured to connect to a drive source, e.g., a motor, to drive rotation of rotary cutting tool 1230 relative to outer shaft 1220. Cutting head 1236 defines one or more cutting features, e.g., according to any of the aspects above or any other suitable cutting features, to facilitate cutting tissue in response to rotary driving of rotary cutting tool 1230.

[0077] Sleeve 1250 is configured to inhibit and / or remove tissue wrap and, as noted above, extends distally from outer shaft 1220 about stem 1234 of rotary cutting tool 1230. Sleeve 1250, more specifically, is translationally coupled to outer shaft 1220 and rotary cutting tool 1230 to enable translation of sleeve 1250 between a retracted position (FIGS. 12A and 12B), wherein sleeve 1250 is more proximally disposed, and an extended position (FIGS. 13 A and 13B), wherein sleeve 1250 is more distally disposed. Sleeve 1250 may be translationally coupled to outer shaft 1220 similarly as detailed above with respect to sleeve 750 and outer shaft 720 (see FIG. 8) or in any other suitable manner. However, sleeve 1250 is rotationally fixed relative to outer shaft 1220 and, thus, rotary cutting tool 1230 rotates relative to sleeve 1250.

[0078] Sleeve 1250 includes a plurality of slits 1258 disposed annularly about distal end 1256 of sleeve 1250 and extending proximally from distal end 1256 of sleeve 1250 such that the distal end portion of sleeve 1250 defines a plurality of tabs 1259 arranged annularly about stem 1234 of rotary cutting tool 1230. Tabs 1259 are angled radially inwardly such that sleeve 1250 defines aAttorney Docket: A0012486W001 smaller diameter at distal end 1256 thereof as compared to locations proximally of tabs 1259. However, in the extended position of sleeve 1250 (FIGS. 13A and 13B), tabs 1259 contact stem 1234 of rotary cutting tool 1230. Stem 1234 of rotary cutting tool 1230 may define an increased diameter section, one or more raised portions, or any other suitable features such that tabs 1529 do not contact stem 1234 in the retracted position (FIGS. 12A and 12B) of sleeve 1250 and such that tabs 1259 are moved into contact with stem 1234, in the extended position (FIGS. 13A and 13B) of sleeve 1250. In aspects, the extended position (FIGS. 13A and 13B) corresponds to a position wherein tabs 1229 are disposed in contact with or approximated relative to cutting head 1236 of rotary cutting tool 1230. In such aspects, contact between tabs 1259 and stem 1234 may occur at the interface between stem 1234 and head 1236. As an alternative to feature(s) on step 1234 enabling contact between sleeve 1250 and tabs 1259 in the extended position (FIGS. 13A and 13B), tabs 1259 may be urged radially inwardly in the extended position (FIGS. 13 A and 13B) to establish contact between sleeve 1250 and tabs 1259.

[0079] In use, with sleeve 1250 disposed in the retracted position (FIGS. 12A and 12B) such that tabs 1259 are spaced from stem 1234 of rotary cutting tool 1230, rotary cutting tool 1230 may be utilized to cut tissue without interference from sleeve 1250. When it is desired to remove tissue about and / or wrapped around step 1234, sleeve 1250 is moved to the extended position (FIGS. 13A and 13B) such that tabs 1259 contact stem 1234 of rotary cutting tool 1230, and with rotary cutting tool 1230 driven to rotate, friction generated as a result of contact between the stationary tabs 1259 and the rotating stem 1234 generates heat to cut and / or burn tissue to thereby inhibit tissue wrap around stem 1234 and / or to remove tissue wrapped around stem 1234.

[0080] In aspects, irrigation fluid may be provided, e.g., within outer shaft 1220, within sleeve 1250, and / or through rotary cutting tool 1230, at least when sleeve 1250 is disposed in the extended position (FIGS. 13 A and 13B) to inhibit overheating of other components of surgical cutting device 1200 resulting from friction heat generation.
