Methods of diagnosing primary aldosteronism

By measuring PTH, aldosterone, and renin levels in a single blood sample, the method accurately diagnoses primary aldosteronism and predicts treatment responsiveness, addressing the limitations of conventional ARR-based methods and facilitating timely treatment.

AI Technical Summary

Technical Problem

Conventional methods for diagnosing primary aldosteronism rely heavily on the aldosterone-renin ratio (ARR) from single blood samples, which can be misleading due to aberrant and fluctuating hormone levels, leading to undetected or misdiagnosed cases, and existing treatments like 24-hour urine samples are operationally challenging.

Method used

A method involving the measurement of plasma parathyroid hormone (PTH) concentration alongside plasma aldosterone and renin levels in a single blood sample to accurately diagnose primary aldosteronism, using specific thresholds to determine the presence of the condition and predict treatment responsiveness.

Benefits of technology

This approach provides a more reliable and rapid diagnosis of primary aldosteronism, reducing false negatives and enabling timely and effective treatment with compounds like (R)-(+)-5-(p-cyanophenyl)-5,6,7,8-tetrahydroimidazo[l,5-a]pyridine, improving patient outcomes.

Abstract

The present invention relates to non-invasive methods of diagnosing primary aldosteronism (PA), preferably in subjects in need of such diagnosis. As described herein, it was discovered that the concentration of plasma parathyroid hormone (PTH) in blood samples isolated from subjects was predictive of a diagnosis of primary aldosteronism depending on the plasma aldosterone concentration (PAC), plasma renin concentration (PRC), and plasma renin activity (PRA). Preferably PAC, PRC, and PRA are measured from the same blood sample as the concentration of plasma parathyroid hormone.
