Wound retraction apparatus and method

A surgical wound retractor is adapted to dilate a wound stretchable to a desired diameter, the retractor includes a first ring having a diameter greater than that desired for the wound and being adapted for disposition interiorly of the wound. A second ring has a diameter greater than that desired for the wound and is adapted for disposition exteriorly of the wound. A plurality of retraction elements are disposed in a generally cylindrical relationship to each other, between the first ring and the second ring. These elements extend through the wound to exert a radial retraction force on the wound which is dependent on the distance separating the first ring and the second ring. Retraction elements, both distensible and non-distensible are contemplated with appropriate attachment elements at the rings to provide for variations in the retraction force. With a suitable retraction sleeve, a third ring can be provided to form a circumferential retainer to vary the retraction force. Rings can also be made inflatable or self-expanding to vary the retraction force. An associated method includes the step of rolling the second ring circumferentially of the third ring to form the circumferential retainer.

The invention is directed to a surgical wound retractor for retracting and sealing an incision and forming a functional opening or channel through which a surgical procedure may be executed. The wound retractor provides a path for a surgeon to insert his hand and / or instruments through the opening formed by the wound retractor. The wound retractor is sized and configured to be easily placed through a small incision and removed without further insult to the body tissue adjacent to the incision. The wound retractor is adapted to dilate a surgical wound incision to a desired diameter, and comprises a first ring having a diameter greater than the desired diameter of the wound incision and being adapted for disposition interiorly of the wound incision; a second ring having an annular axis and a diameter greater than the desired diameter of the wound incision and being adapted for disposition exteriorly of the wound incision; and a flexible sleeve disposed in a generally cylindrical form between the first ring and the second ring, the first ring having at least one notch to facilitate folding or collapsing of the first ring during insertion and removal of the first ring through the wound incision. The first ring may further comprise a second notch disposed on an opposing end of the first notch to further facilitate folding or collapsing of the first ring. With this aspect, the first ring is folded by squeezing between the first and second notches during insertion and removal of the retractor from the incision. In another aspect, the wound retractor may further comprise a tether having a length, a first end attached to the first ring interiorly of the sheath, and a second end disposed outside of the wound incision, wherein the tether facilitates removal of the first ring by pulling on the second end to retrieve the first ring through the wound incision.

A wound dressing system includes a fluid permeable support member for positioning within a wound and adapted to generally conform to a topography of a wound, a plurality of beads supported by the support member, an outer member for positioning over the wound to substantially enclose the beads and a conduit for supplying reduced pressure to the wound. The support member is adapted to permit exudates from the wound to pass therethrough. The beads are characterized by having sufficient rigidity to substantially maintain respective shapes thereof to thereby facilitate passage of the exudates through spaces or passages defined between adjacent beads. The beads may comprise glass, an acrylic or a polymeric material. The support member may comprise a polymeric or fabric material. The support member may be an enclosure member or pouch which houses the beads. Multiple pouches are also envisioned.

The reduced-pressure system may develop a force, which may include a vertical force that is realized at tissue site deeper than the epidermis or a closing force directed towards the incision. A speculum for use with a body opening, such as a vagina, rectum or surgical wound, may have an insertion portion with an arcuately-shaped wall and a slot that extends along a length of the wall. An end face of the wall at the distal end of the insertion portion may be arranged at a non-perpendicular angle to the longitudinal axis of the insertion portion. A flange portion joined at a proximal end of the insertion portion may have a continuous band that extends radially outwardly from the wall and includes a notch that communicates with the slot. A handle portion may have a gripping surface extending proximally at a bottom side of the flange portion with an arcuate shape that is approximately concentric with the wall of the insertion portion. A top side of the flange portion may be free of the handle portion.

Radiopaque marker for a surgical sponge

A surgical sponge comprises a plurality of radiopaque markers having a high radiographic density and a distinctive, visually recognizable shape. The markers have an x-ray density equivalent to at least about 0.1 g / cm2 of BaSO4. The markers produce an x-ray image with high contrast and a shape that is readily recognizable and differentiated from the images produced by other items and structures commonly seen in x-rays of post-operative patients. Owing to the distinctive, high contrast image produced by the markers, the sponge is reliably and unambiguously detected. This is so even in situations where the sponge is inadvertently left in the surgical wound. Discomfort, trauma, and possibly fatal consequences that might otherwise occur are virtually eliminated. The surgical procedure is carried out with decreased likelihood of a sponge being retained inadvertently.

A radio-frequency surgical implement detection system detects surgical implements in a surgical wound during and at the completion of a surgical procedure. Surgical implements, including surgical sponges or laparotomy pads, gauze pads and metallic surgical instruments, are individually attached to a non battery-powered, encapsulated radio-frequency marker. The marker comprises an integrated chip having a burnt-in memory code, which is broadcast through an antenna using a modulated carrier frequency. The code is received by an interrogating antenna of a detector. The interrogating antenna provides a power pulse, which is received by the antenna of the radio-frequency marker. The power pulse charges a capacitor, which proves power for the read function, carrier frequency modulation function and broadcast function of the integrated chip, permitting each marker-containing implement to be specifically identified.

The present invention discloses a method for the preparation of an anti-microbial wound dressing comprises alkalizing chitosan fiber to obtain alkalized chitosan fiber which is then etherified with chloroacetic acid to produce carboxymethyl chitosan fiber. Said fiber is made into dressing by fiber opening, web formation and needling. Alternatively chitosan fiber can be made into chitosan non-woven fabric by non-woven technique first which is then carboxylmethylated. The fabric is then made into wound dressing by cutting, packaging and sterilizing. The present invention also discloses the uses of the anti-microbial wound dressing made according to said method. The dressing according to the present invention can be applied in surgical wound, burn, and other chronic wound. Said dressing, when covering wound, is able to prevent moisture losses in body fluid, provide a favorable moist environment necessary for wound healing and maintain a fluid-free, maceration-free, germ-free wound surface. Said dressing is antiphlogistic, hemostatic and antalgic and promotes wound healing.

The present invention provides an improved surgical blade and trocar system for accessing the retina and other parts of the eye while doing vitreo-retinal and cataract surgeries, including surgeries for macular degeneration. The eye surgeon uses an improved surgical blade for vitreo-retinal and cataract surgeries having a generally flat, V-shaped, W-shaped, or “extended W” shaped cross-section. Using the improved surgical blade, the surgeon creates a multi-planar, self-sealing surgical wound, first by directing the surgical blade substantially perpendicular to the eye surface, then redirecting the blade to follow the general curvature of the eye globe, and finally redirecting the blade to enter the interior of the eye. The improved surgical blade is used with an improved trocar system having two main parts-a relatively rigid, wide-mouthed outer segment and a generally thin-walled, collapsible plastic polymer or metal mesh sleeve that spans the surgical wound and substantially molds to its contour. The improved surgical blade and trocar system can be adapted for use in either vitreo-retinal or cataract surgeries.
