Abstract
Description
[0001] Description of the case
[0002] The invention patent application is a divisional patent application. The original application of this divisional patent application (i.e.: Mother's case Firm Roll No. IIM211329 ), which is an invention patent application with an international application date of November 25, 2019, international application number PCT / US2019 / 063063, Chinese national application number 201980078454.1, and invention name “Syringe-based delivery device for vascular access device”. Background Art
[0003] Catheters are commonly used for various infusion therapies. For example, they can be used to administer fluids, such as saline solutions, various medications, and total parenteral nutrition, to patients. Catheters can also be used to withdraw blood from patients.
[0004] One common type of catheter is an over-the-needle peripheral intravenous ("IV") catheter. As the name suggests, an over-the-needle catheter can be mounted on an introducer needle with a sharp distal tip. The catheter and introducer needle can be assembled so that the distal tip of the introducer needle extends beyond the distal tip of the catheter, with the bevel of the needle pointing upward, away from the patient's skin. The catheter and introducer needle are typically inserted through the skin at a shallow angle into the patient's vasculature.
[0005] To verify proper placement of the introducer needle and / or catheter in the blood vessel, the clinician typically confirms the presence of a "flashback" of blood in the flashback chamber of the catheter assembly. Once needle placement is confirmed, the clinician can temporarily occlude flow in the vasculature and remove the needle, leaving the catheter in place for future blood draws or fluid infusions.
[0006] Drawing blood using a peripheral IV catheter can be difficult for several reasons, particularly when the catheter is in place for longer than a day. For example, when a catheter is inserted into a patient for an extended period, the catheter or vein may be more likely to narrow, collapse, kink, become clogged with debris (e.g., fibrin or platelet clots), and adhere the catheter tip to the vasculature. Consequently, catheters are typically used to collect blood samples while the catheter is in place, but are much less frequently used to collect blood samples while the catheter is in place. Consequently, when a blood sample is needed, an additional needle stick is required to provide venous access for blood collection, which can be painful for the patient and result in higher material costs. Therefore, there is a need for a catheter system and method that facilitates placement of blood sampling instruments, such as catheters and probes, within a patient's vasculature without the need for additional needle sticks.
[0007] The subject matter claimed herein is not limited to embodiments that solve any disadvantages or that operate only in environments such as those described above. Rather, this background is merely provided to illustrate one example technology area where some embodiments described herein may be practiced. Summary of the Invention

[0068] In some embodiments, catheter assembly 30 may include a catheter 36 that may be secured within catheter adapter 32 and may extend distally from catheter adapter 32. In some embodiments, catheter 36 may include a peripheral intravenous catheter that may be placed within the patient's vasculature. In some embodiments, catheter adapter 32 may be integrated, have an integrated extension tube (not shown), or be non-integrated.
[0069] In some embodiments, delivery device 10 can allow device 22 to access the patient's vasculature through catheter 36, which can be inserted into the patient's vasculature. In some embodiments, when device 22 is introduced into the catheter assembly via delivery device 10, the device can access the fluid path of catheter assembly 30 and / or the device 22 can extend through catheter assembly 30 and access the patient's vasculature.
[0070] In some embodiments, catheter assembly 30 can include a needle hub coupled to the proximal end of catheter adapter 32 and an introducer needle (not shown) extending distally from the needle hub. In some embodiments, the needle hub and introducer needle can be removed from catheter assembly 30 in response to placement of catheter 36 within the patient's vasculature, and after the needle hub and introducer needle are removed, delivery device 10 can be coupled to the proximal end of catheter adapter 32.
[0071] In some embodiments, guide feature 18 and channel 20 can be oriented in various ways and angles within barrel 14. [0072] In some embodiments, the barrel 14 can include a liquid 38, such as saline or other suitable flushing liquid. In some embodiments, the liquid 38 can completely fill the barrel 14. In some embodiments, the liquid 38 can be used to flush all or a portion of the catheter assembly 30, as will be explained in further detail.
[0073] In some embodiments, a guidewire 40 can be disposed within the device 22. In some embodiments, in response to the plunger 16 being partially and / or fully depressed, the first end 42 of the guidewire 40 can be advanced in a distal direction beyond the distal end of the syringe 12. In some embodiments, the second end 44 of the guidewire 40 can be fixed. For example, the second end 44 of the guidewire 40 can be fixed within the barrel 14.
[0074] In some embodiments, the guidewire 40 can be used to facilitate placement of the device 22 within the patient's vasculature, which can result in less vein-related trauma. In some embodiments, the guidewire 40 can support the device 22, such as a catheter, during advancement to prevent it from being inserted into the second catheter (e.g., catheter 36, such as a catheter 36). Figures 1A-1B In some embodiments, the guidewire 40 can support the device 22 along all or a portion of the device 22 to improve structural reliability, provide adequate blood collection and fill time performance, maintain an acceptable risk of hemolysis, and minimize the risk of kinking the device 22. In some embodiments, after blood sampling is completed, the guidewire 40 and device 22 can be enclosed within the delivery device 10. It should be understood that in some embodiments, the delivery device 10 may not include a guidewire 40.
[0075] Now refer to Figure 1C, in some embodiments, the channel 20 can contact and support the instrument 22. In some embodiments, the shape of the channel 20 can be configured to reduce the risk of the instrument 22 kinking or bending as the instrument 22 moves through the channel. In some embodiments, the outer diameter of the guidewire 40 can be smaller than the inner diameter of the instrument 22 so that fluid can easily flow through the instrument 22. In some embodiments, the curvature of the U-shape can vary depending on, for example, the bend radius of the material comprising the instrument 22. In some embodiments, the curvature of the U-shape can be configured to prevent the instrument 22 from kinking or being damaged. In some embodiments, the channel 20 can include one or more protrusions 27, such as a lip or a detent, which can help to secure the instrument 22 within the channel 20.
[0076] Now refer to Figure 1D , as Figure 1C , where guide feature 18 only partially spans the lumen of barrel 14, in some embodiments, guide feature 18 may span the entire lumen of barrel 14. More specifically, in some embodiments, guide feature 18 may contact the entire interior surface of the barrel at a cross-section of the barrel, e.g., as shown in FIG. Figure 1D shown.
[0077] Now refer to Figures 1F-1H , in some embodiments, with e.g. Figures 1A-1B As opposed to the horizontal orientation shown, the passage 20 may be oriented vertically within the syringe 12, e.g. Figures 1F-1G In some embodiments, the guide feature 18 may span the entire lumen of the barrel 14 and contact the entire inner surface of the barrel at a cross-section of the barrel, for example, as shown in FIG. Figure 1H In other embodiments, guide feature 18 may only partially span the lumen of barrel 14 .

In some embodiments, due to the U-shape of the channel 20, the second distance can be at least twice the first distance (a "1:2 advancement ratio"). In some embodiments, the delivery device 10 and the 1:2 advancement ratio between the guide feature 18 and the first end 25 of the instrument 22 (or another advancement ratio in which the second distance is greater than the first distance) can provide reliability and structural support when the instrument 22 is advanced distally, while also providing an instrument 22 with a long reach.
[0079] In some embodiments, in response to depression of the plunger 16, the liquid 38 can exit the distal opening 46 of the syringe 12. In some embodiments, the diameter of the distal opening 46 of the syringe 12 can be larger than the outer diameter of the device 22. In some embodiments, in response to depression of the plunger 16, the liquid 38 can flow around the device 22 and into and / or through the catheter assembly 30.
[0080] In some embodiments, the apparatus 22 may include a guide wire 40 and / or a multi-diameter catheter, such as described in U.S. Patent Application No. 62 / 660,646, filed April 20, 2018, entitled "MULTI-DIAMETER CATHETER AND RELATED DEVICES AND METHODS," which is incorporated herein by reference. In some embodiments, the multi-diameter catheter can allow for increased blood flow rates during blood collection. In some embodiments, the distal end of the guide wire 40 can be disposed proximal to a transition portion of the multi-diameter catheter to allow fluid to flow in an annular space between the outer surface of the guide wire 40 and the inner surface of the multi-diameter catheter.
[0081] In some embodiments, in response to the plunger 16 being depressed in the distal direction within the barrel 14, the guidewire 40 can move through the channel 20 within the instrument 22, and the first end 42 of the guidewire 40 can be advanced in the distal direction. In some embodiments, in response to the plunger 16 being depressed, the guide feature 18 can move in the distal direction a first distance, and the first end 42 of the guidewire 40 can be advanced in the distal direction a second distance. In some embodiments, the second distance can be greater than the first distance. In some embodiments, the second distance can be at least twice the first distance.
[0082] Return Reference Figure 1E In some embodiments, the distal end 48 of the conduit 36 can include one or more flow holes 50 extending through the wall of the conduit 36. In some embodiments, the liquid 38 can flow out of the flow holes 50 in response to the plunger 16 being depressed.
[0083] Now refer to Figures 3A-3B In some embodiments, the delivery device 10 can include a support tube 54 that extends from the guide feature 18. In some embodiments, the support tube 54 can be fixed to the guide feature 18. In some embodiments, in response to depression of the plunger 16, the guide feature 18 can be moved distally a first distance, the instrument 22 can be moved through the support tube 54, and the first end 56 of the support tube 54 can be advanced distally a first distance. In some embodiments, the support tube 54 can include a uniform inner diameter and / or uniform outer diameter along all or a portion of the length of the support tube 54.
[0084] In some embodiments, when the delivery device 10 includes a support tube 54, the device 22 can include a catheter constructed of a material softer than polyimide, which is less traumatic to the vein wall. In these and other embodiments, the device 22 can include a tube constructed of one or more of polyurethane, silicone, a thermoplastic elastomer ("TPE"), a thermoplastic polyurethane ("TPU"), and another suitable compliant material.
[0085] In some embodiments, the support tube 54 can include an inner diameter that is larger than the outer diameter of the instrument 22 so that the instrument 22 can be easily moved through the support tube 54. In some embodiments, the support tube 54 can provide support for the first end 25 or free end of the instrument 22.
