Technical Field
[0003] The present disclosure relates to osmotic dosage forms and methods of using those dosage forms to treat hyperkinetic movement disorders arising from conditions such as Huntington's disease, tardive dyskinesia, Tourette's syndrome, levodopa-induced dyskinesias, and movement disorders in cerebral palsy. Background Art
[0004] Deutetrabenazine ((RR,SS)-1,3,4,6,7,11b-hexahydro-9,10-di(methoxy-d3)-3-(2-methylpropyl)-2H-benzo[a]quinolizin-2-one) is a vesicular monoamine transporter type 2 (VMAT2). The biologically active metabolites formed from deutetrabenazine (alpha-dihydrodeutetrabenazine [α-deuHTBZ](alpha-dihydrodeutetrabenazine) and beta-dihydrodeutetrabenazine [β-deuHTBZ](beta-dihydrodeutetrabenazine)) are collectively identified as "deuHTBZ", which are potent inhibitors of VMAT2 binding. Deuterated tetrabenazine exhibits an increased half-life of its active metabolite relative to tetrabenazine (eg, US Pat. No. 8,524,733).
[0005] Deutetrabenazine is sold under the trade name It is approved by the U.S. Food and Drug Administration for the treatment of chorea (involuntary muscle movements) associated with Huntington's disease (HD) and for the treatment of tardive dyskinesia (TD) in adults. The dosage form is administered orally twice daily (bid), with a total daily dose of deutetrabenazine of 12 mg or more.
[0006] One factor that affects gastrointestinal absorption of orally administered drugs is the rate at which the drug is released from the dosage form. The drug release rate of an oral dosage form is usually measured in the form of an in vitro dissolution rate, i.e., the amount of drug released from the dosage form per unit time in, for example, an FDA-approved system. Such systems include, for example, United States Pharmacopeia (USP) dissolution apparatus I and II.
[0007] The therapeutic window of a drug is the time period during which the plasma drug concentration is within the therapeutically effective plasma drug concentration range. However, since the plasma drug concentration decreases over time, multiple doses of the drug dosage form must be administered at appropriate intervals to ensure that the plasma drug concentration remains within the therapeutic window or rises to the therapeutic window again. However, at the same time, it is necessary to avoid or minimize plasma drug concentrations that cause undesirable side effects.
[0008] Several dosage forms containing deuterated tetrabenazine are disclosed in U.S. Pat. No. 9,296,739. A dosage form that can deliver deuterated tetrabenazine in a controlled manner over an extended period of time would allow for a more advantageous dosing regimen, for example allowing once daily (qd) administration while maintaining the current regimen of There is a need for alternative dosage forms that achieve the desired therapeutic effect. [0034] Osmotic dosage forms typically utilize osmotic pressure to generate a driving force for absorbing fluid, for example, from the gastrointestinal (GI) tract into a compartment formed at least in part by a semipermeable wall, layer, or membrane that allows free diffusion of fluid, but does not allow free diffusion of drug or one or more osmotic agents (if present). By designing the system to provide a relatively constant osmotic pressure and equipping it with an appropriate outlet device, a constant drug release rate can be achieved by releasing the drug formulation at a rate corresponding to the liquid intake rate caused by the relatively constant osmotic pressure. Without being limited to theory, an osmotic system can operate independently of pH, so that even if the dosage form passes through the gastrointestinal tract and encounters different microenvironments with significantly different pH values, the operation continues at a rate determined by the osmotic pressure over an extended period of time.
[0035] The example of a type of osmotic device comprises two component layers in a compartment (referred to herein as a core, and interchangeably used with a core) formed by a semipermeable wall. One component layer (referred to herein as an active layer) comprises a drug (i.e., deuterated tetrabenazine) mixed with one or more excipients, and a second component layer (referred to herein as a push layer) comprises one or more osmotic active agents optionally mixed with an excipient, but does not contain a drug. The core is further coated with a semipermeable wall, which allows aqueous fluids (i.e., from the gastrointestinal system) to enter the core. It is not desirable to be limited to theory, when fluid is absorbed into the dosage form, the active layer forms a pharmaceutical preparation, and one or more osmotic agents in the push layer swell and push the pharmaceutical preparation, thereby promoting the release of the pharmaceutical preparation at a substantially constant rate. See, for example, U.S. Patent Nos. 4,327,725, 4,612,008, 4,783,337, and 5,082,668.
[0036] Although constant release dosage forms have been shown to be effective for many different drug therapies, these dosage forms are not completely satisfactory in certain clinical situations. It has been observed that for some patients, despite maintaining a substantially constant drug release that is expected to provide sustained effectiveness, the therapeutic effectiveness of the drug still decreases below the therapeutic effectiveness threshold before the end of the desired treatment period.
[0037] It has surprisingly been found that oral dosage forms comprising deuterated tetrabenazine can be obtained that exhibit a desired release rate and thus a desired pharmacokinetic profile over an extended period of time. In some embodiments, when orally administered to a subject on a once daily (qd) basis, the osmotic dosage forms disclosed herein provide comparable release rates to those administered twice daily (bid). In certain embodiments, the osmotic dosage form provides an in vivo plasma profile of total deuHTBZ at steady state that includes an average AUC of about 410,000 to 800,000 h*pg / mL. 0-24 and an average C of less than about 40,000 pg / mL max .
[0038] The osmotic dosage form of the present disclosure includes a tablet core containing at least a push layer and an active layer, wherein the active layer includes deuterated tetrabenazine and one or more excipients for forming a drug formulation when hydrated, and wherein the push layer includes at least one osmotic agent and one or more excipients. The push layer and the active layer are both contained in the tablet core, and the tablet core is at least partially surrounded by a semipermeable layer, and the semipermeable layer has a port used as an outlet device for the drug formulation to be released from the tablet core. In some embodiments, the two layers are compressed into a double-layer tablet core, and the tablet core is surrounded by a semipermeable membrane and also has a suitable orifice for drug release therethrough.
[0039] One embodiment of the oral osmotic dosage form disclosed herein is Figure 1 . The components are not drawn to scale. The dosage form (2) comprises a bilayer tablet core. The core comprises an active layer (4) and a push layer (6), wherein the active layer (4) contains a drug such as deuterated tetrabenazine and one or more active layer excipients, and the push layer (6) contains at least one permeabilizing agent and one or more push layer excipients. At least a portion of the active layer forms a drug formulation when exposed to an aqueous environment. Suitable active layer and push layer excipients are known in the art and include diluents, carriers, adhesives, fillers, controlled release agents, and processing aids. A semipermeable membrane (8) surrounds the bilayer tablet core, and there is a port (10) of appropriate size extending from the semipermeable membrane into the active layer (4) to allow the drug formulation to be released from the tablet core. As shown, the dosage form can be compressed longitudinally, and the port (10) is present on one side of the dosage form containing the active layer. In other embodiments, the dosage form is compressed along the transverse axis of the dosage form, and the port is present on one end of the dosage form. In all embodiments, there can be more than one port. Through the cooperation of the osmotic dosage form components and in the presence of an aqueous environment, the drug formulation is released from the active layer through the port at a desired release rate for an extended period of time. [0301] Example 1-24 mg deuterated tetrabenazine osmotic tablet manufacturing process
[0302] Figure 2a and Figure 2b A flow chart of a general manufacturing process of an osmotic dosage form according to the present disclosure is provided. Tables 1-13 below provide non-limiting examples of materials and their relative amounts used to prepare the dosage forms described herein. The preparation method is as follows:
[0303] A: Active layer material processing: Deuterated tetrabenazine (micronized) and active layer controlled release agent are passed through a #30 sieve and combined with a binder (previously passed through a #20 sieve). The mixture is introduced into a high shear granulator and dry mixed for about 5 minutes. While mixing, an antioxidant (previously dissolved in alcohol) is added to the mixed powder to granulate the material. Continue to mix additionally until the desired granulation endpoint is reached. The resulting granules are wet screened to break up any oversized agglomerates. The material is fed into a diffusion mixer (V-type mixer) where it is blended for about 15 minutes. The lubricant that has passed through a #30 sieve is added to the blended material in the V-type mixer. The contents are lubricated for about 5 minutes.
[0304] B: Tablet core compression: The active layer materials are discharged into a double-layer rotary tablet press. The push layer materials (osmotic agent, push layer controlled release agent and optional binder, colorant and lubricant) are combined and further fed into a double-layer rotary tablet press. The tablet core is compressed.
[0305] C: Optional Core Seal Coat: A core seal coat comprising a binder solution is applied to the core tablets.
[0306] D: Semipermeable layer: A semipermeable layer comprising a cellulose acetate solution and optionally a pore former is applied to the tablet core or sealed tablet core using a pan coater.
[0307] E: Optional Semipermeable Seal Coat: A semipermeable seal coat comprising a binder solution is applied to the tablet, thereby compromising the semipermeable wall.
[0308] F: Creation of exit means: Laser drilling through the layer to obtain holes into the active layer.
[0309] A final immediate release coat comprising deuterated tetrabenazine is optionally applied using similar materials and following the processing steps as detailed above for the active layer.

[0344] Example 2 - Single-dose bioavailability evaluation
[0345] As disclosed in Example 1, an osmotic dosage form containing 24 mg of deuterated tetrabenazine was prepared and studied in a single-dose pharmacokinetic study.
[0346] The primary objective was to evaluate the efficacy of a single 12 mg tablet given twice 12 hours apart (bid) under fasting conditions. Comparative bioavailability (BA) of deuterated tetrabenazine and deuterated α- and β-dihydrotetrabenazine (deuHTBZ) metabolites following a single administration of a 24 mg once daily (qd) osmotic formulation (Test) compared to a tablet.
[0347] Study Population and Number of Subjects: The study included healthy male and female non-smoking subjects aged between 18 and 45 years. A total of 8 healthy subjects (4 in each series) were enrolled in this study.
[0348] Duration of Subject Participation: The study consisted of a 4-week screening period (Period 1), an open-label treatment period with the test formulation (Test2A) and the reference formulation (R) (Period 2), and a follow-up visit after at least 1 day (Period 3).
[0349] treat:
[0350] Treatment sequence A:
[0351] Day 1 - Administer Test2A.
[0352] Day 2-3 - Wash out Test2A for at least 6 hours, then apply R.
[0353] Treatment sequence B:
[0354] Day 1 - Administer R
[0355] Day 2-3 - Wash out R for at least 6 hours, then apply Test2A.
[0356] The main purpose was solved using the following parameters:
[0357] - Maximum observed concentration (Cmax)
[0358] - Area under plasma concentration-time (AUC) from time 0 to the time of the last measurable plasma concentration (AUC0-t)
[0359] -AUC extrapolated to infinity (AUC0-∞)
[0360] -AUC from time 0 to 24 hours after dosing (AUC0-24h)
[0361] analyze
[0362] AUC0-t, AUC0-∞ and AUC0-24h were calculated using the trapezoidal rule. Before statistical analysis, Cmax, AUC0-t, AUC0-∞ and AUC0-24h data were natural logarithm transformed. A separate parameter analysis of variance (ANOVA) model was used to compare Cmax, AUC0-t, AUC0-∞ and AUC0-24h between treatments (T2A and R), with fixed effect terms for sequence, period, treatment group and random effects for subjects within the sequence. Based on the least squares mean of the ANOVA of logarithmic transformed Cmax, AUC0-t, AUC0-∞ and AUC0-24h, the difference between the reference preparation (R) and the test preparation (Test2A) was evaluated by constructing a 90% confidence interval for the test / reference ratio. Treatment differences and associated 90% confidence intervals estimated from log-scale ANOVA were back-transformed to obtain the estimated ratio of geometric means between treatment groups and the 90% confidence interval for that ratio.
[0363] Figure 3a and Figure 3b Results (mean concentration of deuterated tetrabenazine versus time) for the R treatment compared to the Test2A treatment are shown on a direct scale and a logarithmic scale, respectively. Table 14 below provides the assigned pK parameters for deuterated tetrabenazine observed for Test2A compared to R.

[0370]
[0371] As shown in Tables 14 and 15, a once daily dose of Test2A provided acceptable deuHTBZ plasma concentrations observed for the reference. The osmotic dosage forms disclosed herein are administered once daily and provide Compared with the acceptable treatment effect, there are no safety issues.
[0372] The results of this study with a crossover design further demonstrate that patients can safely and effectively switch from twice daily (two divided doses) administration of deuterated tetrabenazine tablets to a once daily osmotic dosage form. The results of this study with a crossover design also demonstrate that patients can safely and effectively switch from a once daily osmotic dosage form to twice daily administration of deuterated tetrabenazine tablets (two divided doses).
[0373] Example 3 - Multiple-dose bioavailability evaluation
[0374] As disclosed in Example 1, an osmotic dosage form containing 24 mg of deuterated tetrabenazine was prepared and studied in healthy volunteers in an open-label, randomized, multiple-dose, two-way crossover study.
[0375] The primary objective was to evaluate the bioequivalence (BE) of Test2A administered once daily (qd) compared with R administered bid under fasting or fed conditions.
[0376] Treatment consisted of 7 days of repeated dosing of Test2A once daily and 7 days of repeated dosing of R bid.

[0380]
[0381] Study Results
[0382] Administration of treatments Test2A and R resulted in reproducible concentration-time profiles for all analytes.
[0383] The primary objective was achieved, namely to demonstrate the AUC of deutetrabenazine and deuHTBZ (alone and as a sum) between Test2A and R formulations 0-24h,ss Bioequivalence (BE) of deuterated tetrabenazine AUC 0-24h,ss The geometric LS mean ratio was 115.15% and the total deuHTBZ was 95.05%.
[0384] The results of this study with a crossover design further demonstrate that patients can safely and effectively switch from twice-daily administration of deuterated tetrabenazine tablets to a once-daily osmotic dosage form. The results of this study with a crossover design further demonstrate that patients can safely and effectively switch from a once-daily osmotic dosage form to a twice-daily administration of deuterated tetrabenazine tablets.
[0385] Example 4: Food Effect Study
[0386] As disclosed in Example 1, osmotic dosage forms containing 24 mg of deutetrabenazine were prepared and studied in an open-label, randomized, 3-period, 3-treatment, 6-sequence, crossover study to evaluate the comparative bioavailability of deutetrabenazine and deuHTBZ in the fed state compared to the fasted state following a single administration of 24 mg once daily (qd) of the osmotic formulation.
[0387] Treatment includes:
[0388] Subjects were randomized to receive 3 treatments: Test2A in the fed state (QD fed), Test2A in the fasted state (QD fasted), and R in the fed state [twice daily (BID) fed] separated by 12 hours in 1 of 6 treatment sequences, e.g. Figure 5 shown.
[0389] There was a washout period of at least 6 days between the first administration in one phase and the first administration in the next phase. In the morning of the first day (Day 1) of each administration period, subjects were orally administered treatment in a sitting position. The second dose of R in the BID fed group was administered exactly 12 hours after the morning administration.
[0390] All subjects fasted overnight for at least 10 hours before the first dose in each period. Subjects receiving either the R BID meal or the Test2A tablet meal were provided with a standardized high-calorie, high-fat breakfast (containing 800 to 1000 kilocalories [kcal] and 50% fat) 30 minutes before the first dose. A standardized dinner was provided 30 minutes before the second dose for the R BID meal or at the corresponding time for the Test2A tablet meal.
[0391] During all treatment periods, blood samples were collected before and up to 96 hours after dosing for pharmacokinetic analysis.
[0392] The AUCt, Cmax, tmax, Cmin, and Cav of deutetrabenazine and deuHTBZ were analyzed.
[0393] result
[0394] For all analytes, the variability of the Test2A fed treatment was generally similar to the variability of the Test2A fasted treatment, indicating that administration of a single 24 mg tablet in the fed state does not significantly increase the pharmacokinetic variability of the metabolites compared to a single 24 mg tablet in the fasted state.
[0395] Following oral administration of a single 24 mg QD osmotic tablet, all analytes met BE criteria under both fed and fasted conditions. Food apparently had no significant effect on PK, and thus the 24 mg QD tablet can be administered with or without food.
[0396] In healthy subjects, a single dose of 24 mg deutetrabenazine QD osmotic tablets in the fed or fasted state and twice daily administration of 12 mg deutetrabenazine tablets (fed state) appeared to be safe and well tolerated.
