Description

TECHNICAL FIELD

[0001] The present application relates to the technical field of medical devices, in particular to a deep vein thrombectomy stent device. BACKGROUND

[0002] Deep vein thrombosis (DVT) is a common vascular disease, and its most dangerous complication, pulmonary embolism (PE), has a potentially fatal risk. Traditional DVT treatment focuses on anticoagulant therapy, aiming to prevent thrombus spread and recurrence, but for patients with a large thrombus load, simple anticoagulation often cannot effectively eliminate intravascular obstruction, resulting in up to 20%-50% of patients developing post-thrombotic syndrome (PTS), which manifests as chronic limb swelling, pain, skin ulcers and other symptoms that severely affect quality of life. In recent years, with the continuous development of interventional technology and equipment, the DVT treatment strategy has shifted from simple anticoagulation to a comprehensive intervention model of active thrombus removal. In particular for acute DVT, through various thrombus removal techniques to restore venous return as soon as possible, it can effectively reduce venous hypertension and protect valve function, thereby significantly reducing the incidence of PTS.

[0003] However, the traditional thrombectomy device has the following defects: 1. Difficulty in cleaning thrombus in the stent after thrombectomy: the thrombectomy stent is covered with thrombus after single thrombectomy, which requires a long time to clean the captured thrombus, and the thrombectomy stent needs to be inserted into the body multiple times for thrombectomy, greatly increasing the operation time; 2. The length of the thrombectomy stent is too long: the length is too long, which causes the thrombectomy stent to invade the inferior vena cava during use, and if the patient has a filter in the inferior vena cava, it will interfere; 3. Diameter is uncontrollable: the thrombectomy stent can only be expanded to the maximum diameter during use, and then operated. SUMMARY

[0004] Therefore, it is necessary to provide a deep vein thrombectomy stent device, which has the advantages of convenient cleaning, controllable length and diameter.

[0042] This application provides a deep vein thrombectomy stent device, which can be used to treat thrombi in venous vessels.

[0043] It is important to understand that the terms "proximal," "posterior," "distal," and "anterior" used in this article are relative to the clinician (operator) manipulating the deep vein thromboplasty stent device. "Proximal" and "posterior" refer to the portion closer to the clinician, while "distal" and "anterior" refer to the portion farther from the clinician. The term "axial" in this article can be understood as the direction of extension of the deep vein thromboplasty stent device from distal to proximal.

[0044] like Figures 1 to 15 As shown, one embodiment of this application of a deep vein thrombectomy stent device 100 includes a handle assembly 1, an inner tube assembly 2, an intermediate tube assembly 3, an outer tube assembly 4, and a thrombectomy stent assembly 5.

[0045] Handle assembly 1 is used to support inner tube assembly 2. Handle assembly 1 is available for the operator to hold.

[0046] refer toFigure 2 , Figure 16 , the intermediate tube assembly 3 comprises an intermediate tube 31, a stent fixing assembly 32, the proximal end of the intermediate tube 31 is connected to the handle assembly 1, and the distal end is connected to the stent fixing assembly 32. The outer tube assembly 4 comprises an outer tube 41. The outer tube 41 is axially movably sleeved on the intermediate tube 31 to switch between the distal end position and the proximal end position. The stent retrieval assembly 5 comprises a self-expanding stent 51, a woven mesh 52. The proximal end of the self-expanding stent 51 is detachably matched with the stent fixing assembly 32, the proximal end of the woven mesh 52 is connected to the distal end of the self-expanding stent 51, and the distal end of the woven mesh 52 is connected to the conical head assembly 22. When the outer tube 41 is in the distal end position (see Figure 1 ), the outer tube 41 is sleeved on the self-expanding stent 51 to keep the self-expanding stent 51 in a contracted state, and when the outer tube 41 is in the proximal end position (see Figure 2 ), the self-expanding stent 51 is in an expanded state.

[0047] Optionally, as shown in Figure 1 , Figure 2 , Figure 4 and Figure 5 , the handle assembly 1 comprises a handle upper cover 11, a handle lower cover 12, a handle front cover 13 and a handle rear cover 14. The handle upper cover 11 and the handle lower cover 12 are connected by buckling. First axial grooves 121 and first circumferential grooves 122 are formed at the distal end of the handle upper cover 11 and the handle lower cover 12, and the first axial grooves 121 and the first circumferential grooves 122 are communicated. The distal end of the handle lower cover 12 also forms first axial grooves 121 and first circumferential grooves 122, and the first axial grooves 121 and the first circumferential grooves 122 are communicated. The two first axial grooves 121 are arranged in central symmetry. The two first circumferential grooves 122 are arranged in central symmetry.

[0048] The handle front cover 13 is provided with a pair of axial bosses 131 matched with the first axial grooves 121 and arranged in central symmetry. Taking the cooperation of the handle front cover 13 and the handle upper cover 11 as an example: the axial boss 131 of the handle front cover 13 is first slid along the first axial groove 121 of the handle upper cover 11 to the bottom, and then rotated along the first circumferential groove 122 to the bottom of the first circumferential groove 122, and then the handle upper cover 11 is fixed. The first circumferential boss 124 of the handle upper and lower covers matches the handle front cover 13, and then the handle front cover 13 can also fasten the handle upper and lower covers to prevent loosening; the handle lower cover 12 is fixed in the same way as the handle upper cover 11.

[0049] Referring to Figure 3 , Figures 6 to 8The inner tube assembly 2 comprises an inner tube 21, a conical head assembly 22, a guide block 23, a support tube 24, a limiting key 25, and a first spring 26. The inner tube 21 is axially slidable but not rotatable relative to the handle assembly 1. The conical head assembly 22 is arranged at the distal end of the inner tube 21. The limiting key 25 is movably arranged on the handle assembly 1 and selectively acts on the inner tube 21 to limit or release the axial movement of the inner tube 21.

[0050] The inner tube 21 is arranged in the handle assembly 1. Specifically, the inner tube 21 passes through the handle upper cover 11 and the handle lower cover 12. Optionally, the limiting key 25 is rotatably connected to the handle upper cover 11. When the handle upper cover 11 and the handle lower cover 12 are buckled, the limiting key 25 can only rotate. Normally, the limiting key 25 is in a position capable of limiting the axial movement of the inner tube 21. By pressing the limiting key 25 downward, the limiting key 25 can be rotated to a position releasing the inner tube 21. At this time, the inner tube 21 can be axially moved proximally, so that the inner tube 21 drives the conical head assembly 22 to move proximally.

[0051] Optionally, the support tube 24 is arranged in the handle assembly 1 and sleeved on the inner tube 21. The distal end of the support tube 24 is connected to the guide block 23. The surface of the support tube 24 is provided with a plurality of limiting teeth 241 arranged in the axial direction, which are used to cooperate with the limiting key 25. The guide block 23 is axially slidably connected to the handle assembly 1. The two ends of the first spring 26 in the axial direction abut against the handle assembly 1 and the guide block 23, respectively.

[0052] The inner tube assembly 2 further comprises a support tube connecting piece 27 and an inner tube connecting piece 28. The proximal end of the support tube 24 is connected to the support tube connecting piece 27, and the inner tube connecting piece 28 is connected to the proximal end of the inner tube 21. The distal end of the support tube connecting piece 27 circumferentially abuts against the inner tube connecting piece 28. The surface of the support tube 24 is provided with a mark 242 arranged in the axial direction, which is used to prompt the diameter change of the braided mesh 52.

[0053] Specifically, the distal end of the inner tube connecting piece 28 abuts against the distal end of the support tube connecting piece 27 to prevent the circumferential sliding of the inner tube connecting piece 28. Referring to Figure 9 and Figure 10 The circumferential direction of the inner tube connecting piece 28 is uniformly provided with a positioning boss 281 matched with a connecting groove 271 in the inner cavity of the support tube connecting piece 27 to prevent the rotation of the inner tube connecting piece 28. The proximal end of the guide block 23 is provided with a proximal end axial protrusion 231 inserted into the inner cavity of the support tube 24 to be fixed, so that the guide block 23 can move synchronously with the inner tube 21 and the support tube 24. In addition, the guide block 23 is provided with a pair of second circumferential bosses 232 matched with the guide grooves 123 arranged on the handle upper cover 11 and the handle lower cover 12. In this way, when the handle upper and lower covers are buckled, the circumferential movement of the guide block 23 is limited and can only move axially along the guide groove.

[0054] Reference Figure 3 , Figure 8 The support tube 24 is sleeved on the inner tube 21, the proximal end of the support tube 24 is connected with the support tube connector 27, and the distal end is connected with the guide block 23. The first spring 26 is sleeved on the support tube 24, the proximal end of the first spring 26 is abutted against the first circular convex platform 124 of the upper handle cover and the lower handle cover 12, and the distal end is abutted against the second circular convex platform 232 of the guide block 23. The surface of the support tube 24 is provided with axial limit teeth 241 in the axial direction, and the limit key 25 is arranged on the convex circular platform of the handle upper cover 11. The limit key 25 includes a first side arm 251 and a second side arm 252. When the upper and lower covers are buckled, the limit key 25 can only rotate. When the limit key 25 is pressed downward, the first side arm 251 is elastically deformed under the action of force, the limit key 25 rotates clockwise, and after the first side arm 251 restores, the limit key 25 rotates counterclockwise, the second side arm 252 is in contact with the support tube 24, and the limit teeth 241 of the support tube 24 are matched to limit the axial movement of the support tube 24. Since the inner tube 21 and the support tube 24 move synchronously, the axial movement of the inner tube 21 is also limited.

[0055] Optionally, referring to Figure 11 The conical head assembly 22 includes a fixed cap 221 and a conical head 222, wherein the conical head 222 includes an inner layer 2221 and an outer layer 2222 forming a gap. The inner layer 2221 is fixed with the inner tube 21, and the outer layer 2222 is threadedly connected with the fixed cap 221. The distal end of the woven mesh 52 is accommodated in the gap. The fixed cap 221 is threadedly connected with the outer layer 2222, and the fixed cap 221 applies a radial force to make the outer layer 2222 and the inner layer 2221 clamp the woven mesh 52.

[0056] In a specific arrangement, the inner layer 2221 and the outer layer 2222 are connected to the proximal end of a conical body and have a certain elasticity. The proximal end of the fixed cap 221 is provided with an internal thread part 2212, and the distal end of the fixed cap 221 is provided with a conical part 2211. The internal thread part 2212 is threadedly connected with the outer layer 2222. The conical part 2211 can be sleeved on the conical body, and the two have consistent taper. When the proximal end of the fixed cap 221 is rotated from the distal end of the conical head 222 to the proximal end of the conical head 222, a smooth transition can be formed, and the distal end of the woven mesh 52 can be fixed. After the fixed cap 221 is screwed on the outer layer 2222, the fixed cap 221 can extrude the outer layer 2222, and then clamp the woven mesh 52. In this way, when the inner tube 21 moves towards the proximal end, the inner tube 21 drives the conical head 222 to move, and then extrude the woven mesh 52.

[0057] Further, in order to facilitate the removal of the fixed cap 221 from the conical head 222, the surface of the fixed cap 221 is provided with an axial groove 2213 in the axial direction. Referring to Figure 7 , Figure 12The deep vein thrombus removing stent device 100 further comprises a conical head fastener 6 which can be sleeved on the fixing cap 221, and the conical head fastener 6 is provided with a guide boss 61 which cooperates with the axial groove 2213. After the conical head fastener 6 is sleeved on the fixing cap 221, the fixing cap 221 can be mounted on and removed from the conical head 222 through the cooperation of the guide boss 61 and the fixing cap 221.

[0058] In some embodiments, the proximal end of the self-expanding stent 51 is detachably connected with the stent fixing assembly 32. In this way, the thrombus removing stent assembly 5 can be detached as needed, and then a new thrombus removing stent assembly 5 can be replaced, so that the thrombus can be quickly removed, and then secondary thrombus removal or multiple thrombus removal can be performed, thereby greatly reducing the operation time and facilitating the operation of the doctor.

[0059] Optionally, referring to Figures 13 to 21 The stent fixing assembly 32 comprises a support block 321, a second spring 322, a limiting cylinder 323 and a stent fixing block 324. The support block 321 is fixed at the distal end of the middle tube 31. The second spring 322 is sleeved on the middle tube 31, the proximal end of the second spring 322 abuts against the distal end of the support block 321, and the distal end of the second spring 322 abuts against the limiting cylinder 323. The stent fixing block 324 is detachably connected with the proximal end of the self-expanding stent 51. The limiting cylinder 323 and the stent fixing block 324 axially slide to lock the self-expanding stent 51.

[0060] When it is needed to detach the thrombus removing stent assembly 5, the limiting cylinder 323 is moved towards the proximal end against the resistance of the second spring 322, and then the self-expanding stent 51 is removed from the stent fixing block 324.

[0061] For example, the stent fixing block 324 is sleeved on the middle tube 31 and fixed at the distal end of the middle tube 31, and the distal end of the stent fixing block 324 is flush with the distal end of the middle tube 31. The surface of the stent fixing block 324 is provided with an L-shaped groove 3241. The limiting cylinder 323 is provided with a first inner cavity 3231 and a second inner cavity 3232. The first inner cavity 3231 is sleeved on the middle tube 31, and the distal end of the first inner cavity 3231 abuts against the proximal end of the stent fixing block 324. The second inner cavity 3232 is sleeved on the stent fixing block 324, and the second inner cavity 3232 is provided with a first L-shaped boss 3233 which axially slides with the L-shaped groove 3241. The self-expanding stent 51 is provided with a second L-shaped boss 511 which axially slides with the L-shaped groove 3241. When the self-expanding stent 51 is assembled on the stent fixing block 324, the first L-shaped boss 3233 and the second L-shaped boss 511 jointly fill the L-shaped groove 3241, and the first L-shaped boss 3233 limits the circumferential rotation of the second L-shaped boss 511 to avoid the axial separation of the second L-shaped boss 511 from the L-shaped groove 3241.

[0062] Specifically, the L-shaped groove 3241 includes two parts, a second axial groove 3241a and a second circumferential groove 3241b, and the groove width of the second axial groove 3241a in the circumferential direction is W1. The first L-shaped boss 3233 includes a first axial part 3233a and a first circumferential part 3233b perpendicular to each other. The second L-shaped boss 511 includes a second axial part 5111 and a first circumferential part 3233b perpendicular to each other. The width of the first axial part 3233a in the circumferential direction is W2, and the width of the first axial part 3233a in the circumferential direction is W3. The relationship of W1, W2, W3 satisfies W1≥W2+W3.

[0063] Reference Figures 18 to 21 As shown, when the self-expanding stent 51 is assembled with the stent fixing block 324, the second circumferential part 5112 is located in the second circumferential groove 3241b, and the second axial part 5111 is located in the second axial groove 3241a; at the same time, the first circumferential part 3233b is abutted against the proximal end of the stent fixing block 324 under the elastic force of the second spring 322, and the first axial part 3233a is located in the second axial groove 3241a and on one side of the second axial part 5111. In this way, the first axial part 3233a can limit the rotation of the second axial part 5111, and in turn limit the second circumferential part 5112 from being pulled out of the second circumferential groove 3241b.

[0064] When the proximal end of the self-expanding stent 51 needs to be disassembled, the limiting cylinder 323 only needs to be pushed in the axial direction, and the L-shaped protrusion inside the limiting cylinder 323 is drawn out of the groove of the stent fixing block 324, and then the self-expanding stent 51 is removed. The proximal end of the braided mesh 52 is connected with the self-expanding stent 51, and the distal end is connected with the conical head 222. Because the thrombus can be quickly removed, the braided mesh 52 can be used multiple times, the length of the braided mesh 52 can be reduced to 1 / 2 of the existing products on the market, and the treatment effect can be guaranteed.

[0065] In the present application, the above-mentioned L-shaped groove 3241 is provided with two places in the circumferential direction of the stent fixing block 324 and is centrally symmetric. Correspondingly, the first L-shaped boss 3233 and the second L-shaped boss 511 are also provided with two places.

[0066] In some embodiments, reference Figure 13 The intermediate tube assembly 3 further includes an outer tube fixing member 33. The outer tube fixing member 33 is fixed to the handle assembly 1, and the outer tube assembly 4 further includes an outer tube connecting member 42, which is axially slidably sleeved on the intermediate tube 31, the distal end of the outer tube connecting member 42 is connected with the proximal end of the outer tube 41, and the proximal end of the outer tube connecting member 42 can be connected with the outer tube fixing member 33 and the handle assembly 1 through, for example, a Luer connection or the like.

[0067] The outer tube fixing member 33 is provided with a triangular boss 331 in the circumferential direction, the distal end of which abuts against the circular table part 1331 at the proximal end of the distal second boss 133 of the handle front cover 13, thereby limiting the movement of the outer tube fixing member 33 in the axial direction towards the distal end of the thrombus-removing stent device, and the proximal end of the outer tube fixing member 33 abuts against the first boss 132 of the handle front cover 13, thereby completely limiting the axial movement of the outer tube fixing member 33. In addition, the outer tube fixing member 33 is provided with a threaded groove 332 in the axial direction, which can be threadedly connected with the outer tube 41.

[0068] In addition, with reference to Figure 2 , the intermediate tube assembly 3 further comprises a first three-way valve 34 and a first infusion tube 35. The first infusion tube 35 is connected with the handle front cover 13, which is a hollow structure, and the first three-way valve 34 is connected with the first infusion tube 35. By using the first three-way valve 34 and the first infusion tube 35, physiological saline can be injected into the gap between the inner tube 21 and the intermediate tube 31, thereby preventing thrombus from coagulating.

[0069] With reference to Figure 2 , the outer tube assembly 4 further comprises a second three-way valve 43 and a second infusion tube 44. The proximal end of the second infusion tube 44 is connected with the side hole on the outer tube connecting member 42, and the distal end is connected with the second three-way valve 43. By using the second three-way valve 43 and the second infusion tube 44, heparin physiological saline can be injected into the gap between the outer tube 41 and the intermediate tube 31, thereby preventing thrombus from coagulating in the gap.

[0070] The working principle of the deep vein thrombus-removing stent device 100 of the present application is as follows.

[0071] As shown in Figure 1 , at this time, the outer tube assembly 4 completely covers the thrombus-removing stent assembly 5, and the self-expanding stent 51 and the braided mesh 52 are in a compressed state. When using the device, the deep vein thrombus-removing stent device 100 is pushed to the target thrombus area, and then the outer tube assembly 4 is withdrawn, the outer tube connecting member 42 is matched with the outer tube fixing member 33 in the intermediate tube assembly 3, thereby locking the outer tube assembly 4. At this time, the self-expanding stent 51 expands, and the braided mesh 52 continues to maintain a contracted shape, as shown in Figure 2 .

[0072] At this time, the support tube connecting member 27 is pulled backward, driving the inner tube assembly 2 to move backward as a whole, the tapered head 222 compresses the braided mesh 52, making it axially shorten and radially expand, thereby facilitating the accommodation of thrombus, and the mark 242 on the support tube 24 facilitates the accurate control of the radial diameter of the braided mesh 52. Then, the thrombus-removing stent device is withdrawn as a whole, and the thrombus can be taken out of the body.

[0073] Because the thrombus is difficult to remove in the braided mesh 52, the conical head fastener 6 can be selected to remove the fastening cap, so that the conical head 222 is in a non-restricted state, the restriction on the distal end of the braided mesh 52 is released, and the braided mesh 52 can be everted to clean the stent. Alternatively, the limiting cylinder 323 is pushed proximally, the spring is compressed, and until the limiting cylinder 323 loses the restriction on the L-shaped boss of the thrombus removing stent proximal end, at which time the thrombus removing stent assembly 5 can be completely removed, and a new thrombus removing stent assembly 5 is installed. Both methods can quickly remove the thrombus, and then perform secondary thrombectomy or multiple thrombectomy, greatly reducing the operation time and facilitating the operation of the doctor.

[0074] The deep vein thrombectomy stent device 100 of the present application utilizes the movement of the inner tube assembly 2 to control the diameter change of the braided mesh 52, realizes the diameter controllability of the radial thrombectomy stent, can realize the minimum vascular wall damage, and can conveniently remove the thrombus multiple times. The length of the thrombectomy stent can be shortened.
