Abstract
Description
Description Title of the invention: Device for preparing harvested fat by liposuction for autologous fat transfer. 1. Scope of the invention
[0001] — The field of the invention is that of plastic surgery, more particularly the cosmetic or reconstructive surgery.
[0002] = More specifically, the invention relates to the general principle of "lipofilling", or lipo- structure, cosmetic or reconstructive surgery operation, which consists of a injection of fat taken from an area of the patient's body where it is in excess for to reinject it into another part of the patient's body that is lacking it.
[0003] — In particular, the invention relates to an emulsification and filtration device for fat harvested from a patient for autologous transfer. 2. Prior art and its disadvantages
[0004] — Lipostructure is a cosmetic or reconstructive surgery technique that consists of to take small amounts of fat from a patient's body for reinjection in the latter for the purpose of filling or remodeling. The lipo- procedure This structure is also called autologous fat transfer or tissue autograft Eraisseux.
[0005] — The lipostructure technique involves two main steps carried out in series. In the first step, known as fat harvesting, the fat is removed and retrieved from one or more fatty sites on a patient's body, such as the thighs or the stomach, using a number of known techniques, including li- liposuction or lipoplasty.
[0006] — The extracted fat is a complex mixture comprising several components. In particular, the fat harvested is composed of mature adipocytes, pre- adipocytes cursors, other precursor cells, and lipids from mature adipocytes broken. Adipocytes are the primary cells of adipose tissue, which is the tissue loose connective tissue of the body where most of the fat is located. As Connective tissue, adipose tissue also includes fibers, fragments of fibers and other non-fatty materials in addition to adipocytes. Lipids are molecules, including fat molecules, which can be characterized by in a simplistic way such as free fat or fat particles.
[0007] The harvested fat can be classified by size into macro-fat, micro-fat or nano-fat in descending order of particle size.
[0008] — In the second step, known as fat reinjection, the fat the extracted tissue is reinjected into the same patient's body in small quantities at the level of one or more target areas at two or three levels of depth (periosteum, muscles and hypodermis). The reinjected fat increases the volume at the treated reinjection site and improves the patient's appearance. Autologous fat grafting offers distinct advantages: no allergic reactions, correction of significant volumes, and improvement of the trophicity of the overlying skin. Nanofat, compared to larger fats, has proven more suitable for fat grafting because it produces significantly better results in the patient's final appearance. Indeed, nanofat, or "nanofat," is emulsified and filtered adipose tissue with a high concentration of stem cells. To obtain purer "nanofat" with a high stem cell concentration from harvested fat, the harvested fat must be broken down and sized before being reinjected into the patient's body. There are currently several different techniques for emulsifying and filtering fat taken by liposuction to obtain a "nanofat" composed mainly of stem cells derived from adipose tissue or ADSCs ("Adipose Derived Stem Cells" in English). These techniques are generally based on emulsification followed by filtration using a system comprising either a single emulsification / filtration device, or several different devices, used successively to emulsify and then filter the grease. Generally, the emulsification step is performed by successive back-and-forth movements (for example, 30 back-and-forths) of the harvested fat between two syringes connected to each other by a fat preparation device. More specifically, once the fat is harvested by liposuction, it is pre-filtered to obtain micro-fat. This micro-fat is then placed in a first syringe, which is then connected to a second syringe via the fat preparation device. The practitioner then applies force to a piston in the first syringe to force the fat through the fat preparation device into the second syringe. This action is then repeated to force the fat back through the second syringe into the first, and so on, thus achieving the desired emulsification. The filtration of emulsified fat can be achieved through successive filtrations using different devices comprising filter elements of varying sizes to obtain different fat particles. Successive passages of the fat through these different sized filter elements allow for the production of increasingly smaller fat particles, ultimately resulting in nano-fat. One drawback of using different devices successively to obtain nano-grease is that it is necessary to transfer the grease from one device to another. the other, which increases the risk of contamination of the latter. There is also a device for preparing fat harvested by liposuction, designed to perform emulsification and filtration on its own. This device, marketed under the name HY-TISSUE NANOFAT®, consists of a reservoir, or flexible plastic bag, containing a filter, and a T-shaped element. More specifically, the two syringes used to emulsify the harvested fat are connected to the device at two branches of the T-shaped element. In particular, they can be positioned either at 180° to each other (at the two opposite upper branches of the T), or at 90° (one at an upper branch of the T and the other at the lower branch or base of the T). The reservoir is then connected to the device at the third branch of the T-element. One drawback of the HY-TISSUE NANOFAT® device is that its reservoir is not inherently rigid (it's a flexible plastic bag). As a result, gravity causes it to assume a vertical position, preventing it from remaining aligned with the arm of the T-shaped element to which it is connected. It can even "bounce" under the pressure of the practitioner's movements on the T-shaped element during emulsification. This change in angle and the resulting movement relative to the T-shaped element can lead to the reservoir detaching from the element, as well as causing obstruction and discomfort during the practitioner's repeated emulsification movements. In addition, the 180° angle between the two syringes for fat emulsification requires an awkward hand and wrist positioning for the practitioner during the emulsification step, making the operation uncomfortable and quickly tiring for the practitioner. There is therefore a need for a simpler and more ergonomic technique for the practitioner to emulsify and filter fat harvested by liposuction before reinjection. 3. Presentation of the invention The invention addresses this need by providing a device for preparing fat harvested from one area of a patient's body for reinjection into a second area of the patient's body. This preparation device comprises: - a frame comprising a first and a second branch to which a first syringe and a second syringe are respectively connected, - a connecting element for the first and second branches, this connecting element fitting into the frame. The connecting element is attached to a third syringe and is designed to take at least two positions: - an emulsification position in which the connecting element forms, with the first and second branches of the armature, a first circuit for circulating grease between the first syringe and the second syringe, - a transfer position in which the connecting element forms with the first or second branches of the armature a second circuit for circulating the grease between the first or second syringe and a third syringe, and the connecting element includes a grease filtration membrane. Thus, the invention proposes a completely new and inventive approach to preparing fat harvested from a patient before its reinjection. It relies on a single, all-in-one device whose original arrangement allows for the successive emulsification and then filtration of the harvested fat. Indeed, the preparation device according to the invention allows up to three syringes to be directly connected: two syringes for emulsification, and one for filtration and recovery. In other words, the fat harvested in the first syringe can be directly emulsified by successive passes between the first and second syringes, and then filtered by passing through a filtration membrane into the third syringe. The ability to emulsify and then filter without changing devices is provided by the connecting element (hereafter also referred to as the connecting element), which, according to the invention, not only switches the first emulsification circuit to the second filtering and transfer circuit, but also integrates the filtering function for the emulsified grease and connects to the third grease recovery syringe. When the connecting element is in the emulsification position, the first emulsification circuit allows the grease to pass between the first and second syringes through the arms of the armature supporting these syringes. When the connecting element is in the transfer position, the second circuit connects the first or second syringe to the third via the filtration membrane, ultimately leading to the third recovery syringe. Integrating the filtration membrane directly into the connection element has the advantage of making the device more compact. The rigid and all-in-one nature of the device according to the invention facilitates its handling. Furthermore, the risks of leakage are reduced, as are the risks of grease contamination, since the grease is handled in a closed circuit. According to one feature of the invention, the first and second branches of the frame are positioned relative to each other in a V shape. One advantage of this V-shape of the two arms of the frame is to improve The device's ergonomics are designed to simplify its use by the practitioner. Indeed, it facilitates the practitioner's movements during the emulsification phase thanks to a more ergonomic hand and wrist positioning. According to another feature of the invention, the first and second arms of the frame are positioned relative to each other at an angle (“) between 45° and 135°, preferably 90°. The practitioner's hands and wrists are therefore positioned in relation to each other in a more ergonomic way, which facilitates the practitioner's movements and thus limits the risk of injury. According to another feature of the invention, the first and second branches of the frame form a Y with the connecting element. One advantage of this Y-shape is that it offers a more ergonomic and natural grip. Indeed, it facilitates the emulsification of the grease between the two upper arms of the frame while allowing easy recovery of the filtered grease from the bottom of the device, i.e., through the connecting element forming a third lower arm of the preparation device. In addition, the fact that the arms of the frame used for emulsification are associated with a third arm formed by the connecting element which incorporates a filter membrane strengthens the robustness of the device, but also simplifies its use and operation. According to a particular aspect of the invention, the first branch forms a first angle (Ba) with the connecting element of approximately 112.5°- 157.5°, preferably 135°, and the second branch forms a second angle (Pz) with the connecting element of approximately 112.5°- 157.5°, preferably 135°. According to another particular aspect of the invention, the filtration membrane is held in the connecting element by means of a cover attached to the connecting element. This cover includes a connection port with the third syringe. Advantageously, the cap is fixed to the connecting element, thus creating a closed chamber in which the filtration membrane is installed. Furthermore, the cap has a Luer-type threaded fitting on one side, onto which the third syringe is screwed. This allows for the emulsification and subsequent filtration of the harvested fat using a single device. This all-in-one procedure simplifies the fat preparation process prior to reinjection into a patient and reduces the risk of contamination. According to one feature of the invention, the armature comprises at least one slide including a first end for engaging the emulsification position and a second end for engaging the transfer position and the element of connection includes a base comprising at least one fixing stud, said at least one fixing stud being designed to fit and slide into said at least one slide until locked in either of the first and second ends, when the connecting element is placed in the corresponding position, Advantageously, the fixing studs thus allow both the connection element to be mobile relative to the frame and to be reversibly held in either position. Advantageously, the fixing studs prevent the connection element from inadvertently changing position during the emulsification or transfer stage, thereby preventing these stages from being carried out correctly. According to another feature of the invention, the connecting element comprises a central stud fixed on the base and oriented along a longitudinal axis (y) of the armature, the central stud comprising at least two motion transformation interfaces in which at least one follower of the first branch and at least one follower of the second branch of the armature slide for a transformation of a rotational movement of the connecting element relative to the armature, under the effect of a rotational action of the connecting element exerted by a practitioner on the base around the longitudinal axis (y), into a translational movement of the connecting element along the longitudinal axis (y). Advantageously, the transition from an emulsification position to a transfer position is very simple: the practitioner rotates the connecting element around a vertical longitudinal axis. The rotational movement of the connecting element is simultaneously transformed into a translational movement along this longitudinal axis (or the vertical "y" axis in a standard orthonormal coordinate system) through the interaction between motion transformation interfaces located on its central pad and followers located on the arms of the frame. This movement of the connecting element, depending on the direction of rotation chosen, allows for the formation of either the first or second circuit of fat circulation. According to another feature of the invention, the central stud further comprises: - a first grease emulsification channel intended to form, together with the first and second branches of the armature, the first grease circulation circuit between the first syringe and the second syringe, when the connecting element is placed in the emulsification position, - a second grease transfer channel intended to form with the first or second branches of the armature the second grease circulation circuit between the first or second syringe and the third syringe, when the connecting element is placed in the transfer position. The invention also relates to a method for preparing fat taken from a first area of a patient's body for reinjection into a second area of the patient's body. This procedure uses the preparation device as described previously and includes the following steps: - positioning of the connecting element in the emulsification position; - emulsification of the grease taken by successive and repeated pressures of an operator on the first and second syringes, the pressures causing a circulation of the grease in the first circuit from the first syringe to the second syringe and vice versa; - a positioning of the connecting element in the transfer position by a change of position of the connecting element relative to the armature; - a filtration and transfer of the emulsified grease by pressure from an operator on one of the first or second syringes, the pressure causing a circulation of the grease taken from the second circuit through the filtration membrane into the third syringe. 4. Detailed description of the invention The invention relates to a device for preparing fat harvested by liposuction to obtain nano-fat for autologous transfer of the latter into a patient. More specifically, the invention relates to a grease preparation device configured to emulsify and filter grease that is more ergonomic and simpler to use than devices known in the prior art. Furthermore, the grease preparation device according to the invention limits external contamination of the grease by emulsifying and filtering it in a closed circuit, that is, without changing the preparation device during the procedure. To achieve this, the fat preparation device according to the invention connects several standard syringes together and can reversibly switch from a first emulsification position to a second position for transferring the emulsified fat. This allows for the recovery of higher-quality nano-fat very simply and with minimal intervention for the practitioner. We now present in connection with [Fig.1] an exploded view diagram of the different elements constituting the grease preparation device according to an example of an embodiment of the invention. The 100 grease preparation device comprises several separate elements that fit together reversibly or irreversibly. Once assembled, these different elements form the 100 preparation device. Among these elements, at least three main elements can be distinguished: a chassis 1, a crankpin 2 and a hood 3, which are described below, respectively in relation to [Fig.2], [Fig.3A], [Fig.3B], and [Fig.4]. In particular, the crankpin 2 fits into the frame 1, and the cover 3 fits into the crankpin 2. More specifically, the crankpin 2 is attached to the frame 1 in such a way that at least part of the crankpin 2 passes through the frame 1 along its longitudinal axis. Thus, the longitudinal axes of the crankpin 2 and the frame 1 coincide. In other words, if we consider a classical orthonormal coordinate system in the three xyz directions, the crankpin 2 is fitted into the frame 1 along the same vertical y-axis. The cover 3 is fixed onto the crankpin 2 in such a way that its longitudinal axis y coincides with the longitudinal axis y of the crankpin 2. The crankpin 2 is assembled under the frame 1, and the cover 3 is assembled under the crankpin 2 along a vertical y-axis. Furthermore, the crankpin 2 is movable relative to the frame 1 and designed to be operated by a practitioner. More specifically, under the effect of such an action (such as the practitioner applying a clockwise or counterclockwise rotation around the y-axis of the crankpin 2), the crankpin 2 can assume two distinct operating positions relative to the chassis 1. These two positions of the crankpin 2 correspond to the first emulsification position and the second position for transferring the emulsified grease. The preparation device 100 further includes a shell 6 formed of a first part 6a and a second part 6b. These two parts 6a and 6b fit together using a plurality of studs 60a and 60b (not shown) located on the inner face of the first and second parts 6a and 6b of the shell 6. The studs on the inner face of the first part 6a are complementary to the studs on the inner face of the second part 6b and allow the shell 6 to be fixed around the frame 1. The studs 60a and 60b of the shell 6 pass through openings 10 made in the frame 1. Once assembled to the frame 1, the first and second parts 6a and 6b of the shell 6 surround the latter to protect it from possible damage, for example during transport of the preparation device 100 or from mishandling by a practitioner (e.g., fall, impact, etc.). The preparation device 100 also includes a filter membrane 4 positioned in the mandrel 2 and held in place by means of the cap 3. This filter membrane 4 allows the fat to be filtered after emulsification during its transfer for recovery of nano-fat, before autologous transplantation in a patient. Filter membrane 4, for example, is a flexible PET (polyethylene terephthalate) filter membrane. Filtration through this type of flexible membrane is less stressful, or traumatic, for the components of the nano-grease, which contributes to a better quality of nano-grease obtained at the end of the emulsification / filtration process using preparation device 100. In one embodiment of the invention, the filter membrane 4 comprises a mesh size between 420 µm and 460 µm, preferably 440 µm. Thus, it is possible to obtain nano-grease without having to successively change filter sizes to progressively reduce the size of the grease. In other words, a single pass of the emulsified grease through the filter membrane 4 is sufficient to obtain nano-grease. We now present in connection with [Fig.2] a diagram of the chassis according to an example of an embodiment of the invention. The chassis 1 is a frame onto which two standard syringes, including Luer-tip syringes, the mandrel 2 and the shell 6 can be attached. In other words, the syringes, the mandrel 2 and the shell 6 are assembled around this central frame 1 of the preparation device 100. In one embodiment of the invention, the chassis 1 is a rigid frame in the form of a single piece, i.e., a single unit. The chassis 1 is, for example, made of rigid plastic. In particular, the chassis | allows two syringes to be connected to the chassis in a conventional manner, for example via a Luer fitting, at the level of a first branch A and a second branch B. The first branch A and the second branch B are symmetrical to each other along the longitudinal axis y of the chassis 1. In other words, the two syringes are connected to each other via the crankpin 2 by means of the frame 1. To this end, the first branch A and the second branch B of the chassis l each include a syringe connection port 12a and 12b in the form of a female Luer fitting (or "Luer lock") onto which the male Luer fitting of the syringes is rotated. The Luer fitting is a standardized system of small-diameter fluid fittings used to create leak-free connections between a male cone fitting and its corresponding female part. In one embodiment of the invention, the first and second branches A and B of the chassis 1 further comprise at least one protrusion each, also designated as followers 13a and 13b, which each engage in a complementary manner in movement interfaces of a central pin of the crankpin 2. In one embodiment of the invention, these two followers 13a and 13b, once engaged in the movement interfaces of the crank 2, enable, in conjunction with the movement interfaces of the crank 2, the conversion of a rotational movement of the crank 2 around its longitudinal axis y into a translational movement along the y-axis in a conventional orthonormal frame of reference. In other words, if the frame 1 is considered fixed relative to the practitioner, then the followers 13a and 13b, once engaged in the movement interfaces of the crank 2, allow the transformation of a clockwise or counterclockwise rotational movement around the y-axis of the crank 2 relative to the frame 1 into an upward / downward movement along the y-axis of the crank 2 relative to the first and second arms A and B of the frame 1. As a result, the crank 2 can assume two different positions relative to the frame 1: - a low or distant position of the chassis 1 corresponding to the first grease emulsification position, in which the first and second branches A and B, therefore the two syringes are connected, and - a high or close position of the frame 1 corresponding to the second grease transfer position, in which the first and second branches are disconnected, and at least one of these branches is connected to another syringe connected to the crankpin 2. The detail of the arrangement of the frame 1 and crankpin 2 in relation to the different positions is described in connection with [Fig.5] and [Fig.6]. In one variant, the practitioner applies a translational movement relative to the frame 1 to the crank 2 along the vertical axis y, and these are the followers 13a and 13b of the Branches A and B, which, once engaged in the movement interfaces of the crankpin 2, allow the latter to be held in one of the first or second emulsification or transfer positions. In other words, the followers 13a and 13b slide in the corresponding movement interfaces of the crankpin 2 along a vertical y-axis, the positions being held, for example, by means of notches or notches at the movement interfaces of the crankpin 2. Furthermore, the frame 1 includes a base 14 on which the first and second branches A and B are mounted. These first and second branches A and B are then positioned relative to each other on this base 14 in the shape of a V. In other words, the angle α between the longitudinal axis passing through the center of the first branch A and the longitudinal axis passing through the center of the second branch B is between 45° and 135°, preferably 90°. Thus, during a conventional emulsification step involving successive back-and-forth movements in two syringes, the practitioner's wrists are positioned approximately between 45° and 135°, preferably 90°, relative to each other. The practitioner's hands are therefore positioned in a much more comfortable and ergonomic manner, which reduces fatigue and the risk of injury. The base 14 of the chassis 1 comprises at least one, preferably two, slides 1la and 11b. These first and second slides 1la and 11b are, for example, in the form of arc-shaped openings, positioned opposite each other at the external part of the base 14. For example, the slides 1la and 11b have a length corresponding to a quarter circle and are positioned opposite each other symmetrically with respect to the center of the base 14 of the chassis Z:: In this embodiment of the invention, the slides 1a and 11b include a constriction at both ends, allowing the crank 2 to be held in the emulsification position or the transfer position. Switching from one position to the other is reversible and is achieved by simply disengaging the crank 2 from either end of the slides 1a and 11b. In other words, the slides 1a and 11b each include a first end for engaging the emulsification position and an end for engaging the transfer position. Switching from one position to the other is accomplished by removing the crank 2 from the constriction at one end of one of the slides 1a or 11b and engaging it in the constriction at the other end of the slides 1a or 11b. These first and second slides 1a and 11b thus allow the crankpin 2 to be fixed to the chassis 1 while allowing rotational and / or translational movement along the y-axis of the crankpin 2 relative to the chassis 1. In one variant, the slide(s) 11a and 11b are simple circular openings, i.e. holes passing through the base 14 of the chassis 1. We now present in connection with [Fig.3A] and [Fig.3B], respectively, a diagram and a longitudinal section of the crankpin according to an embodiment of the invention. The crankpin 2 is the connecting element of different parts of the preparation device 100. In other words, the crankpin 2 allows either the two syringes fixed on the first and second branches A and B of the frame 1 to be connected to each other, or one of these syringes to be connected to another syringe fixed at the skirt 22 of the crankpin 2 via the hood 3. In particular, it is the change of position (from the emulsification position to the transfer position and vice versa) of the crankpin 2 that makes it possible to create two different circuits for the circulation of the flow of grease, and therefore to connect the different syringes two by two according to these different circuits. In one embodiment of the invention, the crankpin 2 comprises a base 24 supporting at least one, preferably two, mounting studs 20a and 20b, or ring clips, fixed to the base 24 at one end. The mounting studs 20a and 20b are, for example, substantially cylindrical with a conical flare at one end. The mounting studs 20a and 20b fit into the slides 1la and 11b of the frame 1, respectively. The conical flare of the mounting studs 20a and 20b secures the crankpin 2 to the frame 1. More specifically, the particular shape of the mounting studs 20a and 20b first allows the crankpin 2 to fit into the slides 1la and 11b of the frame 1 and then prevents the crankpin 2 from disengaging. Thus, the mounting studs 20a and 20b enable the assembly and securing of the crankpin 2 to the frame 1. The base 24 of the crank 2 includes a grooved skirt 22 to facilitate the gripping of the crank 2 by the practitioner. Once positioned in the slides 1la and 1 Ib, the fixing studs 20a and 20b also allow the crank 2 to be mobile relative to the frame 1 (when we consider that the frame 1 is fixed relative to the practitioner). The crankpin 2 includes on its base 24 a central stud 23. In one embodiment, the central stud 23 is located between the two fixing studs 20a and 20b. The central stud 23 is larger than the fixing studs 20a and 20b and passes through the frame 1 between the two arms A and B. The central stud 23 includes at least one, preferably three circular grooves 25 in which sealing gaskets 5 are positioned. These sealing gaskets 5 ensure the sealing of the preparation device 100 during the emulsification and transfer steps. The crankpin 2 comprises at least one, preferably two, motion interfaces 26a and 26b. In one embodiment of the invention, these two motion interfaces 26a and 26b enable the transformation of the rotation of the crankpin 2 relative to the frame 1 into a translation of the crankpin within the frame 1 according to the longitudinal axis of the crankpin 2 and the frame 1. For example, these motion interfaces 26a and 26b, or motion conversion interfaces, take the form of substantially semi-helical grooves located at the end of the central stud 23 furthest from the base 24, and are positioned opposite each other. It is this particular shape of grooves that allows the transformation of rotational motion into translational motion. Followers 13a and 13b of branches A and B enter these two opposite grooves. Thus, when the practitioner applies a rotational movement to the crank 2, for example by turning the crank 2 a quarter turn relative to the frame 1 (rotation around the longitudinal axis y), the two followers 13a and 13b follow the semi-helical grooves 26a and 26b, which allows the central stud 23, and therefore by extension the entire crank 2, to have a translational movement along the y axis, which has the effect of moving the crank 2 away from or closer to the frame 1. The fixing studs 20a and 20b then slide reversibly in the slides 1la and 11b of the frame 1 until a locking position located at one or the other end of the slides 1la and 11b. In one variant, the grooves, or movement interfaces 26a and 26b, are straight along a vertical axis y of the crankpin 2 and include notches or grooves. The followers 13a and 13b then engage in the notches of the grooves 26a and 26b to hold the crankpin 2 in one position or another. The change in position of the crankpin 2 is then achieved by simple translation along the y-axis of the crankpin 2 relative to the frame 1. The central stud 23 also includes a first grease passage channel 27 which passes through it transversely, i.e. along an x-axis. The central stud 23 also includes a second grease transfer channel 28 which passes through the central stud 23 transversely only over a part of the latter. In one embodiment example, the first channel 27 is positioned above the second channel 28 in the central block 23, the entrance of the second channel 28 being offset by a quarter turn around the vertical axis y relative to the through openings of the first channel 27. In one variant, the first channel 27 is positioned above the second channel 28 in the central block 23, the inlet of the second channel 28 being aligned with one of the through openings of the first channel 27. Figure 3B is a longitudinal section of the crankpin 2. The first grease channel 27 from the central pin 23 connects the two syringes fixed to the first and second branches A and B of the frame 1 during the emulsification step. The second grease transfer channel 28 from the central pin 23 is hollowed out inside from the central pin 23 first transversely (along the horizontal x-axis), then longitudinally (along the vertical y-axis). Advantageously, the second channel 28 is essentially shaped like an inverted L hollowed out inside the central pin 23. In other words, the channel 28 forms a bend inside the pin 23. This second channel 28 allows one of the syringes fixed to the first branch A or the second branch B of the frame 1 to be connected to a third syringe connected to the lower end of the crankpin 2, during the transfer step. In one embodiment of the invention, the channel 28 connects in particular branch B to the crank 2, it is therefore not possible to make a transfer from the syringe connected to branch A to the third syringe connected to the crank 2. Thus, when the crank 2 is in the emulsification position, the connection ports 12a and 12b of the chassis 1 are opposite the first passage, or emulsification, channel of the grease 27 to form a first circuit of circulation of the grease flow between two syringes fixed on the connection ports 12a and 12b via the passage channel 27. Conversely, when the crankpin 2 is in the grease transfer position, only one of the connection ports 12a or 12b is opposite the second grease transfer channel 28 to form a second circuit for circulating grease flow between one of the syringes attached to one of the connection ports 12a or 12b and a third syringe connected to the lower end of the crankpin 2, via this second channel 28. We now present in connection with [Fig.4] a diagram of a hood according to an example of an embodiment of the invention. Cap 3 includes a female Luer connection port, or 30-pin connector, allowing the attachment of a syringe with a male Luer connector to cap 3. The cover 3 includes a skirt 31 enabling it to fit into the crankpin 2 at the face of the base 24 of the crankpin 2 opposite to that comprising the fixing studs 20a and 20b and the central stud 23, or so-called lower face of the crankpin s In one embodiment of the invention, the cover 3 is irreversibly fixed to the crankpin 2, for example by ultrasonic welding. The filter membrane 4 is then fitted into the crankpin 2 by means of the fixed cover 3. In other words, the filter membrane 4 is sandwiched between the crankpin 2 and the cover 3. In another variant, the cover 3 is fixed reversibly, for example by means of an additional thread between the crankpin 2 at the face of the base 24 of the crankpin 2 opposite to that comprising the fixing studs 20a and 20b and the central stud 23 and the skirt 31 of the cover 3 and a sealing gasket. Advantageously, it is possible to change the filter membrane 4 and reuse the preparation device 100. Thus, the crankpin 2 forms a third branch of the grease preparation device 100 which includes a filter membrane held by the hood 3, the first and second branches being branches A and B of the chassis 1. As described above, the preparation device 100 allows for the recovery of nano-fat from fat harvested, for example, by liposuction in an intra-structure procedure. Specifically, in order to recover the nano-fat, the preparation device 100 according to the invention emulsifies and filters the fat within a single fat preparation device. For this purpose, the preparation device 100 is designed to take two positions: - the first position called emulsification for the emulsification step by successive back-and-forth movement of the grease in two standard syringes and in which the connecting element 2 (i.e. the crankpin 2) forms with the first and second branches A and B of the frame 1 a first circuit for the circulation of the grease between a first syringe and a second syringe fixed to the frame 1 via the branches A and B; - the second so-called transfer position for the transfer of grease from one syringe to another through a filter membrane 4 and in which the crankpin 2 forms with one of the first or second branches A and B of the frame 1 a second circuit for the circulation of grease between the first or second syringe and a third syringe fixed via the cover 3 at the level of the lower face of the crankpin 2. It is the movement of the crankpin 2 relative to the frame 1 that enables this change of position. In particular, as described previously, the crankpin 2, thanks to the mounting studs 20a and 20b which slide in the guides 11a and 11b of the frame 1, can rotate a quarter turn clockwise or counterclockwise relative to the base 14 of the frame 1 around the y-axis. The followers 13a and 13b then follow the semi-helical motion conversion interfaces 26a and 26b of the central stud 23 of the crankpin 2, which transforms the rotational movement into a translational movement of the crankpin 2 along the y-axis. Consequently, the crankpin 2 moves from the first emulsification position to the second transfer position (and vice versa) by rotating around the y-axis and simultaneously translating along this axis. One advantage of such a preparation device, based on converting a rotational movement into a translational one, is its ease of use. The practitioner only needs to perform a limited number of steps to carry out the initial emulsification operation, change the device's position, and then perform the second filtration / transfer operation. Furthermore, it is simple to manufacture. Indeed, creating such a preparation device does not require assembling several complex parts. On the contrary, a small number of simple components are sufficient. Furthermore, the preparation device 100 according to the invention is particularly er- ergonomic while guaranteeing the production of excellent quality nano-fat. We now present, in connection with [Fig.5], a diagram of the grease preparation device in the emulsification position according to an example of an embodiment of the invention. During the emulsification step, the connecting element 2, i.e. the crankpin, is in the so-called emulsification position relative to the chassis 1. More specifically, in the emulsification position, the base 24 of the crankpin 2 is away by a distance d, for example from 4 to 5 mm, preferably equal to 4.4 mm, from the chassis 1, i.e. there is a space, or clearance, between the base 24 of the crankpin 2 and the base of the chassis 1. Thanks to the particular shape of the fixing studs 20a and 20b which slide in the guides 11a and 11b in the base 14 of the chassis 1, the crankpin 2 is mobile relative to the chassis 1, while remaining fixed to it. In the emulsification position, the syringe connection ports 12a and 12b can communicate with each other via the fat passage channel 27 located in the central pin 23 of the crankpin 2. Thus, the flow of fat (represented by the gray arrows) to be emulsified can pass from a first syringe connected to a first connection port 12a to a second syringe connected to a second connection port 12b. The successive passage of the fat from one syringe to the other allows, for example, after 30 back-and-forth movements by applying pressure to one or the other of the syringe pistons by the practitioner, the fat to be emulsified to obtain nano-fat. Advantageously, the first arm A of the frame 1 forms an angle Ba with the crankpin 2 of approximately 112.5°–157.5°, preferably 135°, and the second arm B of the frame 1 forms an angle Bp with the crankpin 2 of approximately 112.5°–157.5°, preferably 135°. Thus, the preparation device 100 is more ergonomic in its use. Angle B is the angle between the longitudinal axis passing through the center of the first arm A and the longitudinal axis passing through the center of the crankpin 2, and angle Ps is the angle between the longitudinal axis passing through the center of the first arm B and the longitudinal axis passing through the center of the crankpin 2. In other words, the preparation device 100 according to the invention comprises three branches: a first branch A of the chassis, a second branch B of the chassis and a third branch formed by the crankpin 2. These three branches are then positioned relative to each other in the form of a Y in order to guarantee the best possible ergonomics. We now present, in connection with [Fig.6], a diagram of the grease preparation device in the transfer position according to an example of an embodiment of the invention. During a grease transfer step, the connecting element 2, or crankpin, is in the so-called grease transfer position relative to the frame 1. More specifically, in the transfer position, the base 24 of the crankpin 2 is no longer far from the distance d (d=0) of the base 14 of the chassis 1, that is to say that there is no longer any space between the base 24 of the crankpin 2 and the base 14 of the chassis 1. In addition, one of the branches A or B of the chassis 1 is then connected to the grease transfer channel 28. Thus, the emulsified grease present at the end of the emulsification step in one of the syringes, then called the transfer syringe, passes through the connection port of the latter, then the transfer channel 28 to end up in the syringe fixed on the hood 3, or recovery syringe. In this example, it is the connection port 12b that is then connected to the fat transfer channel 28. Thus, the emulsified fat can be transferred from the transfer syringe connected to the connection port 12b to a third syringe connected to the connection port 30 of the hood 3 via the fat transfer channel 28. In order for the practitioner to identify which branch of the frame is the transfer branch, i.e. to which the transfer syringe is attached, an arrow on the transfer branch is shown. The emulsified grease then passes through the filter membrane 4 sandwiched between the crankpin 2 and the cover 3. In one variation, the practitioner manipulates the crank 2 by applying a longitudinal translational movement (along the y-axis) relative to the frame 1 to move from one position to the other. In other words, the practitioner moves the crank 2 closer to or further from the frame along the direction of the longitudinal y-axis, raising or lowering it relative to the frame 1. Diagrams of the different stages of the emulsification / filtration process implementing the grease preparation device according to an example of an embodiment of the invention are now presented in connection with figures 7A to 7E. In step EO1 (not shown), during a lipostructure procedure, the practitioner harvests the patient's fat in the usual way. This fat may, for example, be further pre-filtered using a known filtration technique to remove all particles that are not fat or fat cells, and in particular to recover micro-fat. In step E02 related to [Fig.7A], the practitioner attaches a standard syringe S3 to the connection port 30 of the cover 3 of the preparation device 100. To do this, the practitioner can hold the preparation device 100 by the skirt 22 of the mandrel 2. He then screws the male Luer tip of the syringe S3 onto the female Luer tip, or connection port 30 of the cover 3 associated with the mandrel 2, on the lower face of the mandrel 2, in the usual way. In step E03, related to [Fig. 7B], the practitioner then screws the male Luer tip of two other syringes S1 and S2 onto the connection ports 12a and 12b of branches A and B of the preparation device 100 in the usual way. One of these syringes, for example syringe S2 contains the previously collected and possibly pre-filtered fat which must be emulsified to recover the nano-fat. In step E04, in connection with [Fig.7C], the practitioner positions the crankpin 2 in the emulsification position. More specifically, the practitioner rotates, for example by a quarter turn, the crankpin 2 until the fixing studs 20a and 20b of the crankpin 2 slide in the guides 1la and 11b of the frame 1 up to the constriction located at one end of the guides 1la and 11b to clip the fixing studs 20a and 20b into the guides la and 11b and thus maintain the emulsification position. The rotational movement around the y-axis of the crank 2 is then simultaneously transformed into a translational movement along this axis of the entire crank 2 by sliding the followers 13a and 13b in the motion conversion interfaces 26a and 26b of the central pin 23. The base 24 of the crank 2 is then separated from the base of the frame 1 by a distance d. In other words, the crank 2 rotates and descends relative to the frame 1 along the y-axis.The central pin 23 therefore rotates a quarter turn and descends along the y-axis inside the chassis 1. In this emulsification position, the grease passage channel 27, or emulsification channel, of the central pin 23 of the crankpin 2 is then opposite the connection ports 12a and 12b of the chassis 1, thus forming a first circuit for the circulation of the grease. The practitioner can then proceed to emulsify the fat by successive back-and-forth movements of the fat in syringes S1 and S2 in the conventional manner. In step E05, related to [Fig. 7D], the practitioner adjusts the position of the crank 2 of the preparation device 100 to move it into the fat transfer position. To do this, the practitioner rotates the crank 2, for example by a quarter turn, in the opposite direction to before, until the fixing pins 20a and 20b of the crank 2 slide into the guides 11a and 11b of the frame 1, up to the constriction located at the ends of the guides 11a and 11b which are opposite the ends maintaining the emulsification position. The fixing pins 20a and 20b are then clipped into the guides 11a and 11b to maintain the transfer position during the transfer step. The rotational movement of the crankpin 2 around the y-axis is then simultaneously transformed into a translational movement along this axis of the entire crankpin 2 by sliding the followers 13a and 13b in the motion transformation interfaces 26a and 26b of the central pin 23. In other words, the crankpin 2 rotates and rises relative to the frame 1 along the y-axis. The central pin 23 therefore rotates a quarter turn in the opposite direction to the previous one around the y-axis and rises along this axis, thus moving up inside the frame 1. In this transfer position, the base 24 of the crankpin 2 is no longer separated from a distance d (d=0) from the base 14 of the chassis 1, but in contact with it. The emulsified grease contained in one of the syringes fixed to the arms of the chassis 1 can then be transferred into the syringe fixed to the cover 3. More specifically, in this transfer position the transfer channel 28 of the central pin 23 of the crankpin 2 is then opposite one of the connection ports 12a or 12b of the chassis 1 thus forming a second circuit for the circulation of the emulsified grease. In other words, one of the syringes S1 or S2 is a so-called emulsified fat transfer syringe, that is to say that at the end of the emulsification step, the practitioner ensures that all the emulsified fat is in a single syringe (identified by an arrow on the transfer branch) which can be connected to the so-called emulsified fat recovery syringe connected to the hood 3 via the transfer channel 28. In step E06, related to [Fig.7E], the practitioner then applies pressure to the piston of the transfer syringe, for example syringe S2 connected to branch B of frame 1. The flow of emulsified fat thus passes from syringe S2 to connection port 12b, then through transfer channel 28 of crankpin 2 to end its journey in the recovery syringe S3 connected to cap 3. During the transfer, the emulsified fat passes through the filter membrane 4 located in crankpin 2 and held by cap 3. The nano-fat obtained by emulsification, then filtration by the preparation device 100 is then recovered in the S3 syringe and is ready for reinjection into the patient in the conventional way. The unique shape of the preparation device 100 according to the invention (Y-shaped, once all the elements are assembled) allows the practitioner to better position their hands and wrists, thus facilitating the emulsification step through successive back-and-forth movements of the fat. The presence of a filtration membrane in one of the device's arms (formed by the crank 2) allows the fat to be filtered directly after emulsification, without having to change devices and therefore maintaining a closed circuit. Thus, the preparation device 100 limits the risk of contamination of the resulting nano-fat. Furthermore, integrating the filter directly into a third arm (formed by the crank 2) of the preparation device 100 reduces the risk of leaks, particularly those related to improper filter connection.
