Abstract
Description
[Technical field]
[0001] The present invention relates to a composition for improving insulin resistance. The present invention also relates to a composition for treating, preventing or improving a disease or disorder associated with insulin resistance. [Background technology]
[0002] Insulin is secreted into the blood from the pancreas after a meal and acts on skeletal muscle cells and fat cells, which take up glucose and lower blood sugar levels. When glucose metabolism disorders occur due to aging or an irregular lifestyle such as overeating, the effect of insulin to lower blood sugar levels weakens, and as a result, it is known that blood sugar levels increase after meals and that elevated blood sugar levels are less likely to return to normal. This state in which insulin becomes less effective is called insulin resistance. If this condition is left untreated and insulin becomes even less effective, type 2 diabetes will develop. In other words, in order to prevent type 2 diabetes, it is important to suppress the increase in blood sugar levels after meals.
[0003] It has been reported that blood glucose levels improved in people who took turmeric extract (Patent Document 1). On the other hand, there are reports that in people who took turmeric, the increase in postprandial insulin levels was promoted, but no change was observed in blood glucose levels (Non-Patent Document 1), and that no change was observed in postprandial blood glucose or insulin levels (Non-Patent Document 2). There are also reports that in people who took curcumin, a component contained in turmeric, no change was observed in postprandial blood glucose levels (Non-Patent Document 3), and that the increase in postprandial blood glucose levels was suppressed (Non-Patent Document 4). [Diagram 3] 1 is a graph showing the Cederholm index (an index of insulin sensitivity) calculated from blood glucose and insulin levels in an OGTT test in a control group that took turmeric extract and a placebo group. * indicates a significant difference of p<0.05 compared to the placebo group. DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS
[0014] The present invention is based on the finding that insulin resistance can be improved by administering or ingesting turmeric extract. Specifically, the present invention is based on the experimental results that in humans, ingesting turmeric extract suppresses the rise in blood glucose level after a meal and, although insulin secretion was not promoted, a decrease in insulin level after a drop in blood glucose level was observed, and thus the intake of turmeric extract improved the effectiveness of insulin (i.e., improved insulin resistance).
[0015] Therefore, in one aspect, the present invention provides a composition for improving insulin resistance, which contains turmeric extract as an active ingredient.
[0016] Insulin resistance refers to a state in which sensitivity to insulin, a hormone that has a blood glucose lowering effect, is reduced, and insulin cannot fully exert its action. Specifically, it refers to a state in which, despite the secretion of insulin from the pancreas into the blood, the sensitivity of target organs (skeletal muscle, adipose tissue, liver) to insulin is reduced, and its action (glucose uptake in skeletal muscle and adipose tissue, inhibition of gluconeogenesis in the liver) is weakened. On the other hand, when the amount of insulin produced by the pancreas decreases, the amount of insulin secreted decreases, and as with insulin resistance, blood glucose levels rise and the risk of diabetes increases. However, this state in which the amount of insulin produced by the pancreas decreases is called insulin secretion disorder (decreased insulin secretion), and is a state that is distinguished from insulin resistance.
[0017] Insulin resistance can be evaluated by measuring blood glucose levels (postprandial blood glucose levels) and insulin levels after a sugar load. Since blood insulin levels increase rapidly after a sugar load, the effect of insulin is greater than that of fasting blood glucose levels without a sugar load (which are controlled by glucagon, the liver, and the like in addition to insulin), and this can be used as an evaluation index for insulin resistance. Blood glucose concentration (blood glucose level) and insulin levels after a sugar load are measured over time, and if insulin levels are higher than normal due to excessive promotion of insulin secretion to lower blood glucose levels, or if blood glucose levels are high despite normal insulin levels, the patient can be evaluated as having insulin resistance. In addition, the Cederholm index is known as one of the indicators of insulin sensitivity (Cederholm, J. and Wibell, L. Diabetes Res Clin Pract. 10(2):167-75, 1990; Radikova, Z. Endocr Regul. 37(3):189-94, 2003), and the higher the value of this Cederholm index, the higher the insulin sensitivity, and the lower the value, the lower the insulin sensitivity. When the value of the Cederholm index is low, it can be evaluated that the insulin resistance is high.
[0018] Improvement of insulin resistance means, compared to when turmeric extract is not administered or taken, suppressing the decrease in insulin action, increasing the decreased insulin action or bringing it closer to normal, making the change from increase to decrease in insulin level after glucose loading normal or bringing it closer to normal, or increasing the Cederholm index value or bringing it closer to normal.
[0019] <Composition containing turmeric extract> The composition according to the present invention contains a turmeric extract. In the present invention, the turmeric extract refers to an extract (turmeric extract) of a plant material derived from a plant of the genus Curcuma of the Zingiberaceae family using an extraction solvent. The turmeric extract is not limited to a solvent extract obtained by extraction with an extraction solvent, but also includes a solvent extract that has been fractionated and purified by column chromatography or the like. The turmeric extract used in the present invention can be in the form of an extract liquid after the completion of an extraction operation (including a fractionation and purification operation when fractionation and purification is performed), a concentrate obtained by partially removing the solvent from the extract liquid, or a dried product obtained by removing the solvent from the extract liquid. The removal of the solvent from the extract can be carried out by volatilizing the solvent by heating and / or reducing pressure, etc. The method of heating and reducing pressure is not particularly limited, and for example, a conventionally known method can be used.
[0020] The plant raw material may be the rhizome of Curcuma longa, Curcuma aromatica, Curcuma zedoaria, Curcuma phaeocaulis, Curcuma kwangsiensis, Curcuma wenyujin, and / or Curcuma xanthorrhiza, which are plants of the Zingiberaceae family, and the rhizome of Curcuma longa is particularly preferred. The rhizome may be collected from the ground, and an appropriate part of the rhizome may be used as is, or cut to an appropriate size or shape, or in the form of a pulverized product. These plant raw materials may be appropriately dried.
[0021] As the extraction solvent, at least one selected from the group consisting of water and hydrophilic organic solvents can be used. As the at least one extraction solvent selected from the group consisting of water and hydrophilic organic solvents, any of water, hydrophilic organic solvents, and mixed solvents of water and hydrophilic organic solvents can be used. The hydrophilic organic solvent can be a mixed solvent of multiple hydrophilic organic solvents. "Water" also includes hot water. As the hot water, for example, hot water of 95°C or higher can be used. As the hydrophilic organic solvent, at least one alcohol (which may be a mixed solvent of multiple alcohols) can be mentioned, and the alcohol is not particularly limited, but ethanol is preferable. When a mixed solvent of alcohol and water is used as the extraction solvent, the mixing ratio is not particularly limited, but for example, a range of 10:90 to 90:10 by weight ratio is preferable, and a range of 20:80 to 50:50 is more preferable. In addition, supercritical carbon dioxide can also be used as the extraction solvent.
[0022] The method for extracting turmeric from plant raw materials is not particularly limited, and any extraction method commonly used in the art can be used.
[0023] In the present invention, the turmeric extract may contain one type of turmeric extract, or may contain two or more types of turmeric extracts with different extraction solvents and / or extraction methods. The ratio of each extract in the mixture is not particularly limited, and may be, for example, a weight ratio of 1:0.01 to 1:1.
[0024] In a preferred embodiment, the turmeric extract comprises at least one of an extract with water, such as hot water, and an extract with supercritical carbon dioxide.

[0028] [ka]
[0029] In the present invention, turmeronol A and turmeronol B each have the above-mentioned planar structure, and may be any optical isomer or a mixture of multiple optical isomers.
[0030] It is known that the configuration of the 6-carbon atom in the partial structure of 2-methyl-2-hepten-4-one in natural products isolated from turmeric extract is S-configuration for turmeronol A and turmeronol B. However, in the present invention, turmeronol A and turmeronol B may have the above-mentioned planar structure, and the configuration may be S-configuration, R-configuration, or a mixture of S-configuration and R-configuration.
[0031] In the present invention, bisaclone is a compound having the following planar structure: Bisaclone has an asymmetric carbon at the position indicated by the * mark in the planar structural formula and may contain multiple optical isomers, but in the present invention, bisaclone may be any optical isomer or a mixture of two or more optical isomers.
[0032] [ka]
[0033] The turmeric extract may be processed by a conventional method, such as drying, powdering, granulation, or solution.

[0070] [Example 1] Oral glucose tolerance test (OGTT) Male and female subjects aged 40-69 were randomly assigned to two groups and given either a test food containing turmeric extract or a placebo food not containing the extract, two tablets per day before dinner, under double-blind conditions, for 12 weeks (placebo group: n=42, turmeric extract group: n=39). After 12 weeks of taking the test food, an OGTT was performed and blood glucose and insulin levels were measured.
[0071] The specific method of OGTT is as follows. The subjects fasted from the night before the test, and on the day of the test, blood was drawn immediately before glucose loading (0 minutes). The subjects then ingested 75 g of Trelan G solution (a partial starch hydrolysate solution), and blood was drawn 30, 60, and 120 minutes later, and blood glucose and insulin levels were measured. The results were shown as the change from immediately before glucose loading (0 minutes).
[0072] In addition, the Cederholm index, an index of insulin sensitivity (ISI), was calculated. The Cederholm index was proposed as an index of insulin sensitivity by Cederholm and Wibell (Cederholm, J. and Wibell, L. Diabetes Res Clin Pract. 10(2):167-75, 1990; Radikova, Z. Endocr Regul. 37(3):189-94, 2003). The ISI, which is the Cederholm index, was calculated. Cederholm is calculated according to the following formula. This is an indicator that the higher the value, the higher the insulin sensitivity, and the lower the value, the lower the insulin sensitivity.

[0074] The changes in blood glucose (mg / dL) and insulin (μU / mL) levels from before (0 min) to 30, 60, and 120 min after ingestion of Trelan G solution are shown in Figures 1 and 2. The data shown in Figures 1 and 2 are the mean ± SEM of the turmeric extract intake group (n=39) or the placebo group (n=42), and ** indicates p < 0.01 vs. placebo group. As can be seen from Figures 1 and 2, when Trelan G solution was ingested, both blood glucose and insulin levels rose rapidly from 0 min (baseline).
[0075] As shown in Figure 1, the blood glucose levels of the turmeric extract group were significantly lower at both 60 and 120 minutes after the glucose load compared to the placebo group. This suggests that ingesting turmeric extract inhibits the rise in blood glucose levels after a meal (or promotes the disappearance of blood glucose levels (=promotes the return to baseline)). In addition, as shown in Figure 2, the insulin levels of the turmeric extract group were significantly lower at 120 minutes compared to the placebo group. After a meal, insulin is secreted in large amounts from the pancreas to lower blood glucose levels, but as blood glucose levels drop, insulin becomes unnecessary and insulin levels return to normal. Therefore, as shown in the results of Figure 2, the blood glucose levels of the turmeric intake group were significantly lower than those of the placebo group, and it is thought that insulin levels also decreased accordingly. Furthermore, as shown in Figure 3, the change from week 0 to week 12 in the Cederholm index (ΔCederholm index), an index of insulin sensitivity, in the turmeric extract group was significantly higher than in the placebo group (p < 0.05), suggesting that turmeric extract improves insulin sensitivity.
[0076] There are various mechanisms for controlling postprandial blood glucose levels, including glucagon and hepatic gluconeogenesis, but because insulin levels rise rapidly, it is characterized by the fact that insulin has a much greater influence than other control factors. Therefore, the improvement in blood glucose levels due to the intake of turmeric extract is likely due to the promotion of insulin secretion or the improvement of insulin resistance. In this study, although an increase in insulin levels (promoting effect on insulin secretion) was not observed in the turmeric extract group, an improvement in blood glucose levels was observed (Figures 1 and 2). Furthermore, improvement in insulin sensitivity was also observed in the turmeric extract group (Figure 3). Therefore, it can be interpreted as an improvement in the effectiveness of insulin, in other words, an improvement in insulin resistance.
[0077] These results demonstrate that intake of turmeric extract improves insulin resistance.
