[0001] Priority This application claims the benefit of priority from U.S. Provisional Application No. 62 / 413,577, filed October 27, 2016, the disclosure of which is incorporated herein by reference in its entirety.
[0002] The present disclosure relates to intravascular and / or intraluminal medical devices configured to retrieve obstructions from a human blood vessel. The obstructions to be retrieved can include thrombus and thrombotic material. Summary of the Invention [Means for solving the problem]
[0003] Disclosed embodiments may include an intraluminal device including an elongated structure formed from a plurality of wires. The intraluminal device may include a plurality of sets of looped wires positioned longitudinally at an intermediate area of the elongated structure. The plurality of sets may be circumferentially spaced about the structure and configured to cooperate with one another to form a plurality of thrombus entry openings. At least one grouping of woven wires may be positioned longitudinally adjacent the intermediate area and may be configured such that when an opening force is exerted on the elongated structure, the at least one grouping provides structural support to hold open first gaps between the plurality of sets of looped wires. In response to the opening force, second gaps may be formed between wires in at least one grouping of woven wires, the first gaps in the set of looped wires being larger than second gaps between wires in each of the at least one grouping.
[0004] In another embodiment, the at least one grouping of the endoluminal device can include at least two groupings of woven wires, each grouping can be longitudinally spaced apart from one another on either side of an intermediate area containing the sets of looped wires. The at least two groupings of woven wires can cooperate with one another such that when an opening force is exerted on the wire structure, the at least two groupings provide structural support to hold open first gaps between the sets of looped wires. In response to the opening force, second gaps can form between the wires in each of the at least two groupings, and the first gaps in the sets of looped wires can be larger than the second gaps between the wires in each of the at least two groupings.
[0005] In another embodiment, the elongate structure of the endoluminal device may be formed from at least eight wires, and each of the multiple sets of looped wires may be formed from the same at least eight wires as at least one grouping of woven wires.
[0006] In another embodiment, the elongate structure of the endoluminal device may be formed from 12 wires, which in an intermediate area may define 6 sets of looped wires. In adjacent areas, the 12 wires may collectively form at least one grouping of woven wires. In another exemplary embodiment, each of the 12 wires may have a diameter of about 80 microns. In another exemplary embodiment, each wire of the endoluminal device may have a diameter of about 75 microns. As another example, each wire of the endoluminal device may have a diameter between 60 microns and 85 microns. In yet another example, the wires may be smaller than 60 microns and larger than 85 microns. In a further example, a single endoluminal device may have wires of varying diameters.
[0007] In another embodiment, the intraluminal device can include an elongate structure formed from a plurality of wires. The intraluminal device can include a first region where the plurality of wires can be twisted to form a shaft, and a second region adjacent to the first region where the plurality of wires can be woven to form a scaffold. The intraluminal device can also include a third region adjacent to the second region where the plurality of wires can be separated into a set of looped pairs forming a thrombus capture structure. The intraluminal device can also include a fourth region where the plurality of wires can be braided to form a dense filter configured to capture thrombus.
[0008] In another embodiment, the elongate structure of the endoluminal device may be configured to transition between a collapsed (or retracted) position for delivery to a treatment site and an expanded position in response to an opening force exerted on the elongate structure.
[0009] In another embodiment, the elongate structure of the endoluminal device may be configured such that, for example, when an opening force is applied, a first gap may form between the wires in the second region and a second gap may form between the wires in the third region, the second gap being larger than the first gap.
[0010] In another embodiment, the elongate structure of the endoluminal device can be configured, for example, such that when an opening force is applied, a third gap is formed between the wires in the fourth region, e.g., such that the third gap is smaller than both the first gap and the second gap.
[0011] In another embodiment, the intraluminal device can further include a fifth region adjacent to the fourth region, and the wires in the fifth region can be twisted to form an additional shaft. In another embodiment, the intraluminal device can include 12 wires, and each wire can have a diameter of about 80 microns. In one embodiment, for example, each wire of the intraluminal device can have a diameter of about 75 microns. As another example, each wire of the intraluminal device can have a diameter between 60 microns and 85 microns.
[0012] In another embodiment, the elongate structure of the endoluminal device can be configured such that the opening force is applied through axial movement of the first region. In yet another embodiment, the endoluminal device can further include an additional second region of the scaffold between the third and fourth regions.
[0013] In another embodiment, the intraluminal device can include an elongate structure formed from a plurality of wires, and the device can also include a plurality of cables, each formed from a subset of the plurality of wires. Pairs of cables can cross each other at a plurality of crossover locations, where wires from each pair of crossover cables can be unwound and woven together with wires from the paired crossover cables. Furthermore, the wires of the crossover cables can be woven together at the crossover locations in a manner that allows the woven together wires to move relative to each other when applied to the elongate structure. Also, in response to an opening force exerted on the elongate structure, at the crossover location, the pair of crossover cables can be configured to pivot relative to each other as the elongate structure transitions between a collapsed (or retracted) position for delivery to a treatment site and an expanded thrombus capture position. [0016] Notes that appear in the figures are merely illustrative and do not limit the invention as claimed.
[0017] Reference will now be made in detail to the present exemplary embodiments of the present disclosure, examples of which are illustrated in the accompanying drawings. Wherever possible, the same reference numbers will be used throughout the drawings to refer to the same or like parts.
[0018] FIG. 1 illustrates an exemplary intraluminal device 1000 including five alternating wire zones 1001, 1002, 1003, 1004, and 1005. Zones 1001, 1003, and 1005 include groups of woven wires 1010 and can provide structural support for zones 1002 and 1004. Additionally, because the openings between the wires 1010 in zones 1 and 5 are much smaller, they can also provide distal and proximal filters. (An example of variable size openings is illustrated in FIG. 10, discussed below.) As a result, thrombus particles that may emerge during retrieval can be captured in these zones, for example. As further illustrated in FIG. 1, zones 1002 and 1004 can be constructed from looped wires 1020, allowing for areas to capture large thrombus. 1, zones 1001, 1003, and 1005 may be constructed by woven wire 1010. The number of zones shown is exemplary; more or fewer zones may be provided.
[0019] 2 illustrates the endoluminal device 2000 in a more open position than that illustrated in FIG 1, highlighting the thrombus entry cell 2020, which may be made from a looped wire 1020. As further shown in FIG 2, the zones 1002 and 1004 may be constructed from looped wire 1020, allowing an area to capture large thrombus.
[0020] FIG. 3 illustrates yet another exemplary intraluminal device 3000. In this example, as shown in FIG. 3, the device 3000 can be configured to include only two distinct zones. Zone 3001 can be constructed from a group of woven wires 3010, for example, tightly braided, that provide structural support for the device 3000. In addition, zone 3001 can also act as a distal filter to prevent emboli from the distal vasculature. Also, as shown in FIG. 3, zone 3002 can be constructed from looped wires that are positioned longitudinally to provide a thrombus entry zone 3020. Additionally, zone 3001 can provide structural support and can also act as a distal filter. As further shown in FIG. 3, zone 3002 can be a zone where thrombus enters.
[0021] FIG. 4 illustrates yet another exemplary intraluminal device 4000 with four regions. In the first region 4001, the wires can be twisted or coiled to form a shaft 4015. In the second region 4002, the wires can be woven to form a scaffold 4017 that supports an opening in the third region 4003. In the third region 4003, the wires can be looped paired woven sets to form a thrombus capture structure 4020. For example, the wires in the third region 4003 can be loosely looped or loosely connected. Additionally, the fourth region 4004 can be woven to form a distal filter 4030 that captures distal emboli or thrombus particles. The fourth region 4004 can also serve as a scaffold for the third region 4003.
[0022] Figure 5 illustrates yet another exemplary intraluminal device 5000. For example, as shown in Figure 5, the thrombus opening region 5001 can be woven from three wires looped together. Additionally, the number of wires looped together can be greater than two.
[0023] Figure 6 illustrates yet another exemplary intraluminal device 6000. For example, as shown in Figure 6, the thrombus opening region 6001 may be woven from three wires loosely looped together. Additionally, the number of wires looped together may be greater than two.
[0024] 7 illustrates yet another exemplary intraluminal device 7000. For example, as shown in FIG. 7, the device 7000 can include six cables 7025, each of which can include paired wires. This can create a strong, yet flexible crossover. This can also further enable the device 7000 to achieve a flexible structure with high radial force, for example.
[0025] 8 illustrates an example of a cable braid 8000, as discussed above. Each cable 7025 may be made, for example, from a looped pair of wires 8027 that are woven together with a pair of wires 8027 from a crossing cable. As a result, for example, a semi-flexible strong crossing point may be realized.
[0026] 9A-9B illustrate yet another exemplary intraluminal device. As shown in FIG. 9A, the cable can be made, for example, from three wires that are unwound and then woven together with wires from the crossing cable. FIG. 9B also illustrates a cable crossing point 9006 where the cables (including cables 9024 and 9025) are unwound and woven back together. As discussed below in connection with FIG. 12 and FIG. 19C, the braided structure of FIG. 9A can include a 12-wire braided structure with twists before and after each splice frame.
[0027] As discussed above in connection with FIG. 1, FIG. 10 illustrates a device 10000 with variable size openings. Region 10001 includes a group of woven wires 10010 adjacent to intermediate locations 10002 and can provide structural support for intermediate locations 10002. Specifically, the grouping of woven wires 10010 in region 10001 can provide support to hold open first gaps 10028. First gaps 10028 are larger than second gaps 10018, which reside in region 10001. Cross section 10050 shows how cable 10025 is displaced circumferentially at intermediate locations 10002. The lines making up the rings shown in cross section 10050 are intended to illustrate the generally circumferential displacement of cable 10025 around the central region. Additionally, the dotted area shown in cross section 10050 within first gap 10028 is used to indicate a relatively large opening provided for thrombus entry in intermediate location 10002.
[0028] According to embodiments consistent with the present disclosure, an exemplary endoluminal device can include, for example, two braiding mechanisms, configurations, or structures, which can help increase the performance of the device relative to devices incorporating standard braided structures.
[0029] For example, as shown in FIG. 12, according to at least some embodiments of an endoluminal device consistent with the present disclosure, the braided structure 12000 can include twists of wires before and after each splice frame 12050. The braided structure 12000 includes three strands of two wire pairs (two wires 12015 and 12025) that are braided with three strands of two wire pairs (two wires 12030 and 12040). FIG. 12 shows nine splice frames in total. The splice locations within the mesh structure can help prevent the wires from slipping across the twists, which may otherwise act to remain uniformly separated over the circumference of the mesh structure of the endoluminal device. As shown in FIG. 11, braided structure 11000 is capable of achieving slippage (illustrated by arrow 11005) until wire 11040 reaches (for example) parallel wire 11030, but the twist as shown in braided structure 12000 shown in FIG. 12 acts to help prohibit substantial slippage across the twist, allowing for a firm structure to be achieved when the intraluminal device is expanded.
[0030] According to at least some embodiments of an intraluminal device consistent with the present disclosure, and as illustrated in Figures 13 and 14, a braided structure including a twisted structure can add strength to a mesh structure by acting as a restraining system to grip the wires and by dividing external forces applied to the mesh over additional elements. Figure 13 shows a cross section 11010 of the braided structure 11000 of Figure 11, and Figure 14 shows a cross section 12010 of the braided structure 12000 of Figure 12. The dotted lines in cross section 12010 of Figure 14 illustrate the twists in wires 12015 and 12025, and also in wires 12030 and 12040. The force distribution mechanism in a braided structure is similar to that of, for example, a loaded beam with a varying number of supports. This is also illustrated in Figures 13 and 14 with diagrams of a loaded beam 13000 (associated with braided structure 11000) and a loaded beam 14000 (associated with braided structure 12000). As illustrated in Figures 13 and 14, a loaded beam 13020 with three supports 14010 will receive and distribute forces more effectively than, for example, a loaded beam 13020 with two supports 13010 because there is a smaller distance between the three supports 14010.
[0031] According to at least some embodiments consistent with the present disclosure, the exemplary intraluminal device may be delivered through a microcatheter having an internal diameter of 0.017". As a result, the retriever can have a low profile (in a retracted or compressed state) that is smaller than the internal diameter. According to at least some embodiments of an intraluminal device consistent with the present disclosure, the device may be comprised of five following parts, e.g.: A control handle; a stiff proximal shaft (e.g., stainless steel hypotube); A flexible shaft (e.g. made from a cable of wire) an expandable mesh made from the same wires of the cable; A core wire / control wire, which may be connected to the distal tip of the mesh and runs through the shaft to the handle; It is possible to have:
[0032] For example, the intraluminal device 19300 (shown in FIG. 9A) can include a flexible shaft with eight wires (each having a diameter of 70 μm) and a mesh including eight wires (each having a diameter of 70 μm). For example, the eight wires can be formed by creating, for example, four strands of wires that are braided together, each strand including two wires each. As shown in FIG. 19A and discussed above, the braided structure 19200 can include twists of the wires before and after each splice frame to help prevent slippage. Also shown in FIG. 19A is a detailed view of the cable braid 8000 (discussed in connection with FIG. 8). The cable braid 8000 details the splices within the braided structure 19200 of the device 19300. The wires can be made of, for example, Nitinol. FIG. 19A also shows a twist-free braided structure 19100 before and after the joining frame.
[0033] In another exemplary embodiment, the intraluminal device can include a flexible shaft with 12 wires and a mesh including 12 wires. The 12 wires can be formed, for example, by creating six strands of wires braided together, where three strands can include two wires each, while the other three strands can include two wires each. As shown in FIG. 19C and discussed above (e.g., with reference to FIG. 8 showing cable braid 8000 and FIG. 12 showing braided structure 12000), the braided structure 12000 for this embodiment can include twists of wires before and after the respective bonding frames to help prevent slippage. FIG. 19C also shows braided structure 11000 without twists before and after the bonding frames. An exemplary intraluminal device with 12 wires includes device 9000 of FIG. 9.
[0034] According to another embodiment consistent with the present disclosure, an exemplary flexible shaft of an intraluminal device can include a flexible shaft with 10 wires and a mesh including 10 wires. The 10 wires can be formed, for example, by creating four strands of wires that are braided together, two strands can include three wires each, while the other two strands can include two wires each. An exemplary braided structure with 10 wires is illustrated in FIG. 19B. The braided structure 19500 can include twists of wires before and after each joint frame. Braided structure 19500 includes two-wire strands (wires 12015 and 12025) and three-wire strands (wires 12015, 12025, and 19025) braided with two-wire strands (wires 12030 and 12040) and three-wire strands (12030, 12040, and 19040). Also shown is a misalignment arrow 19505. Also shown is a braided structure 19400 without twists. The braided structure 19400 includes two-wire strands (wires 11015 and 11025) and three-wire strands (wires 11015, 11025, and 19015) that are braided with two-wire strands (wires 11030 and 11040) and three-wire strands (11030, 11040, and 19030). Also shown are offset arrows 19005. Within each strand of wire, the wires can be, for example, twisted together to create a stable strand. The wires can be made from, for example, Nitinol, and this configuration can be achieved by, for example, cutting two wires at the transition between the cable and the mesh (although this may require another manufacturing step). FIG. 15 depicts an overall view of an exemplary intraluminal device 15000 according to the present disclosure, and as shown in FIG. 15, the strands can cross over one another to create large openings (cells). At the intersections, for example, the strands may be twisted with one another to create a loosely connected joint. Before and after splicing, for example, the wires of the strands may be twisted.
[0035] As shown in FIG. 16, a strand of two wires (i.e., wire 16020) can cross a strand of three wires (i.e., wire 16010). Twisting the wires before and after the bond, as well as the wires inside the bond, can create a loosely interlocked but stable bond and cross section that helps prevent slippage, creates larger cells, and ultimately helps resist collapse of the device when expanded by high radial forces in the tube. And as shown in FIGS. 17-18 (where FIG. 17 is a detail of region 16015 of FIG. 16 in an expanded configuration, and FIG. 18 is a detail of FIG. 15 in an expanded configuration), when the mesh is expanded, the bond structure keeps the wires together even when the mesh is expanded. As a result, the mesh size remains the same.
[0036] Figures 19A, 19B, and 19C illustrate 8, 10, and 12 wire junctions and configurations, respectively. In addition, as discussed above, devices 15000 and 18000, and the detailed views of Figures 16 and 17, can use the 12 wire junction of Figure 19C. Of course, these are merely examples, and wire junctions and configurations with more or fewer wires can be used, and different braid arrangements can be used regardless of the number of wires.
[0037] Moreover, although illustrative embodiments have been described herein, the scope includes any and all embodiments having equivalent elements, modifications, omissions, combinations (e.g., across various embodiments), applications, or substitutions based on the present disclosure. The elements in the claims should be interpreted broadly based on the language used in the claims and not limited to the examples described herein or as non-exhaustive during the prosecution of this application. Furthermore, the steps of the disclosed methods can be modified in any manner (including by rearranging steps or by inserting or deleting steps).
