Abstract
Description
Medical device and method for delivering sheet-shaped delivery material to a target site
[0001] The present invention relates to a medical device and method for delivering a sheet-shaped delivery material to a target site.
[0002] In recent years, attempts have been made to transplant various cells to repair damaged tissues, etc. For example, attempts have been made to use fetal cardiomyocytes, skeletal myoblasts, mesenchymal stem cells, cardiac stem cells, ES cells, iPS cells, etc. to repair myocardial tissue damaged by ischemic heart diseases such as angina pectoris and myocardial infarction (Haraguchi et al., Stem Cells Transl Med. 2012 Feb;1(2):136-41).
[0003] As part of these efforts, cell structures formed using scaffolds and sheet-like cell cultures formed from cells in sheet form have been developed (Sawa et al., Surg Today. 2012 Jan;42(2):181-4). Regarding the therapeutic applications of sheet-like cell cultures, investigations are underway into the use of cultured epidermal sheets for skin injuries such as burns, corneal epithelial sheet cell cultures for corneal injuries, and oral mucosal sheet cell cultures for endoscopic resection of esophageal cancer, and some of these have already reached the clinical application stage.
[0004] Endoscopic surgery is widely used as a minimally invasive surgical method for administering a sheet-shaped cell culture to a target site, and various instruments have been proposed for delivering and administering a sheet-shaped cell culture to a target site. For example, Japanese Patent Laid-Open Publication No. 2009-511 describes a delivery and administration instrument in which a sheet support member is shaped like a cylinder and stored inside an outer cylinder, thereby reducing the degree of invasiveness to the human body and delivering a sheet-shaped therapeutic substance to the transplant site.
[0005] The sheet application device disclosed in JP 2016-187601 A can apply a sheet-like substance to a target site by moving a rod-shaped member arranged along the surface of a sheet support along the surface of the sheet support to peel it off. The delivery and administration device disclosed in JP 2008-173333 A can administer a sheet using a sheet detachment means that can apply negative pressure to the sheet support to adsorb and hold the sheet-like therapeutic substance, and positive pressure to detach the sheet-like therapeutic substance from the sheet support. The transfer instrument disclosed in JP 2012-30045 A has a displacement body that is displaceable relative to a main body, and a belt body that is wound around the displacement body and connected to the main body, and can transfer a biological graft placed on the belt body by displacing the displacement body.
[0006] The inventors of the present invention have encountered the problem that it is difficult to efficiently deliver the above-mentioned sheet-shaped delivery article to the target site while maintaining its shape. Therefore, the present invention aims to solve this problem and realize a medical device that can deliver a sheet-shaped delivery article to the target site while maintaining its shape using a simple mechanism and simple operations.

[0016] In the present invention, "sheet-shaped delivery product" refers to a sheet-shaped pharmaceutical product, regenerative medicine product, medical device, etc. that is delivered to a living body and used for treatment, etc. Sheet-shaped includes film-shaped and membrane-shaped (high-viscosity object). Sheet-shaped pharmaceutical products include tissue adhesives, local anesthetics, etc. Sheet-shaped regenerative medicine products include grafts, cell cultures, etc. Sheet-shaped medical devices include adhesion barriers, hemostatic materials, wound dressings, etc. In the present invention, "graft" refers to a structure for transplantation into a living body, and particularly refers to a transplant structure containing living cells as a component. Examples of grafts of the present invention include, but are not limited to, sheet-shaped cell cultures, spheroids, cell aggregates, etc., and are preferably sheet-shaped cell cultures or spheroids, and more preferably sheet-shaped cell cultures.
[0017] In the present invention, the term "sheet-shaped cell culture" refers to a sheet-like structure in which cells are interconnected. Cells may be interconnected directly (including via cellular elements such as adhesion molecules) and / or via intervening substances. Intervening substances are not particularly limited as long as they can at least physically (mechanically) connect cells, and examples include extracellular matrix. Intervening substances are preferably derived from cells, particularly from the cells that constitute the cell culture. Cells are at least physically (mechanically) connected, but may also be functionally connected, for example, chemically or electrically. Sheet-shaped cell cultures may be composed of one cell layer (single layer) or two or more cell layers (laminated (multilayered) structures, e.g., two, three, four, five, six, etc.). Furthermore, sheet-shaped cell cultures may have a three-dimensional structure with a thickness exceeding the thickness of a single cell, without the cells exhibiting a distinct layered structure. For example, in a vertical cross section of a sheet-shaped cell culture, the cells may not be uniformly aligned in the horizontal direction, but may be arranged unevenly (for example, in a mosaic pattern).
[0018] The sheet-shaped cell culture preferably does not include a scaffold (support). Scaffolds are sometimes used in the art to attach cells to their surface and / or interior and maintain the physical integrity of the sheet-shaped cell culture. The sheet-shaped cell culture of the present invention can maintain its physical integrity even without such a scaffold. Furthermore, the physical strength of the sheet-shaped cell culture of the present invention can be reinforced by applying fibrin or the like.
[0019] The cells constituting the sheet-shaped cell culture are not particularly limited as long as they can form a sheet-shaped cell culture, and include, for example, adhesive cells (adherent cells). Adherent cells include, for example, adhesive somatic cells (e.g., cardiomyocytes, fibroblasts, epithelial cells, endothelial cells, hepatocytes, pancreatic cells, kidney cells, adrenal cells, periodontal ligament cells, gingival cells, periosteal cells, skin cells, synovial cells, chondrocytes, etc.) and stem cells (e.g., myoblasts, tissue stem cells such as cardiac stem cells, embryonic stem cells, pluripotent stem cells such as iPS (induced pluripotent stem) cells, mesenchymal stem cells, etc.). Somatic cells may be differentiated from stem cells, particularly iPS cells (iPS cell-derived adhesive cells). Non-limiting examples of cells that can be used to form sheet-shaped cell cultures include myoblasts (e.g., skeletal myoblasts), mesenchymal stem cells (e.g., derived from bone marrow, adipose tissue, peripheral blood, skin, hair roots, muscle tissue, endometrium, placenta, or umbilical cord blood), cardiomyocytes, fibroblasts, cardiac stem cells, embryonic stem cells, iPS cells, synovial cells, chondrocytes, epithelial cells (e.g., oral mucosal epithelial cells, retinal pigment epithelial cells, nasal mucosal epithelial cells), endothelial cells (e.g., vascular endothelial cells), hepatocytes (e.g., hepatic parenchymal cells), pancreatic cells (e.g., pancreatic islet cells), kidney cells, adrenal cells, periodontal ligament cells, gingival cells, periosteal cells, skin cells, synovial cells, and chondrocytes. Non-limiting examples of iPS cell-derived adhesive cells include iPS cell-derived cardiomyocytes, fibroblasts, epithelial cells, endothelial cells, hepatocytes, pancreatic cells, kidney cells, adrenal cells, periodontal ligament cells, gingival cells, periosteal cells, skin cells, synovial cells, and chondrocytes.
[0020] In the present invention, the term "medical device" refers to a machine, instrument, etc. that is intended to be used for the diagnosis, treatment, or prevention of diseases in humans or animals, or to affect the structure or function of the human or animal body.
[0021] In the present invention, the term "target site" refers to the area where the sheet-like delivery product is applied (attached). Examples of the target site include the surface of an organ, a damaged site of an organ, and an anastomosis site of an organ. Typical target sites include the heart, the surface of the heart, a damaged site of the heart, and an anastomosis site of the heart. The target site also includes a site where damage (wound) exists in a hollow organ, such as a damaged site on the inside of the hollow wall, or the opposite side of the hollow wall corresponding to the damaged site. "Hollow organ" refers to an organ with a lumen housed in a body cavity, i.e., an organ with a tubular or bag-like structure, and examples thereof include organs of the digestive system, circulatory system, urinary system, respiratory system, and reproductive system.
[0022] Examples of organs of the digestive system include the esophagus, stomach, duodenum, pancreas, gallbladder, bile duct, small intestine, large intestine, rectum, etc. Among these, the duodenum is most preferable because of its thin lumen wall, high risk of perforation, and the harsh environment of exposure to various digestive fluids, which makes damage more likely to progress.
[0023] In one embodiment, the sheet-shaped delivery material is immersed in a protective liquid. The "sheet-shaped delivery material" is not particularly limited as long as it is a liquid that can maintain the shape and function of the graft, and examples thereof include physiological saline, phosphate buffered saline (PBS), Hank's balanced salt solution, cell culture medium, water, and the like, or a mixture thereof.
[0024] In the present invention, "delivering a sheet-like delivery item to a target site" refers to moving the sheet-like delivery item from the base end side of the main body to the tip side closer to the target site. The base end side of the main body is the side opposite the base end side, closer to the target site, and the tip side of the main body is the side opposite the base end side, closer to the target site, and from which the sheet-like delivery item is delivered. In the present invention, "main body" refers to a plate-like member having a surface capable of supporting the sheet-like delivery item. The main body has an arch-shaped portion (arch-shaped part), and the arch-shaped part has an inner surface and an outer surface opposite the inner surface. For example, if the main body has a cylindrical shape with a square cross section formed by connecting four plate-shaped members, the arch-shaped part is a hollow rectangle including a U-shape, and the concave inner surface is called the inner surface, and the protruding outer surface opposite the inner surface is called the outer surface.
[0025] For example, if the main body has a cylindrical cross section formed by rolling a single plate-like member into a circle, the arched portion is O-shaped, including a C-shape, and the inner and outer surfaces are curved. For example, if the main body has a semi-cylindrical cross section formed by rolling a single plate-like member into a semi-circular shape, the arched portion is C-shaped, and the inner and outer surfaces are curved. That is, the arched portion refers to a hollow rectangular, U-shaped, O-shaped, or C-shaped arch portion of the main body. A hollow rectangle includes a U-shape, and an O-shape includes a C-shape. In the present invention, the concave side of an arched portion may be referred to as the inner surface, concave surface, inside, or interior, and the protruding side of an arched portion may be referred to as the outer surface, convex surface, outside, or exterior. An arched shape also includes an arc shape.
[0026] Either the inner or outer surface of the arch-shaped portion may be flat; for example, the inner surface may be concave and the outer surface may be flat, or the inner surface may be flat and the outer surface may be convex. The main body is rigid, and by placing a sheet-like delivery item on its concave surface, it can protect the sheet-like delivery item from external forces, and by placing a sheet-like delivery item on its convex or concave surface, it can hold the sheet-like delivery item in a convex or concave shape. The main body may have an arch-shaped portion with a concave and / or convex surface at least in part, and may be a member with a semicircular, circular, rectangular, or polygonal cross section. The main body is preferably elongated so that it can linearly support multiple sheet-like delivery items.
[0027] The material of the main body is not particularly limited, and various known materials can be used alone or in combination. Examples of such materials include metal, soft polyvinyl chloride resin, polyurethane resin, silicone rubber, natural rubber, synthetic rubber, styrene-ethylene-butylene-styrene copolymer (SEBS), styrene-ethylene-propylene-styrene copolymer (SEPS), ethylene-vinyl acetate copolymer (EVA), polyamide resins and polyamide elastomers such as nylon, polyester resins and polyester elastomers such as polyethylene terephthalate (PET), olefin resins such as polyethylene, fluororubber, fluororesin, polystyrene, polymethyl methacrylate, polycarbonate, polyimide, polyetherimide, polyether ether ketone, etc., and these can be used alone or in combination of two or more.
[0028] In the present invention, the term "sliding body" refers to a strip-shaped body on which a sheet-shaped delivery can be placed while maintaining its shape. The sliding body can slide along the surface shape of the body, and when sliding on the outer surface (convex surface), it deforms into a convex shape along the convex surface, and when sliding on the inner surface (concave surface), it deforms into a concave shape along the concave surface. The strip has a length that allows it to slide along the body, and can support one sheet-shaped delivery or multiple sheet-shaped delivery items linearly on it. The sliding body slides along the longitudinal axis of the body and moves from the inner surface to the outer surface and vice versa by folding back at the tip of the body. In other words, when the sliding body is folded back, it is turned upside down, and the support surface on which the sheet-shaped delivery items are placed is also turned upside down.
[0029] The material of the sliding body is not particularly limited, and various known materials can be used alone or in combination. Examples of such materials include metal, soft polyvinyl chloride resin, polyurethane resin, silicone rubber, natural rubber, synthetic rubber, styrene-ethylene-butylene-styrene copolymer (SEBS), styrene-ethylene-propylene-styrene copolymer (SEPS), ethylene-vinyl acetate copolymer (EVA), polyamide resins and polyamide elastomers such as nylon, polyester resins and polyester elastomers such as polyethylene terephthalate (PET), olefin resins such as polyethylene, fluororubber, and fluororesin, and these can be used alone or in combination of two or more.
[0030] In the present invention, "hole" refers to one or more through holes provided in a support surface, such as a slide or support, on which a sheet-like delivery item is placed, and by feeding a fluid from the opposite side of the surface supporting the sheet-like delivery item and discharging the fluid through the hole, the sheet-like delivery item can be peeled off from the support surface and attached. The hole need only be capable of discharging the fluid, and as long as at least one hole is provided in a part of the support surface that contacts the edge of the sheet-like delivery item, the sheet-like delivery item can be peeled off from the edge.
[0031] Preferably, multiple holes are arranged discretely on the support surface so that the peeling position of the sheet-like delivery can be adjusted. For example, at least one of the multiple holes can be provided near the edge of the sheet-like delivery, and the sheet-like delivery can be peeled from that edge first, followed by ejecting fluid from the other holes, thereby peeling the sheet-like delivery in stages. The multiple holes can be arranged in a row-by-column matrix, for example, 2x2 to 40x40, preferably 5x5 to 20x20, with a spacing of, for example, 0.05mm to 10mm, preferably 0.5mm to 2mm, and a diameter of, for example, 0.05mm to 10mm, preferably 0.3mm to 0.6mm, but are not limited thereto. Those skilled in the art can freely select the diameter based on the size of the sheet-like delivery, the degree of adhesion between the sheet-like delivery and the support surface, etc. A matrix arrangement of multiple holes can also be achieved by forming the support surface into a mesh structure.
[0032] In the present invention, "fluid" is a general term for gases and liquids that can be pushed out and peeled off without damaging the sheet-shaped delivery item. The liquid is composed of at least one component, and the component is not particularly limited, but may be, for example, water, aqueous solutions, non-aqueous solutions, suspensions, emulsions, or other liquids. The components that make up the liquid are not particularly limited as long as they have little effect on the sheet-shaped delivery item. When the sheet-shaped delivery item is a membrane made of a biomaterial, it is preferable that the components that make up the liquid are biocompatible, i.e., do not cause undesired effects such as inflammatory reactions, immune reactions, or toxic reactions in biological tissues or cells, or at least have minimal such effects.
[0033] In the present invention, the term "support" refers to a flat plate-like body having a plane capable of supporting the sheet-like delivery item while maintaining its shape. The support is flexible and can be curved while supporting the sheet-like delivery item and stored along the inside of the main body, and when exposed from the main body, it can be unfolded into a flat state due to its resilience. The material of the support is not particularly limited, and various known materials can be used alone or in combination. Examples of such materials include metal, soft polyvinyl chloride resin, polyurethane resin, silicone rubber, natural rubber, synthetic rubber, styrene-ethylene-butylene-styrene copolymer (SEBS), styrene-ethylene-propylene-styrene copolymer (SEPS), ethylene-vinyl acetate copolymer (EVA), polyamide resins and polyamide elastomers such as nylon, polyester resins and polyester elastomers such as polyethylene terephthalate (PET), olefin resins such as polyethylene, fluororubber, fluororesin, etc., and these can be used alone or in combination.
[0034] In the present invention, the term "cylindrical body (tubular body)" refers to a long, pipe-shaped object having an internal space. The cylindrical body has openings at both ends, and the sheet-like delivery material can be delivered to the distal end through the internal space. The cylindrical body is not particularly limited as long as it has sufficient rigidity to protect the sheet-like delivery material from external forces, and any cylindrical body, including commercially available cylindrical bodies, can be used. Examples of cylindrical body materials include, but are not limited to, polyethylene, polypropylene, fluororesin, polyethylene terephthalate, polymethyl methacrylate, polyamide resin, polystyrene, polycarbonate, polyimide, polyetherimide, polyetheretherketone, glass, and metals (e.g., iron, stainless steel, aluminum, copper, and brass).
[0035] The cylindrical body may be made of a flexible material, and in that case, even if the passage through which the cylindrical body is inserted into the body is curved during laparoscopic surgery, thoracoscopic surgery, etc., it can be curved to fit the shape of the passage or bent to adjust the relative angle between the cylindrical body and the target site. The outer diameter of the cylindrical body is not particularly limited, but can be, for example, 2.2 to 25 mm, preferably 5 to 20 mm. The inner diameter of the cylindrical body is not particularly limited, but can be, for example, 2 to 20 mm, preferably 3 to 15 mm.
[0036] In the present invention, the term "release portion" refers to the portion of the sliding body on which the sheet-like delivery is placed, and "low adhesion" refers to low adhesion between the sheet-like delivery and the release portion. The release portion of the sliding body can be made of a low-adhesion material or can be realized by coating the surface with a low-adhesion material. The degree of low adhesion can be quantified, for example, by applying a tensile load to the sheet-like delivery on the release portion and measuring the load at the time of peeling. The peel load is typically 0.0001 N to 0.5 N, preferably 0.001 N to 0.1 N. Those skilled in the art can find the optimal low-adhesion material by examining the adhesion between the sheet-like delivery and the release portion using various materials.
[0037] A low-adhesion release section can also be achieved by processing the portion of the sliding body where the sheet-like delivery is placed. For example, a low-adhesion release section can be achieved by forming multiple protrusions on a portion of the sliding body, and reducing the contact area between the sheet-like delivery and the multiple protrusions. A low-adhesion release section can also be achieved by imparting elasticity to the portion of the sliding body where the sheet-like delivery is placed. That is, for example, when a stretchable release section is folded from the outside to the inside at the tip of the main body, the inwardly pulled release section stretches. As the release section stretches (its area increases), the inextensible sheet-like delivery placed on it detaches from the release section. A stretchable release section can be configured to stretch, for example, 1.1 to 5 times, preferably 1.8 to 4 times, and more preferably 2 to 3 times its original length. Those skilled in the art can identify the optimal stretchable material by testing the adhesion between the sheet-like delivery and the release section using various materials.
[0038] The sizes and materials of the main body, sliding body, convex portions, holes, elongated body, cylindrical body, support, cylindrical body, etc. can be freely selected in consideration of the size and type of the sheet-like delivery article to be delivered to the target site, the number of multiple sheet-like delivery articles to be continuously delivered, the distance to the target site, operability, weight, cost, etc. When the sheet-like delivery article is, for example, a sheet-like cell culture with a diameter of about 60 mm, the diameter of the main body can be 2.2 to 25 mm, preferably 5 to 20 mm, and more preferably 10 to 15 mm.

[0040] First, a first embodiment of the present invention will be described. As shown in FIG. 1 , a medical device M according to the first embodiment of the present invention includes a main body 1 and a sliding body 2. The main body 1 is a long cylindrical body. In this embodiment, the cylindrical body of the main body 1 forms an O-shaped arched portion 11. The inner surface 12 of the main body 1 is a concave curved surface, and the outer surface 13 is a convex curved surface. The sliding body 2 is a long, strip-like body having a cylindrical shape and has an inner diameter larger than the outer diameter of the main body 1. The sliding body 2 has a folded portion 25 at the distal end 14 of the main body 1. The sliding body 2 can slide by folding back the outer surface 13 and the inner surface 12 of the main body 1 through the folded portion 25, and deforms to conform to the outer and inner surface shapes of the main body 1. The distal end 14 of the main body 1 preferably has a smoothly curved surface and an R-shaped cross section to facilitate smooth sliding of the sliding body 2 and reduce wear of the sliding body 2.
[0041] The sliding body 2 can cover the inner surface 12 and the outer surface 13 of the main body 1, for example, by passing it from the base end side (right side in FIG. 1 ) to the outside of the main body 1, pulling it in the distal direction (left direction in FIG. 1 ), folding it back inward at the distal end portion 14 of the main body 1, and pulling the tip of the folded-back portion 25 from the inside of the main body 1 toward the base end. Then, by pulling the inner end portion 21 of the sliding body 2 toward the base end of the main body 1, the sliding body 2 can be slid from the outer surface 13 to the inner surface 12 of the main body 1, or conversely, by pulling the outer end portion 22 of the sliding body 2 toward the base end of the main body 1, the sliding body 2 can be slid so as to fold back from the inner surface 12 to the outer surface 13 of the main body 1.
[0042] An inner end 21 of the sliding body 2 is connected at a connecting portion 31 to an elongated body 3 that can advance and retract in the axial direction inside the main body 1. The elongated body 3 is a cylindrical or rod-shaped member disposed inside the main body 1. An outer end 22 of the sliding body 2 is connected at a connecting portion 41 to a cylindrical body 4 that can advance and retract in the axial direction outside the main body 1. The cylindrical body 4 is a cylindrical member that surrounds the outside of the outer surface 13 of the main body 1. That is, the elongated body 3 and the cylindrical body 4 function as operating portions for operating the sliding body 2, and the sliding body 2 can be slid relative to the main body 1 by pushing them toward the distal end of the main body 1 or pulling them toward the proximal end.
[0043] As shown in Fig. 1, when placing the sheet-like delivery item S in the medical device M, the sheet-like delivery item S is first placed on the support surface 26 of the slider 2 on the outer surface 13 of the main body 1 (placed position). Next, as shown in Fig. 2A, the elongated body 3 is pulled in the proximal direction, thereby drawing the support surface 26 into the inner surface 12 of the main body 1. The support surface 26 is turned upside down from the outside to the inside by folding back at the folding back portion 25 at the distal end 14 of the main body 1, and the sheet-like delivery item S moves onto the inner surface 12 (concave surface) while maintaining its shape. As a result, the sheet-like delivery item S is protected from external forces by the main body 1 (protected position).
[0044] Next, by transperitoneally inserting the medical device M into the body cavity while the sheet-like delivery item S is held on the inner surface 12 of the main body 1, it is possible to bring the sheet-like delivery item S close to the target site T (Fig. 2B) while maintaining its shape. Then, as shown in Fig. 2B, the cylindrical body 4 inserted into the body cavity is grasped with fingers or the like, the long body 3 is pushed into the main body 1, the support surface 26 of the sliding body 2 is exposed from the inside to the outside of the main body 1 (application position), and the sheet-like delivery item S can be peeled off and applied to the target site T.
[0045] When applying multiple sheet-like deliveries S to the target site T, the multiple sheet-like deliveries S can be placed linearly in the sliding direction of the sliding body 2 and operated in the same manner as in Figure 2A, thereby protecting the multiple sheet-like deliveries S on the concave surface (inside) of the main body 1. Then, by operating the medical device M inserted into the body cavity in the same manner as in Figure 2B, the multiple sheet-like deliveries S can be moved from the inside to the outside of the main body 1, and the multiple sheet-like deliveries S can be applied to the target site T.
[0046] The sheet-like delivery item S is applied to the destination T by rotating the main body 1 so that the support surface 26 of the sliding body 2 supporting the sheet-like delivery item S faces the destination T, and then pressing and peeling off the sheet-like delivery item S against the destination T. Alternatively, the sheet-like delivery item S can be peeled off from the sliding body 2 using forceps C or the like and placed on the destination T. When applying multiple sheet-like delivery items S to the destination T, the sheet-like delivery items S can be applied to the destination T by moving them one by one from the inside to the outside of the main body 1.
[0047] The main body 1 may have any shape as long as it has a concave surface that can protect the sheet-like delivery item S from external forces. For example, even if the main body 1 is a plate-like member with a semicircular (C-shaped) or U-shaped cross section and the sliding body 2 is a ribbon-shaped band, the sheet-like delivery item S can be properly protected by using the main body 1 like a spatula to scoop up and place the sheet-like delivery item S while moving the sliding body 2 toward the inner surface 12 (concave surface) of the arch-shaped portion 11. Furthermore, as shown in Fig. 2A, the sheet-like delivery item S can be further properly protected by moving the cylindrical body 4 toward the tip end 14 of the main body 1 to cover it.
[0048] 3A is a conceptual diagram showing a first modified example of the medical device M. In this modified example, the sliding body 2 has a plurality of convex portions 23. When the sliding body 2 is on the outer surface 13 of the main body 1, the diameter of the sliding body 2 is reduced to match the outer diameter of the main body 1, and when the sliding body 2 is on the inner surface 12, the diameter of the sliding body 2 is reduced to match the inner diameter of the main body 1. That is, as shown in FIG. 3B, the sheet-like delivery item S drawn into the inner surface 12 of the main body 1 is gripped with the distance between the convex portions 23 narrowed, so that the sheet-like delivery item S can be more reliably fixed to the sliding body 2.
[0049] FIG. 4A is a conceptual diagram showing a second modified example of the medical device M. In this modified example, the sliding body 2 has a plurality of holes 24 in the support surface 26. Each hole 24 penetrates the sliding body 2 in the thickness direction. The main body 1 ejects the fluid from the holes 24 by feeding the fluid from the side opposite to the support surface 26 supporting the sheet-like delivery item S, i.e., from the gap between the sliding body 2 and the outer surface 13 of the main body 1. By ejecting the fluid, the sliding body 2 can detach the sheet-like delivery item S from the sliding body 2 and drop it at the target site T. The main body 1 has a flow channel 15 that circulates the fluid toward the holes 24. The base end of the flow channel 15 is connected to a valve V. The fluid is introduced into the flow channel 15 from an injection port P on the side of the main body 1. In this embodiment, the fluid enters the gap between the sliding body 2 and the inner surface 12, flows around the tip 14, and flows into the holes 24. In the embodiment shown in FIG. 4A, the flow channel 15 is located on the inner surface 12 side of the main body 1. In this embodiment, when the base end side is closed by the valve V and a fluid is introduced from an injection port P on the side of the main body 1 , the fluid can be sent toward the inner end 21 of the sliding body 2 .
[0050] 4B further includes a cylindrical body 4 that covers the main body 1, and the flow path 15 is located between the outer surface 13 of the main body 1 and the cylindrical body 4. In this embodiment, when the base end side of the cylindrical body 4 is closed with a valve V and a fluid is introduced from an injection port P on the side surface of the cylindrical body 4, the fluid can be sent toward the outer end 22 of the sliding body 2. In this embodiment, the fluid flows into the hole 24 from the gap between the sliding body 2 and the outer surface 13.
[0051] The medical device M of the first embodiment can be used in the following order: (1) Provide the medical device M. (2) Place the sheet-shaped delivery S on the slide 2. (3) Move the sheet-shaped delivery S to the protection position. (4) Bring the medical device M close to the target site T. (5) Move the sheet-shaped delivery S to the application position. (6) Peel off the sheet-shaped delivery S. (7) Apply the sheet-shaped delivery S to the target site T.
[0052] By steps (1) to (5), the sheet-shaped delivery item S can be delivered to the target site T. In (2), the sheet-shaped delivery item S can be placed on the inner or outer sliding body 2 of the main body 1. In (3), the sheet-shaped delivery item S can be covered with a cylindrical body 4. In (4), the medical device M can be inserted into a cylindrical body (e.g., a catheter) inserted transperitoneally into a body cavity, and brought close to the target site T. In (7), the sheet-shaped delivery item S can be applied by dropping it onto the target site T.
[0053] As described above, according to the present invention, a simple mechanism and easy operation can be used to efficiently deliver the sheet-shaped delivery item S to the target location T while maintaining the shape of the sheet-shaped delivery item S, thereby offering great advantages in terms of workability and manufacturing costs.
[0054] (Second Embodiment) A medical device M according to a second preferred embodiment of the present invention will now be described in detail with reference to the drawings. As shown in Fig. 5A, the medical device M according to the second embodiment of the present invention includes a main body 1 and a ribbon-shaped sliding body 2. The sliding body 2 has a peeling portion 27 for supporting the sheet-like delivery item S. The peeling portion 27 is a portion of the sliding body 2 that supports the sheet-like delivery item S, and its surface has low adhesion. When placing the sheet-like delivery item S in the medical device M, first, the peeling portion 27 of the sliding body 2 is moved to the outer surface 13 of the arch-shaped portion 11 of the main body 1 (placement position), and the sheet-like delivery item S is placed on the peeling portion 27.
[0055] As shown in Figure 5B, by pulling the elongated body 3 in the proximal direction, the peeling portion 27 is pulled into the inner surface 12 of the main body 1. The peeling portion 27 is folded back at the distal end 14 of the main body 1 and turned upside down from the outer surface 13 to the inner surface 12, while the sheet-like delivery item S protrudes distally from the distal end 14 of the main body 1 without being turned back. That is, the sheet-like delivery item S held in an arch shape on the outer surface 13 moves distally together with the peeling portion 27, and when the peeling portion 27 is folded back at the distal end 14, the sheet-like delivery item S is released from the low-adhesion peeling portion 27. The arch-shaped sheet-like delivery item S, which has a certain strength, protrudes distally from the main body 1 while maintaining its shape (application position). To impart a certain strength to the sheet-like delivery item S, it may be reinforced by applying fibrin or the like to the sheet-like delivery item S.
[0056] That is, as shown in Fig. 5B, the sheet-like delivery item S is peeled off from the sliding body 2 and protrudes toward the tip, so that it can be easily grasped (clamped) with forceps C or the like and applied to the destination T. Also, as shown in Fig. 5C, the main body 1 can be rotated so that the sheet-like delivery item S faces the destination T, and the sheet-like delivery item S can be placed at the destination T, or the front end 14 of the main body 1 (sheet-like delivery item S) can be pressed against the destination T without rotating. The sheet-like delivery item S can be further protruded from the state shown in Fig. 5B, detached from the sliding body 2, and dropped into the destination T.
[0057] Contrary to Fig. 5A, the sheet-like delivery S can also be placed with the peeling portion 27 of the slider 2 moved to the inner surface 12 of the arch-shaped portion 11 of the main body 1. That is, if the arch-shaped portion 11 of the main body 1 is partially open, such as in a U-shape or C-shape, the sheet-like delivery S can be scooped up and placed using the main body 1 like a spatula with the peeling portion 27 moved to the inner surface 12 (loading position). This protects the sheet-like delivery S on the inner surface 12 (protecting position), and the arch-shaped sheet-like delivery S can be pushed out toward the front end of the main body 1 by sliding the peeling portion 27 from the inside to the outside of the main body 1 (applied position). If the main body 1 is partially open, the slider 2 can be connected to the cylindrical body 4 (Fig. 1) and the cylindrical body 4 can be moved toward the front end of the main body 1 to more appropriately protect the sheet-like delivery S.
[0058] The medical device M of the present invention can be used in the following order: (1) Provide the medical device M. (2) Place the sheet-shaped delivery item S on the peeling section 27. (3) Bring the medical device M close to the target site T. (4) Protrude the sheet-shaped delivery item S toward the tip of the main body 1. (5) Apply the sheet-shaped delivery item S to the target site T.
[0059] The sheet-like delivery item S can be delivered to the target site T through steps (1) to (4). In step (2), the sheet-like delivery item S can be placed on the peeling section 27 inside or outside the main body 1. When the sliding body 2 is connected to the cylindrical body 4, in step (3), the cylindrical body 4 can be moved toward the tip of the main body 1 to cover the sheet-like delivery item S. In step (3), a medical device M can be inserted into the cylindrical body 4 transperitoneally into a body cavity to bring the medical device M close to the target site T. A step (6) of detaching the sheet-like delivery item S from the sliding body 2 may be included between steps (4) and (5). In step (5), the sheet-like delivery item S can be applied by dropping it onto the target site T.
[0060] As described above, according to the present invention, it is possible to efficiently deliver the sheet-shaped delivery item S to the target site T while maintaining its shape using a simple mechanism and simple operations, which offers great advantages in terms of workability and manufacturing costs.
[0061] (Third Embodiment) A medical device M according to a preferred third embodiment of the present invention will be described in detail below with reference to the drawings. As shown in FIG. 6 , the medical device M according to the third embodiment of the present invention includes a cylindrical main body 1, a sliding body 2A, an elongated body 3, a tubular body 4, and a support 5A. The elongated body 3 and the tubular body 4 are similar to the elongated body 3 and the tubular body 4 described with reference to FIG. 1 . The sliding body 2A is a ribbon-shaped band extending in the axial direction of the main body 1. The sliding body 2A has an outer portion 28 that covers the outer surface 13 of the main body 1 and an inner portion 29 that covers the inner surface 12. The distal end of the outer portion 28 of the sliding body 2A and the distal end of the inner portion 29 are integrally connected via a distal end folded portion 30a located at the distal end 14 of the main body 1. Furthermore, the proximal end of the outer portion 28 of the sliding body 2A and the proximal end of the inner portion 29 are integrally connected via a proximal end folded portion 30b located at the proximal end 16 of the main body 1. That is, the sliding body 2A has a structure in which an outer portion 28 and an inner portion 29 are connected in a ring shape like a belt conveyor.
[0062] The sliding body 2A has a connection portion 31 for connecting to the elongated body 3 at a part of its inner portion 29. The inner portion 29 of the sliding body 2A moves integrally with the elongated body 3 via the connection portion 31. Therefore, by moving the elongated body 3 back and forth relative to the main body 1, the outer portion 28 and inner portion 29 of the sliding body 2A can be slid toward the distal end or the proximal end. That is, when the elongated body 3 is moved back toward the proximal end from the illustrated state, the inner portion 29 slides toward the proximal end together with the elongated body 3. When the inner portion 29 slides toward the proximal end, the outer portion 28, which is connected to the inner portion 29 via the distal fold portion 30a and the proximal fold portion 30b, slides toward the distal end. When the elongated body 3 is moved forward toward the distal end, the outer portion 28 and inner portion 29 slide in the opposite direction.
[0063] The support 5A has a connecting portion 51 at one end. The connecting portion 51 is a tongue-like portion extending from the support 5A and is joined to the sliding body 2A. The support 5A is connected to the sliding body 2A via the connecting portion 51. The support 5A slides on the outer surface 13 and inner surface 12 of the main body 1 so as to follow the sliding of the sliding body 2A. As shown in the figure, the support 5A is made of a flexible sheet body. When the support 5A is protruded toward the tip end of the main body 1, it unfolds into a flat surface as shown in the figure, and the sheet-like delivery item S can be placed on that surface.
[0064] The following describes how to use the medical device M of this embodiment. First, as shown in Figure 6, the support 5A is extended toward the tip of the main body 1, and the sheet-like delivery item S is placed on the surface of the support 5A. When placing the sheet-like delivery item S, the support 5A on which the sheet-like delivery item S is placed can be attached to the sliding body 2A.
[0065] Next, as shown in Fig. 7A, an operation of pulling the elongated body 3 toward the proximal end is performed. When this operation is performed, the inner portion 29 of the sliding body 2A slides toward the proximal end, and the support body 5A is pulled into the inner surface 12 of the arch-shaped portion 11 of the main body 1. The support body 5A is pressed against the distal end 14 of the main body 1, causing it to curve, and is stored in the inner surface 12 of the main body 1 with the sheet-like delivery item S supported inside. Next, with the sheet-like delivery item S held on the inner surface 12 of the main body 1, the medical device M is inserted transperitoneally into the body cavity. The sheet-like delivery item S is efficiently placed near the target site T while its shape is maintained by the main body 1.
[0066] Then, as shown in Fig. 7B, an operation is performed to push the elongated body 3 connected to the sliding body 2A into the main body 1. This operation causes the support 5A to protrude toward the tip of the main body 1 and unfold. The sheet-like delivery item S placed on the surface of the support 5A is applied to the target site T (application position). Furthermore, as shown in Fig. 7C, an operation may be performed to push the elongated body 3 further toward the tip of the main body 1. This operation causes one end (connection portion 51) of the support 5A connected to the sliding body 2A to be inverted outward from the arch-shaped portion 11 of the main body 1, allowing the support 5A to be rotated onto the outer surface 13 of the main body 1 (application position).
[0067] As shown in Fig. 7B, when the support 5A is on the outer surface 13 of the main body 1, the sheet-like delivery item S faces the target location T, making operation simple and eliminating the need to rotate the main body 1. The support 5A may have a hole 24 (see Fig. 4A), and the sheet-like delivery item S can be detached from the support 5A and dropped at the target location T by ejecting fluid through the hole 24 from the side opposite to the surface of the support 5A supporting the sheet-like delivery item S.
[0068] 8A to 9B are conceptual diagrams showing a first modified example of the medical device M of the third embodiment. In this modified example, the support 5B is flexible and has connection portions 51 at both the distal and proximal ends in the axial direction. That is, the distal and proximal ends of the support 5B are connected to the sliding body 2A. As shown in FIG. 8A, when placing the sheet-like delivery item S, the sliding body 2A (support 5B) is moved onto the outer surface 13 of the main body 1 to place it (placement position). Next, as shown in FIGS. 8B and 8C, the sliding body 2A connected to the support 5B is retracted into the inner surface 12 of the main body 1, so that the sheet-like delivery item S is protected on the inner surface 12 (protection position).
[0069] When the medical device M is transperitoneally inserted into a body cavity with the sheet-like delivery item S held on the inner surface 12 of the main body 1, the sheet-like delivery item S maintains its shape and is efficiently placed near the target site T. Then, as shown in Figures 9A and 9B, when the elongated body 3 is pushed toward the tip of the main body 1, the support 5B is inverted onto the outer surface 13 of the main body 1 (application position), and the sheet-like delivery item S is peeled off from the support 5B and applied to the target site T.
[0070] 7C, the flexible support 5A makes a large turn as indicated by the arrow, whereas in Fig. 8B and Fig. 9A, when the sliding body 2A is folded back at the tip 14 of the main body 1 (tip folded back portion 30a), the flexible support 5B makes a small turn (folded back) as indicated by the arrow in response to the folding back of the sliding body 2A at the tip 14 of the main body 1. By making a small turn of the support 5B, it is possible to prevent problems such as the support 5B accidentally damaging the target site T.
[0071] The medical device M of the third embodiment can be used in the following order: (1) Provide the medical device M. (2) Place the sheet-shaped delivery material S on the supports 5A and 5B. (3) Move the supports 5A and 5B to the protection position. (4) Bring the medical device M close to the target site T. (5) Move the sheet-shaped delivery material S to the application position. (6) Peel off the sheet-shaped delivery material S. (7) Apply the sheet-shaped delivery material S to the target site T.
[0072] By steps (1) to (5), the sheet-like delivery item S can be delivered to the target site T. In (2), the supports 5A and 5B on which the sheet-like delivery item S is placed may be attached to the sliding body 2A, or the sheet-like delivery item S may be placed on the support 5A attached to the sliding body 2A with the support 5A protruding from the tip side of the main body 1. In (3), the supports 5A and 5B may be covered with a cylindrical body 4. In (4), the medical device M may be inserted into a cylindrical body (e.g., a catheter) inserted transperitoneally into a body cavity, and brought close to the target site T.
[0073] In (6), the sheet-like delivery object S can be peeled off from the supports 5A and 5B by discharging fluid from holes 24 provided in the supports 5A and 5B. In (7), the sheet-like delivery object S can be applied by dropping it onto the target site T. Between (5) and (6), a step (8) of grasping the supports 5A and 5B with forceps C or the like and removing them from the sliding body 2A can be included. In (7), the removed supports 5A and 5B can be dropped onto the target site T to apply the sheet-like delivery object S.
[0074] When one end is connected by a connecting portion 51 as in the case of the support 5A, the application position in (5) can be a state in which the support 5A protrudes toward the tip of the main body 1 (FIG. 7B) or a state in which the support 5A is rotated onto the outer surface 13 of the main body 1 (FIG. 7C). A flow path may be provided inside the support 5A, and a hole 24 for discharging a fluid may be provided in the support 5A. When discharging a fluid from the hole 24, it is preferable to do so with the support 5A protruding toward the tip of the main body 1. When both ends are connected by two connecting portions 51 as in the case of the support 5B, the application position is a state in which the support 5B is rotated onto the outer surface 13 of the main body 1 (FIG. 9B), and the sheet-like delivery item S can be applied by pressing the support 5B side of the main body 1 against the target site T.
[0075] As described above, according to the present invention, a simple mechanism and simple operation can be used to efficiently deliver the sheet-shaped delivery item S to the target site T while maintaining its shape, which offers great advantages in terms of workability and manufacturing costs.
[0076] Although the present invention has been described above with reference to the illustrated embodiments, the present invention is not limited to these. The configurations of the first to third embodiments can be combined as appropriate. In the present invention, each configuration can be replaced with any other configuration that can perform the same function, or any other configuration can be added.
[0077] As a modified example, for example, as shown in Fig. 10A, the outer periphery of the sliding body 2 can be made expandable so as to be larger than the diameter of the sheet-like delivery material S, thereby allowing the sheet-like delivery material S to be placed or applied. Then, as shown in Fig. 10B, by contracting the sliding body 2 to reduce its outer diameter, a minimally invasive medical device M can be realized. Since fluid needs to be injected to expand the sliding body 2, valves V are provided in the elongated body 3 and the cylindrical body 4, and an injection port P is provided in either the main body 1, the elongated body 3, or the cylindrical body 4.
[0078] As a modified example, in order to efficiently accommodate the sliding body 2, whose diameter is significantly larger than the outer diameter of the main body 1, and the large-area sheet-like delivery item S placed thereon on the inner surface 12 of the main body 1 (FIG. 10C), as shown in FIG. 11B, by providing a jagged, wavy unevenness 17 on the tip 14 of the main body 1, the sliding body 2 can be accommodated so as to wrinkle in a star shape on the inner surface 12 of the main body 1, as shown in FIG. 11A. A similar effect can also be achieved by making the cross section of the inner surface 12 of the main body 1 jagged (not shown).
