Abstract
Description
[Technical field]
[0001] The present invention relates to compounds that induce the production of proteins, in particular interleukin-10 (IL-10), by immune cells and their use in the prevention and / or treatment of immune mediated diseases, in particular neurodegenerative diseases, inflammatory diseases, allergic diseases, adverse events associated with transplantation, autoimmune diseases, metabolic diseases with an inflammatory component, inflammatory responses having detrimental effects on the risk of cancer development or progression, and ageing. [Background technology]
[0002] Immune-mediated diseases are chronic inflammatory diseases perpetuated by antibodies and cellular immunity. The immune response damages healthy organs either accidentally as a result of attacking foreign components that have invaded the body, or by attacking self-tissues, a process called autoimmunity. These diseases include many forms of arthritis (e.g., rheumatoid arthritis and psoriatic arthritis), inflammatory bowel disease (e.g., ulcerative colitis and Crohn's disease), endocrine disorders (e.g., type 1 diabetes and Graves' disease), neurodegenerative diseases (e.g., multiple sclerosis, autism spectrum disorders, Alzheimer's disease, amyotrophic lateral sclerosis (ALS), Parkinson's disease, Huntington's disease, Guillain-Barre syndrome, myasthenia gravis, and chronic idiopathic demyelinating diseases (CID)), and vascular diseases (e.g., autoimmune hearing loss, systemic vasculitis, and atherosclerosis). These diseases are not uncommon and have a large socio-economic impact.
[0003] Currently, the main focus of treatment is to suppress immunity and inhibit inflammation by using immunosuppressants, blockers of cytokine or cytokine receptor signaling, or by temporary depletion of leukocyte subsets. However, these approaches do not take into account the need to restore dominant immune regulation, and are often incomplete in restoring a healthy and stable immune system in these pathologies. The majority of patients treated with these drugs do not respond, or become resistant after a period of time, or relapse when treatment is stopped. [0084] The term "pharmaceutical formulation" refers to a preparation that is in a form that allows the biological activity of the active ingredients contained therein to be effective and that does not contain additional ingredients that are unacceptably toxic to the subject to which the formulation will be administered.
[0085] "Pharmaceutically acceptable carrier" refers to an ingredient, other than an active ingredient, in a pharmaceutical formulation that is non-toxic to a subject. Pharmaceutically acceptable carriers include, but are not limited to, buffers, excipients, stabilizers, or preservatives.
[0086] In some embodiments, the subject may be a human, male or female, regardless of age. In some embodiments, the patient may be a non-human animal, advantageously a non-human mammal, such as a dog, cat, mouse, rat, hamster, rabbit, chinchilla, horse, cow, pig, sheep, goat, or primate.
[0087] In some embodiments, the pharmaceutical composition comprises a dose of immune cells induced by at least one compound of formula (I) as defined above and at least one pharma- ceutically acceptable carrier.
[0088] In some embodiments, the pharmaceutical composition comprises a dose of immune cells induced by a compound of formula (I) and at least one pharma- ceutically acceptable carrier, wherein the compound of formula (I) is selected from the group consisting of the following compounds:
[0089] [ka]
[0090] It is one of them.
[0091] Carrier In some embodiments, the immune cells are formulated with a pharma- ceutically acceptable carrier. In some aspects, the choice of carrier is determined in part by the particular cells or agents and / or by the method of administration. Thus, there are a variety of suitable formulations. For example, the pharmaceutical composition may contain a preservative. Suitable preservatives may include, for example, methylparaben, propylparaben, sodium benzoate, and benzalkonium chloride. In some aspects, a mixture of two or more preservatives is used. The preservative or mixtures thereof are typically present in an amount of about 0.0001% to about 2% by weight of the total composition. Carriers are described, for example, by Remington's Pharmaceutical Sciences 16th edition, Osol, A. Ed. (1980). Pharmaceutically acceptable carriers are generally nontoxic to recipients at the dosages and concentrations employed and include, but are not limited to, buffers, such as phosphates, citrates, and other organic acids; antioxidants including ascorbic acid and methionine; preservatives (such as octadecyldimethylbenzyl ammonium chloride; hexamethonium chloride; benzalkonium chloride; benzethonium chloride; phenol, butyl or benzyl alcohol; alkyl parabens, such as methyl or propyl paraben; catechol; resorcinol; cyclohexanol; 3-pentanol; and m-cresol); low molecular weight (less than about 10 residues) amines, such as ethyl esters, butyl ... proteins, such as serum albumin, gelatin, or immunoglobulins; hydrophilic polymers, such as polyvinylpyrrolidone; amino acids, such as glycine, glutamine, asparagine, histidine, arginine, or lysine; monosaccharides, disaccharides, and other carbohydrates, including glucose, mannose, or dextrins; chelating agents, such as EDTA; sugars, such as sucrose, mannitol, trehalose, or sorbitol; salt-forming counterions, such as sodium; metal complexes (e.g., Zn-protein complexes); and / or non-ionic surfactants, such as polyethylene glycol (PEG).

[0093] The formulation may comprise an aqueous solution. The formulation or composition may contain more than one active ingredient useful for a particular indication, disease or condition treated with induced immune cells, where the respective activities do not adversely affect each other. Such second active ingredients are suitably present in an amount that, in combination, is effective for the intended purpose. In an advantageous embodiment, the pharmaceutical composition according to the invention also comprises at least one other active ingredient. In a particularly advantageous embodiment of the invention, at least one other active ingredient may synergistically interact with the immune cells induced by at least one compound of formula (I) according to the invention.
[0094] Examples of other active ingredients include, but are not limited to, estrogen receptor modulators; androgen receptor modulators; glucocorticoids, retinoid receptor modulators; cytotoxic drugs; antiproliferative drugs including adriamycin, dexamethasone, vincristine, cyclophosphamide, fluorouracil, topotecan, taxol, interferons, or platinum derivatives; corticosteroids, TNF blockers, IL-1 Anti-inflammatory drugs including RA, azathioprine, cyclophosphamide, JAK inhibitors (e.g. tofacitinib and baricitinib), SYK inhibitors, BTK inhibitors, or sulfasalazine; prenyl-protein transferase inhibitors; HMG-CoA reductase inhibitors; HIV protease inhibitors; reverse transcriptase inhibitors; angiogenesis inhibitors including sorafenib, sunitinib, pazopanib, or everolimus; immunomodulatory or immunosuppressive drugs including cyclosporine, tacrolimus, rapamycin, mycophenolate mofetil, interferons, corticosteroids, cyclophosphamide, azathioprine, or sulfasalazine; PPAR-gamma agonists including thiazolidinediones; PPAR-delta agonists; inhibitors of multidrug resistance;agents for treating anemia, including erythropoiesis stimulating factors, vitamins, or iron supplements;antiemetics, including 5-HT3 receptor antagonists, dopamine antagonists, NK1 receptor antagonists, H1 histamine receptor antagonists, cannabinoids, benzodiazepines, anticholinergics, or steroids;agents for treating neutropenia;immunostimulants;proteasome inhibitors;HDAC inhibitors;inhibitors of chemotrypsin-like activity in the proteasome;E3 ligase inhibitors;interferon-alpha, which may induce release of cytokines, interleukins, TNF, or death receptor ligands, including TRAIL, Bacillus Calmette-Guerin Modulators of the immune system, including anti-inflammatory drugs such as anti-inflammatory drugs (Anti-inflammatory drugs), ...Antiparkinsonian drugs including anticholinergics, or dopaminergics including dopamine aminergic precursors, monoamine oxidase B inhibitors, COMT inhibitors, dopamine receptor agonists; drugs for treating cardiovascular disease including beta-blockers, ACE inhibitors, diuretics, nitrates, calcium channel blockers, or statins; drugs for treating liver disease including corticosteroids, cholestyramine, or interferons; antiviral drugs including nucleoside reverse transcriptase inhibitors, non-nucleoside reverse transcriptase inhibitors, protease inhibitors, integrase inhibitors, fusion inhibitors, chemokine receptor antagonists, polymerase inhibitors, viral protein synthesis inhibitors, viral protein modification inhibitors, neuraminidase inhibitors, fusion or entry inhibitors; Agents for treating blood disorders, including thyrosteroids, anti-leukemias, or growth factors; agents for treating immune deficiency disorders, including gamma globulins, adalimumab, etanercept, tocilizumab, or infliximab; HMG-CoA reductase inhibitors, including torvastatin, fluvastatin, lovastatin, pravastatin, rosuvastatin, simvastatin, or pitavastatin, or in combination or sequence with radiation, or at least one chemotherapeutic agent, such as asparaginase, busulfan, carboplatin, cisplatin, daunorubicin, doxorubicin, fluorouracil, gemcitabine, hydroxyurea, methotrexate, paclitaxel, rituximab, vinblastine;
[0095] In some embodiments, when the pharmaceutical composition comprises another active ingredient, the pharmaceutical composition is adapted for simultaneous or sequential administration of the immune cells induced by at least one compound of formula (I) as defined above and the other active ingredient.
[0096] Within the meaning of the present invention, "co-administration" means the administration of immune cells induced by at least one compound of formula (I) as defined above and other active ingredients at the same time, contemporaneously to a patient in therapeutically effective amounts to allow the synergistic effect of the pharmaceutical composition. This does not necessarily mean that immune cells induced by at least one compound of formula (I) as defined above and other active ingredients must be administered in the form of a mixture; they can in fact be administered simultaneously, but separately, in the form of separate compositions.
[0097] "Existing in two separate compositions" means that the immune cells induced by at least one compound of formula (I) as defined above and other active ingredients are physically separate, and then they are packaged and administered separately in multiple (at least two) dosage forms (e.g., two separate compositions) without prior mixing, in a therapeutically effective amount that allows the synergistic effect of the pharmaceutical composition.
[0098] In some embodiments, the immune cells induced by at least one compound of formula (I) as defined above and other active ingredients may be present in one and the same composition in a therapeutically effective amount that allows the synergistic effect of the pharmaceutical composition. Within the meaning of the present invention, "present in one and the same composition" means the physical combination of the immune cells induced by at least one compound of formula (I) as defined above and other active ingredients. Thus, the immune cells induced by at least one compound of formula (I) as defined above and other active ingredients should be administered simultaneously, since they are administered together in the form of a mixture, in one and the same dosage form, in a therapeutically effective amount that allows the synergistic effect of the pharmaceutical composition.
[0099] Within the meaning of the present invention, the expression "sequential administration" means that the immune cells induced by at least one compound of formula (I) as defined above and the other active ingredient are not administered simultaneously, but separately from each other over time, in a therapeutically effective amount that allows a synergistic effect of the pharmaceutical composition. The terms "preceding" or "preceding" and "following" or "subsequently" therefore apply. The term "preceding" or "preceding" is used when the components of the pharmaceutical composition according to the invention are administered minutes or hours, or even days before the administration of the other components of the pharmaceutical composition. Conversely, the term "following" or "subsequently" is used when the components of the pharmaceutical composition according to the invention are administered minutes or hours, or even days after the administration of the other components of the pharmaceutical composition. In one particularly advantageous embodiment of the present invention, the immune cells induced by at least one compound of formula (I) as defined above are administered several days before the other active ingredient.
[0100] In some embodiments, when the pharmaceutical composition comprises another active ingredient, the pharmaceutical composition is adapted for sequential administration of immune cells induced by at least one compound of formula (I) as defined above and the other active ingredient.
[0101] In some embodiments, the pharmaceutical composition comprises the induced immune cells in an amount effective for treating a disease or condition, e.g., a therapeutically effective or prophylactically effective amount. Within the meaning of the present invention, the term "therapeutically effective" or "treatment effective amount" or "pharmaceutical effective" refers to the amount of immune cells induced by at least one compound of formula (I) as defined above necessary to inhibit or reverse a disease, advantageously to treat a disease selected from neurodegenerative diseases, inflammatory diseases, immune-mediated diseases, allergic diseases, adverse events associated with transplantation, autoimmune diseases, metabolic diseases with an inflammatory component, inflammation promoting cancer development or progression, and aging. The determination of a therapeutically effective amount depends specifically on factors such as toxicity and efficacy of the pharmaceutical. These factors will vary depending on other factors such as activity, relative bioavailability, patient weight, severity of adverse effects, and preferred route of administration. Toxicity can be determined by using methods well known in the art. The same applies to efficacy. As a result, a pharmaceutical effective amount is an amount that is toxicologically acceptable and deemed effective by the clinician. In some embodiments, therapeutic effectiveness is monitored by periodic evaluation of the subject being treated. For repeated administration over several days or longer, depending on the condition, treatment is repeated until the desired suppression of disease symptoms occurs. However, other administration regimes may be useful and can be determined. The desired dosage can be delivered by a single bolus administration of the composition, by multiple boluses of the composition, or by continuous infusion administration of the composition.
[0102] The induced immune cells of the present invention can be administered using standard administration techniques, formulations, and / or devices. Formulations and devices, such as syringes and vials, are provided for storing and administering the compositions. With respect to cells, administration can be autologous or xenogeneic. For example, immunoresponsive cells or progenitor cells can be obtained from one subject and administered to the same subject or a comparable different subject. Peripheral blood-derived immunoresponsive cells or their progeny (e.g., derived in vivo, ex vivo, or in vitro) can be administered by local injection, including catheter administration, systemic injection, local injection, intravenous injection, or parenteral administration. When a therapeutic composition (e.g., a pharmaceutical composition comprising the induced immune cells of the present invention) is administered, it is generally formulated in a unit dosage injectable form (solution, suspension, emulsion).
[0103] Formulations include those for oral, intravenous, intraperitoneal, subcutaneous, pulmonary, transdermal, intradermal, intratumoral, intralesional, intramuscular, intranasal, buccal, sublingual, topical, local, auricular, inhalation, retinal, intraocular, periocular, subretinal, intravitreal, or suppository administration, septal, subscleral, intrachoroidal, intracameral, subconjectval, subconjuntival, subtenon, retrobulbar, peribulbar, or posterior juxtascleral administration. The term "parenteral" as used herein includes intravenous, intramuscular, subcutaneous, rectal, vaginal, and intraperitoneal administration. In some embodiments, the agent or cell population is administered to a subject using peripheral systemic delivery by intravenous, intraperitoneal, or subcutaneous injection.
[0104] The composition in some embodiments is provided as a sterile liquid preparation, such as an isotonic aqueous solution, suspension, emulsion, dispersion, or viscous composition, which in some aspects may be buffered to a selected pH. Liquid preparations are usually easier to prepare than gels, other viscous compositions, and solid compositions. In addition, liquid compositions are more or less convenient to administer, especially by injection. On the other hand, viscous compositions can be formulated within a suitable viscosity range to provide a longer contact period with specific tissue. The liquid or viscous composition may contain a carrier, which may be a solvent or dispersion medium, such as water, saline, phosphate buffered saline, polyol (e.g., glycerol, propylene glycol, liquid polyethylene glycol), and suitable mixtures thereof.
[0105] Sterile injectable solutions can be prepared by incorporating the cells into a vehicle, such as by mixing with a suitable carrier, diluent, or excipient, such as sterile water, saline, glucose, dextrose, or the like.
[0106] Formulations to be used for in vivo administration are generally sterile. Sterility may be readily accomplished, for example, by filtration through sterile filtration membranes.
[0107] dosage In some embodiments, a dose of the induced immune cells of the present invention is administered to a subject in accordance with the provided methods and / or the provided products or compositions. In some embodiments, the size or timing of the dose is determined according to the specific disease or condition in the subject. In some cases, the size or timing of the dose for a specific disease can be empirically determined in view of the provided explanation.
[0108] In some embodiments, the dose of the induced immune cells of the present invention is 2×10 cells. 5 (or its approximation) / kg to 9 × 10 cells 14 (or an approximation thereof) / kg, advantageously between 2×10 cells 5 (or its approximation) / kg to 9 × 10 cells 13(or an approximation thereof) / kg, advantageously between 2×10 cells 5 (or its approximation) / kg to 9 × 10 cells 12 (or an approximation thereof) / kg, advantageously between 2×10 cells 5 (or its approximation) / kg to 9 × 10 cells 11 (or an approximation thereof) / kg, advantageously between 2×10 cells 5 (or its approximation) / kg to 9 × 10 cells 10 (or an approximation thereof) / kg, advantageously between 2×10 cells 5 (or its approximation) / kg to 9 × 10 cells 9 (or its approximation) / kg, advantageously between 2×10 5 cells (or approximate number) / kg to 9 x 10 cells 8 (or its approximation) / kg.
[0109] In some embodiments, the dose of induced immune cells of the invention is at least, at least about, or at or about 2×10 cells (e.g., IL10-expressing cells) per kilogram of body weight of the subject. 5 (cells / kg), e.g., at least that many, at least about that many, or at or about 3×10 cells. 5 / kg, at least that many, at least that many, or at or near that many, or 4 x 10 cells 5 / kg, many, or at or near that many 11 / kg, dosages may vary depending on the disease or disorder and / or patient and / or other treatment specific attributes. In some embodiments, the dose of cells is a flat dose of cells or a fixed dose of cells, such that the dose of cells is not tied to or based on the subject's body surface area or weight.
[0110] In some embodiments, the dose of induced immune cells of the present invention is administered to a subject as a single dose, or only once within a period of two weeks, one month, three months, six months, one year or longer.
[0111] In the context of adoptive cell therapy, administration of a given "dose" includes administration of a given amount or number of cells as a single composition and / or a single continuous administration, e.g., as a single injection or continuous infusion, and also includes administration of a given amount or number of cells as split doses provided in multiple separate compositions or infusions over a specified period of time, e.g., up to 3 days, or as multiple compositions. Thus, in some situations, a dose is a single or continuous administration of a specified number of cells given or initiated at a single time point. However, in some situations, a dose is administered in multiple injections or infusions over a period of up to 3 days, e.g., once a day for 3 or 2 days, or by multiple infusions over a period of 1 day.
[0112] Thus, in some aspects, the dose of cells is administered in a single pharmaceutical composition, hi some embodiments, the dose of induced immune cells is administered in multiple compositions that together contain the dose of cells.
[0113] In some embodiments, the term "split dose" refers to a dose that is divided to be administered over more than one day. This type of dosing is encompassed by the present methods and is considered to be a single dose.
[0114] Thus, the dose of induced immune cells can be administered as split doses, e.g., split doses administered over time. For example, in some embodiments, the dose can be administered to the subject over two days or over three days. An exemplary method for split administration includes administering 25% of the dose on day 1 and administering the remaining 75% of the dose on day 2. In other embodiments, 33% of the dose can be administered on day 1 and the remaining 67% on day 2. In some embodiments, 10% of the dose is administered on day 1, 30% of the dose is administered on day 2, and 60% of the dose is administered on day 3. In some embodiments, the split dose is not divided over more than three days.
[0115] In some embodiments, administration of a composition or dose, e.g., administration of multiple cellular compositions, involves administration of the cellular compositions separately.
