[0059] In certain embodiments, device can be completely sealed along the edge of the device, thereby forming a completely encapsulated interior space or lumen. In other embodiments, device can be opened at one or more positions at the edge of the device to allow entry into the interior space or lumen of the device. When device includes two openings for entering the lumen of the device, two openings can be positioned relative to each other, such as, on the opposite sides of a planar device. In some cases, device of the present disclosure does not include a support or framework. For example, the first polymer layer and the second polymer layer can be laid on each other and sealed along the periphery of the layer. [0060] In some aspects, cells can be loaded into the lumen of the device prior to implantation in a subject. In some cases, after the cells have been loaded into the lumen of the device, the cells can be embedded within the macroporous scaffold of the second polymer layer ( Figure 4). In other aspects, the device (wherein no cells are loaded) can be implanted in a subject. After the device has been implanted for a period of time, the device can be subsequently loaded with cells. The second end of the pipeline at the far side of the first end can be used as a port so that the fluid is introduced into the lumen via the pipeline. The length of the pipeline can be selected based on a variety of factors. For example, when the cell colony is loaded into the lumen of the device in vitro, for example, before the device is placed in the experimenter, a shorter pipeline can be used. If the pipeline will be used to introduce fluid into the lumen of the device and / or remove fluid from the lumen of the device after the device is placed in the experimenter, a longer pipeline can be used. In some cases, after the device has been implanted in the experimenter, the port can be accessible so that the device can be pre-implanted and no cell is accommodated in the lumen, and the lumen can be loaded with cells when implanting a period of time later. In some aspects, the device can be pre-loaded with supporting cells or factors (for example, growth factors) before the introduction of therapeutic cells.
[0061] The device herein can be designed so that therapeutic molecules (e.g., those produced or secreted by therapeutic cells) can diffuse out of the lumen or internal compartment of the device. In addition, the device can allow molecules present outside the device to penetrate into the lumen or internal compartment of the device. In some cases, the device can allow oxygen to diffuse into and / or diffuse out of the device. In some aspects, the device can promote blood vessel growth into or around the implantation site of the device (e.g., promote vascularization of the device).
[0062] When the implantable device that holds cells is enclosed in the cell-impermeable layer and isolated relative to the immune system, the implantable device often stimulates a local inflammatory reaction called foreign body reaction (FBR), which has long been considered to limit the function of the implanted device that needs solute transport. FBR has been fully described in the literature and is composed of three main layers. The innermost layer of the FBR adjacent to the implanted device is composed of macrophages and foreign body giant cells. These cells form a single layer of cells that are closely relative to the surface of the implanted device. The FBR middle layer located at the far side of the first layer relative to the device is a wider area (30-100 microns) mainly composed of fibroblasts and fibrous matrix. The outermost layer of the FBR is a loose granular connective tissue comprising new blood vessels. Once FBR is induced, the implanted device is isolated from the internal environment, thereby limiting the molecular exchange with the implanted device, and also limiting the effectiveness of the implanted device, causing the death of any cell provided in the implanted device. In some cases, the devices of the present disclosure may not provoke a foreign body response, may not provoke a significant foreign body response, or may provoke a limited foreign body response such that cells encapsulated within the device remain viable for a period of time. In some cases, the foreign body response may be evidenced by fibrosis around the implanted device. In some cases, the devices of the present disclosure may not provoke fibrosis around the implanted device, may not provoke significant fibrosis, or may provoke limited fibrosis. Fig. 8A and Figure 8B Depicting a device of the present disclosure showing the absence of fibrosis surrounding the implanted device.
[0063] In some aspects, a device disclosed herein, once implanted into a subject, can support the survival of cells present in the lumen of the device for at least one week, two weeks, three weeks, four weeks, one month, at least 2 months, at least 3 months, at least 4 months, at least 5 months, at least 6 months, at least 7 months, at least 8 months, at least 9 months, at least 10 months, at least 11 months, at least 12 months, at least 2 years, at least 3 years, at least 4 years, at least 5 years, or longer.
[0064] In some aspects, the device disclosed herein can be substantially impermeable to cells present outside the device, so that cells present outside the device cannot enter the device. For example, in some cases, the device is substantially impermeable to immune cells, so that immune cells present outside the device cannot enter the lumen of the device. In some aspects, the device disclosed herein can be substantially impermeable to molecules present outside the device, so that the molecules cannot enter the device. For example, in some cases, the device is substantially impermeable to immunoglobulins, so that the concentration of any immunoglobulin entering the lumen of the device is lower than the level required for the immune response of cells present in the lumen of the device. In another example, the device disclosed herein can substantially limit cytokines from entering the lumen of the device, so that the concentration of cytokines entering the lumen of the device is lower than the level required for the immune response of cells present in the lumen of the device. In some aspects, the device is configured to promote nutrient exchange with encapsulated cells and / or promote the diffusion of metabolic waste to the outside of the device. In some cases, the devices provided herein are configured to allow the passage of small molecules (including therapeutic molecules), but substantially inhibit the passage of large molecules.
[0065] In some aspects, the outer surface of the device can be modified by coating with one or more agents that improve the function of the device. For example, molecules that promote vascularization of the device or inhibit immune or inflammatory responses to the device can be disposed on the outside of the device. In some cases, the agent can be disposed within the device (e.g., loaded within the lumen of the device or coated on the inner surface of the device). In some cases, the agent can diffuse out of the lumen of the device and act on the external environment. Additionally or alternatively, the agent within the lumen of the device can act on cells disposed within the lumen of the device (e.g., a maturation factor that acts on partially mature stem cells). Such molecules or agents may include, but are not limited to, VEGF (vascular endothelial growth factor), PDGF (platelet-derived growth factor), FGF-1 (fibroblast growth factor), angiopoietin MCP-1, α v β3, α v β5, CD-31, VE-cadherin, ephrin, plasminogen activator, angiopoietin, Del-1, aFGF (acidic fibroblast growth factor), vFGF (basic fibroblast growth factor), follistatin, G-CSF (granulocyte colony-stimulating factor), HGF (hepatocyte growth factor), leptin, placental growth factor, PD-ECGF (platelet-derived endothelial cell growth factor), etc.

In some cases, the device can be implanted in a subject when cells are preloaded into the lumen of the device. In other cases, the device can be implanted in a subject when cells are not preloaded into the lumen of the device. In such cases, the device can be loaded with cells after a period of time (for example, by using a pre-implanted port or the pipeline of the device). In some cases, the method can include using the subject device to transplant cells that produce and secrete therapeutic molecules into a subject. In some cases, therapeutic molecules can diffuse from the device and enter the environment of the subject so that the therapeutic molecules exert a therapeutic effect on the subject.
[0067] In some cases, the method can be used to treat a disease or condition of a subject. A disease or condition can be a disease or condition characterized by lack of functional cells or insufficient protein production. In such cases, the method can be used for treating a subject suffering from or suspected of having diabetes. Diabetes can be type 1 diabetes. The device can include islet cells, or can include stem cells that can differentiate into islet cells. In certain embodiments, pluripotent stem cells (PSC) can differentiate into islet cells in the device, and the device for accommodating differentiated islet cells is then placed in a subject (e.g., in omentum, adjacent to pancreas or liver). In some cases, the device can include PSC and the device can be implanted near the pancreas or liver of a subject.

[0110] Example
[0111] Example 1. Cell encapsulation device.
[0112] Figure 1A and Figure 1B Depicted are non-limiting examples of cell encapsulation devices according to embodiments of the present disclosure. Figure 1A A first polymer layer is depicted. The first polymer layer comprises a plurality of pores having an average connected diameter of about 200 nm and an average pore size of about 2 μm. Figure 1B The second polymer layer is depicted. The second polymer layer comprises a plurality of pores having an average pore diameter of about 500 μm to about 3 mm and an average connectivity diameter of about 100 μm to about 500 μm.
[0113] Example 2. Polymer Scaffold
[0114] The three-dimensional polymer scaffold was designed to facilitate islet transplantation in a manner that allows optimal packing of islets with better spatial distribution. The scaffold was designed with a random macroporous structure, which functions to allow islets to embed and blood vessels to grow within, so that the islets are vascularized ( Figure 2A-2E ).
[0115] Example 3. Cell encapsulation device.
[0116] The device is constructed with a nanoporous layer and a macroporous layer composed of polycaprolactone (e.g., the stent according to Example 2) ( Figure 3A-3C ). The nanoporous membrane is about 20 μm thick and consists of pores with a pore diameter of about 2 μm and a connectivity diameter of about 200 nm. The macroporous membrane is about 500 μm thick and consists of macroscopic pores with a connectivity diameter and pore diameter of 200-400 μm. To make the device, the macroporous membrane and the nanoporous membrane are stacked and sealed directly along the periphery.
[0117] Example 4. Implantation of syngeneic islets into diabetic mice
[0118] The device described in Example 3 was loaded with approximately 500 syngeneic C57BL / 6 islets, sealed, and implanted subcutaneously in C57BL / 6 mice ( Figure 5 ). On day 21, an intraperitoneal glucose tolerance test (IPGTT) was performed. The glucose tolerance test measures the clearance of an intraperitoneally injected glucose load from the body and can be used to detect glucose metabolism disorders that may be associated with human conditions such as diabetes or metabolic syndrome. The animals were fasted for approximately 16 hours, and fasting blood glucose levels were determined before a glucose solution was administered by intraperitoneal (IP) injection. Blood glucose levels were then measured at different time points during the subsequent 2 hours.
[0119] IPGTT was performed at 21 days with 500 islets encapsulated in the device implanted subcutaneously and 500 islets implanted directly into the renal capsule. The islets in the device had the same glucose profile as the islets in the renal capsule, indicating similar glucose responses ( Fig. 6A ).
[0120] In addition, insulin secretion was analyzed using a glucose-stimulated insulin secretion assay. The glucose stimulation index is a measure that quantifies beta cell function by comparing the ratio of insulin release in a high glucose state to that in a resting state. The ratio of insulin secreted under high to low glucose conditions is used to calculate the glucose stimulation index. Figure 6B As depicted in FIG. 5 , islets encapsulated within a device as described in Example 3 have a greater glucose stimulation index than islets implanted directly into the kidney capsule (KC).
[0121] Example 5. Device evaluation after implantation
[0122] The device was implanted subcutaneously for more than 30 days to allow colonization. After sacrifice, the tissue was excised and fixed at 4°C for more than 24 hours using Z-Fix and then washed in PBS. The tissue and device were imaged using CLARITY (Clear Lipid Exchange Acrylamide Hybridization Fine Imaging / Immunostaining / In Situ Hybridization Compatible Tissue Hydrogel), a technology that embeds tissue into a hydrogel matrix. Lipids are removed to produce an optically transparent mixture that is light-transmissive and molecularly permeable. The markers included are DAPI for nuclear staining, and CD31 and alpha smooth muscle actin (α-SMA) for staining blood vessels. Figure 7 Staining depicts blood vessels that have penetrated into and around the device.
[0123] Fig. 8A and Figure 8BDepicts immunohistochemical staining of sections after implantation of the device in mice. The image depicts pancreatic islets (arrows) embedded within the macroporous scaffold layer of the device.
