Description

[0001] This application is a divisional application of the Chinese patent application with application number 2020100370625, application date January 14, 2020, and invention name “Multi-purpose handle”. Technical Field

[0002] The present disclosure generally relates to a medical device delivery system. More specifically, the present disclosure relates to a multi-purpose handle for use in a medical device delivery system and methods of using the handle, the multi-purpose handle being configured to control relative motion associated therewith. Background Art

[0003] The statements in this section merely provide background information related to the present disclosure and may not constitute prior art.

[0004] In many medical procedures, implants are inserted into the patient for treatment. For example, a vena cava filter can be deployed in the patient's vascular system to prevent pulmonary embolism. In order to successfully deploy and retrieve the implant, the user (e.g., physician) needs to be able to control the relative motion of the various structures of the delivery and retrieval system. Many known systems require two-handed operation. However, such operation is difficult for the user to move multiple structures in different directions while performing other functions (such as monitoring the condition of the surgical site to ensure patient safety).

[0005] Conventional medical device delivery systems are typically operated by moving the medical device along a desired vascular path or other body passageway within a catheter located within the patient's body until the medical device reaches the desired treatment site. The deployment operation requires a certain degree of skill on the part of the user, as he / she must monitor the relative positions of the medical device and other components within the delivery system, as well as use his / her hands to operate the delivery system. More specifically, in many conventional systems, the user must hold a proximal hub attached to an inner shaft member of the delivery system in a fixed position with one hand while simultaneously withdrawing the proximal hub attached to a tubular outer sheath with the other hand.

[0006] Accurate deployment and positioning of the device is important to ensure patient safety. Therefore, a system that provides accurate control of the movement of the medical device can improve the safety associated therewith. At the same time, it is also desirable to provide an integrated and ergonomic handle to make the system easy and efficient to operate. A handle that can be operated with one hand through a locking mechanism can provide such benefits. Therefore, a delivery and retrieval system with a handle configured for multi-purpose operation can be improved. Summary of the invention

[0007] The present disclosure generally provides a medical device delivery system. More specifically, the present disclosure provides a multi-purpose handle for use in a medical device delivery system, the multi-purpose handle being configured to control relative motion associated therewith.

[0008] The multi-purpose handle generally includes an elongated handle body, an actuation button and a locking member. The elongated handle body has a proximal end extending to a distal end, defining a longitudinal axis. The elongated body further includes an incision defining a motion space therein, the actuation button being arranged in the incision and being connected to the medical device. The actuation button can move along the longitudinal axis in the incision and can rotate in the incision. The locking member is connected to the elongated handle body and can move between a locked position and an unlocked position. When in the locked position, the locking member contacts the actuation button and is in a configuration that limits the actuation button from moving along the longitudinal axis.

[0009] According to another aspect of the present disclosure, a medical device delivery system is provided. The medical device delivery system includes: an outer sheath; a medical device, the medical device being at least partially located within the outer sheath in an initial configuration ready for deployment; and a multi-purpose handle as described above and further defined herein. The multi-purpose handle is connected to the outer sheath and the medical device.

[0010] According to yet another aspect of the present disclosure, a use of a medical device delivery system is described that incorporates a multi-purpose handle to deliver or retrieve a medical device from a desired or target site within the vascular system of a patient's body. During such use, the multi-purpose handle provides the user with control over longitudinal movement of the attached medical device along with rolling rotation. The user can perform such longitudinal and rotational movements using his / her thumb and index finger with a single hand.

[0045] (vii) The ability to maintain delivery system components in fixed relative positions during intermediate stages of deployment of the medical device.

[0046] For purposes of this disclosure, the term "proximal" refers to a direction generally toward a user (e.g., a physician) during a medical procedure, while the term "distal" refers to a direction generally toward a target site within a patient's anatomy during a medical procedure.

[0047] For the purposes of this disclosure, the terms "about" and "substantially" are used herein with respect to measurable values and ranges due to expected variations known to those skilled in the art (eg, limitations and variability in measurements).

[0048] For the purposes of this disclosure, the terms "at least one of" and "one or more of" an element are used interchangeably and may have the same meaning. These terms indicating that a single element or a plurality of elements are included may also be indicated by the suffix "(s)" at the end of the element. For example, "at least one metal," "one or more metals," and "metal(s)" may be used interchangeably and are intended to have the same meaning.

[0049] The focus of the design of the multi-purpose handle and medical device delivery system is to improve patient safety. In this regard, the multi-purpose handle utilizes ergonomic design and one-handed operation, which gives the user (e.g., physician) more control when conventional products rely on both hands to operate the device. Safety features are also embedded in the thumb button for both longitudinal and rolling motions, and increase precision for the user, which is not available in conventional products. The multi-purpose handle also includes a locking mechanism triggered by the index finger, which ensures that the device can be fixed without compromising control. In general, the user can perform all relevant steps required in the medical procedure while only moving the thumb and index finger while maintaining a firm grip on the delivery system. Compared to conventional delivery systems, the medical device delivery system of the present disclosure can be manipulated by the user to easily perform all necessary steps with one hand without having to take the line of sight away from the commonly used radiographic imaging screen.

[0050] refer to Figures 1 to 15 , the medical device delivery system 1 includes a multi-purpose handle 5, which includes an elongated handle body 10, an actuation button 15, and a locking member 20. The elongated handle body 10 has a proximal end 25 extending to a distal end 30, defining a longitudinal axis (A). The elongated body 10 further includes a cutout 35, which creates a movement space therein. The actuation button 15 is arranged in the cutout 35 and is connected to the medical device 40. The actuation button 15 can be movable along the longitudinal axis (A) in the cutout 35 and can be rotated in the cutout 35. The locking member 20 is connected to the elongated handle body 10 and can be moved between a locked position and an unlocked position. The locking member 20 can contact the actuation button 15 and is configured to limit the movement of the actuation button 15 along the longitudinal axis (A) when in the locked position.

[0051] like Figure 1 and Figure 2 As shown, the actuation button 15 may include a sliding portion 15A for controlling the longitudinal movement of the medical device 40 and a rotating portion 15B for controlling the rotational movement associated with the medical device 40. The sliding portion 15A and the rotating portion 15B may be manipulated by different fingers of the user's hand. For example, the sliding portion 15A may be manipulated using a thumb, while the rotating portion 15B may be manipulated by the user's index finger.

For example, this sliding member 45 may be, but is not limited to, a clamp 46 that secures the medical device 40 using fasteners 46A or other types of anchors.

[0053] According to this disclosure, Figure 3 and Figure 4 In another aspect shown in FIG. 1 , the longitudinal and rotational movements associated with the actuation button 15 can be combined so that one finger of the user can control both the longitudinal and rotational movements of the medical device through a sliding motion or a rolling motion, respectively. For example, the actuation button 15 can be a thumb wheel that can be manipulated by only the thumb of the user (e.g., a physician) to move the medical device 40 longitudinally and rotationally through the sheath 50. When desired, as Figure 5 The illustrated actuation button 15 may include one or more fins 17 and / or a recessed edge 18 configured to engage the thumb 3 to provide additional grip during rotational and longitudinal movement. When the actuation button 15 is a thumb wheel, the fins 17 may be disposed around the circumference of the wheel.

[0054] Reference now Figure 6 , the elongated handle body 10 may include one or more grip knurls 55, alternatively including multiple grip knurls 55 for providing a secure gripping surface for the user. The elongated handle body 10 may represent a single unified component, or include multiple components (such as an upper body portion and a lower body portion) attached together by gluing, welding, fusion bonding, etc. The size of the elongated handle body 10 can be customized to fit a specific predetermined hand size, or can be optimized so that various hand sizes can obtain a secure grip when using the multi-purpose handle 1. Additional ergonomic features may also be included in the elongated handle body without exceeding the scope of the present disclosure.

[0055] The multi-purpose handle 5 (comprising the elongated handle body 10 and each part contained therein) can be formed by polymeric material or plastic material, ceramic material, metal or metal alloy, and / or its combination. The material used in the multi-purpose handle 5 is selected so that the material exhibits desired or required performance characteristics, such as biocompatibility, flexibility and strength, etc. The polymeric material or plastic material can include one or more thermoplastic materials or thermosetting materials individually or in combination. Alternatively, the elongated handle body 10 is formed by ABS. Any metal part used in the multi-purpose handle 5 can be formed by but not limited to stainless steel, brass, nitinol or its combination. Alternatively, the metal part is formed by brass.

[0056] Reference now Figure 6 , Figure 7 and Figure 8A-8B The elongated handle body 10 may also include a cutout 35 in the form of a longitudinal slot that defines a channel 65 configured to accommodate the sliding action of the actuation button 15. This cutout or longitudinal slot 35 can limit the extent of longitudinal movement within the multi-purpose handle 5. The actuation button 15 or thumb wheel that fits within the cavity 60 of the cutout 35 may include a tactile protrusion 65, which may be Figure 7 This tactile projection 65 can help the user determine the orientation of the attached medical device 40. When desired, this longitudinal slot can be in the shape of a droplet 35A, such as Fig. 8A The size of the drop-shaped cutout 35A can vary as long as the actuation button 15 is fixed and cannot slide out of the cavity 60 ( Figure 8B ).

[0057] Reference now Figure 9A-9C The locking mechanism 20 may include a locking ring 20A configured to be in an unlocked position or a locked position. When the locking ring 20A is unlocked, the actuating button 15 may be moved ( Fig.9A However, when the locking ring 20A is in the locked position, the actuating button 15 is prevented from moving ( Fig. 9B The locking ring 20A may include at least one slit 67 that allows a portion 20A-1 of the locking ring to abut against the actuation button 15 when the locking ring 20A is rotated to the locked position. Fig. 9C The other portion 20A-2 of the locking ring is positioned so that it grasps or holds a portion of the elongated handle body 10 ( Fig. 9B). Such a locking mechanism 20 can resist accidental or unintentional movement or retraction of the actuation button 15 or other components in the multi-purpose handle 5 during packaging, sterilization, transportation, storage, handling, and preparation for use.

[0058] Reference now Fig.10 and Fig.11 The elongated handle body 10 may include a fin 11 aligned with the longitudinal axis (I) of the elongated handle body 10. The fin 11 further includes a hole 13 located at a position that is easily accessible to the index finger of the user during operation of the medical device delivery system 1 (see Fig.10 The difference (Δ) between the longitudinal axis (I) of the elongated handle body 10 and the longitudinal axis (II) of the locking member 20 causes the locking member 20 to rotate into the elongated handle body 10 of the multi-purpose handle 5 (see Fig.11 ). The locking member 20 may include one or more fins 21 to assist in gripping the mechanism in order to roll or rotate the member between the locked and unlocked positions.

[0059] Reference now FIG. 12A to FIG. 12E , various other forms of locking mechanisms are described. Fig. 12A In the embodiment, a sliding locking button 23 is used. This locking button 23 slides against a portion of the actuation button 15 to prevent its movement when in the locked position. Alternatively, the locking button 23 abuts against the side and / or face 15A of the actuation button 15 or thumb wheel. Alternatively, the locking member 20 or button 23 contacts the distal side 15A of the actuation button 15 or thumb wheel. The locking button 23 can slide in the opposite direction to unlock and allow the actuation button 15 to move. In this embodiment, the user's index finger 4 can provide greater control of the actuation button 15 together with the thumb.

[0060] exist Fig. 12B , the locking button 23 is shown as being in abutment against the face 15A of the actuating button 15. In this embodiment, the fin 11 in contact with the user's middle or index finger 4 can be moved (m) to engage the locking button 23 with the face 15A of the actuating button 15 to establish the locked position. The locking button 23 can be held in place using one or more fasteners 24 formed as part of the locking button 23.

[0061] exist Fig. 12C In another configuration shown, the actuation button 15 can slide on a protrusion 10B rising from the wall of the elongated handle body 10 to produce a locked position. In this locked position, the actuation button 15 is held in place by the spring 70. The actuation button 15 can be released to produce an unlocked position by using the locking button 23. In this case, the locking button 23 is a button that overcomes the retention of the spring 70 and moves the actuation button 15 past the protrusion 10B in the wall of the elongated handle body 10.

[0062] Another form of the locking member 20 is Fig.12D , where the locking button 23 is a spring-loaded lever that can pivot about its fulcrum 75. The spring 71 is positioned so that the locking button 23 is held in the locked position by the spring 71 forcing the lever to contact the face 15A of the actuating button 15. The user can release the actuating button 15 by pushing the locking button 23, thereby overcoming the hold of the spring 71 and producing the unlocked position.

[0063] exist Fig.12E In the embodiment of the present invention, the multi-purpose handle 1 may include the ability to provide a variable locking position during operation of the delivery system. The variable locking position may be produced by using, without limitation, a plurality of splines, ridges or teeth 80A located in the surface of the actuation button 15, which are appropriately matched or meshed with a plurality of grooves 80B formed in the locking member 20, and vice versa. The alignment or meshing of the splines 80A and the grooves 80B produces a locking position that prevents the actuation button 15 from moving. Different numbers of splines 80A and grooves 80B that can be meshed or matched together establish the variability of the position of the actuation button 15. The locking member 20 may include a locking button 23, wherein pushing the locking button 23 separates the splines 80A and the grooves 80B, thereby producing an unlocked position that allows the actuation button 15 to move.

[0064] Reference now Fig.13A and Fig. 13B , the locking member 20 may be integral with the fin 11 formed as part of the elongated handle body 10. In this embodiment, the locking member 20 includes a pivot point 90 that allows the user's index finger to apply pressure and push the locking member 20 to contact the surface of the face 15A of the actuation button 15. The locking member 20 acts like a lever that is normally in the unlocked position and is forced into the locked position when the user applies external force or pressure.

[0065] exist Fig.14 Another example of a medical device delivery system incorporating a multi-purpose handle of the present disclosure is described in . In this configuration, the locking button 23 includes at least one protrusion 93 located on each side of the locking button 23 to hold the button 23 in place and prevent the button 23 from being removed (i.e., falling) through an opening present in the elongated handle body 10. These protrusions 93 can also be used as a pivot point 90A relative to the axis (III) to allow the locking button to establish a locked position and an unlocked position. When desired, the locking button 23 can also be ergonomically bent 95 to make it more appropriately fit the user's index finger.

[0066] According to one aspect of the present disclosure, the locking button 23 is forced into abutment against the face 15A of the actuating button, thereby producing the locked position. The locking button 23 may be spring loaded, in that the spring forces the multi-purpose handle to be in an unlocked or locked state in the absence of pressure applied to the locking button 23 by the user. According to another aspect, as Fig.15 As shown, the spring 72 can provide a perceptible and / or audible click associated with locking / unlocking the button 23. In the absence of pressure, the button 23 remains in a state in which it is retained for locking or unlocking that must be performed manually later. In other words, an external force applied to the locking member that overcomes the force of the spring can move the locking member from the unlocked position to the locked position or from the locked position to the unlocked position.

[0067] According to another aspect of the present disclosure, there is provided a medical device delivery system incorporating a multi-purpose handle as described above and as further defined herein. Figures 1 to 15 As shown, the medical device delivery system includes an outer sheath, a medical device and a multi-purpose handle. The medical device is at least partially located in the outer sheath with an initial configuration ready for deployment, alternatively completely located in the outer sheath. The outer sheath can be a catheter that can hold and protect the medical device when the medical device is pushed through the patient's vascular to reach remote treatment or target site in the cavity. Advantageously, for a given application, the diameter of the outer sheath or catheter is as small as possible to improve movement in the vascular and to properly keep the medical device in place until it is deployed. The outer sheath can accommodate the use of a guide wire to track the delivery of the medical device to the desired or target position. The type of medical device is not limited, and can take the form of any known device, including but not limited to stents, embolic coils and vena cava filters.

[0068] According to another aspect of the present disclosure, the use of a medical device delivery system incorporating a multi-purpose handle as described herein to deliver or retrieve a medical device from a desired or target site within the vasculature of a patient's body is considered to be within the scope of the present invention. During such use, the multi-purpose handle provides the user with control of longitudinal movement of the attached medical device along with rolling rotation. The user can perform such longitudinal and rotational movements using his / her thumb and index finger with a single hand.
