Abstract
Description
Technical Field
[0001] The present disclosure relates to a medical motor handpiece for driving a distal end effector (tool) having a handle section. Background Art
[0002] In modern minimally invasive surgery, this tool / apparatus and the instrument handpiece / handheld instrument to which it belongs are for example used for processing bones, cartilages and for processing organic materials in neurosurgery in arthroscopic surgery, spinal surgery and similar orthopedics / surgery. The tool / apparatus has a handpiece / handle / grip section, and optionally replaceable effectors, such as milling cutters, rotary cutters, polishing heads, etc. The effector is supported in the handle of the tool / apparatus on its distal end, and can be supported in a rotatable driving manner if necessary.According to the tool speed of use purpose and expectation, hydraulic, pneumatic or electric drive is set to tool drive, which is effectively connected with tool head (effector) by the torque transmission path in tool and / or handheld instrument or grip section. At this, these drives can be integrated in tool and / or handheld instrument or be configured as external drive units, which are coupled with tool or handheld instrument by energy supply line or torque transmission system.
[0003] It is advantageous to bend the distal handle section of the handle of the medical handheld instrument in order to be able to perform an operation in a very small space, for example, in an operation on the spine. In other words, in surgical operations, especially minimally invasive operations, the structural space and good operability of the instruments used play an important role. Therefore, the instrument handle should be able to be actively (intentionally implemented by an operating mechanism) bendable in the smallest possible space, especially in the area of the distal effector.
[0004] Furthermore, it is advantageous if tools or effectors can be replaced quickly and easily in order to ensure rapid adaptation to changing surgical conditions and / or working conditions and to keep the burden on the patient as low as possible. Prior art
[0005] Bendable handles for medical handheld instruments are well known and usually have a proximal handle section and a bendable distal handle section. Both the distal handle section and the proximal handle section have an inclined end / end side tilted relative to the corresponding handle section axis. This means that the ends / end sections / end sides of the distal and proximal handle sections are not straight lines, but inclined / tilted. These inclined portions / tilted end sections / end sides have approximately the same inclination angle respectively. Therefore, the inclined portions cooperate with each other so that the proximal and distal handle sections form a straight handle / straight tube in a specific relative rotational position. If the distal handle section now rotates around its longitudinal axis relative to the proximal handle section and the proximal handle section remains stationary, the distal handle section is inevitably bent by the inclined end side / end section.
[0006] Furthermore, DE 10 2017 010 033 A1 also discloses a medical device having a guide unit and an actuating tube axially movable in the guide tube and connected to a pivot head, the guide unit having the guide tube with a longitudinal axis, a proximal first coupling part fixedly connected to the guide tube, and a distal cylindrical sleeve-shaped pivot head, the actuating tube causing a pivoting of the pivot head via a proximal operating element. In order to more precisely orient the distal guide element for a rotatable surgical tool and thus the angular orientation of a working head of such a tool, the operating element can be pivoted about the longitudinal axis and causes a pivoting of the pivot head when the actuating tube is axially displaced.
[0007] Furthermore, US 7,585,300 B2 or US 10,070,872 B2 disclose examples of surgical instruments having a handpiece and a handle accommodated in the handpiece, to which a tool head is pivotably articulated at the distal end. The pivoting of the tool head can be achieved here by a handwheel rotatably arranged on the handpiece. Similarly, US 8,303,594 B2 discloses a surgical handpiece, wherein the pivoting of the tool head is achieved by a lever arranged on the handpiece.
[0008] Furthermore, US Pat. No. 9,597,093 B2 shows a tool which can be coupled with a drive unit at its proximal end in a torque-transmitting manner and has a tool head at its distal end. The tool head can be pivoted relative to the tool shaft by rotating a sleeve arranged in the region of the tool head.
[0009] Further examples of surgical hand instruments having a rotation tool which can be bent / pivoted relative to the handpiece are disclosed, inter alia, in US 10,178,998 B2, US 10,307,180 B2 or US 10,524,820 B2.
[0010] However, the prior art always has the disadvantage that the tool with its bendable handle requires a complicated coupling. Therefore, a simple and fast decoupling is not easy to achieve. Summary of the invention
[0011] The object and purpose of the present disclosure is to overcome or at least reduce the disadvantages of the prior art and in particular to provide an intuitive motor handpiece or handheld instrument which allows simple and rapid tool changes and at the same time allows relative movement of the tools during operation.
[0012] With regard to a medical motor handpiece of this type, this object and purpose are achieved according to the disclosure by the subject matter of claim 1. The disclosure is therefore based on the recognition that two functions are integrated into the operating element.
[0013] Therefore, according to the present disclosure, the medical motor handpiece is characterized by a preferably sleeve-shaped operating element, which is rotatably held on the distal end section of the handle section about the longitudinal axis of the handle and can be coupled to the end effector in such a way as to convert the rotational movement of the operating element along a first rotational direction into the movement of the end effector and to induce a function on the end effector when the operating element rotates in a second rotational direction opposite to the first rotational direction.
[0014] In other words, two different functions can be realized with the rotatable operating element depending on the direction of rotation. This 2-in-1 operation of the motor handpiece makes it possible to eliminate the need for additional operating elements, which in turn leads to more intuitive operation.
[0015] Advantageous embodiments are claimed in the dependent claims and are explained below.
[0016] In a preferred variant, the motor handpiece can be coupled to the end effector via a shaft, and the operating element bends the end effector relative to the shaft during a rotational movement of the operating element in a first rotational direction.
[0017] In addition, the end effector can preferably have a first coupling device and the handle can have a second coupling device, and the first coupling device and the second coupling device are operatively engaged with each other in a coupled state, wherein the rotational movement of the operating element along the second rotational direction releases the operative engagement between the first coupling device and the second coupling device.
[0018] According to the present disclosure, a motor handpiece according to an advantageous design may be configured such that in a top view of the end effector, the first rotational direction is oriented in a clockwise rotational direction and the second rotational direction is oriented in a counterclockwise rotational direction.
[0019] Furthermore, it is advantageous if the handle section and / or the operating element has at least one indicator, preferably in the form of a numerical scale and / or a pictogram, for indicating a movement and / or a function of the end effector.
[0020] According to a preferred improvement, at least one latching and / or stop unit (locking device) can be provided, in particular, in the form of a spherical pressure element, which is arranged and constructed to limit the rotational movement of the operating element at an end position and / or to increase the resistance to the rotational movement of the operating element in at least one rotational intermediate position.
[0021] An advantageous design solution according to the present disclosure may also be characterized by a locking unit, which in a locked position locks the rotational movement of the operating element in the first rotational direction and in the second rotational direction and unlocks it in an unlocked position. The locking unit may preferably be configured as a locking slide, which is arranged on the operating element so as to move axially between a locked position and an unlocked position. Furthermore, it may be particularly advantageous here that the locking slide has a spring element, which presses the locking slide into the locked position.
[0022] Particularly preferably, the latching and / or stop unit can be designed in the form of at least one spherical pressure element and have a spring force adjustment element, preferably in the form of a countersunk screw, for steplessly adjusting the spring force of the spherical pressure element.
[0023] In an advantageous design, the motor hand piece can be coupled to the end effector by a handle, and the handle can be coupled to the rotation transmission sleeve of the motor hand piece in a rotationally resistant manner. Here, it is also preferred that the operating element transmits its rotational motion to the rotation transmission sleeve via a radial adjusting pin.
[0024] According to an advantageous improvement, the motor handpiece may have a joint for coupling the motor handpiece to the drive unit and a locking unit at a proximal end section, the locking unit being connected to the operating element so that when the motor handpiece is coupled to the drive unit, the rotation of the operating element in the second rotational direction is locked. Preferably, the locking unit may have a locking bolt, which is coupled to the operating element and is displaced in the proximal direction when the operating element is rotated in the second rotational direction. For this purpose, the drive unit may also be configured with a stopper so that the movement of the locking bolt is blocked / locked by the drive unit in the coupled state of the motor handpiece and the drive unit.
[0025] Furthermore, the present disclosure relates to a medical handheld instrument having a medical motor handpiece according to the present disclosure and an end effector, wherein the end effector is coupled to the motor handpiece via a handle to transmit torque from the motor handpiece to the end effector.
[0026] In other words, the present disclosure relates to a (motor) handpiece with a rotary operating unit, with which tool ejection can be initiated and the angle of the distal tip can be set in multiple steps. As in the known handpieces, the safety function (on / off) ensures that the tool cannot be accidentally ejected in the switched-on state. BRIEF DESCRIPTION OF THE DRAWINGS

[0045] Figure 1 A medical handheld instrument 1 according to a first embodiment of the disclosure is shown in perspective. The medical handheld instrument 1 has a medical motor handpiece 2, which has a proximal handle section 4 and is Figure 1 As schematically shown in FIG. 2 , the motor handpiece can be coupled to a drive unit 6. Alternatively, the drive unit 6 can also be accommodated in the motor handpiece 2 and supplied with energy via a power supply connection.
[0046] The handheld instrument 1 also has a distal tool (end effector / effector section) 8, which can be coupled to the motor handpiece 2, in particular the handle section 4, or can be decoupled from the motor handpiece via a (tool) shaft 10. The tool 8 can be designed as a milling cutter, a drill or a polishing head, for example.
[0047] When the handle 10 is coupled to the handle section 4, the torque is transmitted from the drive unit 6 to the tool 8 via the torque transmission path arranged in the handheld instrument 1, in particular arranged in the handle section 4 and the handle 10, so as to rotate the tool. When the handheld instrument 1 is used in the scope of surgery or medical treatment, it is known that the tool 8 can be bent relative to the handle 10 (in Figure 1 That is, the tool 8 and the handle 10 can be bent relative to each other as described in more detail below, so that the longitudinal axis S1 of the tool 8 and the handle longitudinal axis S2 of the handle 10 are configured at an angle not equal to 180°, in particular less than 180° (see Figure 6).
[0048] like Figure 1 As shown in , for this purpose, a sleeve-shaped operating element 12 is provided in the handheld instrument 1 according to the present disclosure. The operating element 12 is arranged at the distal end section of the handle section 4 and is rotatable about the handle longitudinal axis in a first rotational direction A and in a second rotational direction B opposite to the first rotational direction A. In other words, the motor handpiece 2 has a handle section 4 arranged coaxially with each other and a distally arranged operating element 12, wherein the operating element 12 can be rotated relative to the handle section 4.
[0049] The rotation of the operating element 12 from a neutral zero position (in which the tool 8 is not bent relative to the handle 10) in the first rotational direction A is performed as follows: Figure 1 The tool 8 is bent relative to the handle 10. In the handheld instrument 1 according to the present disclosure, the first rotation direction A is defined as the rotation of the operating element 12 to the left relative to the handle section 4. In other words, the first rotation direction A corresponds to the rotation of the operating element 12 in the clockwise direction in the top view of the distal side of the tool 8.
[0050] like Figure 2 As shown in , the operating element 12 in the handheld instrument 1 according to the present disclosure can also be rotated relative to the handle section 4 along a second rotation direction B opposite to the first rotation direction A. That is, the second rotation direction B is defined as a clockwise rotation of the operating element 12 or as a rightward rotation in a top view of the tool 8. Figure 2 As shown by the arrow D in , the rotation of the operating element 12 along the second rotation direction B causes the coupling of the tool 8 and the handle 10 to be loosened. Therefore, as will be explained in more detail below, the rotation of the operating element 12 along the second rotation direction B can ensure simple and fast tool replacement.
[0051] Figure 3 and Figure 4 A partial cross-sectional view of a handheld instrument 1 according to a first embodiment is shown. In order to transmit the rotational movement of the operating element 12 to the handle 10, an adjusting pin 14 is arranged in the radial direction of the operating element 12. Here, the radially outer end section of the adjusting pin 14 is accommodated in an axial groove 16, which is configured on the inner circumference of the operating element 12. The adjusting pin 14 is accommodated in a rotation transmission sleeve 18 on the radial inner side. The rotation transmission sleeve 18 is connected to the handle 10 in a rotationally fixed manner, so that the movement of the adjusting pin 14 in the circumferential direction causes the rotation of the rotation transmission sleeve 18 and the tool 8. In other words, when the operating element 12 is twisted in the first rotation direction A or in the second rotation direction B, the adjusting pin 14 moves in the axial groove 16 together with the operating element 12 and causes the tool 8 to be bent or thrown out depending on the rotation direction.
[0052] Figure 5 The longitudinal cross section of the distal end section of the handheld instrument 1 is shown in the shape of a straight handle. This means that the operating element 12 is not twisted relative to the handle section 4 (zero position). Therefore, the tool 8 and the handle 10 are not bent, and the longitudinal axis S1 of the tool 8 is colinear with the longitudinal axis S2 of the handle. Figure 5 As can be seen in the figure, the handle 10 has a handle section 20 facing the handle section 4 and can be coupled to the handle section, and a handle section 22 on the far side that can be coupled to the tool 8. When the operating element 12 is not twisted relative to the handle section 4 and the handle 10 is in a straight handle shape, the handle section 20 on the near side and the handle section 22 on the far side are therefore arranged on a line or have an angle of 0° between each other. The handle section 20 on the near side is roughly designed as a tube and has an end side 24 inclined along the handle longitudinal axis S2 on its distal end section. The handle section 22 on the far side is also implemented in an approximately tubular manner. The tube extends pointedly toward the distal end. The handle section 22 on the far side has an end side 26 inclined along the handle longitudinal axis S2 on its proximal end section.
[0053] These inclined end sides 24, 26 each have an inclination of preferably 22.5° relative to the normal plane of the shank longitudinal axis S2. When the shank 10 is in a straight shank shape or extended, the two inclined end sides 24, 26 are offset from each other so that the long ends of the inclined end sides 24, 26 are opposite to each other with respect to the shank longitudinal axis S2. The inclined end sides 24, 26 are superimposed on each other. Here, the inclined end sides 24, 26 do not necessarily have an inclination of 22.5°. An inclination of, for example, 10°, 18°, 30°, 45° or any other inclination is also conceivable.
[0054] As described above, the distal shank section 22 is designed to be coupled to the tool 8 . This means that the distal shank section 22 is a tool holder, wherein the tool 8 is received in the tool holder and is rotatably mounted relative to the tool holder, ie the distal shank section 22 .
[0055] For this purpose, a first coupling device 28 is arranged in the distal shank section 22. A second coupling device 30 is provided on the tool 8, which in the coupled state realizes the coupling between the tool 8 and the distal shank section 22 by cooperating with the first coupling device 28, so as to fix the tool 8 in the axial direction A in the distal shank section 22.
[0056] As described above, the longitudinal axis S1 of the tool 8 or the distal shank section 22 and the shank longitudinal axis S2, ie the longitudinal axis of the proximal shank section 20, are located at Figure 5 In addition, Figure 52 shows the coupling state between the tool 8 and the distal shank section 22. That is, the first coupling device 28 arranged in / at the distal shank section 22 and the second coupling device 30 arranged on the tool 8 engage or cooperate with each other. The tool 8 has a tool head / effector 32, such as a milling head, and a tool shank 34. The tool head 32 and the tool shank 34 are connected to each other in a rotationally fixed manner.
[0057] exist Figure 5 It can also be seen that the tool shank 34 is rotatably supported in the distal shank section 22 by means of a rolling bearing unit 36. A drive shaft 38 extends through the proximal shank section 20, which is connected to the tool shank 34 in a rotationally fixed manner and applies torque from the drive unit 6 to the tool shank 34 in the form of a torque transmission system. The tool shank 34 rotates relative to the distal shank section 22 due to this torque application. The rolling bearing unit 36 has at least one, here exactly two rolling bearings 40, more precisely ball bearings, which are spaced apart from each other in the axial direction A, but alternatively, the rolling bearings can also be configured as sliding bearings. The rolling bearing 40 is accommodated in a bearing housing 42, which is part of the rolling bearing unit 36.
[0058] The second coupling device 30 is introduced into the bearing housing 42 and is therefore not arranged directly on the tool 8. The rolling bearing unit 36 and therefore also the bearing housing 42 are fixedly arranged on the tool shank 34 in the axial direction A. However, as an alternative to this, it is also conceivable that the second coupling device 30 is arranged directly on the tool shank 34. The second coupling device 30 is configured as an axial securing groove 44 that extends continuously or circumferentially in the circumferential direction of the bearing housing 42. The axial securing groove 44 preferably has a semicircular or sector-shaped cross section.
[0059] The first coupling device 28 arranged in the distal handle section 22 has at least one locking ball 46. The diameter of the locking ball 46 is selected so that the locking ball 46 can be accommodated in the axial safety groove 44. Preferably, the axial safety groove 44 surrounds at least one section of the locking ball 46 that contacts the axial safety groove 44 over the entire circumference. In the coupled state, the locking ball 46 is retained in the axial safety groove 44 by the distal end of the locking pin / slider 48 on its side opposite to the axial safety groove 44. The locking pin 48 is part of the first coupling device 28. The locking pin 48 is fixed in the radial direction R. The locking pin 48 is arranged in the distal handle section 22 movably or movably in the axial direction.
[0060] Therefore Figure 5In the coupled state shown in FIG, the locking ball 46 is accommodated in the axial securing groove 44 and is held in the axial securing groove 44 in the radial direction by the latching pin 48. This position of the latching pin 48 in the axial direction A is referred to as the coupled position. The edge 63 on the distal end of the proximal shank section 20 prevents the latching pin 48 from moving in the axial direction toward the proximal shank section 20.
[0061] The proximal shank section 20 has a fixed outer tube 50, a hollow gear 52 with an internal toothing 54, a pinion 56 with an external toothing 58, and an eccentric securing bushing 60. The hollow gear 52 is positioned in the outer tube 50 and the longitudinal axis of the outer tube 50 corresponds to the longitudinal axis of the hollow gear 52. The outer tube 50 and the hollow gear 52 are arranged concentrically. The hollow gear 52 is connected to the rotation transmission sleeve 18, that is, to the operating element 12, via a hollow shaft 62 accommodated in the proximal shank section 20.
[0062] As described above, the outer tube 50 is designed as a fixed tube and therefore does not move. The distal end of the outer tube 50 has an inclined end side 24. The distal end of the outer tube 50 also has a receiving hole 64 and a receiving pin for a rolling bearing 66. The outer tube 50 has a groove / recess for the balls of the rolling bearing 66. The distal shank section 22 is supported on the rolling bearing 66.
[0063] The inner toothing 54 meshes with the outer toothing 58 of the pinion 56. Thus, the rotation of the internal gear 52 controlled by the operating element 12 is transmitted to the pinion 56. The direction of rotation of the pinion 56 is equal to the direction of rotation of the internal gear 52. The pinion 56 is driven by the internal gear 52, but the pinion 56 rotates in the eccentric safety bushing 60. The safety bushing 60 is arranged eccentrically relative to the internal gear 52. That is, although the longitudinal axis of the eccentric safety bushing 60 is parallel to the longitudinal axis of the internal gear 52, the longitudinal axes do not overlap or stagger with each other. The distal handle section 22 has an adjustment bushing 68. The adjustment bushing 68 is supported in the receiving hole 64 of the proximal handle section 20. The adjustment bushing 68 is connected to the pinion 56 by a flexible silicone hose 70 so that the rotation of the pinion 56 is transmitted to the adjustment bushing 68. For this purpose, a flexible silicone hose 70 is fastened, for example by welding or adhesive bonding, to the adjustment bushing 68 and the pinion 56. Furthermore, the adjustment bushing 68 is connected to the distal handle section 22 in a form-locking manner via a driver pin (not shown), so that the rotation of the adjustment bushing 68 is transmitted to the distal handle section 22.
[0064] Figure 6 A longitudinal cross section through the distal end section of the shank 10 is shown, wherein the distal shank section 22 is bent at 45° relative to the proximal shank section 20 with an inclination of 22.5° at both end faces 24 , 26 . Figure 51 is rotated 180° about its own longitudinal axis S1. In this position, the oblique end sides 24, 26 overlap each other completely / flatly again. However, due to the rotation of the distal shank section 24, the long ends of the oblique end sides 24, 26 are positioned next to each other. The inclination angles of the oblique end sides 24, 26 are thus added up. As a result, the distal shank section 22 is bent at twice the inclination angle of the oblique end sides 24, 26 compared to the proximal shank section 20.
[0065] exist Figure 6 In the position shown in , the driver pin is arranged relative to the safety bushing 60. This means that the adjusting bushing 68 is rotated 180° from the extended position to the maximum bend. For this structure, the 45° position represents the reversal point. The adjusting bushing 68 is rotated 180° in this position. When the hollow gear 52, that is, the operating element 12, is rotated further, the distal handle section 22 will be rotated back to the initial position (zero position).
[0066] In addition, Figure 6 It can be seen that even if the distal shank section 22 is bent relative to the proximal shank section 20 in a maximum adjustable angle position or in a maximum bent manner, the locking ball 46 is held in the axial securing groove 44 by the latching pin 48 in the radial direction R. In this way, the tool 8 is fixed in the distal shank section in the axial direction A even in this angular position by the engagement between the first coupling device 28 and the second coupling device 30.
[0067] The torque train, in particular the drive shaft 38, is flexible toward the tool shaft 34, which is arranged in the transition region between the distal shaft section 22 and the proximal shaft section 20. This flexible section of the torque train allows the distal section of the tool shaft 34 with the tool head 32 to be bent together with the distal shaft section 22 relative to the torque train in the proximal shaft section 20. At the same time, the flexible section of the torque train is designed so that it can continue to transfer the torque applied to the proximal section of the tool shaft 34 to the tool head 32.
[0068] Figure 7 8 and the distal shank section 22 in a longitudinal section through the distal end section of the shank 10 in the released state. Figure 7 In the position shown in FIG. 1 , the first coupling device 28 and the second coupling device 30 are not in active engagement with each other, so that the tool 8 can be removed or replaced. In order to release the active engagement of the first coupling device 28 and the second coupling device 30, the operating element 12 is rotated in the second rotational direction B (see FIG. 1 ). Figure 2 ). As will be explained in more detail below, the locking ball 46 is no longer accommodated in the axial securing groove 44. Instead, the locking ball is held by the latch pin 48 in the radial direction R against the outer circumference of the bearing housing 42.
[0069] In order to make the locking ball 46 Figure 5 In the coupling state shown in FIG, the locking pin 48 must move in the axial direction A toward the proximal handle section 20 in order to move out of the axial safety groove 44 and into the release state. This is prevented in the coupling state by the surrounding edge 63 of the proximal handle section 20. However, the edge 63 is interrupted at the position of the locking pin receiving recess 72. When the operating element 12 is rotated in the second rotational direction B, the distal handle section 22 rotates in the direction opposite to the bend relative to the proximal handle section 20, for example, by (-) 18°, until the locking pin 48 is positioned relative to the proximal handle section 20 so that the locking pin is at the height of the locking pin receiving recess 72 in the circumferential direction. The preload element 74 as part of the first coupling device 28 presses the locking pin 48 in the axial direction A toward the proximal handle section 20. Therefore, the preload element 74 pushes the locking pin 48, more precisely, the proximal end thereof configured as a locking projection 76, into the locking pin receiving recess 72. The position of the latching pin 48 when the latching projection 76 engages in the latching pin receiving recess 72 is referred to as the released state or released position.
[0070] In the release position, the distal end of the latching pin 48 no longer lies opposite the axial securing groove 44. The locking ball 46 is therefore not held in the axial securing groove 44 in the radial direction R. The tool 8 is therefore no longer fixed in the axial direction A relative to the distal shank section 22. If a tensile force (in the axial direction A) is now applied to the distal end of the tool 8 starting from the coupled state, the locking ball 46 is released from the axial securing groove 44. In this way, the tool 8 can be decoupled from the distal shank section 22.
[0071] Figure 8 A stereoscopic view of the distal end section of the motor handpiece 2 of the first embodiment is shown, without the handle 10 being coupled to the handle section 4. As described above, rotation of the operating element 12 in the first rotational direction A causes bending of the tool 8, and rotation of the operating element 12 in the second rotational direction B releases the coupling between the tool 8 and the handle 10 or the distal handle section 22 by releasing the effective engagement between the first coupling device 28 and the second coupling device 30. To facilitate operation, indicators 78 in the form of arrows are provided on the operating element 12, which indicate the direction of rotation. In combination with the indicator 78 on the indicator sleeve 80 which is mounted on the handle section 4 on the distal side of the operating element 12 in a rotationally fixed manner, the user can quickly identify which direction of rotation corresponds to which function when operating the handheld instrument 1. For this purpose, as shown in Figure 8As shown in FIG, different angular positions and a lock symbol or an open lock symbol are applied to the outer circumference of the indicator sleeve 80, which respectively correspond to a limited rotation of the operating element 12 along the first rotation direction A and thus to a limited bending of the tool 8 relative to the handle 10. The user can thus assign a corresponding function, i.e. bending or decoupling, to each rotation direction A, B. Therefore, the motor handpiece 2 together with the operating element 12 enables intuitive operation and integration of these two functions.
[0072] As in Figure 8 As shown in FIG. 1 , a locking slide 82 is arranged on the motor handpiece 2 according to the first embodiment. Here, the locking slide 82 is fixedly received in the operating element 12 in the radial direction and the circumferential direction, so that the locking slide 82 can only move in the axial direction relative to the operating element 12 between a (distal) locking position and a (proximal) unlocking position. In the locking position, as described in more detail below, the operating element 12 cannot rotate relative to the handle section 4.
[0073] For this reason, as in Fig. 9 As shown in the figure, the distal journal section 83 of the locking slide 82 is locked with the locking ring 84 accommodated in the indicator sleeve 80 in the locking position. The locking ring 84 has a plurality of recesses distributed on the circumference, into which the journal section 83 extends in the locking position, so as to fix or hold the operating element 12 relative to the handle section 4 in the circumferential direction. Here, the recess of the locking ring 84 preferably corresponds to the indicator 78 mounted on the outer circumference of the indicator sleeve 80. Therefore, the user can simply set the bending of the tool 8 at a defined angle and fix the bent tool 8 at this angle.
[0074] In order to be able to rotate the operating element 12 relative to the handle section 4 for adjusting the angle or for releasing the coupling, the locking slide 82 must therefore be displaced from the locking position into the unlocking position, ie in the proximal direction.
[0075] In the motor handpiece 2 according to the first embodiment, the locking slide 82 is prestressed in the axial direction against the operating element 12 via a spring element 86. Here, the spring element 86 is arranged between the locking slide 82 and the stop surface of the operating element 12, so that the spring element presses the locking slide 82 into the locking position, that is, in the distal direction. The spring element 86 thus exerts an automatic restoring force on the locking slide 82 in order to keep it in the locking position.
[0076] Alternatively, it is of course also conceivable that the motor handpiece 2 does not have a spring element 86. In such a motor handpiece according to a modification of the first embodiment with manual reset, the user needs to pull the locking slide 82 out in the proximal direction for unlocking and actively press it in the distal direction for locking.
[0077] As in Fig.10 and Fig.11 As shown in FIG. 1 , an additional ball push element 88 is provided in the motor handpiece 2 according to a modification of the first embodiment. The ball push element has a spring element 90, which presses the ball 92 in the proximal direction against a sliding slot (ball cap locking ring) 94 (see FIG. 1 ) which is fixedly received in the handle section 4. Fig.11 A spherical cap-shaped latching recess 96 is formed on the sliding slot 94, which can at least partially accommodate the ball 92.
[0078] Similar to the recesses of the latching ring 84, the latching recesses 96 correspond to a defined angle at which the tool 8 can be bent relative to the handle 10 when the operating element 12 is twisted by a certain angle. That is, when the operating element 12 is twisted by a defined angle relative to the handle section 4, the ball push element 88 rotates together with the operating element 12 until a defined angle is reached, at which the ball 92 engages in the corresponding latching recess 96 of the sliding guide 94 due to the preload force of the spring element 90. The additional ball push element 88 thus provides the user with tactile feedback when setting the angle.
[0079] Fig.12 The medical handpiece 1 according to the first embodiment is shown in an operating state, wherein the motor handpiece 2 is connected to the drive unit 6. For this purpose, as described above, the motor handpiece 2 has a connection at its proximal end section, which is Fig.12 The motor hand piece 2 also has a locking slide 100, which is in the closed position (see Fig.13 ) can be displaced in the proximal direction in order to loosen the connection between the handle 10 and the motor handpiece 2.
[0080] In order to secure the unlocked position of the tool 8, Fig.13 As can be seen in the figure, the locking slide 100 has a locking bolt 102. The locking bolt 102 is connected to the operating element 12 and extends in the proximal direction when the tool 8 is unlocked by rotating the operating element 12 in the second rotational direction B, that is, when the active engagement between the first coupling device 28 and the second coupling device 30 is released.
[0081] When the medical handpiece 1 is in the operating state, this proximal extension of the locking bolt 102 is not possible due to the motor cable 98, in particular the nose of the motor cable 98 blocking such extension. It is thus possible to prevent the tool 8 from being thrown out during operation due to an unintentional rotation of the operating element 12 in the second rotational direction B. However, the rotation of the operating element 12 in the first rotational direction A is still possible, so that the tool 8 can also be bent relative to the handle 10 during operation.
[0082] exist Fig.14 2 shows a motor handpiece 2 for a medical handheld instrument 1 according to a second embodiment. It can be seen here that the motor handpiece 2 according to the second embodiment does not have a locking slide 82. The omission of the locking slide 82 enables an improved view of the operating area, more intuitive operation and reduced cleaning effort.
[0083] However, in order to ensure a certain degree of security of the operating element 12 against undesired rotation even without the locking slide 82 and to enable locking of the operating element 12 at a defined angle, as in Fig.15 As shown in , two ball push elements 104 are arranged in the operating element 12. These ball push elements each have a spring element 106, which presses the ball 108 as a locking body against the locking ring (locking plate) 84. This means that the spring element 106 presses the ball 108 against the locking ring 84 in the distal direction. Fig.16 As shown in FIG. , the locking ring has a recess with which the balls 108 are locked.
[0084] As described above, the ball push element 104 presses against the locking ring 84 with its distal end section, i.e. the ball 108. At its proximal end section, the ball push element 104 has a spring force setting device in the form of a countersunk screw 110. By screwing in the countersunk screw 110, the preload of the spring element 106 and thus the spring force acting on the locking ring 84 can be set. A higher spring force makes it difficult to twist the operating element 12 unintentionally, while a lower spring force allows easier adjustment of the angle and thus enables one-handed operation.
