A surgical instrument for delivering energy to lung tissue, for example to cause lung volume reduction. In one embodiment, an elongated catheter has a handle portion that includes an interior chamber that is supplied with a biocompatible liquid media under pressure. An energy source delivers energy to the media to cause a liquid-to-vapor phase change within the interior chamber and ejects a flow of vapor media from the working end of the catheter. The delivery of energy and the flow of vapor are controlled by a computer controller to cause a selected pressure and selected volume of vapor to propagate to the extremities of the airways. Contemporaneously, the vapor undergoes a vapor-to-liquid phase transition which delivers a large amount of energy to airway tissue. The thermal energy delivered is equivalent to the heat of vaporization of the fluid media, which shrinks and collapses the treated airways. The treated tissue is the maintained in a collapsed state by means of aspiration for a short interval to enhance tissue remodeling. Thereafter, the patient's wound healing response causes fibrosis and further remodeling to cause permanent lung volume reduction.

A non-linear fin heat sink is provided for dissipating / removing heat uniformly from a device, where the heat generation is non-uniform over that device, while also providing a small and relatively lightweight heat sink. Furthermore the heat sink features turbulence enhancement of the coolant stream by a pin array through which coolant stream passes, such fin array featuring a non-linear shape, spacing, and height pattern to provide optimal cooling while simultaneously reducing volume and flow resistance.

Methods and devices for reducing gastric volume

The present invention involves new interventional methods and devices for reducing gastric volume, and thereby treating obesity. The procedures are generally performed laparoscopically and may generally be described as laparoscopic plication gastroplasty (LPG) in which, after obtaining abdominal access, spaced apart sites on a gastric wall are engaged and approximated to create one or more tissue folds that are then secured by placing one or more tissue fasteners to produce one or more plications projecting into the gastrointestinal space. The serosal tissue may optionally be treated during the procedure to promote the formation of a strong serosa-to-serosa bond that ensures the long-term stability of the tissue plication. These procedures are preferably carried out entirely extragastrically (i.e. without penetrating through the gastrointestinal wall), thereby minimizing the risks of serious complications. Minimally invasive devices for approximating and fastening soft tissues are disclosed that enable these new interventional methods to be carried out safely, efficiently and quickly. Methods for reversing the procedure are also disclosed.
