[0040] Endoscopy is widely used to treat and / or diagnose a variety of conditions. Some of these conditions (such as hemorrhagic ulcers) can be treated by delivering fluid to the treatment site using a syringe in conjunction with an endoscope. Understandably, delivering fluid in conjunction with an endoscope can be challenging. This document discloses systems and apparatuses for delivering fluid in conjunction with an endoscope (e.g., in conjunction with an endoscope).
[0041] Figure 1 An exemplary system 10 for use with endoscope 12 is shown. System 10 can be secured to endoscope 12 and can be used, for example, to deliver fluid to a target object. At least in some cases, the target object can be a hemorrhagic ulcer. Therefore, system 10 can deliver fluid and / or an attachment patch (e.g., a fluid or fluid-like substance that can form an attachment patch), which can, for example, help cover the hemorrhagic ulcer, slow or stop bleeding, reduce perforation, reduce stenosis formation, combinations thereof, etc. In addition to treating hemorrhagic ulcers, system 10 can also be used to treat patients after tissue resection (e.g., endoscopic mucosal resection, endoscopic submucosal dissection, polyp removal, etc.), reinforce sutures, seal fistulas, treat or close transoral endoscopic incision sites, etc. Some additional interventions are considered for using system 10.
[0042] System 10 may include a sleeve 14. Generally, sleeve 14 may be configured to couple to and / or be disposed along the outer surface of endoscope 12. In some cases, sleeve 14 may contain silicone. Other materials are also considered. Sleeve 14 may facilitate securing system 10 to endoscope 12 (e.g., friction fixation). Other mechanisms for securing system 10 to endoscope 12 are also considered, including mechanical bonding, adhesive bonding, thermal bonding, combinations thereof, etc. In some cases, system 10 may be permanently fixed to endoscope 12. In this case, endoscope 12 may be included in system 10. In other cases, system 10 may be attached to endoscope 12. In other words, system 10 may be an add-on component that can be added to endoscope 12.
[0043] A fluid sleeve or housing 16 may be coupled to a sleeve 14. The fluid sleeve 16 may include an inner wall 18 and an outer wall 20. A fluid chamber 22 may be defined within the fluid sleeve 16, for example, between the inner wall 18 and the outer wall 20. The fluid sleeve 16 may have a distal region and / or a distal surface 24. In some cases, the distal surface 24 may be arcuate. One or more openings 26 may be formed on the distal surface 24.
[0044] Generally, the fluid cannula 16 may be configured to contain fluid for delivery to a suitable target location. For example, for an intervention aimed at treating a hemorrhagic ulcer, one or more therapeutic fluids may be contained in the fluid chamber 22. Exemplary fluids may include acrylates, methacrylates, n-butyl 2-cyanoacrylate, other materials and / or adhesives, etc. Such fluids may be present in monomeric form within the fluid chamber 22 and may polymerize to adhere to wound tissue upon delivery and / or contact with tissue. Other fluids may be considered. In at least some cases, the fluids may be biocompatible, at least partially visible and perceptible during the intervention, effective and / or efficient for treating appropriate conditions (e.g., covering a hemorrhagic ulcer, stopping bleeding, etc.), relatively flowable and / or easy to apply, relatively easy to deliver along the digestive tract, combinations thereof, etc. In some cases, the fluid may form an attachment patch and / or otherwise take the form of an attachment patch.
[0045] In some cases, the fluid may comprise multiple portions or components. In this case, the two portions may be kept separate by a partition (not shown) within the fluid chamber 22. Delivery of the two portions results in the portions mixing together and / or combining to form a therapeutic material. For example, one component may include fibrinogen (e.g., lyophilized mixed human fibrinogen) and another may include thrombin (e.g., bovine thrombin, human thrombin, etc.). In some cases, one or both of the components may include calcium salts. A variety of different portions / components are possible.
[0046] System 10 may include a distal annular member 28. In at least some cases, the distal annular member 28 may be disposed within the fluid sleeve 16. The distal annular member 28 may be axially fixed relative to the sleeve 14 and / or relative to the fluid sleeve 16. As will be described in more detail herein, the distal annular member 28 may function similarly to a plunger or plunger tip of a syringe. For example, because the distal annular member 28 may be axially fixed relative to the fluid sleeve 16, movement or translation of the fluid sleeve 16 may move the distal annular member 28 within the fluid sleeve 16 in a manner that facilitates the ejection of fluid contained in the fluid chamber 22 through the opening 26 from the fluid sleeve 16.
[0047] Actuator 30 may be coupled to fluid sleeve 16. For example, actuator 30 may be coupled to and extend proximally from the proximal region of fluid sleeve 16. In some cases, actuator 30 may be a pull cable. Pull cable 30 may be configured to move fluid sleeve 16. In some cases, system 10 may include one actuator 30. In other cases, system 10 may include one or more additional actuators 30.
[0048] One or more coil members 32 may be disposed along the outer surface of the endoscope 12. For example, one or more coil members 32 may be wound around the endoscope 12 and the actuator(s) 30. Thus, the actuator 30 may extend between the one or more coil members 32 and the outer surface of the endoscope 12. Alternatively, one or more coil members 32 may be wound around each actuator 30 and extend proximally along the actuator 30.
[0049] In some cases, system 10 may include a proximal annular member 34. The proximal annular member 34 may be disposed proximal to the distal annular member 28. In some cases, actuator 30 may extend through the proximal annular member 34. Spacer 36 may extend between the distal annular member 28 and the proximal annular member 34. The proximal annular member 34, the distal annular member 28, and the spacer 36 may be axially fixed relative to each other. For example, the proximal annular member 34 may be axially fixed relative to the distal annular member 28 by the spacer 36.
[0050] As described in this article, the fluid sleeve 16 can be as follows: Figures 2 to 3 The device is configured to move relative to endoscope 12 between a first position and a second position. For example, when... Figure 2 When the fluid sleeve 16 is in the first position, the distal annular member 28 can be disposed adjacent to the proximal region of the fluid sleeve 16. When... Figure 3When the fluid sleeve 16 is in the second position, the distal annular member 28 can be positioned distal to the proximal region of the fluid sleeve 16. Since the fluid sleeve 16 is movable relative to the distal annular member 28, the distal annular member 28 can move within the fluid sleeve 16 and function as a plunger to inject fluid from the fluid sleeve 16 through the opening 26.
[0051] Figure 4 A handle 40 coupled to actuator 30 is shown. Generally, handle 40 can be used to move actuator 30 and thus move fluid cannula 16. Handle 40 may include a finger-pull member 42 slidable along handle 44. Thumb member 46 may be disposed at the proximal end of handle 40. Actuator 30 may be coupled to finger-pull member 42 such that retraction of finger-pull member 42 along handle 44 retracts actuator 30 and thus retracts fluid cannula 16. As described above, one or more coil members 32 may be disposed around endoscope 12, or as... Figure 4 As shown, one or more coil components 32 may be wound around the actuator 30. Figure 4 The diagram shows one or more coil members 32 from which the actuator 30 extends distally. In practice, one or more coil members 32 may extend from the handle 40 to the proximal annular member 34.
[0052] Figure 5 Another exemplary system 110 is shown, which is similar in form and function to other systems disclosed herein for use with endoscope 12. System 110 may include a sleeve 114. Generally, sleeve 114 may be configured to couple to and / or be disposed along the outer surface of endoscope 12. In some cases, sleeve 114 may contain silicone. Other materials are also considered. Sleeve 114 may facilitate securing system 110 to endoscope 12 (e.g., friction fixation). Other mechanisms for securing system 110 to endoscope 12 are also considered, including mechanical bonding, adhesive bonding, thermal bonding, combinations thereof, etc. In some cases, system 110 may be permanently fixed to endoscope 12. In this case, endoscope 12 may be included in system 110. In other cases, system 110 may be attached to endoscope 12. In other words, system 110 may be an add-on component that can be added to endoscope 12.
[0053] A fluid sleeve or housing 116 may be coupled to a sleeve 114. The fluid sleeve 116 may include an inner wall 118 and an outer wall 120. A fluid chamber 122 may be defined within the fluid sleeve 116, for example, between the inner wall 118 and the outer wall 120. The fluid sleeve 116 may have a distal region and / or a distal surface 124. In some cases, the distal surface 124 may be arcuate. One or more openings 126 may be formed on the distal surface 124. Similar to the fluid sleeve 16, the fluid sleeve 116 may be configured to contain fluid for delivery to a suitable target location.
[0054] System 110 may include a distal annular member or a distal annular region 128. In at least some cases, the distal annular region 128 may be disposed within the fluid sleeve 116. The distal annular region 128 may be axially fixed relative to the sleeve 114 and / or relative to the fluid sleeve 116. As will be described in more detail herein, the distal annular region 128 may function similarly to a plunger or plunger tip. For example, since the distal annular region 128 may be axially fixed relative to the fluid sleeve 116, movement or translation of the fluid sleeve 116 may move the distal annular region 128 within the fluid sleeve 116 in a manner that facilitates the ejection of fluid contained in the fluid chamber 122 through the opening 26 from the fluid sleeve 116.
[0055] Actuator 130 may be coupled to fluid cannula 116. For example, actuator 130 may be coupled to and extend proximally from the proximal region of fluid cannula 116. In some cases, actuator 130 may be a pull cable. Pull cable 130 may be configured to move fluid cannula 116. In some cases, system 110 may include one actuator 130. In other cases, system 110 may include one or more additional actuators 130. In some cases, one or more coil members (not shown) may be disposed along the outer surface of endoscope 12 and / or wound around each actuator 130.
[0056] In some cases, system 110 may include a proximal annular member or a proximal annular region 134. The proximal annular region 134 may be located proximal to the distal annular region 128. In some cases, actuator 130 may extend through the proximal annular region 134. A spacer member or spacer region 136 may extend between the distal annular region 128 and the proximal annular region 134. The proximal annular region 134, the distal annular region 128, and the spacer region 136 may be axially fixed relative to each other.
[0057] In some cases, the proximal annular region 134, the spacer region 136, and the distal annular region 128 may be a single structure with three segments. For example, the proximal annular region 134 may have an outer diameter that is wider than the remaining segments and may be located proximal to the fluid sleeve 116. The distal annular region 128 may be located within the fluid sleeve 116. The spacer region 136 may extend between the proximal annular region 134 and the distal annular region 128, coupling the proximal annular region 134 to the distal annular region 128.
[0058] As described in this article, Figures 6 to 7 As shown, the fluid cannula 116 can be configured to move relative to the endoscope 12 between a first position and a second position. For example, when... Figure 6 When the fluid sleeve 116 is in the first position, the distal annular region 128 can be disposed adjacent to the proximal region of the fluid sleeve 116. When... Figure 7 When the fluid sleeve 116 is in the second position, the distal annular region 128 can be positioned distal to the proximal region of the fluid sleeve 116. Since the fluid sleeve 116 is movable relative to the distal annular region 128, the distal annular region 128 can move within the fluid sleeve 116 and function as a plunger to inject fluid from the fluid sleeve 116 through the opening 126.
