[0001] USE OF AN EXTRACT OF HERICIUM ERINACEUS FOR THE TREATMENT OF VAGINAL AND CERVICAL DISEASES

[0002] The present invention belongs to the field of medicine, specifically to the treatment of vaginal and cervical diseases and / or lesions. The present invention relates to an extract of Hericium erinaceus, preferably obtained from the carpophore of said fungus, for use in the treatment and / or prevention of vaginal or cervical epithelial diseases and lesions, caused by an infectious agent, in particular by the human papillomavirus (HPV).

[0004] Vaginal infections are considered the most common gynaecological problem in women of reproductive age, affecting millions of people each year and being the most common cause of gynaecological medical care. The most common infections include bacterial vaginosis, aerobic vaginitis, vulvovaginal candidiasis, trichomoniasis and viral infections. Viral infections are the most difficult to treat. In particular, human papillomavirus (HPV) infection is among the most common sexually transmitted diseases in the world, with the highest prevalence among women under 25 years of age. HPV infection is the leading cause of cervical cancer and is linked to many other types of cancer.

[0005] The human papillomavirus (HPV) is an infectious agent belonging to the virus family Papillomaviridae, the members of which exhibit tropism for the cutaneous epithelium and the mucosal epithelium.

[0006] There are more than 100 subtypes of HPV, of which 13 are considered high risk. The HPV-16 and HPV-18 subtypes are the most oncogenic and prevalent, accounting for about 70% of cervical cancer cases. It is estimated that 80% of sexually active women will have been infected at some point. More than 90% of HPV infections are transient and cleared by an immune response within 6 to 18 months, although a new HPV reinfection may occur. The persistence of the virus can lead to the development of low- grade squamous intraepithelial lesions (LSIL) in the cervix. LSILs are characterised by the presence of abnormal cells on the surface of the cervix. LSILs can lead to high-grade squamous intraepithelial lesions (HSILs) that can ultimately lead to cervical cancer. Factors that correlate with higher persistence rates include immunodeficiency. Incidence data indicate that cervical cancer is the second most common tumour in women worldwide. It is estimated that each year there are more than 500,000 new cases of cervical cancer and approximately 280,000 deaths.

[0007] There are vaccines for the treatment of HPV but they are not effective against all serotypes. Furthermore, when a person has already been exposed to the virus before vaccination, the efficacy of the vaccine against serotypes such as HPV-16 or HPV-18 decreases.

[0008] Despite the large number of successfully vaccinated women around the world, many populations still have not had the opportunity to be vaccinated. In particular, current HPV immunisation programmes target only 12% of young adolescent women worldwide, while the remaining women will miss the opportunity to get vaccinated without rapid implementation of the HPV vaccine. Many countries in Africa and Asia, which represent the majority of the world’s population, have very vulnerable populations at a very high risk of developing cervical cancer and other HPV-related diseases.

[0010] Therefore, it is necessary to provide alternative strategies to combat HPV infection and cervical cancer, particularly in the early stages of HPV infection.

[0011] DESCRIPTION OF THE INVENTION

[0012] The present invention relates to an extract of Hericium erinaceus, preferably obtained from the carpophore of said fungus, for use in the treatment and / or prevention of vaginal or cervical epithelial diseases and lesions, preferably caused by an infectious agent, in particular by the human papillomavirus (HPV).

[0013] The inventors have obtained greater in vitro therapeutic activity in HeLa cell lines with the extract of Hericium erinaceus than with extracts derived from other species of fungi such as, for example, Lentinula edodes, Coriolus versicolor, Grifola frondosa or Ganoderma lucidum (see Table 1 of the examples). Furthermore, the inventors have discovered that with the carpophore extract of Hericium erinaceus a surprisingly higher percentage inhibition of HeLa cells is obtained than when a mycelium extract of this fungus is used (see Table 2 of the examples). By way of example, when using a carpophore extract at concentrations of 1 mg / mL, 0.1 mg / mL and 0.02 mg / mL, a percentage inhibition of HeLa cells of 42%, 46% and 41 %, respectively, is obtained, while the use of a mycelial extract of Hericium erinaceus, at a concentration of 0.01 mg / mL, gives an inhibition of between 1 and 8%.

[0014] Therefore, in a first aspect, the present invention relates to an extract of Hericium erinaceus obtained from the carpophore, hereinafter “the extract of the invention” or “the carpophore extract of the invention”. Hericium erinaceus is a basidiomycete of the family Hericiaceae, considered a natural probiotic that helps to restore microbiota and contains a large number of bioactive compounds, especially p-glucan polysaccharides, which are responsible for the anti-cancer, immunomodulatory, antioxidant, neurostimulatory and neuroprotective activities of this species.

Compounds that inhibit the growth of this human cervical adenocarcinoma cell line could be used in the prevention and / or treatment of vaginal and / or cervical epithelial lesions (re-epithelialisation) and / or the treatment or prevention of diseases caused by the HPV infectious agent, particularly cervical cancer. 2.1. Hela 229 cell line culture conditions

These cells were cultivated in DMEM (Dulbecco’s Modified Eagle Medium) cell culture medium supplemented with 10% FBS (foetal bovine serum) and 2 mM L-glutamine in an atmosphere of 95% air and 5% CO2 at 37°C.

[0169] 2.2. Cytotoxicity study

[0170] The inhibition of cell growth induced by the different fungal extracts was evaluated using a system based on MTT tetrazolium salts (3-[4,5-dimethylthiazole-2-yl]-2,5- diphenyltetrazolium bromide), and on its ability to be transformed into formazan. Metabolically active living cells are able to reduce MTT, with a yellow colour in solution, to formazan, an insoluble compound that precipitates in the form of violet crystals, thus demonstrating metabolic activity. Formazan absorbs at a wavelength of 550 to 570 nm.

[0171] Hela 229 cells were seeded in a sterile 96-well plate at a density of 4,000 cells / well and incubated for 4-6 hours in the aforementioned growth medium. Subsequently, the different tested extracts dissolved in H2O or DMSO were added to the cells, depending on the solubility of the ingredient, maintaining the same ratio of solvent in each well. [0177] With a commercial carpophore extract of Lentinula edodes at concentrations of 0.1 mg / mL and 0.01 mg / mL, a percentage inhibition of 11 % and 34%, respectively, is obtained (Table 1 ). The carpophore extract of Lentinula edodes used is a hydroalcoholic extract obtained by percolation, using water and ethanol as a solvent in a 1 :1 ratio, at a solid-liquid ratio of 1 :20 for 3 hours at a temperature of 45°C. The composition of the carpophore extract of Lentinula edodes comprises 9.07% m / m of beta-glucans, 205.7 pg / g of GABA, 584.21 pg / g of ergosterol and 12.01 pg / g of lovastatin.

The composition of the carpophore extract of Coriolus versicolor comprises 66.75% m / m of beta-glucans, 610.29 pg / g of ergosterol and 12.69 pg / g of lovastatin.

[0187] High Risk Human Papillomavirus (HPV) infection is correlated with precancerous cervical lesions. E6 and E7 proteins, present in subtypes such as HPV-18, are responsible for increased interleukins levels which can promote viral replication and for premalignant cells to avoid attack by the immune system, leading to the slow accumulation of microscopic lesions (low-grade squamous intraepithelial lesions, LSIL). LSILs can lead to high-grade squamous intraepithelial lesions (HSIL).

[0188] Hela Cells is an HPV-18 positive immortalized cervical cancer line, isolated from cervical cancer patient Henrieta Lacks, with integrated genes encoding HPV E6 and E7 proteins identified as major viral oncogenes. The available evidence of HPV and cervical neoplasia suggests that a relationship between the likelihood of precursor lesions persisting and progressing to cancer can be dependent on the characteristics of the HPV infection. Due to this, the inventors have used a cell line that contains subtype HPV-18, which is among the 5 most frequently detected in cervical intraepithelial neoplasia (CIN) grades Il-Ill and stages l-IV cervical cancer. The results revealed that the extract of Hericium erinaceus (carpophore or mycelium extract) exhibited a stronger growth inhibitory effect in HeLa cells, particularly compared to carpophore extracts from other mushroom species as shown above. It can be concluded that Hericium erinaceus extract, particularly carpophore extract, presents a high cytotoxic activity against malignant high risk cervical cancer cells, and thus preventing the accumulation of cervical microscopic precancerous lesions.

[0189] The development and progression of cervical lesions can lead to the formation of cancer, especially high-risk strains such as RH-18 present in the selected cellular model HeLa cells. These lesions are due to a precancerous neoplastic process characterized by the presence of mild dysplastic cytological changes that may be due to HPV. Therefore, Hericium erinaceus extract will contribute to prevent or reverse cervical intraepithelial lesions related to HPV. Inhibiting the growth of precancerous cells infected with HPV could prevent and / or reverse the HPV associated lesions.

[0190] 4. Isolation of primary fibroblasts, cell culture and treatments.

[0191] Primary cells isolated from patients were used in this study. Cell culture was performed in 10 centimeter (cm) diameter Petri dishes in Modified Eagle Medium (DMEM, Lonza) supplemented with 10% fetal bovine serum (FBS) and 1 % penicillin / streptomycin (P / S)). The cells were incubated in culture ovens at 37eC and 5% CO2. Dermal fibroblasts were cultured for 24 h.

[0192] Subsequently, the Hericium erinaceus carpophore extract is added at a concentration of 10 ng / mL. The extract was diluted in water to the mentioned concentrations. Fibroblasts to which no compound was added were used as a control. After 24 hours, the samples are collected and the mRNA expression levels of the cytokine IL-8 are analyzed following standardized methodologies. RNA extraction was carried out using Tryzol® Reagent (Invitrogen) following the protocol described in the instruction manual. 1 mL of T ryzol was added for cell lysis, allowing it to rest for 5 minutes at 4°C. Subsequently, 200 pL of chloroform was added, and centrifugation was carried out for 15 minutes at 12,000 rpm and 4°C. The supernatant was selected and diluted in 500 pL of isopropanol, homogenizing the preparation and centrifuged at 12,000 rpm for 15 min and 4eC. Finally, 1 mL of cold 70% ethanol was added and the samples were centrifuged at 12,000 rpm for 5 minutes and 4eC. RNA concentration was quantified using spectrophotometric techniques (Nanodrop ND-1000). Complementary DNA (cDNA) synthesis was then carried out from 1 mg of RNA using Superscript® VILO™ cDNA Synthesis kit (Invitrogen). To analyze gene expression, real-time quantitative PCR (RT-qPCR) was carried out. In each reaction, 5pL of the cDNA dilution was used; 5 pL of primer mix in a 1 :1 ratio (10 pM sense primer, 10 pM antisense primer); 10pL of SYBR Green (PowerUPTM SYBRTM Green Master Mix, Thermo Fisher) in a real-time thermocycler (LightCyclera480 System, Roche). The tests were carried out in triplicate.

[0193] High risk HPV E6 and E7 proteins are responsible for increased levels of certain proinflammatory interleukins levels leading to viral replication in malignant cells and the slow accumulation of microscopic precancerous cervical lesions.

[0194] Hericium erinaceus carpophore extract at a concentration of 10 ng / mL decreases the expression of inflammatory cytokines compared to untreated fibroblasts. Specifically, the inventors have observed that for IL-8, the extract of Hericium erinaceus carpophore reduces the expression of these cytokine by 13% (Table 3).

For cervical intraepithelial lesions the most common cause is an infection with the human papilloma virus (HPV). The pathogenesis of HPV induced cervical lesions includes imbalance of cytokine expression in epithelium and fibroblasts. It must be noted that the expression of IL-8 is higher in cervical intraepithelial lesions when compared to normal tissues. The balanced expression of these cytokine suggests that pro- and anti-inflammatory cytokine balance has an important role in precancerous cervical lesions morfopathogenesis. Hericium erinaceus carpophore extract decreases the expression of inflammatory cytokine IL-8 in fibroblasts. Therefore, Hericium erinaceus carpophore extract will contribute to counteracting the effect of proinflammatory cytokines such as IL-8, thus maintaining immune balance and healthier cervical epithelium.

[0197] Most HPV infections are transient and are eliminated by a medium-term immune response. A proinflammatory immune response in fibroblast, which would be associated with a transition of this precancerous cervical lesions to cervical cancer, can also be directly inhibited by the Hericium erinaceus carpophore extract. These results underlined the effect of Hericium erinaceus carpophore extract to prevent or reverse cervical intraepithelial lesions related to HPV.

Claims

CLAIMS1. An extract obtained from the mycelium or from the carpophore of Hericium erinaceus or a composition comprising it for use in the treatment and / or prevention of vaginal or cervical epithelial lesions, wherein the vaginal or cervical epithelial lesion is caused by an infectious agent and wherein the infectious agent is human papillomavirus (HPV).

2. The extract for use according to claim 1 , wherein the extract is obtained from carpophore of Hericium erinaceus.

3. The extract for use according to claim 2, wherein the extract comprising 24.37% m / m of beta-glucans, 51.2 pg / g of GABA, 348.2 pg / g of ergosterol and 0.2% m / m of Hericenone C.

4. The extract for use according to claim 2 or 3, wherein the extract is comprised in a composition, preferably wherein the composition comprises the extract of Hericium erinaceus in a concentration between 0.5% and 25% m / m.

The extract for use according to claim 4, wherein the composition is a nutritional composition or a pharmaceutical composition, preferably wherein the pharmaceutical composition further comprises a pharmaceutically acceptable carrier and / or excipient or wherein the nutritional composition is a food or nutritional supplement.

6. The extract for use according to claim 4 or 5, wherein the composition is in liquid, gel or solid form.

7. The extract or the composition for use according to any one of claims 1 to 6, wherein the composition is administered orally, topical vaginally, parenterally, rectally and / or cervically, preferably wherein the composition is administered simultaneously orally and topical vaginally.
