[0001] Injectable device

[0002] DESCRIPTION

[0003] The present invention relates to a composition comprising hyaluronic acid and at least one collagen component.

[0004] STATE OF THE ART

[0005] Hyaluronic acid (HA) is a member of a vast family of glycosaminoglycans (GAGs) which are the main components of the extracellular matrix. The unique characteristics distinguishing it from other GAGs are its simple structure and large molecular size. The HA molecule is made up of D- glucuronic acid and N-acetyl-D-glucosamine linked with [3-glycosidic bonds. Its peculiar properties and its role in important biological functions have made it the subject of innumerable biochemical and medical studies. The numerous uses this molecule has found, in both the medical and cosmetic fields, have obviously contributed to the extraordinary interest surrounding HA. Since it became possible to source it for industrial applications, the properties of gelling and retaining a large amount of water have made it an ideal component for cosmetic moisturising preparations, so much so that it has today become the ingredient par excellence of mid to high-end cosmetics, and also the subject of particularly effective communication. But it is the properties of HA in different fields of application that make it most interesting from a scientific viewpoint, and for which applications have been developed that have in any case made it very popular and widely used for a vast range of medical applications. Firstly, HA is administered by injection to compensate for its loss due to aging, diseases or surgical procedures. Clinical uses include dermal filling, viscosupplementation in deteriorated joints, as ophthalmic surgical aids and as a tear replacement. For such applications, HA has the advantage of being biocompatible and safe and induces a minimum reaction as a foreign body. Because of its important physiological role in tissue repair, as mentioned above, HA is also used for wound healing.

[0006] Many of the applications mentioned above are not obtained using HA in its native form: in fact, “crosslinked” molecules are often used. Crosslinking of a polymer is a process whereby linkages are created between independent chains which make the structure of the polymer stiffer. In the case of HA, crosslinking not only maintains the physiological function of HA, but also imparts the skeletal function of a hydrogel, which further expands the possible applications, starting from the possibility of modulating the rheological properties of the gel. For hyaluronic acid, moreover, crosslinking also has another considerable effect: crosslinked structures are less subject to attack by degrading enzymes (hyaluronidase), and this is why they remain for a longer time at the application site.

[0007] Another compound that is very useful in maintaining the physiological conditions of numerous body structures is collagen. In fact, collagen is one of the body’s most important structural and biological components; it is a structural protein with minimal immunoreactivity which plays structural roles in the main connective tissues, such as the skin, cornea, ligaments, tendons, joint capsules, and in cartilage in general. Many different uses of collagen are described in the literature, particularly for the treatment of diseases affecting the skin and joints. Collagen has shown to be very effective and safe; its side effects are wholly negligible. However, the halflife of collagen, when administered by injection, is very brief. In fact, it is rapidly attacked by enzymes present in body tissues, such as, for example, collagenases, which very quickly degrade collagen.

[0008] Therefore, there is a greatly felt need to provide a system that can transfer collagen in a controlled manner, thus providing an availability and effectiveness that are prolonged over time.

[0024] DETAILED DESCRIPTION OF THE INVENTION

[0025] A first aspect of the present invention regards a composition comprising hyaluronic acid and at least one collagen component.

[0026] In one embodiment, the composition comprises crosslinked hyaluronic acid with at least one collagen component. In other words, the invention relates to a crosslinked hyaluronic acid composition comprising at least one collagen component.

[0027] In fact, the invention preferably relates to a composition for the extended and / or delayed release of the at least one collagen component or derivative. In one embodiment, the collagen, i.e. the collagen from which the derivative or component is obtained, is chosen from collagen type I, II, III, IV, V, VI, VII, VIII, IX, X, XI, XII, XIII, XIV, XV, XVI, XVII, XVIII, XIX, XX, XXI, XXII, XXIII, XXIV, XXV, XXVI, XXVII, preferably type I, II or III, preferably type I, preferably type II, preferably type III.

Said at least one collagen component or derivative, for example a peptide, is linked to the hyaluronic acid.

[0051] In fact, the at least one collagen component remains included in the network formed by the hyaluronic acid, thanks to a crosslinking process, and it is thus not available outside the network of the gel. The gel thus obtained, therefore, is capable of releasing the peptides and / or amino acids of the collagen component or derivative in a controlled manner when used in vivo. In this manner, at the application sites, the peptides and / or amino acids are freed during the process of absorption of the hyaluronic acid. The at least one collagen component or parts thereof, once freed from the hyaluronic acid network, is available for attack by enzymes (e.g. collagenase) and degraded, in turn, to amino acids, which are thus available at the application site. In this manner, the at least one collagen component or derivative or a part thereof is released from the network in a controlled manner.

[0052] One application of choice of the composition according to the invention is the use thereof as an injectable filler for aesthetic medicine. In this context, the filler acts like a typical hyaluronic acid-based filler, compensating for the lack of tissue in wrinkles or in other skin depressions. As occurs with this type of filler, over time the hyaluronic acid is absorbed. During this process, and from the start, the amino acids are released on the application site and form a reserve of components for the production of new collagen, also stimulated by the presence of the hyaluronic acid. The new collagen can, at least in part, replace the hyaluronic acid that is degraded and prolong the efficacy of the filler even after all the hyaluronic acid has been absorbed. In this manner, the aesthetic effect of the filler is prolonged, and a collagen substrate is formed which can make a possible subsequent application of a filler more effective.

[0053] A second aspect of the present invention regards a process for preparing the composition described above, preferably for preparing a crosslinked hyaluronic acid hydrogel. The process comprises at least one step of crosslinking the hyaluronic acid in the presence of the at least one collagen component or derivative.

[0054] In one embodiment, the process comprises the steps of:

[0056] II) adding hyaluronic acid to the composition;

[0057] III) adding to the composition obtained in step II) at least one crosslinker to obtain a crosslinking composition; and IV) keeping the crosslinking composition at a temperature between 10 and 35°C, more preferably between 15 and 25°C for between 1 hour and 15 days, more preferably between 1 and 7 days.

[0058] In one embodiment, steps l-lll are carried out in any order, thereby obtaining a composition with different characteristics.

[0059] Preferably, the at least one collagen component or derivative is a peptide or at least a portion thereof.

[0060] In a preferred embodiment of the invention, the at least one collagen component or derivative is as described above in detail.
