Abstract
Description
[0001] Highly concentrated dermally applicable permethrin preparation with increased drug load in the target compartment for improved treatment of ectoparasite infestation
[0002] Description
[0003] Subject of the invention
[0004] The present invention relates to a semi-solid cream preparation for cutaneous = dermal-topical application in / treatment of ectoparasites / ectoparasite infestation, wherein the cream preparation comprises the following components: water, one or more hydrocarbon (lipid) components, selected from non-polar, in particular aliphatic liquid, semi-solid and / or solid hydrocarbons (HC), especially one or more petrolatum, one or more paraffins or mixtures thereof; one or more polar excipients with emulsifying properties (EMU), wherein the excipients with emulsifying properties are selected from C 10 - C 28 -fatty alcohols, C 10 - C 28-fatty alcohol sulfates, C 10 - C 28 - fatty alcohol sulfate salts and mixtures thereof; and the active ingredient permethrin (WS) in an amount of 8 to 13 wt.%, wherein the ratio of hydrocarbon component (KW) to (WS) active ingredient permethrin is 1.5 to 2.7, or 1.8 to 2.4, or even 1.9 to 2.3, in one embodiment 2.0 to 2.2, based on the wt.% of (KW) and (WS) in the total preparation. In one embodiment, the semi-solid cream preparation therefore contains water, at least one hydrocarbon (lipid) component (KW), at least one excipient with emulsifying properties (EMU), and the active ingredient permethrin WS.
[0005] In a further embodiment, the ratio of the wt. % of hydrocarbon component (KW) to (WS) active ingredient permethrin is 1.5 or more, 1.6 or more, 1.7 or more, 1.8 or more, 1.9 or more, or 2.0 or more. In a further embodiment, the ratio of hydrocarbon component (KW) to (WS) active ingredient permethrin is 2.7 or less, 2.7 or less, 2.6 or less, 2.5 or less, 2.4 or less, 2.3 or less, or 2.2 or less. Most preferably, in the use according to the invention, the ratio (KW) to (WS) is approximately 2.1.
[0006] The semi-solid cream preparation may contain additives selected from preservatives, skin conditioning agents, antihistamines, corticoids, or mixtures thereof. Common preservatives for the semi-solid cream preparation include phenoxyethanol, parabens (such as methylparaben and propylparaben), benzyl alcohol, ethylhexylglycerin, sorbic acid, potassium sorbate, sodium benzoate, chlorphenesin, benzalkonium chloride, and dehydroacetic acid. These substances are used to prevent the growth of microorganisms in skin care products, thus ensuring the shelf life and safety of the products.
[0007] Common skincare ingredients include hyaluronic acid, retinol, vitamin C, niacinamide, ceramides, peptides, glycolic acid, salicylic acid, squalane, allantoin, aloe vera, panthenol, shea butter, jojoba oil, and collagen. These ingredients offer various benefits, such as moisture retention, skin renewal, free radical protection, skin soothing, and skin barrier support.
[0008] Common antihistamines include cetirizine, loratadine, fexofenadine, diphenhydramine, desloratadine, levocetirizine, dimenhydrinate, hydroxyzine, promethazine, and ebastine. These medications are often used to treat allergies, hay fever, and allergic skin reactions by blocking the action of histamine, which is released during allergic reactions.
[0009] Common corticosteroids include hydrocortisone, mometasone, betamethasone, clobetasone, triamcinolone, clobetasol, fluticasone, desoximetasone, diflorasone, and fluocinolone. These corticosteroids are used in dermatology to treat inflammation, itching, and allergic skin reactions by modulating the skin's immune response.
[0010] The additives in the semi-solid cream preparation may be contained in an amount of 0 to 7% by weight, in particular 0.01 to 5% by weight.
[0011] In a further embodiment, the semi-solid cream preparation contains additives in an amount (in wt%) of 0.01 or more, 0.05 or more, 0.1 or more, 0.5 or more, 1.0 or more, 1.5 or more, 2.0 or more, 2.5 or more, 3.0 or more, or 4.0 or more. In a further embodiment, the semi-solid cream preparation contains additives in an amount (in wt%) of 7.0 or less, 6.8 or less, 6.5 or less, 6.2 or less, 6.0 or less, 5.8 or less, 5.5 or less, 5.2 or less, or 5.0 or less. In a further embodiment, the semi-solid cream preparation contains additives in an amount (in wt. %) of 0 to 7.0, of 0.01 to 7.0, of 0.05 to 6.8, of 0.1 to 6.5, of 0.5 to 6.2, of 0.5 to 6.0, of 1.0 to 5.8, of 1.5 to 5.5, or of 2.0 to 5.0.
[0012] The application according to the invention relates in particular to the control of mites, such as scabies mites in humans, and the areas affected therein and, in this respect, the treatment of associated diseases such as scabies (scabies).
[0013] Scabies, medically known as scabies, is a contagious skin disease caused by tiny mites called Sarcoptes scabiei. These mites burrow into the top layer of skin, where they lay their eggs and leave behind waste products. This leads to intense itching and a characteristic rash.
[0014] The itching associated with scabies is often worst at night and is caused by the body's immune response to the mites, their eggs, and their waste products. Affected skin areas are commonly found in the interdigital spaces (between the fingers), on the wrists, in the armpits, around the waist, on the elbows, in the groin, and on the buttocks. In infants and young children, the face, scalp, and soles of the feet may also be affected.
[0015] Scabies is primarily transmitted through close skin-to-skin contact, but transmission can also occur through shared clothing, bedding, or towels. The disease is more common in overcrowded or unsanitary living conditions, but can affect anyone, regardless of hygiene or social status.
[0016] The preparation according to the invention can be easily applied cutaneously and / or dermally to the skin and / or appendages of mammals, especially humans. Surprisingly, it exhibits an unexpectedly high penetration capacity into the relevant skin layers (middle to lower stratum corneum) and a higher and thus superadditive effectiveness compared to the active ingredient preparation applied twice and concentrated according to current standards (5%). This was demonstrated particularly in the complete elimination of scabies mite infestation (cure of scabies) just two weeks after the first application. The invention also relates to the use of permethrin (WS) and one or more non-polar hydrocarbons = hydrocarbon (lipid) components (KW), in particular of paraffins and / or petrolatums such as white petrolatum, yellow petrolatum, thick paraffin, microcrystalline paraffin, hard paraffin (solid paraffin), thin paraffin or mixtures thereof, in a weight ratio (KW) to (WS) of 1.5 to 2.7, or 1.8 to 2.4, or also 1.9 to 2.3, especially 2.2 to 2.0, based on wt.% of (KW) and (WS) in the total preparation, for producing a semi-solid cream preparation with improved application properties in topical-cutaneous use against ectoparasites, especially mites, in particular scabies mites and for treating scabies (scabies).
[0017] The semi-solid cream preparation comprises or consists of 8 to 13 wt.% permethrin (WS), 38 to 73 wt.% water, 13 to 34 wt.% hydrocarbon component (HC), 6 wt.% to 15 wt.% of one or more excipients with emulsifying properties (EMU), especially selected from C 10 - C28 fatty alcohols and fatty alcohol sulfates, fatty alcohol sulfate salts, emulsifying cetylstearyl alcohol type A or type B and mixtures thereof, 0 to 7%, in particular 0.01 to 5% by weight of additives, in particular selected from preservatives, skin care substances, antihistamines, corticoids or mixtures thereof.

[0022] Background of the invention
[0023] Semi-solid preparations for cutaneous (topical) application generally comprise a suitable base with one or more active ingredients dissolved or dispersed within it. The base can be monophasic or multiphasic and, depending on the ingredients, exhibit hydrophilic or hydrophobic properties. The term 'semi-solid' means that the preparations' properties lie between solid and liquid preparations. See Stephanie Jacobsen, "Arzneiformlehre: Ein Wegweiser in Bildern," 2nd revised edition, 2013, ISBN 13: 978304731615, Chapter 9.
[0024] Suitable preparations for cutaneous application include creams. These are multiphase systems, particularly two-phase systems, consisting of a water and a lipophilic phase. Creams can be divided into hydrophilic creams (oil-in-water creams) and lipophilic (hydrophobic) creams (water-in-oil creams), hydrophilic ointments or creams, amphiphilic creams, etc.
[0025] Of importance for the cutaneous effect of such preparations are, among other things, the structure and condition of the skin. This is essentially divided into three layers: the epidermis, the dermis (cutis), and the subcutis. The skin layers exhibit different polarities and different water contents, see Jacobsen, op. cit.
[0026] Active ingredients from cutaneous preparations can penetrate the skin directly through the cells (transcellular route) or by passing through the intercellular space (intercellular route).
[0027] The penetration of an active ingredient into the upper layers of the skin (epidermis) is referred to as penetration, and subsequent penetration into deeper layers (into the dermis) is referred to as permeation. Penetration into (deeper) skin layers can be enhanced by excipients (penetration accelerators, enhancers). See also Stephanie Jacobsen, "Arzneiformlehre: Ein Wegweiser in Bildern," 2nd revised edition, 2013, ISBN 13: 978304731615, Chapter 9.
[0028] Penetration results in a local, and therefore essentially non-systemic, effect, whereas in the case of relevant permeation, a relevant systemic effect due to penetration into the area of the skin's blood vessels cannot be ruled out.
[0029] Cutaneous application may be necessary for various influences on the skin, e.g., to protect against dehydration, radiation, injuries, wounds, allergies, eczema, and others, which can have various causes. A common cause is parasites such as ectoparasites, which can infest humans as well as their livestock and pets. These include anthropods such as ticks, mites, fleas or lice, flies, mosquitoes, and the like, and can lead to serious illnesses, both through direct effects (e.g., skin lesions, rashes) and through the transmission of pathogenic substances. Examples of such illnesses include encephalitis, Lyme disease, malaria, and especially scabies (the technical term: scabies). Scabies is a contagious disease caused by specific scabies mites and is transmitted through close physical contact. The mites burrow into the uppermost layer of the skin and lay eggs there. The skin itches, and a rash, redness, and blisters develop. If the itchy areas are scratched, small wounds form. If the mites spread, a bacterial infection can occur. The skin then becomes very red and oozing, sometimes with pus and scabs.
[0030] The health problems caused by the described parasites, especially scabies, require treatment.
[0031] State of the art
[0032] EP 2816898 B1 describes liquid combination agents consisting of 30 to 55 wt.% permethrin, 2-15 wt.% Fibronil with a special solvent system consisting of Myglyol and N-methylpyrrolidone in order to achieve improved solubility and stability and thus improved effectiveness of the active ingredients against parasites such as mites.
[0033] EP 1379138 B1 (US7728011 B2) relates to dermally applicable liquid formulations containing permethrin and agonists or antagonists of the nicotinic acetylcholine receptors of insects for controlling fleas and ticks, comprising 35-60 wt% of the active ingredient permethrin, 2.5-12.5 wt% imidacloprid or imidacloprid analogue, 27.5-62.5 wt% N-methylpyrrolidone, 0-5 wt% water, 0-0.5 wt% phenolic antioxidants and 0-0.5 wt% organic acids.
[0034] The use of topical formulations containing permethrin (3-phenoxyphenyl) methyl 3-(2,2-dichloroethenyl)-2,2-dimethylcyclopropanecarboxylate (CAS No. [52645-53-1]) for controlling parasitic insects on animals is known, see, for example, WO 95 / 17090, relating to a non-aqueous liquid preparation. EP-A-0567368 relates to liquid anhydrous pesticide compositions for controlling insects and mites in animals, which contain oils and aromatic solvents, in particular comprising 200 to 800 g / l of vegetable oil and 300 to 500 g / l of biphenyl aromatic solvent and a pyrethroid, especially deltamethrin or permethrin, which are to be applied in the form of baths, aerosols or by the so-called "pour-on" method. They can also be used in shampoos, lotions or creams, for example to control lice in humans. An exemplary preparation is composed of (per g / L): Permethrin: 159.6, BHT (butylhydroxytoluene): 1, calcium phenylsulfonate 30, Emuisogen EL 360: (PEG-36 Castor Oil) = ethoxylated castor oil: 20, cyclohexanone: 150, cottonseed oil 300, biphenyl solvent 273. As a biphenyl aromatic solvent, for example, dibenzyltoluene (Marlotherm SH, mixture of aromatic hydrocarbons) is mentioned here.
[0035] EP 0892606 B1 relates to formulations for the dermal control of parasitic insects and mites on humans by means of agonists or antagonists of the nicotinic acetylcholine receptors of insects, such as chloronicotinyl insecticides, in a concentration of 0.0001 to 20 wt.%, solvents from the group of cyclic carbonates in a concentration of 2.5 to 99.9999 wt.%, optionally further solvents from the group of alcohols in a concentration of 0 to 95 wt.%.
[0036] EP 1587368 B1 describes the use of antiparasitic preparations such as permethrin (40 / 60 cis / trans) in amounts of 15, especially 40 and more wt.% together with nicotinic acetylcholine receptor agonists in solid or N-methylpyrrolidone-containing liquid preparations for repelling anthropods in warm-blooded animals.
[0037] WO 93 / 24002 A discloses that certain 1-[N-(halo-3-pyridyl-methyl)]-N-methylamino-1-alkylamino-2-nitroethylene derivatives are suitable for systemic application against fleas on pets. According to this WO 93 / 24002, the non-systemic—i.e., dermal—application route is unsuitable for controlling fleas on pets.
[0038] EP0003251 A1 relates to a liquid electrostatically sprayable insecticide preparation comprising a solution of 0.5 to 50% of an insecticide such as permethrin in an organic solvent, the preparation having a resistance at 20°C of 1 x 10 6 up to 1 x 1010 Ohm centimetres, and a viscosity at 20°C in the range of 1 to 50 centistokes.
[0039] EP 4122468 A1 = WO2021 / 213483 A1 discloses the use of a pharmaceutical preparation for controlling and combating mites, in particular also for combating eye diseases caused thereby, such as marginal blepharitis, meibomian gland dysfunction, tarsitis, madarosis, abnormal eyelash alignment, glaucoma, and others, or for combating skin diseases such as seborrheic dermatitis, acne, acne rosacea, and the like. The active ingredient should be selected from taraxasterol, taraxerol, taraxerone, roburic acid, taraxasterol acetate, taraxeryl acetate, lupenone, diosmetin, or swertiamarin, sweroside, gentiopicroside, and loganic acid, in particular as ophthalmic drops, gel, tincture, injection, ointment, powder, or aerosol.
[0040] EP 4129314 A1 / WO2021 / 213485 A1 relates to pharmaceutical preparations for controlling mites containing dandelion active ingredients, namely monomer compounds selected from taraxerol, diosmetin, and taraxasterol acetate. The preparation is to be used as described in EP 4122468 A1.
[0041] US 6506396 B1 relates to liquid shampoo formulations for use in animals, with 0.1-10% agricultural / care or veterinary substances including insecticides, b) 0.0002-40% of a microemulsion concentrate comprising: (i) 0-10% castor oil ethoxylate or tristyrylphenol ethoxylate, (ii) 0-1% ethoxylated phosphoric acid as pH buffer, and N-C8-C18-alkyl-pyrrolidones and N-C1-C4-alkyl-pyrrolidones, surfactants with shampoo properties, and water.
[0042] For the control of parasites such as mites, the state of the art therefore discusses the use of various combinations of active ingredients or liquid, predominantly non-aqueous permethrin-containing products or combination products with permethrin, whereby organic solvents, in particular aromatic solvents such as N-alkylpyrrolidones and / or biphenyl solvents, are required to achieve suitable efficacy.
[0043] Furthermore, the state of the art for mite infestation discusses the topical application of a semi-solid product comprising 5% by weight permethrin in paraffin and petrolatum, cetylstearyl alcohol, emulsifying agent (Type A), and purified water. This 5% preparation can be obtained from a non-use formulation concentrate of 25% permethrin in Unguentum emulsificans aquosum DAB by diluting it with the cream. If the cream does not kill the mites, such as in the contagious scabies crustosa, or is not suitable for other reasons, it is possible to take tablets or use other creams containing other active ingredients.
[0044] However, there is evidence in the literature that the effectiveness of such formulations containing 5% w / w permethrin has recently decreased, so that the product in question may now need to be applied multiple times or more frequently. This could be due to a reduction in the sensitivity of the scabies mites or even to new parasitic variants of the mites.
[0045] Object of the present invention
[0046] The object of the present invention is therefore to provide an agent for use against ectoparasites that, on the one hand, is easy to handle, user-friendly, skin-friendly, and stable, while, on the other hand, offering improved effectiveness. In particular, reliable efficacy against ectoparasites, especially mites, in living organisms, such as scabies mites in humans, on or in the skin, on the neck, head, legs, abdomen, and entire body, should be ensured with the shortest possible application time and as few treatment repeats as possible, especially for the adequate treatment of scabies / scabies.
[0047] Solution
[0048] This object is achieved according to the invention by the topical dermal (external) application of a semi-solid, non-liquid cream preparation for ectoparasites and for the treatment of infestations caused by them such as scabies (scabies) by mites, comprising, in particular consisting of, water, a non-polar hydrocarbon (lipid) component (KW), especially selected from one or more petrolatums, one or more paraffins and mixtures thereof, one or more polar excipients with emulsifying properties (EMU), optionally additives such as preservatives in particular, and the active ingredient (WS) permethrin, used as a cis / trans isomer mixture with cis content < trans content, in an amount of 8 wt.% to 13 wt.%. According to the invention, the ratio of hydrocarbon component (HC) to active ingredient (AI) is 1.5 to 2.7, or else 1.9 to 2.3 or 1.8 to 2.4, in particular 2.0 to 2.2.The ratio of hydrocarbon component(s) (HC) to permethrin active ingredient (AC) is based on the weight proportions of the two components (wt.% in the total preparation). For the application according to the invention, the ratio (HC) to (AC) is particularly preferably 2.1.
[0049] The hydrocarbon component (HC) is preferably present in an amount of 13 to 34 wt. %, especially 15 to 26 wt. %.
[0050] According to the invention, it may also be advantageous if the weight ratio of emulsifying auxiliary (auxiliary with emulsifying properties) (EMU), especially emulsifying fatty alcohol such as C -C 28 - fatty alcohols and Ci0-C28 fatty alcohol sulfates and their salts, in particular alkali salts, and mixtures thereof, and permethrin active ingredient (WS), 0.8 to 1.3, in particular 0.8 to 1.0, in particular 0.9, based on the weight of the components in the total preparation (wt.% ratio).
[0051] Additives selected from conditioning agents, skin care ingredients, additional active ingredients such as antihistamines, corticoids, or mixtures of such additives may also be present. Conditioning agents include, in particular, preservatives.
[0052] The excipient(s) with emulsifying properties (EMU) are preferably selected from C 12 -C 18 - fatty alcohols and C 12 -C 18 - Fatty alcohol sulfates and / or sulfate salts, especially sodium salts thereof, and mixtures thereof, especially lauryl sulfate (sodium salt), cetyl alcohol, stearyl alcohol, cetylstearyl alcohol, cetylstearyl sulfate (sodium salt), in particular from emulsifying cetylstearyl alcohol type A or type B.

[0055] • Cetyl alcohol: A C16 fatty alcohol used as both a thickener and emulsifier in creams and lotions, providing a conditioning effect on the skin. • Stearyl alcohol: A C18 fatty alcohol used in a similar way to cetyl alcohol to improve the texture of skincare products and stabilize emulsions.
[0056] • Cetylstearyl alcohol: A mixture of cetyl and stearyl alcohol (also known as cetearyl alcohol), often used as a consistency agent and emulsifier in creams and lotions.
[0057] • Cetyl stearyl sulfate (sodium salt): An anionic emulsifier particularly used in skin care products to ensure the stability of emulsions.
[0058] • Emulsifying Cetylstearyl Alcohol Type A: An emulsifier containing a mixture of cetylstearyl alcohol and a small amount of sodium cetylstearyl sulfate, which makes it particularly effective in emulsification.
[0059] • Emulsifying cetylstearyl alcohol type B: A variant of type A that contains sodium lauryl sulfate instead of sodium cetylstearyl sulfate to meet specific requirements for stability and skin compatibility.
[0060] In a further embodiment, the following emulsifiers can also be added:
[0061] • Polysorbates (e.g. Polysorbate 20, Polysorbate 80): These non-ionic surfactants are often used in cosmetics and pharmaceuticals to stabilize oil-in-water emulsions.
[0062] • Glyceryl monostearate: A non-ionic emulsifier commonly used in the food and cosmetic industries to stabilize emulsions and improve texture.
[0063] • Lecithin: A natural emulsifier derived from soy or egg yolk, often used in natural or hypoallergenic skin care products due to its mild properties.
[0064] • Sorbitan stearate: Another non-ionic emulsifier used in combination with polysorbates to create stable emulsions.
[0065] • PEG-40 Stearate: A polyethylene glycol derivative of stearic acid that serves as a nonionic emulsifier and solubilizer in many cosmetic formulations. The antiparasitic active ingredient permethrin (WS) comprises two chiral centers and is present as an isomer mixture. According to the present application, all cis:trans isomer mixtures with a cis content < trans content, in particular a cis content < 50%, preferably < 40%, especially the 25:75 cis:trans isomer mixture thereof, are included and are present in an amount of 8 wt.% to 12 wt.%, or also 9 wt.% to 11 wt.%, in particular 10 wt.%. The 25:75 cis:trans mixture is very particularly preferably used in an amount of 10 wt.%.
[0066] Hydrocarbon (lipid) component (KW) according to the present invention means non-polar, in particular aliphatic liquid and / or semi-solid and / or solid hydrocarbons and especially mixtures of these, which are preferably selected from paraffins and petroleum jelly, such as white petroleum jelly, yellow petroleum jelly, microcrystalline paraffin, hard paraffin, thick paraffin, thin paraffin (paraffin liquid and paraffin perliquid), or mixtures thereof, in particular mixtures of paraffin liquid and white petroleum jelly and / or petroleum jelly Flavium or paraffin perliquid (thin) and petroleum jelly, such as yellow petroleum jelly, or white petroleum jelly or petroleum jelly Flavium or mixtures of two or three of these petroleum jelly with paraffin liquid or perliquid.
[0067] Ectoparasites according to the present invention are insects or arachnids whose development occurs entirely or partially on or in the skin of a host, or which require blood meals, cell or lymph fluids, or even skin components of the host for their development. They primarily include ticks and mites, as well as fleas, lice, biting lice, mosquitoes, flies, and sand flies.
[0068] For the purposes of this invention, mites are tiny arachnids (Arachnida). They can infest animals (e.g., dogs, cats) as well as humans. Specific types of mites include Demodex mites and scabies mites (Sarcoptes scabiei, especially the human-specific Sarcoptes scabiei var. hominis). Female scabies mites grow to 0.3 to 0.5 mm in size, while male mites grow to 0.21 to 0.29 mm. Infestation by Sarcoptes scabiei causes scabies in humans, with the associated skin symptoms and often severe, especially nighttime, itching.
[0069] The application according to the invention primarily relates to mammals. Mammals according to the present invention include domestic animals (such as dogs), farm animals (such as cattle, sheep, goats, or even horses and pigs), and in particular humans.
[0070] For the purposes of the present invention, semi-solid means a non-liquid, non-solid preparation that can be filled into solid containers such as tubes or jars at rest and applied in a formable manner upon pressure, shear, or the like. This does not constitute an emulsion, which is liquid (and not semi-solid as in the invention) and has a higher flow behavior than a semi-solid preparation.
[0071] Topical cutaneous = dermal (external) application, according to the present invention, means that the preparation is suitable for application to a specific small or large area of skin, in particular to the entire body, as needed. The amount can be, for example, 25 g to 60 g per whole-body application (depending on body weight and size).
[0072] Penetration in the sense of the present invention means, as explained above, the penetration of an active ingredient into a skin layer, here in particular the lower and middle stratum corneum, but not the relevant penetration into the area of the dermis or blood vessels.
[0073] Application effectiveness
[0074] Surprisingly, a higher amount of permethrin than previously (5 wt.%), namely 8 to 13 wt.%, or 9 to 12 wt.%, in particular 10 wt.%, based on the total weight of the preparation, can be used in one application according to the invention. Furthermore, no higher amounts of organic solvents are required for this, nor are anhydrous preparations (i.e., a maximum of up to 5 wt.% water) necessary to ensure adequate penetration and effectiveness.
[0075] It has been shown that when using the active ingredient permethrin (WS) and liquid, solid and / or semi-solid, primarily aliphatic hydrocarbons (HC) or mixtures thereof, in particular as mentioned above (paraffins, petrolatum), in a weight ratio (HC) to (WS) of 1.5 to 2.7, or else 1.9 to 2.3 or 1.8 to 2.4, especially 2.0 to 2.1, semi-solid cream preparations can be produced and used with unexpectedly high, super-additive effectiveness in combating / treating ectoparasite infestations such as mites / scabies in particular. The semi-solid preparation used according to the invention is stable and is applied topically to the skin, i.e. cutaneously. It acts dermally (not systemically) in the upper layers of the skin, here in particular in the lower and middle stratum corneum, where an increased active ingredient concentration is achieved. An improved penetration profile can be achieved compared to the prior art. Thus, the increased availability of the active ingredient in the relevant skin layers unexpectedly leads to a more than additive effect compared to the same applied amount from standard products with half the concentration (as previously known and the 5% permethrin creams described above). This was demonstrated by clinical studies in connection with mites, in which a higher therapeutic success than would have been expected was demonstrated: in patients with dermatologically confirmed scabies and failure of a double permethrin therapy, after two weeks of applying a standard cream with half the dosage compared to the cream (applied) according to the invention, namely 2 x 5% permethrin, within 24 hours, a therapeutic success of only 11% was achieved, whereas a single application of the 10% permethrin cream according to the invention (1 x 10%) resulted in a therapeutic success (cure of scabies / complete eradication of all scabies mites) of 33% (see Table 2 below). This is surprising, since when the same total amount of permethrin is applied within 24 hours, a three-fold increase in effect can be achieved with the 10% permethrin cream according to the invention, and this with only one application. In addition, in the above-mentionedA study after a second application of 10% permethrin cream found a total treatment success rate of 50% in patients who were not yet cured after 14 days.
[0076] Surprisingly, the application according to the invention leads to a cure in every second patient, even when 5% permethrin cream was previously ineffective, using a more suitable permethrin cream preparation, namely the permethrin cream preparation according to the invention.
[0077] The semi-solid preparation used according to the invention for dermal topical use in ectoparasites, in particular mite infestation, scabies, can thus have a superadditive effect in the elimination of mite infestation, or in the treatment of scabies, in relation to a semi-concentrated, double-applied preparation according to the standard 5% permethrin cream (i.e. 2x 5% w / w active ingredient concentration compared to 1x 10 w / w).
[0078] Detailed explanation of the invention and the individual components of the preparation The semi-solid preparation used according to the invention for cutaneous (dermal) application is in particular a hydrophilic cream and comprises, in addition to water, one or more non-polar hydrocarbon (lipid) components (HC), one or more excipients with emulsifying properties (EMU) and, if appropriate, additives.
[0079] The preparation also contains the active ingredient permethrin (WS), which is present in the preparation as an antiparasitic active ingredient.
[0080] Other antiparasitic active ingredients may be present in the preparation if desired, but are preferably not included.
[0081] 1. Active ingredient (WS) Permethrin
[0082] Permethrin according to the present invention is an insecticide and acaricide from the pyrethroid group. It acts as a contact and ingestion poison; its spectrum of activity is very broad. Permethrin has been commercially available since around 1977. It is a type I pyrethroid, meaning it does not have a cyano-3-phenoxybenzyl group.
[0083] The chemical name is:
[0084] 3-(2,2-Dichloroethenyl)-2,2-dimethylcyclopropanecarboxylic acid (3-phenoxyphenyl)methyl ester
[0085] Or 3-(Phenoxy-1-yl) benzyl 3-(2,2-dichloroethen-1-yl)-2,2-dimethylcyclopropanecarboxylate (see https: / / de.wikipedia.org / wiki / permethrin).
[0086] The chemical formula is:
[0087] Permethrin is solid at room temperature (melting point approx. 34-35°C) and almost insoluble in water (0.2 mg l -1 at 20 °C).
[0088] Due to the two stereocenters in the cyclopropane ring, permethrin has four stereoisomers (two enantiomer pairs). The trans enantiomer pair is known as transpermethrin, see https: / / de.wikipedia.org / wiki / Permethrin. The enantiomer pair cis / trans 25:75 is preferred. Other cis / trans ratios may also be suitable if the cis portion is lower than the trans portion.
[0089] 2. l / l / water
[0090] The amount of water, in particular purified water, is in particular 38 wt.% to 73 wt.%, in particular 42 wt.% to 65 wt.%, or 30 to 65 wt.%, in particular 45 wt.% to 65 wt.%, in particular 60 wt.%, in each case based on the total amount of the preparation.
[0091] 3. Hydrocarbon (=lipids) component (HC)
[0092] Nonpolar hydrocarbons (HC) form the lipid component. Depending on their chain length, these hydrocarbons are solid, semi-solid, liquid, or mixtures thereof. The weight percentages (13 wt.% to 34 wt.%, preferably 15 wt.% to 26 wt.%, in particular 17 wt.% to 22 wt.%) are based on the sum of all hydrocarbons contained in the entire preparation.
[0093] These hydrocarbons (HC) suitable for the cream used according to the invention include, in particular, petrolatum and paraffins. These are predominantly aliphatic hydrocarbons, which, depending on their chain length and degree of branching, are liquid to semi-solid or solid hydrocarbons derived from petroleum, with predominantly aliphatic components. These products are familiar to those skilled in the art. They are also available in pharmaceutical grades.
[0094] Such paraffins and petroleum jelly are described below as examples:
[0095] Vaseline (= petrolatum) is generally known as a nonpolar, semi-solid hydrocarbon mixture. It can consist of a liquid portion of highly branched isoparaffins and olefins, which have a melting point of 35 °C to 60 °C, and a solid (crystalline) portion (10-30%) of long-chain components (n-paraffins and sparsely branched isoparaffins). See https: / / www.chemie.de / lexikon / Vaseline.
[0096] Depending on the proportion of aromatic hydrocarbons, petroleum jelly is available as yellow or white petroleum jelly. The chain length of the hydrocarbons can be C 15 -C 60 , the middle C range C26, or hydrocarbons with a C content > 20 may also be present.
[0097] It can also be a mixture of C24-C 34 hydrocarbons (as described in EP2550954), in particular in white petrolatum.
[0098] Pharmaceutical qualities (according to the European Pharmacopoeia Ph.Eur.) of white and yellow petrolatum (Latin Vaselinum album and Vaselinum flavum, also known as paraffin white soft and yellow soft respectively) differ not only in their appearance but also in that white petrolatum is said to contain fewer undesirable aromatic polycyclic hydrocarbons (“MOAH”) than yellow petrolatum.
[0099] The dropping point (solid-liquid transition) for white petrolatum can be between 35°C and 75°C, for yellow petrolatum between 40-60°C (https: / / de.wikipedia.org / wiki / Vaseline) or even between 35° and 70°C (Ph. Eur.).
[0100] In the preparations used according to the invention, white petrolatum such as Vaseline white = Vaselinum album = Paraffin white soft = semi-solid hydrocarbon mixture of petroleum, e.g. with a dropping point of 35°C to 75°C or 35°C to 70°, is preferably used.
[0101] Paraffins are mixtures of acyclic alkanes (saturated hydrocarbons); their molar mass can range from 275 to 600 g / mol, for example. They can exist as solids, semi-solids, or liquids.
[0102] Solid to semi-solid examples include hard paraffin, also known as microcrystalline wax, with a melting point between 50°C and 62°C, or soft paraffin with a melting point of approximately 45°C.
[0103] Microcrystalline paraffins (waxes) are purified mixtures of solid straight-chain, branched, and cyclic saturated hydrocarbons with a pot point > 65°C, according to Ph. Eur. Microcrystalline waxes can have solidification points between 70°C and 80°C and contain chain lengths of up to 75 carbon atoms (see https: / / www.chemie.de / -lexikon / Paraffin).
[0104] In the case of liquid paraffins, which can have a density of 0.81-0.89 g / cm), a general distinction can be made, depending on the chain length and degree of branching of the hydrocarbons, between thin-bodied paraffins (Paraffinum perliquiduni), e.g. with a viscosity of 25 to 80 mPas, and thick-bodied paraffins (Paraffinum subliquidum), which are present as an oily liquid (e.g. with a viscosity of 110 to 230 mPas).
[0105] Accordingly, the viscosities of such paraffin products vary (wax: melting point 50-62 °C, Paraffinum Light: viscosity > 100 cp, 20 °C, medicinal mineral oil: 65 cp / 20 °C).
[0106] Examples include:
[0107] Thin liquid paraffin (Paraffinum perliquidum, Paraffin light liquid (Ph.Eur. with density 0.810-0.875), liquid paraffin (Paraffinum Liquid, e.g. according to Ph.Eur: Paraffinum Liquid, density: 0.827-0.890) or thick liquid paraffin (Paraffinum subliquiduni).
[0108] Such paraffin products are also known as paraffin, paraffinum white (= microcrystalline wax), paraffinum light (light mineral oil).
[0109] The hydrocarbons and mixtures thereof as explained above can have different compositions according to the chain lengths and branching of the components and the general, non-exclusive formula C n -H2n+2 with n = 16-75, especially C n H2n+2 with n = 18-60, especially 18-36 (e.g., for paraffins). These are mainly nonpolar, aliphatic, acyclic, saturated, possibly unsaturated, straight-chain, liquid, semi-solid, or solid hydrocarbons or mixtures, as well as aliphatic, saturated, branched (iso-)hydrocarbons, possibly with small amounts of unsaturated compounds or aromatic compounds, the amount of which may depend on the degree of purity.
[0110] The petrolatum and paraffin products used in the preparations according to the invention are equally suitable. Hydrocarbon mixtures (HC) such as petrolatum (petrolatum), e.g., as described above, consisting of 70 to 90% of a liquid portion of highly branched isoparaffins and olefins and a solid crystalline portion (10-30%) of long-chain components (n-paraffins and sparsely branched isoparaffins), are particularly preferred in a (total) amount of 20 to 30 wt.%.
[0111] Also preferred are paraffins such as paraffin liquid (purified mixture of liquid hydrocarbons), light paraffin or paraffinum perliquid (liquid or thin paraffin), medical (liquid) paraffin, paraffinum subliquidum (thick paraffin) or mixtures thereof, especially in total amounts of 18 to 30 wt.%, especially 20 to 25 wt.%.
[0112] According to the application, particularly suitable mixtures of hydrocarbons (HC) are Vaseline + thick paraffin (Paraffinum subliquidum), Vaseline white, Vaselinum album and / or Paraffinum white soft + thick paraffin (Paraffinum subliquidum).
[0113] 4. Excipients
[0114] As excipients, those with emulsifying (emulsifying) properties are particularly chosen. These are used in amounts of 6 wt.% to 15 wt.%, preferably 7 wt.% to 11 wt.%, and especially 9 wt.%, based on the total preparation.
[0115] These excipients include mainly C6-C3o, especially C 10 -C 28-Aliphatic, monohydric fatty alcohols, such as those found in waxes (bound in esters) (hence also called wax alcohols), especially in mixtures with ionic, especially anionic, sulfate salts thereof, especially sulfate alkali salts thereof (surfactant sulfates from fatty alcohols). The salts, especially sulfate salts or alkali sulfate salts of the fatty alcohols, can be used as ionic, especially anionic (emulsifying) auxiliary components.
[0116] The aliphatic fatty alcohols include, for example, saturated representatives such as 1-decanol (010), dodecanol = lauryl alcohol (C 12 ), lauryl alcohol, myristyl alcohol (C 14 ), cetyl alcohol (C 16 ), stearyl alcohol (C 18 ), 1 docosanol (C 22 ), Ligoceryl alcohol C 24 )or monounsaturated compounds such as cis-9-octadecen-1-ol (oleyl alcohol (C 18 ), or linoeyl alcohol Oc-tradecatrien-1-ol (C 18, 3-fold unsaturated) or sulfates such as sodium lauryl sulfate, cetyl sulfate, (sodium) sodium stearyl sulfate, sodium cetylstearyl sulfate.
[0117] Particularly suitable are products known under the name 'Lanette waxes'. These contain cetylstearyl alcohol. The latter is a mixture of solid aliphatic alcohols, mainly cetyl alcohol (hexadecan-1-ol) and stearyl alcohol (octadecan-1-ol), of animal or plant origin.
[0118] Cetylstearyl alcohol, also known as cetearyl alcohol, is a mixture of fatty alcohols consisting mainly of cetyl alcohol (C 16 H 34 O) and stearyl alcohol (C 18 H 38 O). Cetylstearyl alcohol typically consists of a mixture of cetyl alcohol (C16) and stearyl alcohol (C18) in a ratio of approximately 30:70 to 50:50. The exact ratio may vary depending on the specific application.
[0119] Cetylstearyl alcohol becomes more solid the higher the proportion of stearyl alcohol (C18) in the mixture. Stearyl alcohol has a higher melting point (approximately 59-60°C) compared to cetyl alcohol (approximately 49-50°C). Therefore, a higher stearyl alcohol content results in a firmer consistency of the cetylstearyl alcohol.
[0120] A ratio of 30% cetyl alcohol to 70% stearyl alcohol (i.e., 30:70), or a ratio with an even higher stearyl alcohol content, would result in a rather solid cetylstearyl alcohol. The more stearyl alcohol in the mixture, the more solid the final product will be at room temperature.
[0121] In the preparations according to the invention, a ratio of cetyl alcohol to stearyl alcohol (in wt. %) between 50:50 and 30:70, or between 45:55 and 35:65, or 40:60 is preferred.
[0122] This ratio influences the properties of cetyl stearyl alcohol, including its melting temperature, texture, and emulsifying ability, making it versatile for use as an emulsifier, bodying agent, and skin care ingredient.
[0123] This substance belongs to the group of long-chain aliphatic alcohols and has both hydrophobic (water-repellent) and lipophilic (fat-loving) properties, making it an effective emulsifier and consistency regulator in cosmetic and pharmaceutical formulations. Cetylstearyl alcohol helps stabilize oil-in-water emulsions by facilitating the formation of droplets of oil in water and preventing phase separation.
[0124] Additionally, cetyl stearyl alcohol improves the texture of skin care products by giving them a creamy consistency and making the skin feel pleasant after application. It also acts as a thickener and emollient, softening the skin and locking in moisture. Due to its mild and nourishing properties, cetyl stearyl alcohol is commonly used in a variety of products such as creams, lotions, hair conditioners, and sunscreens.
[0125] Cetylstearyl alcohol is available in Type A and Type B. Type A cetylstearyl alcohol is a mixture of fatty alcohols consisting primarily of cetyl alcohol (C16) and stearyl alcohol (C18), combined with a small amount of anionic surfactant, typically sodium cetylstearyl sulfate. This combination gives Type A a pronounced emulsifying effect, enabling the formation of stable oil-in-water emulsions. The additional amount of sodium lauryl sulfate increases the effectiveness of this emulsifier in stabilizing emulsions and improves the dispersibility of hydrophobic components. Type A cetylstearyl alcohol is widely used in cosmetics and skincare products to improve consistency, condition the skin, and ensure even distribution of ingredients.
[0126] Cetylstearyl alcohol Type B is also a mixture of cetyl and stearyl alcohols, but with or without sodium lauryl sulfate instead of sodium cetylstearyl sulfate compared to Type A. This composition makes Type B a milder emulsifier, particularly used in products that require a gentler, less foaming action. Type B is often used in skin-friendly and hypoallergenic formulations because it has a lower irritating effect on the skin while still ensuring good emulsion stability.
[0127] Cetylstearyl alcohol type A can therefore also be defined as a mixture of cetyl and stearyl alcohols with an anionic surfactant, ideal for emulsifying and stabilizing oil-in-water emulsions. It is an emulsifying fatty alcohol with added sodium lauryl sulfate to improve dispersibility in cosmetic formulations. Cetylstearyl alcohol type A is therefore a cetylstearyl alcohol with enhanced emulsifying properties through the addition of an anionic surfactant. Key properties:
[0128] • Cetylstearyl alcohol with an added anionic surfactant to increase emulsifying ability.
[0129] • An emulsifying mixture of C16 and C18 fatty alcohols with anionic surfactant.
[0130] • Cetylstearyl alcohol with sodium cetylstearyl sulfate.
[0131] Cetylstearyl alcohol type B can therefore also be defined as a mixture of cetyl and stearyl alcohols without anionic surfactant, for mild and skin-friendly emulsions. It is a non-ionic fatty alcohol emulsifier with or without added sodium lauryl sulfate, suitable for sensitive skin. Cetylstearyl alcohol type B is therefore a cetylstearyl alcohol with little or no added surfactant for gentle emulsion systems. Key properties:
[0132] • Cetylstearyl alcohol without anionic surfactant for gentle emulsions.
[0133] • An emulsifying mixture of C16 and C18 fatty alcohols with little or no added surfactant.
[0134] • Cetylstearyl alcohol with or without sodium lauryl sulfate.
[0135] Cetylstearyl alcohol is a waxy mass, white to pale yellow in color. Pharmaceutical grades contain at least 40.0 percent stearyl alcohol, and the sum of the stearyl alcohol and cetyl alcohol contents is at least 90.0 percent. The melting point is between 49 °C and 56 °C. Cetylstearyl alcohol has an HLB value of approximately 1 and is only weakly surface-active (water / oil emulsifier).
[0136] Such fatty alcohols can be combined with anionic components: emulsifying cetylstearyl alcohol is a (pharmaceutically usable) mixed emulsifier consisting of at least 80.0 percent cetylstearyl alcohol and 7.0 percent of the ionic O / W emulsifiers sodium cetylstearyl sulfate (Type A) or sodium lauryl sulfate (Type B). Both emulsifying cetylstearyl alcohol Type A and Type B are soluble in hot water, forming an opalescent solution, but practically insoluble in cold water. These components can be used to prepare hydrophilic bases such as "Anionic Hydrophilic Creams DAB."
[0137] Commercially available products are Lanette O (cetylstearyl alcohol), Lanette E (sodium cetylstearyl sulfate) and Lanette N (emulsifying cetylstearyl alcohol (type A: cetearyl alcohol and sodium cetearyl sulfate), Lanette SX (emulsifying cetylstearyl alcohol type B: cetearyl alcohol + sodium lauryl sulfate + sodium cetearyl sulfate), see also https: / / de. Wikipedia.org / wiki / Cetylstearylalkohol.
[0138] According to the invention, cetylstearyl alcohol type A or cetylstearyl alcohol type B is particularly preferably used, each in an amount (wt%) of 6-15, in particular 7 wt% to 11 wt%, especially 8 wt% -10 wt%, in particular 9 wt%
[0139] 5. Additives Additives can preferably be selected from skin care ingredients, conditioning agents, additional active ingredients, or mixtures thereof. They are present in amounts of 0 to 7% by weight, preferably 0 to 5%, especially 0.01 to 5% by weight, especially 0.01 to 3% by weight, based on the weight of the total composition.
[0140] 5. 1. Skin care ingredients
[0141] Suitable skin care ingredients, especially for moisture regulation and protection against external undesirable influences, primarily include lipophilic polar components from the group of fats and fatty oils. For the purposes of the present invention, these are not non-polar lipid components (HC) nor are they excipients with emulsifying properties as defined in the invention (which include fatty alcohols and their salts / sulfate salts).

[0204] Production and examples
[0205] The preparation according to the invention is prepared from an aqueous phase and a lipid phase according to the usual practice of the skilled person. For the lipid phase, the hydrocarbons and the emulsifier (e.g., emulsifying cetylstearyl alcohol, viscous paraffin, and petrolatum) are first melted. The heated active ingredient is added and homogenized. For the aqueous phase, the heated water is prepared with any additives present, such as dissolved preservatives (e.g., sorbic acid). The aqueous phase is then incorporated into the lipid phase while stirring. Once the finished cream has cooled, production is complete.
[0206] Overall, these are particularly pharmaceutically acceptable ingredients, and no complex mixtures need to be prepared due to the small number of individual components. The cream prepared in this way is filled into containers and / or devices such as tubes, jars, or dispensers, usually containing 20 to 100 g, in particular 30, 50, or 60 g, from which the skin application can be carried out as per the application.
[0207] The following preparations were prepared as described:
[0208] Example 1: 10% Permethrin
[0209] Example 2: 8% Permethrin
[0210] Example 3: 12% Permethrin
[0211] Comparative example 4: 5% permethrin
[0212] Application
[0213] The preparation according to the invention is applied from suitable devices such as tubes, jars or dispensers directly onto the affected person's skin, usually over the entire body (excluding only the mouth and eye areas). Particular attention must be paid to complete coverage of the pre-delicacy areas, i.e. the hands and feet, as well as the skin around the fingernails and toenails, the genital area and the perianal region. For this purpose, 25-60 g of cream are usually used once, depending on body size and weight. The preparation is skin-friendly and absorbs quickly. It should usually be left on the skin for at least several hours, more precisely, at least 8 hours. If there is no healing or if, for example, the scabies mites are not completely eliminated, the treatment is usually repeated after 7 to 14 days.
[0214] The effectiveness is explained in more detail using the tests described below in application examples A and B.
[0215] Application example A: Skin penetration examinations
[0216] To investigate the skin penetration of the preparation according to the invention, a study was carried out on human skin in a specialized laboratory in which the penetration into the skin, particularly in the stratum corneum, was determined in comparison with the conventional 5% permethrin cream.
[0217] Formulations used:
[0218] Permethrin 5% cream according to comparative example 4
[0219] Permethrin 10% cream according to Example 1
[0220] 20 mg of each formulation was applied, except for the double application of permethrin from the 5% cream, which was also carried out according to Comparative Example 4, where 2 x 20 mg = 40 mg were applied. The respective skin samples were analyzed by determining the radioactive decay rate using a scintillation counter (TRICARB). For this purpose, radioactively labeled (H)-permethrin was added to the test formulations prior to application (20 pCi per 20 mg formulation).
[0221] Test conditions:
[0222] For each test formulation, three human skin samples, i.e., from three different donors, were examined. Each skin sample yielded three punch biopsies at three different time points, i.e., a total of 27 samples per test formulation. Skin from the same donors was used for all formulations.
[0223] Skin preparation:
[0224] The studies were conducted on excised human breast skin (3 different donors). The tissue sections were cleaned postoperatively with swabs and isotonic NaCl solution. The subcutaneous fat was mechanically dissected and discarded. At the time of examination, the skin pieces (diameter 20 mm; area: 3.1416 cm 2 ) were completely thawed at room temperature and the surface was dried with swabs. Skin integrity was determined using a corneometer.
[0225] Experimental procedure
[0226] Distilled water was used as the acceptor. The individual skin samples were applied to filter gauze and placed on the diffusion cell preheated to 32 °C (direct contact between filter gauze and acceptor liquid). The thickness of the diffusion layer was reduced by continuous stirring, thus simulating physiological conditions by continuously removing permeated active ingredients (sink conditions with a glass cover as evaporation protection). After homogeneous application of the test formulation to the
[0227] Skin samples were incubated for 100 minutes, after which the remaining formulation was removed with a swab. The skin samples were then taken, and three biopsies measuring 0.6 mm (0.848 cm 2) were punched out of the skin. The punch biopsies were then frozen at -40°C and sectioned horizontally using a cooling microtome. To determine the concentration-time profile, the skin layers relevant for penetration determination were prepared according to the following scheme:
[0228] Stratum corneum: SC: 20 adhesive film tears of 2 pm each
[0229] Vital epidermis: EP: 10 cuts of 20 pm each Dermis: DR: 15 cuts of 40 pm each
[0230] For the subsequent analysis, two adhesive film tears were combined into one fraction, then processed and measured together.
[0231] Data analysis and discussion
[0232] The active ingredient concentrations in the various skin sections were calculated taking into account section thickness, area and applied amount.
[0233] Results based on the preparation according to the invention
[0234] It has been shown that the highest permethrin concentration is generally found in the stratum corneum.
[0235] Since the natural habitat of scabies mites is in the stratum corneum (SC), particularly in the middle and lower layers and possibly at the border to the epidermis (EP), these skin layers will be examined in more detail below, as shown in Table 1.
[0236] Table 1
[0237] Table 1: Permethrin penetration profiles into the middle and lower stratum corneum (SC, each with sample number according to increasing skin depth) and the upper epidermis (EP, each with sample number according to increasing skin depth) for Permethrin Cream 5%, Permethrin Cream 10% (according to the invention) and 2x Permethrin Cream 5% (double amount) after 100 min application time in Franzzell test with human skin (C mean - medium concentration).
[0238] As can be seen from this, the penetration of Permethrin 10% Cream into the middle and lower layers of the stratum corneum and the upper epidermis is not only significantly higher than for Permethrin 5% Cream (factor > 2), which is not necessarily expected for lipophilic substances such as permethrin, but surprisingly also significantly higher than when applying the equivalent amount of active ingredient (2x Permethrin 5% Cream). Here, Permethrin 10% Cream shows a disproportionate effect of > +50% for stratum corneum layers 4-10, and when considering only the lowest SC layer (SC10) even > +100%, despite the same amount of active ingredient being applied. A similar picture is seen in the upper epidermis (EP1-2), with > +100% for Permethrin 10% Cream compared to both Permethrin 5% Cream and 2x Permethrin 5% Cream.
[0239] Application example B: Clinical testing
[0240] Methodology and implementation:
[0241] To test the clinical efficacy of the patented Permethrin 10% Cream in cases of scabies confirmed by mite detection and medically confirmed, a larger prospective clinical trial was conducted in which patients aged between 8 and 85 years were randomized to receive treatment with Permethrin 10% Cream in direct comparison with a double application of Permethrin 5% Cream in a full-body setting (including the head, excluding the face and the mouth and eye areas) with 60 g of cream applied by trained medical personnel in dermatological clinics (for the
[0242] All patients received the same amount of active ingredient (double application, twice within 24 hours). Treatment success was defined as complete eradication of all mites, i.e., a negative detection of scabies mites after a dermatological whole-body inspection by trained dermatologists. Efficacy was assessed on day 14 after application. If scabies mites were still detectable, patients who received Permethrin 10% cream underwent a further treatment with a follow-up after 14 days (day 28).
[0243] Results and discussion:
[0244] The following results were obtained, shown in Table 2:
[0245] Table 2
[0246] Table 2: Clinical therapeutic success / eradication of all scabies mites for 2x Permethrin 5% cream and Permethrin 10% cream (* same patients), N= number of patients.
[0247] The test data show that despite increasing the application quantity (mandatory 60 g compared to the previously failed standard application with Permethrin 5% cream) and double application within 24 hours, treatment with Permethrin 5% cream showed hardly any clinical effect on the scabies mites (11% treatment success rate). In contrast, a single application of Permethrin 10% cream, despite applying the same amount of active ingredient, surprisingly showed a disproportionately high, three-fold effect (compared to Permethrin 5% cream), thus demonstrating the superiority of the new, inventive formulation. This is all the more remarkable given that this patient had previously been unsuccessfully treated with the same active ingredient (permethrin). In this context, the additional effect of the second treatment with Permethrin 10% cream is also worth highlighting. The overall efficacy of 50% achieved with a maximum of two applications is also surprising and demonstrates the potential increase in efficacy that can be achieved with the 10% permethrin cream according to the invention after failure of regular permethrin therapy. It is also interesting to note that 2 out of 3 (67%) of the cured patients achieved therapeutic success with just one application of permethrin 10% cream.
