Abstract
Description
Technical Field
[0001] The present invention belongs to the field of medicine and relates to pharmaceutical use of a traditional Chinese medicine composition, specifically to pharmaceutical use of a dendrobium preparation, and especially to use of a composition of honeysuckle, honeysuckle, Achyranthes bidentata, Scrophularia ningpoensis and dendrobium or a composition of honeysuckle, Achyranthes bidentata, Scrophularia ningpoensis and dendrobium in preparing medicines and / or health products for treating and / or preventing obesity. Background Art
[0002] Obesity is a chronic metabolic disease caused by the combined effects of multiple factors such as genetics and the environment, which causes an increase in the total amount of body fat or a local increase. A large amount of data research shows that obesity increases the probability of suffering from a variety of diseases such as diabetes, heart disease, hypertension, coronary atherosclerosis, and obesity. Obesity is often diagnosed clinically based on information such as the patient's body mass index, waist circumference, and body fat percentage. At present, the drugs approved by the US FDA for the treatment of obesity mainly include naltrexone / bupropion, phentermine / topiramate, orlistat, and liraglutide. The only drug approved by my country's Food and Drug Administration for the treatment of obesity is orlistat. Orlistat is an inhibitor of gastric and intestinal lipase. It improves obesity by inhibiting the decomposition and absorption of fat in food, which often causes adverse reactions such as increased gastrointestinal gas, urgency to defecate, and fatty stools. Therefore, further research and development of safe and effective drugs is of great significance for the treatment of obesity. Summary of the invention

[0118] Embodiment 15
[0119] The therapeutic effect of the Chinese medicine composition on obesity was evaluated using a high-fat diet (HFD)-induced obesity animal model. The specific method is as follows:
[0120] (1) Experimental Materials
[0121] Experimental animals: SPF-grade C57BL / 6 male mice, weighing 12-16 g (4 weeks old), were purchased from Shanghai Slake Experimental Animal Co., Ltd. and raised in the Experimental Animal Center of Shanghai University of Traditional Chinese Medicine. The mice were raised at a temperature of (22±1)°C, a relative humidity of (65±10)%, and a 12h / 12h light cycle.
[0122] Test drugs: Using Achyranthes bidentata, Scrophularia ningpoensis, Dendrobium officinale, Flos Lonicerae, and Flos Lonicerae (purchased from the medicinal material market and identified as genuine products) as raw materials, oral liquid was prepared according to the preparation method of Example 1. The positive drug was Orlistat (commercially available).
[0123] (2) Experimental methods
[0124] Animal experiment scheme: C57BL / 6 male mice were randomly divided into 5 groups, 6 mice in each group. Grouping: NCD group (blank control group); HFD group (model group); HFD+Orlistat group (positive drug group, the dosage is 20 mg / kg, using 0.5% sodium carboxymethyl cellulose solution to prepare a suspension, oral administration); HFD+low-dose group of traditional Chinese medicine composition (the dosage is equivalent to 13.26 g / kg of the original oral solution, oral administration); HFD+high-dose group of traditional Chinese medicine composition (the dosage is equivalent to 66.30 g / kg of the original oral solution, oral administration).
[0125] After 1 week of adaptive feeding, the mice in the NCD group were fed with ordinary feed every day, and the rest of the mice were fed with high-fat feed. After 8 weeks of feeding, the mice fed with high-fat feed were randomly and evenly divided into HFD group, HFD+Orlistat group, HFD+low-dose Chinese medicine composition group, and HFD+high-dose Chinese medicine composition group. The four groups of mice were given drug intervention every day for 4 weeks. The body weight and food intake of the mice were monitored during the administration period, and the glucose tolerance and insulin resistance of the mice were tested at the end of the administration. After the animals were killed, the liver tissue was taken, quickly frozen in liquid nitrogen, and stored at -80°C for later use.
[0126] Glucose tolerance test: After 4 weeks of administration, mice in each group fasted for 16 hours, and their fasting blood glucose was measured by tail tip blood sampling. Then, glucose (2 g / kg) was injected intraperitoneally, and the result was recorded as 0 min. Blood glucose levels were measured by tail tip blood sampling at 15, 30, 60, and 120 minutes after injection.
[0127] Insulin tolerance test: After 4 weeks of administration, mice in each group fasted for 4 hours, and their fasting blood glucose was measured by tail tip blood sampling. Then, insulin (0.75 U / kg) was injected intraperitoneally, and the result was recorded as 0 min. Blood glucose levels were measured by tail tip blood sampling at 15, 30, 60, and 120 minutes after injection.
[0128] H&E staining of liver tissue and adipose tissue for morphological observation: Liver tissue, epididymal white adipose tissue (eWAT), inguinal white adipose tissue (iWAT) and brown adipose tissue (BAT) from the same part (liver lobule) of mice were taken for pathological examination. The samples were fixed with 10% formaldehyde solution, embedded in paraffin and sliced. Some of the paraffin sections were kept for later use, and some were stained with hematoxylin-eosin and observed under an optical microscope to evaluate the morphological changes in each group.
[0129] Oil red staining of liver tissue for morphological observation: liver tissues from the same site (liver lobule) were taken for pathological examination, fixed with 4% paraformaldehyde solution, embedded in paraffin, sliced, and stained with Oil red O dye to evaluate the lipid accumulation in rat liver.
[0130] Detection of total cholesterol (TC) content in serum: Take serum samples from each group of mice as test samples. Select a blank well, a standard well, and the others as sample wells in a 96-well plate. Add 2.5 μL of distilled water, standard, and sample to the blank well, standard well, and sample well, respectively. Add 250 μL of working solution to each well, mix well, incubate at 37°C for 10 minutes, and use a microplate reader to measure the absorbance of each well at a wavelength of 510 nm. The concentration of the standard is 5.17 mmol / L, and the specific calculation formula is as follows:
[0131] TC content in serum sample (mmol / L) = (sample OD value - blank OD value) ÷ (standard OD value - blank OD value) × standard concentration (mmol / L)
[0132] Energy metabolism measurement of mice: 4 weeks after administration, the mice were placed in energy metabolism cages (single cage housing), and each mouse was given sufficient feed and drinking water. After the mice adapted to the metabolic cages for 24 hours, the metabolic cage detection instrument was turned on, and after the machine stabilized, metabolic indicators such as oxygen consumption, carbon dioxide exhalation, and heat generation of each mouse within 24 hours were detected.
[0133] Total RNA extraction: 100 mg of adipose tissue was cut and placed in a 1.5 mL centrifuge tube. After adding 1 mL of Trizol to homogenize, the tube was allowed to stand on ice for 5 min. 200 μL of chloroform was added and shaken for 15 s. The tube was centrifuged at 4°C and 12,000 × g for 15 min. The colorless water layer was transferred to a new tube and 250 μL of isopropanol was added and shaken thoroughly and allowed to stand for 10 min. The tube was centrifuged at 4°C and 12,000 × g for 10 min to obtain RNA precipitation. The supernatant was discarded. 1 mL of pre-cooled 75% ethanol was added. The tube was centrifuged at 4°C and 7,500 × g for 5 min. After centrifugation, the ethanol was discarded. After the precipitate was dried, 200 μL of DEPC water was added to fully dissolve it. The resulting solution was the RNA solution. The concentration of RNA in each sample was measured and stored at -80°C for later use.
[0134] Reverse transcription: Prepare the reverse transcription system, add the corresponding reagents according to the instructions of the kit, and mix thoroughly. React at 37℃ for 30min, and 85℃ for 1min. Perform reverse transcription according to this program temperature. After reverse transcription, store the cDNA sample at -80℃ for later use.
[0135] Real-time PCR: Prepare the reaction system according to the kit instructions, with a sample volume of 10 μL. Perform the Real-time PCR amplification experiment according to the instrument instructions.-ΔΔ The Ct method was used to calculate the difference in gene expression between the two groups.

[0141] Effects of Chinese herbal medicine combination on body weight, peripheral adipose tissue weight and morphology of HFD-induced obese mice
[0142] The final body weight (Final body weight) and relative weight of white adipose tissue (eWATweight / body weight: weight of epididymal white adipose tissue relative to body weight; iWAT weight / body weight: weight of inguinal white adipose tissue relative to body weight) of mice in different groups were statistically analyzed. The results showed that compared with the mice in the NCD group, the body weight and relative weight of white adipose tissue of the mice in the HFD group were significantly increased, and the lipid droplets in the adipocytes were enlarged, indicating that the obese mouse model was successfully established. The administration of the Chinese medicine composition to high and low dose groups can reduce the body weight of obese mice to varying degrees, and the efficacy is not significantly different from that of the positive drug ( Figure 1 After administration of the Chinese medicine composition, the relative weight of the white adipose tissue of obese mice was also significantly reduced, and the efficacy was not significantly different from that of the positive drug (Table 4). The results of H&E staining of adipose tissue showed that the lipid droplets in the adipocytes of mice in the HFD group increased, and the lipid droplets in the adipose tissue were significantly reduced after administration of the Chinese medicine composition ( Figure 2 ). The above results suggest that administration of the Chinese medicine composition can significantly reduce the body weight, peripheral adipose tissue weight and its morphology of obese mice, and reduce the formation of lipid droplets in adipose tissue.

[0146] Chinese herbal medicine composition reduces elevated TC content in serum of HFD-induced obese mice
[0147] The experimental results are shown in Table 5, which show that the TC content in the serum of the HFD group mice was significantly increased compared with the NCD group mice; different doses of the Chinese medicine composition can reduce the TC content in the serum of the mice, and the efficacy is not significantly different from that of the positive drug. This indicates that the Chinese medicine composition can reduce the elevated TC content in the serum of obese mice.

[0151] Chinese herbal medicine combination improves liver lipid accumulation in HFD-induced obese mice
[0152] The results of liver tissue Oil Red O and H&E sections are shown in Figure 3 , showing that the administration of the Chinese medicine composition can significantly reduce the accumulation of lipids in the liver, lipid vacuoles and ballooning of hepatocytes. The above results suggest that the Chinese medicine composition can significantly improve the lipid accumulation and degeneration of the liver in the process of HFD-induced obese mice.
[0153] SHZJ improves glucose tolerance and insulin resistance in HFD-induced obese mice
[0154] Figure 4 , Figure 5Table 6 shows the area under the curve of glucose tolerance (AUC of GTT) and the area under the curve of insulin resistance (AUC of ITT) of each group of mice. The experimental results show that after intraperitoneal injection of glucose in each group of mice, compared with the NCD group mice, the blood sugar of the HFD group mice increased rapidly and then slowly decreased. After the administration of the Chinese medicine composition, the increased blood sugar decreased rapidly, and the return time was significantly reduced ( Figure 4 ). After intraperitoneal injection of insulin in each group of mice, the blood sugar level of the HFD group mice decreased slowly compared with the NCD group, indicating that their sensitivity to insulin was reduced. After administration of the Chinese medicine composition, the blood sugar level of the obese mice decreased rapidly ( Figure 5 ), wherein the efficacy of the Chinese medicine composition was not significantly different from that of the positive drug. The above results suggest that the Chinese medicine composition can improve glucose tolerance and insulin resistance in HFD-induced obese mice.

[0158] Chinese herbal medicine combination improves energy metabolism in HFD-induced obese mice
[0159] The experimental results showed that compared with the NCD group mice, the oxygen consumption, carbon dioxide exhalation and heat generation of the HFD group mice were significantly reduced, indicating that the energy metabolism level of obese mice was significantly reduced. After drug intervention with the Chinese medicine composition, the oxygen consumption of obese mice ( Figure 6 ), exhaled carbon dioxide ( Figure 7 ) and the heat generated and the total energy metabolism during the day and night ( Figure 8 , Table 7) were significantly increased, and there was no significant difference in efficacy between the positive drugs and the positive drugs, indicating that the Chinese medicine composition can significantly improve the energy metabolism level of obese mice.

[0163] Chinese herbal medicine combination improves thermogenesis and lipid decomposition-related gene expression in inguinal white adipose tissue of HFD-induced obese mice
[0164] Adipose tissue is divided into white adipose tissue and brown adipose tissue. White adipose tissue is composed of a large number of lipid droplets and is used for energy storage in the body, while brown fat contains abundant mitochondria and can consume energy through non-shivering thermogenesis dependent on mitochondrial uncoupling protein 1 (UCP1). It is currently believed that promoting the transformation of white adipose tissue to brown fat to increase energy consumption is an important intervention measure to improve obesity. High levels of UCP1 can weaken the proton electrochemical gradient on the mitochondrial membrane, thereby uncoupling the energy of oxidative phosphorylation to generate heat and promote energy consumption. UCP1 and lipid droplet-associated protein-cell death-inducing DFFA-like effector A (Cidea) are considered to be markers of brown fat. In addition, adipocytes can promote the conversion of thyroxine to triiodothyronine through iodothyronine deiodinase II (DIO2), thereby regulating UCP1 expression and promoting increased thermogenesis in adipose tissue. At the same time, PR domain-containing 16 (PRDM16), peroxisome proliferator-activated receptor gamma (PPARγ) and peroxisome proliferator-activated receptor-gamma coactivator 1-α (PGC-1α) are considered to be important proteins involved in promoting adipocyte differentiation. The main hydrolases used to hydrolyze triglycerides in adipose tissue are triglyceride lipase (ATGL) and hormone-sensitive lipase (HSL), and the fatty acids generated by hydrolysis partially act on adipocytes for metabolic consumption.
[0165] Real-time-PCR experimental results are shown in Fig. 9 , Fig.10The results showed that the Chinese medicine composition can significantly increase the expression of genes such as Pparγ, Atgl, Hsl, Pgc1α, Ucp1, Dio2, Prdm16, and Cidea. Among them, the increased expression of Atgl and Hsl genes can promote the hydrolysis of triglycerides in adipose tissue, and the increased expression of Pparγ, Pgc1α, Ucp1, Dio2, Prdm16 and Cidea genes can promote the browning of white fat, thereby causing adipose tissue to consume fatty acids in the form of heat energy.
[0166] In summary, the Chinese medicine composition can promote fat decomposition and browning of white fat, enhance its heat production capacity, increase energy consumption, and thus play a role in improving obesity.
