Description

Technical Field

[0001] The present invention relates to an oral magnetic resonance imaging (MRI) composition and its use in the in vivo diagnosis of liver metastasis and / or primary liver cancer in a subject consuming a predetermined meal. Background Art

[0002] Orviglance is an oral liver imaging drug (i.e., a liver-specific contrast agent) for detecting and visualizing focal lesions of the liver using magnetic resonance imaging ("MRI") in subjects where the use of current gold standard gadolinium-based contrast agents ("GBCAs") may not be medically advisable or can not be administered. MRI is a medical imaging technique used in radiology to form pictures of the body's anatomy and physiological processes. MRI scanners use strong magnetic fields, magnetic field gradients, and radio waves to generate images of organs in the body.

[0003] A study by Leander et al. (Investigative Radiology, 45, 2010) has previously shown that administration of an oral manganese-based MRI composition to non-fasting subjects followed by MRI resulted in very low signal enhancement in the liver and the subjects were excluded from further efficacy analysis.

[0004] Therefore, fasting is routine procedure prior to administration of Orvigran and other contrast agents to ensure that imaging quality is not affected by interactions with various food components that affect contrast agent absorption and / or directly affect the imaging properties of manganese (II) ions or their complexes. Summary of the invention

[0005] The inventors have demonstrated that the consumption of a meal by a subject prior to the administration of Orvigran results in image quality significantly lower than that observed for fasting subjects. Fasting for long periods of time (>8h) is generally undesirable and may limit the willingness to use Orvigran, and for some subjects, fasting for very long periods of time may present health risks or significant inconveniences. Surprisingly, the inventors have now demonstrated that a meal having a caloric content of a predetermined value or less may be consumed prior to the administration of Orvigran to obtain an imaging quality at least as high as that obtained for fasting subjects. These findings support the diagnostic potential of Orvigran, particularly for subjects in whom fasting is significantly inconvenient or presents a health risk.

[0006] In a first aspect, an MRI composition is provided, comprising, alone or together, a physiologically acceptable manganese (II) compound, proteinogenic amino acids and / or vitamin D and one or more water-soluble excipients; wherein the MRI composition is for use in the in vivo diagnosis of liver metastasis and / or primary liver cancer in a subject; wherein the subject consumes a meal 0 to 8 hours before or up to about 2 hours after administration of the MRI composition and wherein the calorie content of the meal is about a predetermined value or lower.

[0007] In a second aspect, an MRI composition is provided, comprising, alone or together, a physiologically acceptable manganese (II) compound, proteinogenic amino acids and / or vitamin D and one or more water-soluble excipients; wherein the MRI composition is for use in the in vivo diagnosis of liver metastasis and / or primary liver cancer in a subject; wherein the subject consumes a meal 0 to 8 hours before or up to about 2 hours after administration of the MRI composition and wherein the caloric content of the meal is about 500 kcal or less.

[0008] In a third aspect, a method for magnetic resonance imaging (MRI) of a subject is provided, the method comprising administering to the subject a composition comprising, alone or together, a physiologically acceptable manganese (II) compound, proteinogenic amino acids and / or vitamin D, and one or more water-soluble excipients; and performing MRI on the subject; wherein the subject consumes a meal 0 to 8 hours before or up to about 2 hours after administration of the MRI composition, and wherein the caloric content of the meal is about 500 kcal or less.

[0009] In a fourth aspect, there is provided a method of reducing or preventing nausea, diarrhea and / or headache in a subject receiving an MRI composition comprising, alone or together, a physiologically acceptable manganese (II) compound, proteinogenic amino acids and / or vitamin D and one or more water-soluble excipients; the method comprising:

[0010] i) administering a meal to the subject; and

[0011] ii) administering the MRI composition to the subject;

[0012] wherein the meal is administered 0 to 8 hours before administration of the MRI composition or up to about 2 hours after administration of the MRI composition.

[0013] definition

[0014] As used herein, the term "administering" refers to the act of entering the body of a subject with an MRI composition. Thus, the time before or after administration is calculated relative to the time point at which the MRI composition enters the body of a subject.

[0015] The terms "meal", "predetermined meal", "snack" or "light meal" as used interchangeably herein refer to a serving of food having a calorie content of about a predetermined value or less (e.g., about 500 kcal or less). The predetermined value refers to a calorie content significantly lower than a regular meal having a calorie content of about 900 kcal or more.

[0016] The term "water-soluble" is used herein to describe compounds that dissolve in water at room temperature (e.g., about 25° C.). The degree of water solubility ranges widely, from infinitely soluble (unrestricted) (completely miscible) (e.g., ethanol in water) to poorly soluble (e.g., silver chloride in water). The term "insoluble" is generally applied to compounds that are poorly soluble or very poorly soluble.

[0017] The term "water-insoluble" is used herein to describe compounds that are insoluble in water at room temperature (eg, about 25°C).

[0018] Many other descriptive terms are also used to quantify the degree of solubility of a given solute (i.e., a compound that undergoes a dissolution process):

[0019] the term The mass of water required to dissolve 1 mass of solute Very soluble <1 Soluble 1 to 10 Soluble >10 to 30 Poorly soluble >30 to 100 Slightly soluble >100 to 1000 Very slightly soluble >1000 to 10,000 Almost insoluble or insoluble ≥10,000

[0020] The water solubility of a given solute is typically temperature dependent. Depending on the nature of the solute, the solubility may increase or decrease with temperature. For most solids and liquids, their solubility in water increases with temperature.

[0021] The term "hygroscopic" is used herein to describe a compound that absorbs water by absorption, adsorption, or a combination of these two processes to any significant extent at a relative humidity (rH) between 40% and 60%, such as to an extent where the mass and / or volume of the compound increases by about 5% or more over a period of time (e.g., after about 1 hour).

[0022] The term "non-hygroscopic water-soluble" is used herein to describe compounds that are not hygroscopic as defined herein and at the same time are water-soluble as defined herein.

[0023] The term "absorption enhancer" is used interchangeably herein with the term "uptake enhancer" and refers to a compound capable of enhancing the transport of manganese across the gastrointestinal membrane. These compounds are well known in the art, for example from WO 96 / 05867, and include physiologically tolerated reducing compounds containing an α-hydroxy ketone group, physiologically tolerated acids or salts thereof containing an α- and / or β-hydroxy or amino group, and / or vitamin D. BRIEF DESCRIPTION OF THE DRAWINGS

[0024] Figure 1 : Overview of the study design from Example 1, where two time periods were fasting and two groups of subjects received a meal or snack prior to dosing with Orvigran.

[0025] Figure 2 : Relative change in MRI signal intensity (SI%) from liver imaging at 1, 4, 8 and 24 hours after dosing in fasting subjects (black line) compared to the same subjects after receiving a meal (dashed line). [0201] All subjects abstained from xanthine-containing foods or beverages (eg, chocolate, tea, coffee, or cola drinks), tobacco, and tobacco-containing products (eg, pan, pan masala, gutkha) 24 hours prior to enrollment until their overnight stay in the clinical facility.

[0202] Dietary iron supplements (including multivitamins containing iron) were prohibited from 6 hours before enrollment in Period 1 until after the last contrast-enhanced MRI on study. Magnesium and / or calcium supplements may reduce manganese absorption and were prohibited from 6 hours before enrollment in Period 1 until after the last contrast-enhanced MRI on study.

[0203] Supplementation with vitamin C or vitamin D3 (which promotes manganese absorption) was prohibited from 6 hours prior to enrollment in Period 1 until after the last contrast-enhanced MRI on study. Magnesium-containing antacids and laxatives were prohibited from 6 hours prior to enrollment in Period 1 until after the last contrast-enhanced MRI on study. Quinolone and tetracycline antibiotics were prohibited from 6 hours prior to enrollment in Period 1 until after the last contrast-enhanced MRI on study.

[0204] All subjects abstained from grapefruit or its products within 72 hours prior to enrollment in Period 1 until the last sample collection of the study. All subjects abstained from alcohol or alcohol products, recreational drugs within 48 hours prior to enrollment in Period 1 until the last sample collection of the study. Smoking was prohibited and subjects were to quit smoking 6 months prior to enrollment in Period 1 until the completion of the study.

[0205] Posture restrictions

[0206] Orvigran was administered to subjects in a sitting position. For the first 4 hours after dosing in each period, subjects were in a sitting or ambulatory position. In the event of one or more adverse events, the subject's position was changed appropriately. Thereafter, subjects were allowed to participate only in normal activities while avoiding any strenuous physical activity. Note: Subjects were in the supine position during MRI.

[0207] Fasting duration and meal distribution:

[0208] For time period 1 :

[0209] In Period 1, overnight fasting will be at least 4 hours prior to dosing and lunch will be provided after completion of MRI 4 h post-dose.

[0210] For time period 2 :

[0211] All subjects will be required to fast overnight for at least 6 hours before providing a full meal according to Table 1 or a snack according to Table 2, requiring them to be completely eaten within 30 minutes of providing the full meal or snack. Lunch will be provided in time period 2 after 4 ± 1 hours after dosage administration. Nutritionists / trained researchers weigh the remaining food and calculate the calories of any subject who does not fully consume a full meal or snack. Standardized meals are provided to them at the appropriate time during the subject's stay in the clinical facility. Subjects receive lunch at least 4 ± 1 hours (after 4 hours MRI) (for time period 1 and time period 2) after administration, and from then on, other meals are provided at appropriate intervals until registration and departure.

[0212] In addition to the administration of 200 mL of water during dosing, in each time period, the subjects refrained from drinking water from 1 hour before each dosing until 1 hour after each dosing. Before and after this, water could be drunk as needed.

[0213] result

[0214] The results of the study are shown in the following tables: Table 4 (comparison of meal and fasting); and Table 5 (comparison of meal and fasting). The best SI (%) increase is most common at about 4 hours after dosing; therefore, comparison of SI (%) at 4 hours after dosing is important to examine the difference in SI (%) between non-fasting subjects and fasting subjects.

[0215]

[0216] Table 4. MRI signal intensity (SI%) from liver imaging and relative change in signal intensity at 1, 4, 8 and 24 hours post-dose in fasted subjects (T1) compared to the same subjects after receiving a meal (T21). The SI% obtained from imaging of subjects after a meal was significantly lower than after the fasting period.

[0217]

[0218]

[0219] Table 5. MRI signal intensity (SI%) from liver imaging and relative change in signal intensity at 1, 4, 8 and 24 hours post-dose in fasted subjects (T1) compared to the same subjects after receiving a meal (T22). Surprisingly, the SI% obtained from imaging of fed subjects was at least as good as that after fasting.

[0220] in conclusion

[0221] This study supports the existing hypothesis that consumption of a meal according to Table 1 prior to administration of an oral manganese-based MRI agent such as Orvigran significantly reduces the image quality of subsequent liver MRI. Surprisingly, this study also supports that subjects who have consumed a meal with a caloric content of about 500 kcal or less (snack) are still able to undergo MRI using Orvigran and the image quality is at least as high as that of fasting subjects. These findings significantly support the diagnostic potential of Orvigran, especially for subjects for whom fasting may be inconvenient or problematic.
