Description

[0001] METHOD FOR THE PRODUCTION OF EXTRACELLULAR VESICLES LOADED WITH A MOLECULE OF INTEREST AND COMPOSITION COMPRISING SUCH EXTRACELLULAR VESICLES

[0002] Technical Field

[0003] The present invention relates to a method for the production of extracellular vesicles loaded with a molecule of interest compatible with the pharmaceutical industry, for example exosomes loaded with a molecule of interest.

[0004] The present invention also relates to a pharmaceutical composition containing extracellular vesicles loaded with a molecule of interest obtained by the method according to the present invention.

[0005] The present invention also pertains to a pharmaceutical composition containing extracellular vesicles loaded with a molecule of interest.

[0006] State of the art

[0007] Exosomes are small extracellular vesicles with a diameter between 30 and 150 nm and are secreted by a multitude of cell types. Exosomes are involved in several cellular processes such as intercellular communication, antigen presentation, transfer of proteins, messenger RNAs, siRNAs, and / or microRNAs from one cell to another. The function of exosomes is altered in many diseases such as cancers, which suggests their importance as a diagnostic tool and / or in therapeutic applications, for example in acellular therapies as a vehicle to deliver therapeutic molecules, in immunotherapy, to treat different types of cancer and / or inflammatory and cardiovascular neurodegenerative diseases, diabetes, etc.

[0008] Some protocols for isolating and purifying exosomes and extracellular vesicles are known, and exosomes have already been successfully used in cell-free therapy studies.

[0009] However, although some exosome isolation and purification protocols exist, it is important to develop exosome production and purification methods that allow for batch-to-batch consistent pharmaceutical-grade exosome production at large scale, with high yield, while being fast and inexpensive so as to increase the use of exosomes as a cell-free therapy tool in medical practice, for the benefit of the greatest number of patients. Exosomes can also be loaded with small-molecules and can act as a therapeutic cargo delivery system. Different strategies already exist for the loading of exosomes with small molecules allowing a targeted delivery of drug molecules by exosomes (Zeng H. et al. 2023, Current strategies for exosome cargo loading and targeting delivery. Cells 12(10): 1416, DOI: 10.3390 / cellsl 2101416) .

[0010] In that context, EP3706796 describes extracellular vesicle (EV) therapeutics, wherein the EVs comprise endogenously loaded RNA therapeutics such as mRNAs, circular RNAs, miRNAs, shRNAs and various other RNA therapeutics agents. However, even if this document and the prior art extol the merits of their technology, currently, no efficient production and loading strategies allow reaching a sufficient amount of loaded exosomes with sufficient loading of molecules of interest (therapeutic molecules) to be used as a therapeutic means in daily medical practice while being reliable, sufficiently generalizable and reproducible and remaining affordable in terms of production costs.

[0011] Consequently, there is a need to improve the current production and loading methods by making them more efficient, industrially applicable both in terms of the quantity of loaded exosomes produced and in terms of production costs, and this with a production of loaded exosomes with constant pharmaceutical quality regardless of the production batch.

[0012] Objectives of the invention

[0013] The present invention aims to overcome the drawbacks of the state of the art, particularly those described above.

[0014] In particular, the present invention proposes to provide a method for producing loaded exosomes with a molecule of interest making it possible to reach an industrial scale of production which is profitable by minimizing production costs.

[0015] Another objective of the present invention is to provide a method of producing loaded extracellular vesicles of constant pharmaceutical quality, regardless of the production batch, which meets GMP standards.

[0016] The present invention also aims to provide a pharmaceutical composition containing extracellular vesicles loaded with a molecule of interest having high and efficient anti-inflammatory properties able to be used as a therapeutic option in daily medical practices for chronic inflammatory pathology and / or an age-related pathology. Summary of the invention

[0017] To achieve the aforementioned objectives, the present invention provides a method to produce extracellular vesicles loaded with one or several molecule of interest compatible with the pharmaceutical industry, for example exosomes loaded with one or several molecules of interest comprising the following steps:

[0018] - a supply of a bioreactor chamber containing a porous three- dimensional scaffold by a cell culture medium via a supply port, and, after a predefined duration, an exit via an outlet port,

[0019] - seeding said porous three-dimensional scaffold with a starting number of cells (starting number of adherent cells resuspended after detachment) in a volume of culture medium,

[0020] - a phase of attachment of said cells to form adherent cells on said porous three-dimensional scaffold, wherein preferably said adherent cells are adherent immortalized cells,

[0021] - an expansion of the adherent cells on said porous three-dimensional scaffold up to a second number of adherent cells, said second number of adherent cells divided by said starting number defining a multiplication factor being preferably of at least 5 for a single expansion step in the said scaffold,

[0022] - a production of extracellular vesicles by said adherent cells during a production phase, after or concomitantly with the expansion, during which the vesicles are produced and excreted in the culture medium to give a culture medium enriched with extracellular vesicles,

[0023] - preferably, a first purification of the culture medium enriched with extracellular vesicles to produce a purified culture medium enriched with extracellular vesicles,

[0024] - a loading of said extracellular vesicles with said one or several molecules of interest, subsequently to the production,

[0025] - preferably, a (second) purification arranged to remove unloaded extracellular vesicles to produce a purified culture medium enriched with extracellular vesicles loaded with a molecule of interest, - optionally, a harvest of extracellular vesicles.

[0026] As it can be seen, according to the present invention, a bioreactor with a porous three-dimensional scaffold (fixed bed) is seeded with adherent cells.

[0027] According to the present invention, the adherent cells resuspended after detachment in a volume of culture medium are preferably obtained by detachment, for example by trypsinisation, of the adherent cells from a culture of adherent cells and resuspension of said adherent cells in the volume of culture medium.

[0028] After or concomitantly to the expansion (growth phase), extracellular vesicles production phase is started, for example by stress or chemical induction, and the vesicles are produced and excreted in the culture medium to give a culture medium enriched with extracellular vesicles. After the production phase, the extracellular vesicles are further purified and loaded with at least one molecule of interest leading to a high production of extracellular vesicles of high and constant quality loaded with molecules of interest.

[0029] Such method for the production of extracellular vesicles loaded with one or several molecules of interest compatible with the pharmaceutical industry, wherein preferably the adherent cells are adherent immortalized cells, allows having a stable production of the extracellular vesicles with the formation of high cell density at the end of the expansion phase, wherein the cells are of high quality. This permits having a method according to the present invention generating a large quantity of extracellular vesicles of high and stable quality that is compatible with the pharmaceutical industry.

[0030] Surprisingly, while the density of cells after the growing phase is very high, it was found that high extracellular vesicle (EV) concentration is achieved with EV of high quality in the presence of few death cells in the harvesting solution of the extracellular vesicles. EV of high quality in the presence of few death cells in the harvesting solution will also facilitate the loading of EV with molecules of interest when the loading occurs after the harvest phase and subsequent purification. By the unique combination of this adequate environment with a culture of an adherent cell line, which are, advantageously, very stable between each generation, and with, preferably, a multiplication factor of at least 5, allows not only to reach but also to maintain a very high yield of cells of high and constant quality with few death cells. This cell culture of high yield and constant quality regardless of the production batch promotes a production of loaded extracellular vesicles of high and constant pharmaceutical quality, which meets GMP standards while reaching an industrial scale of production which is profitable by minimizing production costs of a cycle of production.

[0033] Other embodiments of the pharmaceutical composition of the present invention obtained by the method according to the present invention are indicated in the appended claims.

[0034] The present invention also relates to a pharmaceutical composition comprising a therapeutically effective amount of extracellular vesicles loaded with a molecule of interest wherein said molecule of interest comprises at least a circular RNA, and wherein said extracellular vesicles are extracellular vesicles obtained from adherent immortalized mesenchymal stem cell.

[0119] Examples.-

[0120] Example 1.- Pharmaceutical composition according to the present invention comprising extracellular vesicles loaded by mechanoporation with a miRNA

[0121] The inventors have encapsulated by a mechanoporation step hsa- miR-146a into extracellular vesicles obtained from HEK293 cells and immortalized Wharton's Jelly derived-mesenchymal stem cells.

[0122] The mechanoporation step were performed in a micro-nano fluidic chip at a flow rate comprised between 10 pL / min and 10000 pL / min, preferably between 50 pL / min and 5000 pL / min and a flow pressure up to 50 bar. In each condition of mechonoporotion tested, o high stability of the loading process (loading of hsu-miR-146a into extracellular vesicles) was achieved.

[0123] After purification steps using nuclease and tangential flow filtration (TFF), the extracellular vesicles were characterized by size distribution (NTA and MRPS), protein, cholesterol, and residual nucleotides concentrations. The absence of GM130 and presence of tetraspanins CD9 / CD63 / CD81 were also confirmed.

[0124] Results: loading efficiency and anti-inflammatory effect of the extracellular vesicles loaded by mechanoporation with hsa-miR-146a

[0125] The loading efficiency was assessed by specific qRT-PCR, and the antiinflammatory effect was assessed by cytokines quantification from RAW 264.7 macrophages (20 000 cells / well in a 96-well plate). A concentration of 5.10A9 extracellular vesicles / ml of solution were used.

[0126] The anti-inflammatory assay showed that unloaded extracellular vesicles obtained from HEK cells had limited anti-inflammatory effects, even when mixed with miRNA, while loaded extracellular vesicles obtained from HEK cells showed very potent anti-inflammatory effects, demonstrating the efficiency of the loading.

[0127] In that context, Figure 1 shows the expression of IL-6 mRNA in different culture conditions (Cl , C2, C3, C4, C5, and C6) of RAW 264.7 macrophage with different addition of component (condition A to condition E). Condition A is a condition in the presence of lipopolysaccharide, LPS; condition B is a positive control in the presence of dexamethasone; condition C is a condition in the presence of unloaded extracellular vesicles obtained from HEK cells; condition D is a condition in the presence of miR-146a; while condition E is a condition in the presence of loaded extracellular vesicles obtained from HEK cells with miR-146a wherein the loading is performed by mechanoporation. Cl represents the cell culture condition without the addition of any component from condition A to E, Cl is the negative control; C2 represents the cell culture condition in the presence of LPS; C3 represents the cell culture condition in the presence of LPS and dexamethasone; C4 represents the cell culture condition in the presence of LPS and unloaded extracellular vesicles obtained from HEK cells; C5 represents the cell culture condition in the presence of LPS, unloaded extracellular vesicles obtained from HEK cells, and miR-146a; while C6 represents the cell culture condition in the presence of loaded extracellular vesicles obtained from HEK cells with miR-146a wherein the loading is performed by mechanoporation. As shown, the presence of loaded extracellular vesicles obtained from HEK cells with miR-146a wherein the loading is performed by mechanoporation allows reducing the expression level of the pro-inflammatory cytokine IL-6. Unloaded extracellular vesicles from Wharton's Jelly derived- mesenchymal stem cells presented anti-inflammatory effects, but loaded extracellular vesicles from Wharton's Jelly derived-mesenchymal stem cells (by mechanoporation) presented similar effects when used at a 10 times lower concentration. Loading of extracellular vesicles (by mechanoporation) enhances the loading of cargo into extracellular vesicles. The loading efficiency in this controlled and robust approach enables the production of high-quality drug-loaded extracellular vesicles with yields compatible for the manufacturing of highly potent therapeutic products.
