AI Technical Summary
Current fat injection methods for lipofilling face challenges such as poor injectability, high risk of post-operative bruising, and limited precision due to the viscosity and consistency of fat, which affects the accuracy and effectiveness of the procedure.
A malleable gel composition is created by combining a hydrogel, specifically hyaluronic acid, with fat or its derivatives in emulsion form, achieving a viscoelastic property that allows for precise injection through small cannulas and improved stability, reducing bruising and enhancing procedural ease.
The gel-like emulsion improves the rheology of fat for lipofilling, enabling precise injections, reducing bruising, and allowing for more satisfactory results with longer-lasting visible effects on the skin, such as skin remodeling and improved radiance.

[000260] The gel, gelled emulsion or emulsion according to the invention also finds an important application in joint surgery and in dental surgery for the filling of periodontal pockets for example. [000261] These examples of use are in no way limiting, the gel, the gelled emulsion or the emulsion according to the present invention being more broadly intended for: - fill volumes; - generate spaces between certain tissues; - replace deficient physiological fluids; - Reshape any body area. [000262] The invention also relates to the process for preparing gels, gelled emulsions or emulsions according to the invention, characterized in that it comprises the following steps: a) Fat or fat derivative is provided, b) A hydrogel is provided, c) The hydrogel and the fat or fat derivative are mixed aseptically, d) The mixture is emulsified until an emulsion, a gelled emulsion or a homogeneous gel is obtained. [000263] In one embodiment, the hydrogel contains an active ingredient as previously defined. [000264] In one embodiment, an active ingredient is provided which is introduced at the time of mixing the hydrogel and the fat or fat derivative. [000265] The fat, the fat derivative, the hydrogel and the active ingredients are as previously defined. [000266] In different embodiments of the invention, the gel, gelled emulsion or emulsion comprising different hydrogels, different fat products or fat derivatives can be injected into the patient's body at different levels of anatomical depth, on different areas of the body and at different concentrations or dilutions. These different compositions and injection methods will be adapted according to the area to be treated and the different needs for remodeling or improving the trophicity of the treated areas. Thus, different gels, gelled emulsions or emulsions can be used at the same time or successively for the same area or for different areas of the body. [000267] The invention makes it possible to significantly improve the rheology of fat for lipofilling by making it more suitable for injection. The gels, gelled emulsions or emulsions obtained are stable and have a consistency close to, or equivalent to, that of mayonnaise. [000268] Gels, gelled emulsions or emulsions have great malleability allowing them to be injected via very small diameter cannulas which allows practitioners to be extremely precise and to limit the undesirable effects which are quite common in the case of fat injections for lipofilling alone. [000269] The use of an emulsion, a gelled emulsion or a gel according to the invention makes it possible to significantly reduce the appearance of post-operative bruising compared to the injection of fat for lipofilling alone. [000270] In addition, this malleability makes it easier for the surgeon to perform the procedure and makes it more fluid because the injectability of the emulsions is good. [000271] Finally, this malleability allows the surgeon to reshape different areas of the body. It is possible to modify the implantation area or the shape of the implant once it is injected. These post-injection modifications make it possible to obtain the desired effects more satisfactorily. [000272] The injection of the emulsion, the gelled emulsion or the gel according to the invention makes it possible to obtain visible effects on the skin, such as for example a remodeling effect or even an improvement in the radiance of the skin. [000273] The visible effects on the skin are still visible at least 3 months after the injection. [000274] The visible effects on the skin are still visible at least 6 months after the injection. [000275] The visible effects on the skin are still visible at least 12 months after the injection. [000276] The visible effects on the skin are still visible at least 18 months after the injection. Use syringe 1 to collect the fat for lipofilling, or the previously prepared MICROFAT or NANOFAT (medium-sized syringe). 2. Take cartridge 4 pre-filled with hyaluronic acid and connect syringe 1 to cartridge 4 using Luer-Lock connectors 1a and 4a. 3. Add syringe 2 (large volume syringe VI + V4) to the second tip 4-b of cartridge 4 and operate pistons 1b and 2b of both syringes 1 and 2 using back and forth movements to create an emulsion between the fat for lipofilling and the hyaluronic acid gel. 4. After several back and forth movements (at least 5 movements), completely draw the emulsion into syringe 2 (large volume syringe). 5. Disconnect syringe 2 from the cartridge, the gel, gelled emulsion or emulsion contained in this syringe 2 is ready to be injected. 6. Fit a cannula onto syringe 2, tip 2a, to perform the injections. The choice of these cannulas is left to the surgeon's discretion depending on the area to be treated. [000284] Optional steps can be carried out to allow the incorporation of active ingredients into the gel, the gelled emulsion or the emulsion of hyaluronic acid and fat for lipofilling. [000366] Figure 37 represents the front view of the connector (14). [000367] The following examples are given to illustrate the invention. EXAMPLES: Example 1: Hyaluronic acid gels [000368] A commercial non-crosslinked hyaluronic acid gel, the product Stylage® Hydro (VIVACY) and a commercial crosslinked hyaluronic acid gel comprising a local anesthetic, the product Juvederm Ultra® 2 (ALLERGAN), were used for the preparation of the fat and hyaluronic acid emulsions according to the invention. These commercial hyaluronic acid gels are available in 1 mL syringes. Example 2: Process for recovering the fat used for the preparation of the fat emulsion. [000369] Fatty tissue, called lipofilling fat, was harvested during surgery with the appropriate consent of the donor, preferably from the abdomen or knees. [000370] The fat collection was carried out following standard surgical techniques under anesthesia according to a protocol involving in particular identification of the area of the body to be collected, making an incision in the collection area followed or not by infiltration with Klein serum, collection of the fat for lipofilling using a specific Tonnard cannula of the Tulip ® brand and transfer of the fat from the cannula to a sterile syringe under vacuum using back and forth movements in the tissues and fan-shaped movements of the cannula to avoid bleeding and obtain a regular appearance after collection. [000371] The choice of material used for the sample as well as its technical characteristics is left to the surgeon's discretion depending on the area to be treated. [000372] The grease-filled syringe is ready to use or can be stored in the cold before use. Example 3: Process for preparing a fat emulsion for lipofilling [000373] Recovered fat for lipofilling is collected using a sterile 5 mL syringe equipped with a Luer lock system. [000374] This sterile syringe is connected to one of the inlets of a sterile 3-way valve or to one of the inlets of a sterile double Luer lock connector, while to one of the other inlets is connected an empty sterile 5 mL syringe equipped with the Luer lock system. [000375] In order to obtain a homogeneous emulsion based solely on fat for lipofilling, rapid back and forth movements of the pistons between the two sterile syringes connected to each other by the 3-way valve or by the double Luer lock connector are carried out. At least 40 back and forth movements are necessary to obtain a satisfactory emulsion of fat for lipofilling. Example 4: Process for preparing a fat emulsion for lipofilling and hyaluronic acid according to the invention [000376] 4 mL of recovered fat for lipofilling is collected using a sterile 5 mL syringe equipped with a Luer lock system. [000377] This sterile syringe is connected to one of the inlets of a sterile 3-way valve or to one of the inlets of a sterile double Luer lock connector, while to one of the other inlets is connected a syringe containing 1 mL of hyaluronic acid gel. [000378] The hyaluronic acid gel is then introduced into the sterile syringe containing the fat for lipofilling. [000379] The syringe containing the hyaluronic acid gel is replaced by an empty sterile 5 mL syringe equipped with a Luer lock system. [000380] In order to obtain a homogeneous emulsion based on fat for lipofilling and hyaluronic acid, rapid back and forth movements of the pistons between the two sterile syringes connected to each other by the 3-way valve or by the double Luer lock connector are carried out. At least 40 back and forth movements are necessary to obtain a satisfactory emulsion of fat for lipofilling and hyaluronic acid. [000381] In a variant, the homogeneous emulsion is obtained using the kit comprising the cartridge as defined above. Example 5: Storage stability of a fat emulsion for lipofilling and hyaluronic acid according to the invention. [000382] The stability of a fat emulsion for lipofilling and hyaluronic acid prepared according to the method of Example 2 from the product Stylage® Hydro was monitored visually during horizontal storage on a table at 20°C, 2 hours and 4 hours after its preparation. [000383] The stability of a fat emulsion for lipofilling prepared according to the method of Example 1 was also evaluated in parallel according to the same protocol. [000384] The results of this stability study are described in Table 1. Table 1 [000385] A pure fat emulsion for lipofilling degrades rapidly as indicated by the formation of two phases after 2 hours of storage at room temperature. [000386] The fat emulsion for lipofilling and hyaluronic acid according to the invention is stable at room temperature for at least 4 hours. [000387] The stability of the fat emulsion for lipofilling and hyaluronic acid according to the invention offers flexibility and comfort to the surgeon in the context of its use during a surgical procedure. Example 6: Injectability of fat emulsions for lipofilling and hyaluronic acid according to the invention [000388] The injectability of a fat emulsion for lipofilling and hyaluronic acid prepared according to the method of Example 2 from the product Stylage® Hydro and a fat emulsion for lipofilling and hyaluronic acid prepared according to the method of example 2 from the product Juvederm Ultra® 2 was evaluated through 27G and 25G cannulas from freshly prepared emulsions and after 4h30 of storage at 20°C. [000389] For comparison, the injectability of a freshly prepared fat emulsion for lipofilling was evaluated through 27G and 25G cannulas. [000390] The injectability results are shown in Table 2. Table 2 [000391] The results obtained show that the fat emulsions for lipofilling and hyaluronic acid according to the invention allow the surgeon to use cannulas of a smaller size than those which can be used for the injection of a pure fat emulsion for lipofilling. [000392] The fat emulsions for lipofilling and hyaluronic acid according to the invention can still be injected with small cannulas at least 4.5 hours after their preparation, which provides comfort and flexibility in terms of use during a surgical procedure. Example 7: Appearance of fat emulsions for lipofilling and hyaluronic acid according to the invention after passage through a cannula [000393] The stability of fresh emulsions according to the invention and 4.5 hours after their preparation was evaluated by visual observation of the deposit obtained after passing through a 25G cannula. [000394] The emulsions tested are identical to those of example 4. [000395] The observation results of these different emulsions are presented in Table 3. Table 3 [000396] The fat emulsions for lipofilling and hyaluronic acid according to the invention have better resistance after passing through a cannula than the pure fat emulsion for lipofilling, even after storage for 4.5 hours at room temperature. [000397] The emulsions of fat for lipofilling and hyaluronic acid according to the invention have a malleability close to a gel, this characteristic is different from that of fat for lipofilling alone. The emulsions obtained according to the invention behave like a modeling clay, which is a different property from fat for lipofilling alone. Example 8: Rheological properties of fat emulsions for lipofilling and hyaluronic acid according to the invention [000398] The viscoelastic properties were measured using an AR2000 rheometer from TA instrument. The measurements are carried out at 25 °C with a 2 ° 40 mm conical geometry using a frequency sweep from 0.08 to 5 Hz with a deformation of 0.8%. [000399] The viscoelastic properties were measured for a fat emulsion for lipofilling prepared according to Example 1 and for an emulsion of fat for lipofilling and hyaluronic acid prepared according to Example 2 from the product Stylage® Hydro. [000400] The results of the measurements of G' and Tan 5 at 1 Hz, as well as the cut-off frequency of the G' and G", / "cut-off curves, of the different samples studied are presented in Table 4 below. Table 4 [000401] The rheological properties of the fat emulsion for lipofilling and hyaluronic acid according to the invention prepared from the Stylage® Hydro product are not affected by passage through a 25G size cannula. [000402] It can be noted that the fat emulsion for lipofilling and hyaluronic acid according to the invention prepared from the product Stylage® Hydro has a Tan 5 value close to that of the commercial hyaluronic acid gel Stylage® Hydro. [000403] The rheological properties of the fat emulsion for lipofilling and hyaluronic acid according to the invention prepared from the product Stylage® Hydro are much more interesting than those of the pure fat emulsion for lipofilling. Example 9: Microscopic analysis of fat emulsion for lipofilling and hyaluronic acid according to the invention. [000404] The microscopic analyses were carried out using an Axio scope Al optical microscope from Zweiss, equipped with Filtrex software and with x20 magnification. [000405] Three types of samples were analyzed under a microscope: a sample of a fat sample for lipofilling, a sample of a fat emulsion for lipofilling prepared according to example 1, a sample of a fat emulsion for lipofilling and hyaluronic acid according to the invention prepared according to example 2 from the product Stylage® Hydro. [000406] The results of the various microscopic observations of the samples are presented in Table 5 below. Table 5 [000407] The fat and hyaluronic acid emulsion according to the invention has a more regular appearance with small fat globules incorporated in a hyaluronic acid matrix. Example 10: Comparative clinical study [000408] Two groups A and B of patients are formed. A contains 40 patients and B contains 40, making 80 patients in total. [000409] Group A is injected for facial rejuvenation with an emulsion of fat for lipofilling and hyaluronic acid while group B is injected for facial rejuvenation with fat for lipofilling. [000410] For each patient in group A, an emulsion of fat for lipofilling taken from the patient and hyaluronic acid is prepared according to the method of example 4, the hyaluronic acid used being the product Stylage® Hydro, with a concentration of 14 mg / g in a mannitol phosphate buffer (VIVACY Laboratory), the fat being washed and decanted fat for lipofilling of the MICROFAT type and the hyaluronic acid:fat volume ratio being 1:4. [000411] For each patient in group B, the fat for lipofilling is taken from the patient and is of the MICROFAT type. [000412] On D0, each patient has fat taken from the stomach area in accordance with the protocol described in example 2. [000413] The injection at the facial level will be carried out using a 25G Steriglide cannula from TSK brand after having made a pre-hole using a 23G Steriglide pre-hole needle from TSK brand. [000414] The practitioner in charge of the injection evaluates the volume injected per hemiface and assesses the ease of injection of the product using the injectability score. This score of 0 to 3 is assessed as follows: 0: injection impossible: the product does not come out. 1: Poor injectability: need to apply very high force to eject the product from the syringe, irregular product ejection flow and cannula clogging 2: medium injectability: need to apply high force to eject the product from the syringe, product ejection flow with jerks 3: good injectability: medium to low force to eject the product, smooth and continuous injection, without jolts. [000415] The person in charge of administration also assesses the malleability of the product when setting up the product using a 2-level scale: 0: the product is not malleable: it cannot be deformed and remains in a block or disperses when massaged or under digital pressure in an uncontrollable manner 1: the product is malleable: it remains in place and under massage or digital pressure. [000416] On D+1, the practitioner in charge of the injection evaluates the adverse effects in terms of occurrence and severity, while the patient assesses the rejuvenation effect using a patient satisfaction score of 1 to 3: 1: unsatisfactory rejuvenation effect 2: moderately satisfactory rejuvenation effect 3: satisfactory rejuvenation effect 4: very satisfactory rejuvenation effect, intention to start again [000417] The results are summarized in table 6 below: Table 6 [000418] These examples show that the gels according to the invention, composed of fat for lipofilling and hyaluronic acid, make it possible to obtain results of rejuvenation much more satisfactory than fat injections for lipofilling alone. [000419] The gels according to the invention make it possible to improve the malleability of fat for lipofilling. [000420] The gels according to the invention make it easier to inject fat for lipofilling because there is no problem of obstruction of the cannulas, unlike tests with fat for lipofilling alone. [000421] In addition, the injections of gels according to the invention make it possible to significantly limit the occurrence of post-injection side effects. Indeed, it appears that side effects, such as bruises, are observed in only 5% of cases with the gels according to the invention, compared to 50% in the case of fat injections for lipofilling alone. Example 11: In vitro injectability study on 10 samples [000422] Mixture of 4 cc of fat for lipofilling, MICROFAT type or NANOFAT type, with 1 cc of Stylage® Hydro product, which is a hyaluronic acid gel with a concentration of 14 mg / g in a mannitol phosphate buffer sold by VIVACY Laboratories, the hyaluronic acid:fat volume ratio being 1:4. [000423] Manual injectability test with TSK Steriglide brand cannulas and BD brand 10 cc Luer-Lock syringes. [000424] The choice of cannulas is made from among the cannulas dedicated to the injection of filler products (22G to 27G), that is to say for sizes smaller than the lipofilling cannulas (19-21G) usually used during surgical interventions. [000425] The practitioner assesses injectability blindly and sets an injectability score using the following injectability scale: 0: injection impossible: the product does not come out. 1: Poor injectability: need to apply very high force to eject the product from the syringe, irregular product ejection flow and cannula clogging 2: medium injectability: need to apply high force to eject the product from the syringe, product ejection flow with jerks 3: good injectability: medium to low force to eject the product, smooth and continuous injection, without jolts [000426] The results are summarized in Table 7 below: Table 7 [000427] For the same cannula size, the injectability is at least identical, or even more often, better with the subject of the invention. This makes it possible, on the one hand, to inject with smaller cannulas, therefore less traumatic for the patient, and on the other hand, to facilitate the practitioner's gesture because the gestures are more fluid and more precise. Example 12: Non-comparative clinical study 45 cases [000428] The gel studied in this example is made up of NANOFAT, resulting from the transformation of lipofilling fat, and the product Stylage® Hydro, which is a hyaluronic acid gel with a concentration of 14 mg / g in a mannitol phosphate buffer sold by Laboratoires VIVACY, the hyaluronic acid:fat volume ratio being 1:4. [000429] A non-comparative study of 45 clinical cases was carried out over a period of 2 years, combining a mixture of 4cc of NANOFAT and 1 cc of Stylage® Hydro product. [000430] On D0, each patient has fat taken from the stomach area, then injected into the face using a 25G cannula in the malar area for facial rejuvenation. Each patient is injected on each side of the face and therefore undergoes 2 injection procedures. [000431] 90 injection procedures are therefore carried out. [000432] The results are summarized in the following Table 8: Table 8 [000433] This clinical study has demonstrated that this new method of skin rejuvenation gives satisfaction to all patients treated, a large majority of whom declare themselves very satisfied. In addition, the effect is still visible more than a year after the procedure was carried out. Finally, side effects are rare, not very widespread and mild.
