Abstract
Description
[Technical field]
[0001] The present invention relates to a lighting device. [Background technology]
[0002] Currently, there is a two-step treatment that involves administering to the patient a drug containing a complex composed of a photosensitive substance and a component that selectively accumulates in specific cells, and then irradiating the patient with light of a specific wavelength to which the photosensitive substance reacts. A photosensitive substance is a substance that reacts with light of a specific wavelength range. After administering the drug and allowing it to selectively accumulate in specific cells, the photosensitive substance is activated by irradiating the cells with light of a specific wavelength, causing necrosis or elimination of the specific cells through biochemical and physical processes. The above treatment is a treatment that places a low burden on the patient, as it is unlikely to cause damage to cells other than the specific cells and has few side effects.
[0003] The photosensitive substance contained in the above-mentioned drug reacts with light other than the light irradiated in the above-mentioned treatment (hereinafter referred to as therapeutic light), such as light from sunlight and lighting, which may cause the composition of the drug to change and may not provide the desired therapeutic effect. In addition, a patient administered with the above-mentioned drug may develop photosensitivity when exposed to light other than therapeutic light. In order to prevent this, a lighting device that does not react with the photosensitive substance and can suppress a decrease in the illuminance in the room is required in the room where the patient is staying, the room where the administration of the above-mentioned drug is prepared, and the manufacturing room where the above-mentioned drug is manufactured. In addition, a lighting unit that does not react with the photosensitive substance and can suppress a decrease in the illuminance is required in medical equipment equipped with a lighting unit that illuminates the affected area, such as an endoscope, which can be used to illuminate the affected area. Summary of the Invention [Problem to be solved by the invention]
[0004] The present invention provides a lighting device capable of suppressing the reaction of photosensitive substances and suppressing a decrease in the illuminance in a room and the illuminance illuminating an affected area. [Means for solving the problem]

DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS
[0008] Hereinafter, an embodiment of the present invention will be described, but the present invention is not limited to the following description. In addition, various modifications and improvements can be made to the embodiment, and such modifications and improvements can also be included in the present invention.
[0009] <Embodiment> Hereinafter, a lighting device according to an embodiment will be described with reference to FIGS. An illumination device 1 according to the embodiment illuminates through a filter 3 that reduces the illuminance of light in a specific wavelength range to which a photosensitive substance reacts. The illumination device 1 includes a filter 1 and a light source 2. The illumination device 1 may further include a filter holder 4 that holds the filter 3, a support 6 that supports the light source 2 and the filter 3, and a clip 7 that fixes the support 6.
[0010] The filter 3 reduces the illuminance of light in a specific wavelength range to which the photosensitive substance reacts. The photosensitive substance is a substance that reacts to light in a specific wavelength range, such as a pigment that is activated by absorbing light in a specific wavelength range. An example of such a pigment is tetrasodium salt of sarrothalocane (6-({[3-({(OC-6-13)-bis({3-[bis(3-sulfopropyl)(3-sulfonatopropyl)azaniumyl]propyl}dimethylsilanolato-κ0,κ0')[(phthalocyaninato(2-)κN29,κN30,κN31,κN32)-1-yl]silicon}oxy)propoxy]carbonyl}amino)hexanoyl (IRDye (registered trademark) 700DX, manufactured by Rakuten Medical Co., Ltd.), which is activated by absorbing light in a wavelength range of 650 nm or more and 750 nm or less. When the photosensitive substance is IRDye (registered trademark) 700DX (Rakuten Medical Co., Ltd.), In the case of a filter 3 made of a material other than fluororesin, it is preferable that the filter 3 reduces the illuminance of light in the wavelength range of 650 nm to 750 nm, and more preferably reduces the illuminance of light in the wavelength range of 675 nm to 750 nm. Moreover, it is preferable that the filter 3 reduces the illuminance of light in the wavelength range of at least 650 nm, 660 nm or more, or 670 nm or more, and reduces the illuminance of light in the wavelength range of 750 nm or less, 740 nm or less, 730 nm or less, or 720 nm or less, and it is preferable that the filter 3 reduces the illuminance of light in the wavelength range of 650 nm to 750 nm, 670 nm to 750 nm, 650 nm to 720 nm or less, or 670 nm to 720 nm.
[0011] The filter 3 preferably reduces the illuminance of light in a specific wavelength range by 90% or more, for example, 90%, 91%, 92%, 93%, 94%, 95%, 96%, 97%, 98%, or 99%, and more preferably by 99% or more. Also, it is preferable that the filter 3 does not reduce the illuminance of light outside the specific wavelength range.
[0012] The form of the filter 3 may be any form that allows the lighting device 1 to illuminate through the filter 3, and may be, for example, a film formed or attached on an LED element. In the case of a fluorescent tube type LED lighting device, the filter 3 may be a film attached to the inner or outer surface of the fluorescent tube.
[0013] For example, a dielectric multilayer film can be used as the above-mentioned filter 3. When the photosensitive substance is IRDye (registered trademark) 700DX (manufactured by Rakuten Medical, Inc.), a dielectric multilayer film (TECHSPEC (registered trademark) OD2.0 Short Pass Filter, manufactured by Edmund Optics) with a transmission blocking band of 675 to 850 nm is used as the filter 3, and the illuminance of light in the wavelength range of at least 650 nm to 750 nm can be reduced by 99% or more by adjusting the angle between the filter 3 and the light source 2.
[0014] The light source 2 is not particularly limited, and may be, for example, a commercially available white LED, halogen lamp, xenon lamp, or the like, which is composed of a continuous arrangement of at least three LED elements (blue, green, and red) and can be recognized as white in the human visible range.
[0015] 1 and 2, the filter 3 may be placed in front of the light source 2 while being held by a filter holder 4. It is preferable that the filter holder 4 does not transmit light in a specific wavelength range to which the photosensitive substance reacts, or has an extremely low transmittance, and is preferably made of, for example, metal or white-colored ABS resin.
[0016] With reference to FIG. 3 and FIG. 4 (cross section 4-4 in FIG. 3), the filter holder 4 according to the embodiment will be described. The filter holder 4 includes a tube 41 and a frame 42. The tube 41 has a substantially cylindrical shape with a through hole having a diameter smaller than that of the filter 3. The tube 41 has a step portion formed inside the substantially cylindrical shape for holding the filter 3. The frame 42 has a through hole having a diameter smaller than that of the filter 3. The filter holder 4 holds the filter 3 installed on the step portion of the tube 41 by fixing the tube 41 and the frame 42 with a screw. Such a filter holder 4 can suppress leakage of light that does not pass through the filter 3 from between the light source 2 and the filter 3.
[0017] The lighting device 1 may further include a hood 5 that surrounds the light source 2, the filter 3, and the filter holder 4. The hood 5 can limit or adjust the direction of light (hereinafter, referred to as illumination light) emitted from the lighting device.
[0018] The lighting device 1 may further include a flexible support portion 6. The support portion 6 can hold the light source 2, the filter 3, the filter holder 4, and the hood 5 in predetermined positions.
[0019] The lighting device 1 may further include a clip 7 at one end of the support portion 6. The clip 7 allows the lighting device 1 to be attached to any desired location.
[0020] The lighting device illuminates light through the above filter, thereby reducing the illuminance of light in a specific wavelength range to which the photosensitive substance reacts from the light source. It is also preferable that the lighting device does not reduce the illuminance of light other than the specific wavelength range to which the photosensitive substance reacts. This makes it possible to suppress the reaction of the photosensitive substance with the illumination light, and also to suppress a decrease in the illuminance of the room in which the lighting device is installed and the illuminance illuminating the affected area.
[0021] It is preferable that the lighting device reduces the light energy of the light in a specific wavelength range to which the photosensitive substance reacts from the light generated from the light source by illuminating through the above-mentioned filter. It is also preferable that the lighting device maintains the light energy of other wavelength ranges without reducing it. For example, when the photosensitive substance is IRDye (registered trademark) 700DX (manufactured by Rakuten Medical, Inc.), it is preferable that the ratio of the light energy when passing through the filter to the light energy when not passing through the filter is 0.1 or less in the wavelength range of 650 nm to 750 nm, and 0.9 or more in the wavelength range of 380 nm to 600 nm. This makes it possible to suppress the reaction of the photosensitive substance with the illumination light, and to suppress the decrease in the illuminance in the room in which the lighting device is installed and the illuminance illuminating the affected area.
[0022] It is preferable that the lighting device does not reduce the light energy in the wavelength range of 600 nm to less than 650 nm, or reduces the light energy only slightly, thereby preventing the illumination light from becoming bluish-white and providing illumination light with a color close to that of natural light or white LED lighting.
[0023] The light energy of each wavelength range is obtained by measuring the light energy of each wavelength of the illumination light using a spectrometer and calculating the integral value of the light energy for each wavelength range. The light energy can be measured using, for example, a compact spectrometer CCS175 and a cosine corrector CCSA1 manufactured by Thorlabs.
[0024] The lighting device according to the embodiment can be used as a lighting device for medical treatment. The lighting device for medical treatment can be, for example, lighting used in a room where a patient who has been administered a drug containing a complex composed of a photosensitive substance that reacts to a specific light and a component that selectively accumulates in a specific cell is staying, and in a room where preparations are made to administer the drug, such as a reading lamp, a desk lamp, direct lighting and indirect lighting installed on the ceiling or wall of the room, lighting used in an operating room, such as an operating lamp and a shadowless lamp, and a lighting unit of a medical device, such as an endoscope, that has a lighting unit that illuminates an affected area. The lighting device according to the embodiment can also be used as a lighting unit used in a manufacturing room where the drug is manufactured.
[0025] The room in which a patient who has been administered a drug is staying refers to any room that a patient enters, such as a hospital room, an operating room, a treatment room, a waiting room, and the corridors connecting these rooms.
[0026] Examples of rooms where preparation for administering medication is carried out include pharmacy departments, wards, nurse stations, and treatment rooms where preparation for administering the above-mentioned medications, such as mixing medications, is carried out.
[0027] The drug is used in a two-step treatment consisting of, for example, administering to a patient a drug containing a complex composed of the above-mentioned photosensitive substance and a component that selectively accumulates in specific cells, and irradiating the specific cells with specific light to which the photosensitive substance reacts. The above treatment is performed, for example, by administering the above-mentioned drug to a patient, allowing it to selectively accumulate in specific cells, and then irradiating the specific cells with light to which the photosensitive substance reacts, thereby activating the photosensitive substance and causing necrosis or elimination of the specific cells.
[0028] The specific cells include cancer cells, etc. Examples of cancer cells include head and neck cancer cells, etc. Examples of head and neck cancer include pharyngeal cancer such as nasopharyngeal cancer, oropharyngeal cancer, and hypopharyngeal cancer, laryngeal cancer such as glottic cancer, supraglottic cancer, and subglottic cancer, nasal and paranasal sinus cancer such as maxillary sinus cancer, oral cancer such as tongue cancer, salivary gland cancer, and thyroid cancer.
[0029] An antibody or the like can be used as a component that selectively accumulates in specific cells. For example, cetuximab, which is a chimeric anti-human epidermal growth factor receptor (EGFR) monoclonal antibody (IgG1), can be used.
[0030] The complex may be a compound in which a photosensitive substance is bound to a component that selectively accumulates in a specific cell. As such a complex, cetuximab sarcotalocan sodium, a compound in which IRDye (registered trademark) 700DX (manufactured by Rakuten Medical, Inc.) is bound to cetuximab, a chimeric anti-human epidermal growth factor receptor (EGFR) monoclonal antibody (IgG1), may be used.
[0031] The drug can be any drug that contains the above complex without any particular limitations. For example, Acallux (registered trademark) intravenous infusion 250 mg (manufactured by Rakuten Medical, Inc.), which contains cetuximab sarlotalcohan sodium, can be used.
[0032] The method of administering the drug is not particularly limited, and the drug can be administered, for example, by intravenous drip injection.
[0033] The photosensitive substance contained in the above-mentioned drug may react with light other than the therapeutic light, such as sunlight and light from lighting, and the composition of the drug may change. In order to prevent the photosensitive substance from reacting with light other than the therapeutic light, the illuminance is controlled to a certain value or less in the room where the patient who has been administered the drug enters, the room where the drug is prepared for administration, and the manufacturing room where the drug is manufactured. However, the room where the brightness is controlled in this way is extremely dark, which may interfere with the patient's life and reduce the quality of life (QOL) or reduce the work efficiency of medical staff. In addition, there is a risk of reducing the work efficiency in the manufacture of the above-mentioned drug.
[0034] The lighting device according to the embodiment used as lighting for medical treatment illuminates through a filter that reduces the illuminance of light of a specific wavelength to which a photosensitive substance reacts, and therefore can suppress the reaction of the drug and the reduction in illuminance. Therefore, the lighting device according to the embodiment can suppress the reduction in the QOL of patients who have been administered the drug, and can suppress the reduction in the work efficiency of medical staff.
[0035] The lighting device according to the embodiment is used in a room where a patient who has been administered a drug containing a complex composed of a photosensitive substance and a component that selectively accumulates in specific cells is staying, and illuminates through a filter that reduces the illuminance of light of a specific wavelength to which the photosensitive substance reacts, so that it is possible to suppress the reaction of the drug administered to the patient and to suppress a decrease in the illuminance in the room. Therefore, the lighting device according to the embodiment can suppress a decrease in the QOL of the patient who has been administered the drug, and also suppress a decrease in the work efficiency of medical staff.
[0036] The lighting device according to the embodiment can also be used in a room where a drug containing a complex composed of a photosensitive substance and a component that selectively accumulates in a specific cell is prepared for administration. The lighting device according to the embodiment can suppress the drug from reacting and can suppress a decrease in the illuminance in the room. Therefore, it is possible to prevent the composition of the drug from changing when preparing to administer the drug and to suppress a decrease in the work efficiency of medical personnel.
[0037] The lighting device according to the embodiment can also be used in a manufacturing room for manufacturing a drug containing a complex composed of a photosensitive substance that reacts to a specific light and a component that selectively accumulates in a specific cell. The lighting device according to the embodiment can suppress the reaction of the drug and suppress a decrease in illuminance in the manufacturing room. Therefore, it is possible to prevent changes in the composition of the drug during drug manufacturing and suppress a decrease in work efficiency.
[0038] The illumination device according to the embodiment can also be used as an illumination unit of a medical device having an illumination unit that illuminates an affected area, such as an endoscope. Hereinafter, an illumination device used as an illumination unit of an endoscope will be described with reference to Fig. 5. Descriptions of configurations similar to those of the above embodiment will be omitted by citing the above description.
[0039] The illumination device 101 according to the embodiment illuminates through a filter 103 that reduces the illuminance of light in a specific wavelength range to which the photosensitive substance reacts. The illumination device 101 includes, for example, a light source 102, a condenser lens 108 that condenses light from the light source 102, a light guide 109 that guides the light condensed by the condenser lens 108 to the distal end of the endoscope, and an illumination lens 110 that illuminates the light guided by the light guide 109 from the distal end of the endoscope. For example, a halogen lamp or a xenon lamp can be used as the light source 102. For example, a bundle of optical fibers can be used as the light guide 109. In addition, the illumination device 101 can include a known configuration as an illumination unit of an endoscope without any particular limitation.
[0040] The filter 103 may be placed between the light source 102 and the condenser lens 108, or may be placed between the condenser lens 108 and the light guide 109. FIG. 5 shows the filter 103 placed between the light source 102 and the condenser lens 108. Although not shown, the filter 103 may be placed inside or outside the illumination lens 110. Although not shown, the filter 103 may be attached to the light source 102. The illumination device 101 having such a configuration can illuminate the affected area of the patient and its surroundings with light from the light source 102 via the filter 103.
[0041] This type of lighting device illuminates through a filter that reduces the illuminance of light of a specific wavelength to which the photosensitive substance reacts, thereby preventing the above-mentioned drug from reacting and preventing a decrease in illuminance at the illuminated affected area of the patient, thereby preventing a decrease in the work efficiency of medical staff.
[0042] The lighting device according to the embodiment described above illuminates through a filter that reduces the illuminance of light in a specific wavelength range to which photosensitive substances react, and therefore can suppress a reaction of the photosensitive substance and can also suppress a decrease in the illuminance in the room in which the lighting device is installed and in the illuminance illuminating the affected area.
[0043] <Example> The present invention will be described in more detail below by way of examples, but is not limited to these examples.
[0044] Example 1 (Measurement of the rate of reduction in illuminance due to a filter) A dielectric multilayer film with a transmission blocking band of 675 to 850 nm (TECHSPEC® OD2.0 Short Pass Filter, Edmund Optics) was prepared as a filter. The diffuser of the BioBlade® laser system (Rakuten Medical) was used to measure the irradiance of 150 mW / cm. 2The sample was irradiated with a laser beam of 690 nm wavelength. The illuminance when irradiated through a filter and when irradiated without a filter was measured with an illuminometer (RQ-881E, manufactured by WapoRich). The reduction rate of the illuminance of the light of 690 nm wavelength due to the filter was calculated using the following formula (1). The results are shown in Table 1. Illuminance reduction rate (%) = (1 - (illuminance when irradiated through a filter / illuminance when irradiated without a filter)) × 100 (1)
[0045] [Table 1]
[0046] The photosensitizer IRDye® 700DX (manufactured by Rakuten Medical, Inc.) of Acallux® Intravenous Infusion 250 mg (manufactured by Rakuten Medical, Inc.), which is a drug containing a complex of a photosensitizer and a component that selectively accumulates in specific cells, reacts to light in the wavelength range of 650 nm to 750 nm. The maximum absorption wavelength of the drug is 690 nm.
[0047] As is clear from Table 1, by irradiating light through the filter of Example 1, the illuminance of light with a wavelength of 690 nm, which is the maximum absorption wavelength of the photosensitive substance IRDye (registered trademark) 700DX (manufactured by Rakuten Medical, Inc.), was reduced by more than 99%.
[0048] Example 2 (Preparing the lighting equipment) An LED clip light (YP200CL, manufactured by Nitori), a filter (TECHSPEC® OD2.0 short pass filter, manufactured by Edmund Optics), and a filter holder were prepared. The filter holder was the same as the filter holder 4 described above, which has a cylindrical body 41 and a frame 42 shown in Figs. 4 and 5. The material of the filter holder was a white-colored ABS material. The filter holder holding the filter was installed in front of the LED light source so that the light generated from the LED light source of the LED clip light would pass through the filter. The lighting device thus prepared was the lighting device of Example 2. In addition, an LED clip light (YP200CL, manufactured by Nitori) without a filter and a filter holder was prepared as a lighting device of a comparative example.
[0049] (Measurement of illuminance) The total illuminance (illuminance in the entire wavelength range that the illuminometer can detect) of the lighting device of Example 2 was measured using an illuminometer (RQ-881E, manufactured by WapoRich). The total illuminance was measured directly under the hood of the lighting device when the lighting device was irradiated from the ceiling, and at a position 15 cm below the hood. Illuminance was also measured in the same manner for the lighting device of the comparative example. The rate of decrease in illuminance was calculated using the following formula (2). The results are shown in Table 2. Reduction rate of illuminance (%)=(1-(illuminance of Example 2 / illuminance of Comparative Example))×100 (2)
[0050] [Table 2]
[0051] As is clear from Table 2, even when the lighting device of Example 2 was passed through a filter that reduces the illuminance of light in the specific wavelength range to which the photosensitive substance IRDye (registered trademark) 700DX (manufactured by Rakuten Medical, Inc.) reacts by 99% or more, the total illuminance was reduced by only 15% or less at both positions directly below the hood and 15 cm below the hood when the lighting device was illuminated from the ceiling, compared to the lighting device of the comparative example that did not use a filter.
[0052] (Measurement of light energy) The light energy of the illumination light of the illumination device of Example 2 was measured for each wavelength using a spectrometer (compact spectrometer CCS175 and cosine corrector CCSA1, manufactured by Thorlabs), and the light energy was plotted against the wavelength. The light energy of the illumination light of the illumination device of the comparative example was also measured in the same manner, and the light energy was plotted against the wavelength. The ratio of the light energy of the comparative example to the light energy of Example 2 for each wavelength, that is, the ratio of the light energy of the filter to the light energy of the case without a filter, was calculated and plotted. The results are shown in FIG. 6. The integral values of the light energy of Example 2 and the comparative example in the wavelength ranges of 650 nm to 750 nm and 380 nm to 600 nm were also calculated, and the ratio of the light energy of the filter to the light energy of the case without a filter in each wavelength range was calculated, and the ratio was 0.1 or less in the wavelength range of 650 nm to 750 nm, and 0.9 or more in the wavelength range of 380 nm to 600 nm.
[0053] In the illumination device of Example 2, the ratio of the light energy when passing through a filter to the light energy when not passing through a filter in each wavelength range is 0.1 or less in the wavelength range of 650 nm to 750 nm to which the photosensitizer IRDye (registered trademark) 700DX (manufactured by Rakuten Medical, Inc.) reacts, and is 0.9 or more in the wavelength range of 380 nm to 600 nm. Therefore, the illumination device of Example 2 significantly reduces the light energy in the wavelength range to which the photosensitizer reacts, while the light energy in other wavelength ranges is hardly reduced. In addition, as is clear from FIG. 6, the illumination device of Example 2 was able to provide illumination light with a color similar to natural light and white LED with reduced bluish whiteness without reducing the light energy in the wavelength range of 600 nm to 650 nm.
[0054] (Patient QOL assessment) On the first day, the patient was administered 250 mg of Acallux (registered trademark) intravenous infusion (manufactured by Rakuten Medical, Inc.) by intravenous infusion. After that, the patient was admitted to a hospital room where the illuminance was controlled to 120 lux or less. The lighting device of Example 2 was provided to the patient and allowed to use it freely. On the second day, the patient was treated with a 690 nm wavelength laser light to which Acallux (registered trademark) reacts, using a BioBlade (registered trademark) laser system. After the treatment, the patient was allowed to re-enter the hospital room. After the patient re-entered the hospital room, the patient was allowed to use the lighting device of Example 2 freely from the intravenous infusion to the fifth day. Then, the patient was observed on the third, fourth, and fifth days after the intravenous infusion. After the fifth day of observation, the lighting device of Example 2 was collected. On the sixth day, the patient was observed. After the observation, the first questionnaire was given to the patient about the comfort of living in the hospital room. After the questionnaire, the lighting device of Example 2 was provided again and allowed to be used freely. On the seventh day, the second questionnaire was given to the patients regarding the comfort of their hospital room.
[0055] The questionnaire was conducted using a 100 mm visual analog scale (VAS). The left end of the VAS line was set to 0 mm, and the right end was set to 100 mm. For the following evaluation items (1) to (10), 0 mm and 100 mm were defined as follows. For each evaluation item, the patient was asked to indicate the position on the corresponding line, and the distance (mm) from 0 mm of each position was measured. While using the lighting device of Example 2, the patient did not use a mobile phone or a personal computer, and therefore did not respond to evaluation item (6) of the second questionnaire.

[0057] The results of the survey are shown in Table 3 below and Figure 7.
[0058] [Table 3]
[0059] As is clear from the evaluation items (1) to (5) and (7) to (10) in Table 3 and Fig. 7, the presence of the lighting device of Example 2 suppressed the decrease in the patient's comfort compared to the absence of the lighting device. In this way, the lighting device of Example 2 was able to suppress the decrease in the patient's QOL.
[0060] (Evaluation of medical worker work efficiency) A questionnaire was given to five medical staff, Nurses A to D and Doctor E, regarding the work efficiency when the lighting device of Example 2 was used and when the lighting device of Example 2 was not used. Nurses A to D and Doctor E were those who performed work in the above-mentioned hospital room in the above-mentioned QOL evaluation. The questionnaire was given using a 100 mm visual analog scale (VAS) in the same manner as the above-mentioned QOL evaluation. For the following evaluation items (1) to (4), 0 mm and 100 mm were defined as follows. For each evaluation item, each medical staff member was asked to answer the position on the corresponding line, and the distance (mm) from 0 mm of each position was measured. In the evaluation, Nurses A and B did not read the characters on the medical records, etc., so Nurses A and B did not answer evaluation item (3).
[0061] <Evaluation items> (1) Compared to normal hospital rooms, was the room bright enough for interacting with patients, such as examining the surgical site? 0mm: It was very dark. 100mm: It was bright enough.
