[0010] The endoscope treatment system 300 according to one embodiment of the present invention is described with reference to the accompanying drawings. In addition, in the following drawings, in order to make each component easier to see, the size ratio may be different according to the component. In addition, in the present invention, the term "equal" includes the situation of being substantially equal. Figure 1 1 is an overall diagram showing an endoscope treatment system 300 according to the present embodiment.

[0011] [Endoscopic treatment system 300] like Figure 1 As shown in FIG. 1 , the endoscope treatment system 300 includes an endoscope treatment instrument 100 and an endoscope 200. The endoscope treatment instrument 100 is used by inserting the endoscope 200.

[0012] [Endoscope 200] The endoscope 200 is a well-known flexible endoscope, and includes an insertion portion 210 inserted into the body from the distal end, an operation portion 220 attached to the proximal end of the insertion portion 210 , and a universal cable 230 attached to the operation portion 220 .

[0013] The insertion part 210 is a long and thin member that can be inserted into the cavity. The insertion part 210 has a front end 211, a curved part 214 and a soft part 215. The front end 211, the curved part 214 and the soft part 215 are connected in sequence from the front end side. Inside the insertion part 210, a channel 216 for inserting the treatment instrument 100 is provided. The front end 211 is provided with a front end opening 212 of the channel 216 and a camera part 213.

[0014] The imaging unit 213 includes an imaging element such as a CCD or a CMOS, and can image a portion to be treated. The bending portion 214 bends in response to a user's operation of the operating unit 220. The soft portion 215 is a flexible tubular portion.

[0015] The operation part 220 is connected to the soft part 215. The operation part 220 has a grip part 221, an input part 222 and a forceps opening 223. The grip part 221 is a member supported by the user. The input part 222 receives an operation input for bending the bending part 214. The forceps opening 223 is a base end opening of the channel 216.

[0016] The universal cable 230 connects the endoscope 200 and external equipment. An imaging cable, an optical fiber cable, or the like that outputs an imaging signal captured by the imaging unit 213 to the outside is inserted into the universal cable 230 .

[0017] [Endoscope treatment instrument 100] Figure 2 It is an overall view showing the endoscopic treatment instrument 100 . The endoscope treatment instrument 100 (also referred to as the treatment instrument 100) includes a sheath 1, an operation wire 2 (see Figure 7 ), a support member (cover member) 3, a forceps (jaw member) 5 and a wire operating part 8. In the following description, as Figure 2 As shown, in the length direction A along the central axis of the treatment instrument 100, the side inserted into the patient's body is called "front end side (distal side) A1", and the wire operation part 8 side is called "base end side (proximal side) A2".

[0018] Figure 3 It is a perspective view of the front end portion of the treatment instrument 100 in a closed state. Figure 4 FIG. 2 is a perspective view of the front end of the treatment instrument 100 in an open state. Figure 3 As shown, a support member 3 and forceps 5 are provided at the front end of the treatment instrument 100. The support member 3 serves as a connection mechanism and supports the forceps 5 in an openable and closable manner. The support member 3 and forceps 5 constitute a "treatment section 110" for treating an affected part.

[0019] In the following description, Figure 3 and Figure 4 As shown, the direction in which the forceps 5 is opened and closed is called the "opening and closing direction B" or the "upper and lower direction B". The direction in which the forceps piece 6 is opened is called the "lower side B1" in the opening and closing direction B, and the direction in which the forceps piece 7 is opened is called the "upper side B2" in the opening and closing direction B.

[0020] In addition, the direction perpendicular to the longitudinal direction A and the opening and closing direction B is referred to as the "width direction C" or the "left-right direction C". The direction toward the right when viewing the base end side A2 from the front end side A1 is referred to as the "right side C1" in the width direction C, and the direction toward the left is referred to as the "left side C2" in the width direction C.

[0021] In addition, if Figure 3 As shown in FIG. 1 , a plane that is horizontal to the longitudinal direction A and the width direction C is referred to as a "horizontal plane HP". Figure 4 As shown in FIG. 1 , a plane horizontal to the longitudinal direction A and the opening and closing direction B is referred to as a “vertical plane VP”.

[0022] The sheath 1 has flexibility, such as Figure 1 and Figure 2As shown in FIG. 1 , the sheath 1 is a long member extending from the front end 1a to the base end 1b. The sheath 1 has an outer diameter that can be inserted into the channel 216 of the endoscope 200. Figure 1 As shown, in a state where the sheath 1 is inserted into the channel 216 , the front end 1 a of the sheath 1 can protrude from the front end opening 212 of the channel 216 and retract.

[0023] The sheath 1 is a coil sheath formed by spirally winding a metal wire. Figure 3 and Figure 4 As shown, the sheath 1 is formed by winding the wire 11 in the circumferential direction D of the longitudinal direction A.

[0024] [Disposal Department 110] Figure 5 It is an exploded perspective view of the treatment section 110 . like Figure 1 As shown, the base end side A2 of the support member 3 is attached to the front end 1 a of the sheath 1 , and the support member 3 supports the forceps 5 at the front end side A1 .

[0025] The support member 3 includes a bracket 31, a forceps opening and closing pin (rotation axis, rotation axis member) 36, forceps piece connecting pins 37a, 37b, a pair of support plates 38A, 38B, a connecting portion 39 and a support plate connecting pin 40. Figure 3 and Figure 4 As shown, the forceps 5 are connected to the bracket 31 so as to be openable and closable by the forceps opening and closing pin 36 mounted on the bracket 31, with the forceps opening and closing pin 36 serving as a rotation axis.

[0026] Figure 6 It is a perspective view of the bracket 31. The bracket 31 is formed into a substantially U-shaped shape by a metal such as stainless steel. The bracket 31 has a support portion body (coil sheath connection portion) 32 formed in a cylindrical shape and a pair of bracket pieces 33 (a first bracket piece 34 and a second bracket piece 35). The central axis O2 in the longitudinal direction A of the support member 3 (the bracket 31) is substantially consistent with the central axis O1 in the longitudinal direction A of the sheath 1.

[0027] In addition, in the present embodiment, the support body 32 is fixed to the front end 1a of the sheath 1 by laser welding. Therefore, the length of the hard portion can be shortened compared to conventional bonding by brazing.

[0028] like Figure 7 As shown, a pair of bracket pieces 33 (a first bracket piece 34 and a second bracket piece 35) are protruded from the support portion body 32 toward the front end side A1. The first bracket piece 34 and the second bracket piece 35 are evenly arranged on both sides of the central axis O2 in the left-right direction C. The first bracket piece 34 and the second bracket piece 35 are symmetrical with respect to the vertical plane VP passing through the central axis O2.

[0029] The first bracket piece 34 is formed in a flat plate shape extending in the longitudinal direction A. The plate thickness direction T1 of the first bracket piece 34 substantially coincides with the left-right direction C. The base end side A2 of the first bracket piece 34 is connected to the support portion main body 32 .

[0030] A semicircular first front end portion 34a is formed at the front end side A1 of the first bracket piece 34. An inclined edge 34b is partially formed outside the first front end portion 34a in the width direction C. The thickness of the first bracket piece 34 in the plate thickness direction T1 is constant in the length direction A. A first through hole 34h penetrating in the plate thickness direction T1 is formed at the front end side A1 of the first bracket piece 34.

[0031] The second bracket piece 35 is formed in a flat plate shape extending in the longitudinal direction A. The plate thickness direction T2 of the second bracket piece 35 substantially coincides with the left-right direction C. The base end side A2 of the second bracket piece 35 is connected to the support portion main body 32 .

[0032] A semicircular second front end portion 35a is formed on the front end side A1 of the second bracket piece 35. An inclined edge 35b is partially formed on the outer side of the second front end portion 35a in the width direction C. The thickness of the second bracket piece 35 in the plate thickness direction T2 is the same as the thickness of the first bracket piece 34 in the plate thickness direction T1. A second through hole 35h penetrating in the plate thickness direction T2 is formed on the front end side A1 of the second bracket piece 35. The center of the second through hole 35h coincides with the center of the first through hole 34h.

[0033] The forceps opening and closing pin (rotation axis, rotation axis member) 36 is formed into a substantially cylindrical shape by metal such as stainless steel. Figure 5 As shown, the forceps opening and closing pin 36 is engaged with the first through hole 34 h of the first bracket piece 34 and the second through hole 35 h of the second bracket piece 35 , and is mounted on the bracket 31 .

[0034] The pair of support plates 38A and 38B are formed into a plate shape by metal such as stainless steel. The pair of support plates 38A and 38B have the same shape and overlap in the plate thickness direction with the upper and lower directions facing each other. The support plates 38A and 38B are respectively formed with a first through hole 38a on the front end side A1 and a second through hole 38b on the base end side A2. The pair of support plates 38A and 38B overlap in a state where the second through holes 38b on the base end side A2 are connected to each other and the first through holes 38a are offset from each other in the up-down direction B (opening and closing direction of the forceps 5).

[0035] The support plate 38A is connected to the forceps piece (first forceps piece) 6 of the forceps 5 via the forceps piece connecting pin 37a inserted into the first through hole 38a. The support plate 38B is connected to the forceps piece (second forceps piece) of the forceps 5 via the forceps piece connecting pin 37b inserted into the first through hole 38a. The support plates 38A and 38B are connected to the connecting portion 39 via the support plate connecting pin 40 inserted into each other's second through holes 38b, and are rotatably supported with the support plate connecting pin 40 as an axis.

[0036] The connecting portion (wire connecting portion) 39 has a pair of connecting pieces 39A located at the front end side A1 and a cylindrical (or cylindrical) tubular body 39B connected to the base end side A2 of the connecting piece 39A. An internal space 39s is formed in the tubular body 39B, the front end side A1 is closed, and the base end side A2 is open. A through hole 39b is formed on the front end side A1 of the tubular body 39B, which is connected to the internal space 39s from the right side C1. The through hole 39b does not penetrate the left side C2 of the tubular body 39B. It should be noted that the tubular body 39B may not form a through hole 39b.

[0037] A pair of connecting pieces 39A are arranged in parallel with a gap in the width direction C. A pair of supporting plates 38A, 38B are inserted between the pair of connecting pieces 39A. A through hole 39a that penetrates the plate thickness direction is formed in the center of each connecting piece 39A. The through holes 39a are concentric circles with each other and are connected to each other in the width direction C. The connecting portion 39 supports the supporting plates 38A, 38B rotatably via the supporting plate connecting pin 40 inserted into each through hole 39a formed in the pair of connecting pieces 39A. It should be noted that in the present embodiment, the pair of connecting pieces 39A, the supporting plate connecting pin 40, and the supporting plates 38A, 38B are not subjected to riveting or other joining processes.

[0038] The cylindrical body 39B of the connection part 39 is inserted into the internal space 32s of the support part body 32 from the front end side A1 of the stent 31, and the pair of connection pieces 39A are locked to the front end surface 32a of the support part body 32. The operation wire 2 described later is connected to the cylindrical body 39B of the connection part 39. In addition, the sheath 1 is connected to the support part body 32.

[0039] The forceps (forceps member) 5 is a member for collecting biological tissue. The forceps 5 is formed of a metal material such as stainless steel, and has a first forceps piece 6 and a second forceps piece 7 that are opposite to each other in the vertical direction B when in a closed state. When the first forceps piece 6 and the second forceps piece 7 are in a closed state, Figure 3 As shown, the pliers 5 are in the shape of a tube.

[0040] The first forceps piece 6 and the second forceps piece 7 are rotatably supported by the forceps opening and closing pin 36 with the rotation axis of the forceps opening and closing pin 36 as the rotation center. In addition, in the present embodiment, the forceps opening and closing pin 36 is connected to the first bracket piece 34 and the second bracket piece 35 by laser welding. Therefore, the sliding property between the forceps opening and closing pin 36 and the first forceps piece 6 and the second forceps piece 7 can be improved. The forceps opening and closing pin 36 can also be connected to only one of the first bracket piece 34 and the second bracket piece 35 by laser welding. If the forceps opening and closing pin 36 is connected to one of the first bracket piece 34 and the second bracket piece 35 by laser welding, the sliding property between the forceps opening and closing pin 36 and the first forceps piece 6 and the second forceps piece 7 can be improved.

[0041] The base end sides A2 of the first forceps piece 6 and the second forceps piece 7 are connected to the support plates 38A and 38B, respectively. Furthermore, the support plates 38A and 38B are connected to the connection part 39 and are operated by the operation wire 2 connected to the connection part 39. It should be noted that one of the first forceps piece 6 and the second forceps piece 7 may be fixed to the support member 3, and only the other may be rotatably supported by the support member 3.

[0042] The first forceps piece 6 is formed, for example, from a flat plate or a bar by mainly cutting or stamping. Figure 5 As shown, the first forceps piece 6 includes a forceps cup (first forceps cup) 61 provided on the front end side A1 in the longitudinal direction A and a first plate 63 provided on the base end side A2 in the longitudinal direction A. As shown in FIG.

[0043] Figure 7 It is a cross-sectional view showing the distal end portion (treatment portion 110 ) of the treatment instrument 100 in a closed state. Figure 8 It is a cross-sectional view showing the distal end portion (treatment portion 110 ) of the treatment instrument 100 in an open state.

[0044] The first forceps cup 61 is formed in a substantially hemispherical shape and opens toward the second forceps piece 7 side (upper side B2) in the opening and closing direction B. Figure 7 and Figure 8 As shown, a plurality of protruding teeth 62 a and recessed portions 62 b are formed on the opening edge 62 of the first forceps cup 61 . The protruding teeth 62 a and recessed portions 62 b are concave and convex in the opening and closing direction B in the forceps 5 in the closed state.

[0045] like Figure 5As shown, the first plate 63 is provided at the base end side A2 of the first clamp cup 61, and is formed into a roughly plate shape. The plate thickness direction of the first plate 63 is roughly consistent with the width direction C. The first plate 63 is inserted into the bracket 31, and is arranged adjacent to the support plate 38A at the left side C2 of the support plate 38A. The first plate 63 is rotatably connected to the support plate 38A by the pin 37a. In addition, in the present embodiment, the first plate 63, the pin 37a and the support plate 38A are connected by riveting. Therefore, sufficient bonding strength can be ensured.

[0046] The first plate 63 has a first portion 63A and a second portion 63B. The first portion 63A is formed on the front end side A1 of the first plate 63. Figure 5 On the right side C1 of the first bracket piece 34 of the bracket 31 shown, the first portion 63A is arranged adjacent to the second forceps piece 7 and is located between the first bracket piece 34 and the second plate 73 of the second forceps piece 7 .

[0047] The second portion 63B is formed on the base end side A2 of the first portion 63A. The second portion 63B is disposed adjacent to the support plate 38A on the right side C1 of the first bracket piece 34 and is located between the first bracket piece 34 and the support plate 38A. The thickness of the first portion 63A in the width direction C is greater than the thickness of the second portion 63B, and the difference between the thickness of the first portion 63A and the second portion 63B is substantially equal to the thickness of the support plate 38A.

[0048] The first plate 63 has a first hole 631 and a second hole 632. The first hole 631 is a through hole that penetrates the first portion 63A in the plate thickness direction. Figure 5 The second hole 632 is a through hole that penetrates the second portion 63B in the plate thickness direction and communicates with the first through hole 34h of the first bracket plate 34. Figure 5 The first through hole 38a of the support plate 38A is shown to be in communication.

[0049] like Figure 5 As shown, the forceps opening and closing pin 36 can be inserted into the first hole 631. In addition, the pin 37a can be inserted into the second hole 632. The first portion 63A of the first forceps piece 6 is rotatably connected to the bracket 31 via the forceps opening and closing pin 36. The second portion 63B of the first forceps piece 6 is rotatably connected to the support plate 38A via the pin 37a.

[0050] The second forceps piece 7 is formed, for example, from a flat plate or a rod by mainly cutting or stamping. Figure 5 As shown, the second forceps piece 7 includes a forceps cup (second forceps cup) 71 provided on the front end side A1 in the longitudinal direction A and a first plate 73 provided on the base end side A2 in the longitudinal direction A. As shown in FIG.

[0051] The second forceps cup 71 is formed in a substantially hemispherical shape and opens toward the first forceps piece 6 side (lower side B1) in the opening and closing direction B. Figure 7 and Figure 8 As shown, a plurality of protruding teeth 72 a and convex portions 72 b are formed on the opening edge 72 of the second forceps cup 71 . The protruding teeth 72 a and convex portions 72 b are concave and convex in the opening and closing direction B in the forceps 5 in the closed state.

[0052] like Figure 5 As shown, the second plate 73 is provided at the base end side A2 of the second forceps cup 71 and is formed in a substantially plate shape. The plate thickness direction of the second plate 73 is substantially consistent with the width direction C. The second plate 73 is inserted into the bracket 31 and is arranged adjacent to the support plate 38B on the right side C1 of the support plate 38B. The second plate 73 is rotatably connected to the support plate 38B by the pin 37b.

[0053] The second plate 73 has a first portion 73A and a second portion 73B. The first portion 73A is formed on the front end side A1 of the second plate 73. Figure 5 On the left side C2 of the second bracket piece 35 of the bracket 31 shown, the first portion 73A is arranged adjacent to the first forceps piece 6 and is located between the second bracket piece 35 and the first plate 63 of the first forceps piece 6 .

[0054] The second portion 73B is formed on the base end side A2 of the first portion 73A. The second portion 73B is disposed adjacent to the support plate 38B on the left side C2 of the second bracket piece 35 and is located between the second bracket piece 35 and the support plate 38B. The thickness of the first portion 73A in the width direction C is greater than the thickness of the second portion 73B, and the difference between the thickness of the first portion 73A and the second portion 73B is substantially equal to the thickness of the support plate 38B.

[0055] The second plate 73 has a first hole 731 and a second hole 732. The first hole 731 is a through hole that penetrates the first portion 73A in the plate thickness direction. Figure 5 The second hole 732 is a through hole that penetrates the second portion 73B in the plate thickness direction and communicates with the second through hole 35h of the second bracket plate 35. Figure 5 The first through hole 38a of the support plate 38B is shown in communication.

[0056] like Figure 5 As shown, the forceps opening and closing pin 36 can be inserted into the first hole 731. In addition, the pin 37b can be inserted into the second hole 732. The first portion 73A of the second forceps piece 7 is rotatably connected to the bracket 31 via the forceps opening and closing pin 36. The second portion 73B of the second forceps piece 7 is rotatably connected to the support plate 38B via the pin 37b.

[0057] like Figure 7As shown, the protruding teeth 62a and the recessed portion 62b of the first forceps piece 6, and the protruding teeth 72a and the convex portion 72b of the second forceps piece 7 have shapes that mesh with each other in the closed forceps 5. In addition, in the closed forceps 5, the first forceps piece 6 and the second forceps piece 7 that mesh with each other form a forceps space 5s.

[0058] The operating wire (wire) 2 is a wire made of metal, such as Figure 7 As shown, the operation wire 2 penetrates the internal space 11s of the sheath 1, wherein the sheath 1 is a coil sheath formed by spirally winding a metal wire. The base end of the operation wire 2 is connected to the wire operation part 8. Figure 7 In the cross section shown, the coil sheath 1 is formed by connecting the wound wires 11 in the length direction A.

[0059] in addition, Figure 5 and Figure 6 The internal space 32s of the support body 32 is connected to the internal space 11s of the sheath 1. The operation wire 2 passes through the internal space 32s of the support body 32. The front end of the operation wire 2 is connected to the connection part (wire connection part) 39. In addition, in the present embodiment, the front end of the operation wire 2 and the connection part 39 are fixed by laser welding. Therefore, compared with the conventional connection by brazing, the length of the hard part can be shortened.

[0060] The forceps 5 has a structure in which the first forceps piece 6 and the second forceps piece 7 rotate around the axis of the forceps opening and closing pin 36. A connecting portion 39 is connected to the base end side A2 of the first forceps piece 6 and the second forceps piece 7 via supporting plates 38A and 38B. By operating the operating wire 2 connected to the connecting portion 39, the first forceps piece 6 and the second forceps piece 7 rotate relatively in the opening and closing direction B, thereby opening and closing the forceps 5.

[0061] The user can open and close the forceps 5 by moving the operating wire 2 forward and backward in the longitudinal direction A. Figure 7 In the closed state of the forceps 5 shown in FIG. 1 , the user advances the operating wire 2 in the longitudinal direction A to open the forceps 5 to the open state. Figure 8 In the forceps 5 shown in the open state, the user moves the operating wire 2 back in the longitudinal direction A to close the forceps to the closed state.

[0062] The user brings the open forceps close to the biological tissue, closes the first forceps piece 6 and the second forceps piece 7 by operating the operating wire 2, and clamps the biological tissue 5 with the forceps 5. At this time, the protruding teeth 62a and the recessed portion 62b of the first forceps piece 6 and the protruding teeth 72a and the convex portion 72b of the second forceps piece 7 are engaged with each other, and the biological tissue can be clamped without sliding. In addition, a part of the clamped biological tissue is accommodated in the forceps space 5s of the forceps 5, so that the action of closing the forceps 5 is suppressed from being hindered by the clamped biological tissue, and the biological tissue can be clamped stably.

[0063] [Line operation unit 8] like Figure 1 and Figure 2 As shown in FIG. 1 , the wire operation part 8 is provided on the proximal end side A2 of the sheath 1 . The wire operation part 8 includes an operation part body 81 , a body cover 82 , and a slider 83 .

[0064] The operation portion body 81 is connected to the proximal end 1b of the sheath 1. Specifically, the proximal end 1b of the sheath 1 is provided in a groove (not shown) formed along the longitudinal direction A of the operation portion body 81 and is connected to the proximal end of the groove.

[0065] The main body cover 82 is detachably provided in the aforementioned groove of the operation portion main body 81. When the main body cover 82 is attached to the operation portion main body 81, the base end 1b of the sheath 1 is fixed to the operation portion main body 81.

[0066] The slider 83 is attached to the operation portion body 81 so as to be movable in the longitudinal direction A. The base end of the operation wire 2 is connected to the slider 83. When the user moves the slider 83 forward and backward relative to the operation portion body 81, the operation wire 2 moves forward and backward.

[0067] like Figure 7 As shown, in the closed state of the treatment part 110, the base end 39c of the connection part (wire connection part) 39 is located at the base end side A2 compared to the base end 32b of the support part body (coil sheath connection part) 32. At this time, the length of the support part body (coil sheath connection part) 32 in the longitudinal direction A can be prevented from becoming longer than necessary, and the length of the hard part in the treatment part 110 can be prevented from becoming longer.

[0068] In addition, Figure 8 In the treatment portion 110 in the open state shown, the base end 39c of the connection portion (wire connection portion) 39 is located at the front end side A1 compared to the base end 32b of the support portion body (coil sheath connection portion) 32, and is located in the internal space of the support portion body (coil sheath connection portion) 32.

[0069] For example, when the length of the hard portion in the treatment portion 110 becomes longer because the support portion main body 32 has a length longer than necessary in the length direction A, the curvature radius of the treatment instrument 100 becomes larger in the curved insertion portion 210 of the endoscope 200, and the channel 216 in which the treatment instrument 100 is inserted and the treatment instrument 100 become easier to contact.

[0070] When the user moves the operating wire 2 forward and backward through the operating wire operating unit 8 to clamp the living tissue and pulls out the treatment instrument 100 clamping the living tissue from the endoscope 200, if the channel 216 contacts the treatment instrument 100, a large force is required to pull out the treatment instrument 100, which reduces operability.

[0071] Since the base end 39c of the connection part (wire connection part) 39 in the treatment part 110 in the closed state is located on the base end side A2 compared to the base end 32b of the support part main body (coil sheath connection part) 32, the length of the hard part in the treatment instrument 100 can be prevented from being lengthened, the channel 216 and the treatment instrument 100 can be prevented from contacting in the curved insertion part 210, and the operability of the treatment instrument 100 can be improved.

[0072] Fig. 9 3 is a cross-sectional view showing a connection portion between the tubular body 39B and the operation wire 2 in the treatment section 110 in a closed state. Fig.10 1 is a cross-sectional view showing the wire material 11 forming the sheath (coil sheath) 1 .

[0073] Here, the direction perpendicular to the longitudinal direction A of the sheath 1 is defined as a “radial direction R.” In the radial direction R, a direction approaching the central axis O1 of the sheath 1 is defined as an “inner side IN,” and a direction away from the central axis O1 is defined as an “outer side OU.”

[0074] The surface (inner surface) 11a of the inner side IN of the wire 11 forming the coil sheath 1 forms the inner circumferential surface of the coil sheath 1. The base end 11e of the inner surface 11a of the wire 11 is closer to the central axis O1 of the coil sheath 1 than the front end 11f of the inner surface 11a. That is, the inner surface 11a of the wire 11 is inclined in such a way that the base end 11e is closer to the central axis O1 than the front end 11f. Here, the base end 11e and the front end 11f of the inner surface 11a of the wire 11 do not refer to the base end and the front end of the coil sheath 1 as a whole, but are as follows. Fig. 9 As shown, it refers to the base end 11 e and the front end 11 f in the width direction of the wire 11 in the cross section of the wire 11 .

[0075] When the forceps 5 in the open state are changed to the closed state, the operation wire 2 and the connection portion (wire connection portion) 39 connected to the front end of the operation wire 2 retreat toward the base end side A2 in the internal space 11s of the coil sheath 1. At this time, if, for example, the base end 39c of the connection portion 39 contacts the corner portion of the front end side A1 of the inner surface 11a of the wire 11, the connection portion 39 will be hooked on the wire 11 and hinder the retreat of the operation wire 2, thereby reducing the operability of the treatment instrument 100.

[0076] exist Fig. 9 In the cross section shown, as described above, the coil sheath 1 is formed by connecting the wire 11 in the length direction A in a manner that the base end 11e of the inner surface 11a is closer to the central axis O1 than the front end 11f, so the base end 39c of the connecting portion 39 retreating toward the base end side A2 will not contact the corner portion of the front end side A1 of the inner surface 11a of the wire 11. For example, when the base end 39c of the connecting portion 39 contacts the inner surface 11a of the wire 11, the base end 39c of the connecting portion 39 contacts the base end 11e of the inner surface 11a of the wire 11. Therefore, the connecting portion 39 can retreat without being hooked on the wire 11, and the operability of the disposal device 100 can be improved.

[0077] In addition, in the surface (outer surface) 11b on the outer side OU of the wire 11 forming the outer peripheral surface of the coil sheath 1, the base end 11d of the outer surface 11b of the wire 11 is closer to the central axis O1 of the coil sheath 1 than the front end 11c of the outer surface 11b. That is, the outer surface 11b of the wire 11 is inclined in such a way that the base end 11d is closer to the central axis O1 than the front end 11c. Here, the base end 11d and the front end 11c of the outer surface 11b of the wire 11 do not refer to the base end and the front end of the coil sheath 1 as a whole, but are Fig. 9 As shown, it refers to the base end 11 d and the front end 11 c in the width direction of the wire 11 in the cross section of the wire 11 .

[0078] Thus, the coil sheath 1 is formed by connecting the wire 11 whose base end is inclined closer to the central axis O1 than the front end in the inner surface 11a and the outer surface 11b in the length direction A. The coil sheath 1 is formed by winding, for example, the wire 11 whose shapes of the front end side A1 and the base end side A2 are symmetrical with respect to the center of the length direction A in a state where the base end is inclined closer to the central axis O1 than the front end.

[0079] In addition, if Fig. 9 As shown, the support body 32 includes a large diameter portion 32c provided at the distal end side A1 and a small diameter portion 32d provided at the proximal end side A2 of the large diameter portion 32c. The diameter of the small diameter portion 32d is smaller than that of the large diameter portion 32c on the outer peripheral surface of the support body 32 which is cylindrical and extends around the central axis O1. Fig. 9As shown, a step portion 32e is provided.

[0080] Here, the distance from the central axis O1 to the front end 11c of the outer surface 11b of the wire 11 is equal to the distance from the central axis O1 to the base end of the small diameter portion 32d. That is, on the outer peripheral surface of the support portion body (coil sheath connection portion) 32 and the coil sheath 1, the base end 11d of the outer surface 11b of the wire 11 is closer to the central axis O1 than the front end 11c of the outer surface 11b, the front end 11c of the outer surface 11b and the small diameter portion 32d are at the same distance from the central axis O1, and the small diameter portion 32d is closer to the central axis O1 than the large diameter portion 32c.

[0081] Therefore, when the treatment instrument 100 is pulled out of the endoscope 200 , the outer peripheral surfaces of the support body 32 and the coil sheath 1 are less likely to be caught on the inner surface of the channel 216 , and the operability of the treatment instrument 100 can be improved.

[0082] It should be noted that the positional relationship between the front end 11c of the outer surface 11b of the wire 11, the large diameter portion 32c, and the small diameter portion 32d is not limited to this. The distance from the central axis O1 to the front end 11c of the outer surface 11b of the wire 11 may be equal to the distance from the central axis O1 to the large diameter portion 32c. In this case, when the treatment instrument 100 is pulled out of the endoscope 200, the outer peripheral surface of the support body 32 and the coil sheath 1 is difficult to be hooked on the inner peripheral surface of the channel 216, which can improve the operability of the treatment instrument 100.

[0083] Alternatively, the front end 11c of the outer surface 11b of the wire 11 may be closer to the central axis O1 than the large diameter portion 32c, and the small diameter portion 32d may be closer to the central axis O1 than the front end 11c of the outer surface 11b of the wire 11. In this case, when the treatment instrument 100 is pulled out of the endoscope 200, the outer peripheral surfaces of the support body 32 and the coil sheath 1 are less likely to be hooked on the inner peripheral surface of the channel 216, and the operability of the treatment instrument 100 can be improved.

[0084] in addition, Fig. 9 The distance G between the cylindrical body 39B of the support body (coil sheath connection part) 32 and the connection part (wire connection part) 39 in the radial direction R is 0.02 mm or more and 0.2 mm or less. It should be noted that at this time, the central axis of the support body 32 and the central axis of the connection part 39 are consistent with the central axis O1 of the coil sheath 1. In addition, when the coil sheath 1 is not bent, the distance between the support body 32 and the inner surface of the coil sheath 1 in the radial direction R is consistent with the aforementioned distance G. Preferably, the distance G is 0.04 mm or more and 0.1 mm or less.

[0085] When the insertion portion 210 is bent, the coil sheath 1 inserted into the channel 216 is also bent. When the coil sheath 1 is bent, the connection portion (wire connection portion) 39 formed by the hard member does not bend like the coil sheath 1, so the connection portion 39 may hinder the bending action of the coil sheath 1.

[0086] By setting the distance G to be not less than 0.02 mm and not more than 0.2 mm, the gap between the outer peripheral surface of the connecting portion 39 and the inner peripheral surface of the supporting portion main body 32 is made sufficiently large, and compared with the case where the gap is insufficient, the interference of the connecting portion 39 with the bending action of the coil sheath 1 can be suppressed, thereby improving the operability of the treatment instrument 100. That is, compared with the case where the gap is insufficient, the actual hard length caused by the connecting portion 39 can be shortened.

[0087] Below, refer to Fig.10 Various dimensions of the cross-sectional shape of the wire material 11 will be described. like Fig.10 As shown, the outer shape of the wire material 11 is formed by an inner surface 11a, an outer surface 11b, a first corner portion 11g, a second corner portion 11h, a third corner portion 11i and a fourth corner portion 11j.

[0088] As described above, the inner surface 11 a is a surface located on the inner side IN close to the central axis O1 in the coil sheath 1 formed by spirally winding the wire 11 .

[0089] As described above, the outer surface 11 b is a surface located on the outer side OU in the radial direction R in the coil sheath 1 formed by spirally winding the wire 11 .

[0090] like Fig.10 As shown, the first corner portion 11g and the second corner portion 11h are surfaces connected to the outer surface 11b. Fig. 9 The front end 11c of the outer surface 11b shown is the boundary portion where the outer surface 11b and the first corner portion 11g contact. Fig. 9 The base end 11d of the outer surface 11b shown is a portion where the outer surface 11b and the second corner portion 11h are in contact.

[0091] like Fig.10 As shown, the third corner portion 11i and the fourth corner portion 11j are surfaces connected to the inner surface 11a. The third corner portion 11i is connected to the first corner portion 11g. The fourth corner portion 11j is connected to the second corner portion 11h.

[0092] The first corner 11g, the second corner 11h, the third corner 11i, and the fourth corner 11j are rounded by R chamfering. The shape of the wire material 11 is formed by rolling or drawing.

[0093] The R dimension (radius) of the inner surface 11a is R0.30 mm or more. The inner surface 11a may include a flat surface.

[0094] The R dimension of the outer surface 11b is R0.3 mm or more. The outer surface 11b may include a flat surface or a concave surface.

[0095] The R dimension of the first corner portion 11g and the second corner portion 11h is R 0.05 mm or more and 0.30 mm or less. The first corner portion 11g and the second corner portion 11h are connected to the outer surface 11b at the boundary portions without sharp edges and are connected smoothly.

[0096] The R dimension of the third corner portion 11i and the fourth corner portion 11j is R 0.05 mm or more and 0.30 mm or less.

[0097] In addition, the thickness T of the wire 11 is not less than 0.15 mm and not more than 0.40 mm. The width W of the wire 11 in the width direction H is not less than 0.20 mm and not more than 0.80 mm, which is not less than 1.33 times and not more than 2.0 times the thickness T. Fig.10 As shown, the thickness T and the width W of the wire 11 are dimensions in directions perpendicular to each other.

[0098] It should be noted that, as described above, the wire 11 forming the coil sheath 1 is wound in an inclined state with the base end closer to the central axis O1 than the tip end on the inner surface 11a and the outer surface 11b. Therefore, the radial direction R of the coil sheath 1 and the direction representing the thickness T of the wire 11 are not necessarily the same. For the sake of convenience, Fig.10 The cross section shown is shown with the radial direction R and the direction of thickness T being consistent.

[0099] According to the present embodiment, the endoscopic treatment instrument 100 comprises: a coil sheath (sheath) 1 formed by spirally winding a wire 11; a coil sheath connection portion (supporting portion body) 32 connected to the front end of the coil sheath 1; a wire (operating wire) 2 passing through the coil sheath 1 and capable of advancing and retreating in the length direction A of the coil sheath 1; a forceps 5 having a first forceps piece 6 and a second forceps piece 7, supported to be openable and closable toward the front end side A1 in the length direction A; and a wire connection portion (connecting portion) 39 connected to the front end of the wire 2 and passing through the coil sheath connection portion 32, so as to open and close the forceps 5.

[0100] Furthermore, when the forceps 5 is in the closed state, the base end 39c of the wire connection portion 39 is located closer to the base end side A2 in the length direction A than the base end 32b of the coil sheath connection portion 32. Therefore, it is possible to suppress the length of the hard portion of the endoscope treatment instrument 100 from increasing, and it is possible to suppress the channel 216 from contacting the endoscope treatment instrument 100 in the curved insertion portion 210. As a result, it is possible to provide the endoscopic treatment instrument 100 with improved operability.

[0101] In addition, on the surface (inner surface 11a) on the inner side IN of the wire 11 forming the inner peripheral surface of the coil sheath 1, the base end 11e of the inner surface 11a is closer to the central axis O1 of the coil sheath 1 than the front end 11f of the inner surface 11a. Therefore, when the operation wire 2 is retracted, the connection portion 39 connected to the front end of the operation wire 2 can be prevented from being hooked on the front end 11f of the inner surface 11a of the wire 11, thereby improving the operability of the endoscope treatment instrument 100.

[0102] In the surface (outer surface 11 b ) of the outer side OU of the wire 11 forming the outer peripheral surface of the coil sheath 1 , the base end 11 d of the outer surface 11 b is closer to the central axis O1 of the coil sheath 1 than the front end 11 c of the outer surface 11 b .

[0103] Furthermore, the coil sheath connection part 32 has a large diameter part 32c and a small diameter part 32d, the large diameter part 32c is provided at the front end side A1 of the coil sheath connection part 32, and the small diameter part 32d is provided at the base end side A2 of the large diameter part 32c, and has a smaller diameter than the large diameter part 32c. The distance from the central axis O1 to the front end 11c of the outer surface 11b is equal to the distance from the central axis O1 to the base end of the small diameter part 32d.

[0104] Therefore, when the endoscope treatment instrument 100 is pulled out from the endoscope 200 , the outer peripheral surfaces of the coil sheath connection portion 32 and the coil sheath 1 are less likely to be caught on the inner peripheral surface of the channel 216 , thereby improving the operability of the endoscope treatment instrument 100 .

[0105] In addition, the distance G (gap) between the coil sheath connection part 32 and the wire connection part 39 in the radial direction R is not less than 0.02 mm and not more than 0.2 mm. Therefore, the gap between the outer peripheral surface of the wire connection part 39 and the inner peripheral surface of the coil sheath connection part 32 can be made sufficiently large, and compared with the case where the gap is insufficient, the wire connection part 39 can be suppressed from hindering the bending action of the coil sheath 1, thereby improving the operability of the endoscope treatment instrument 100.

[0106] Thus, in the endoscope treatment instrument 100 of this embodiment, the coil sheath 1 and the channel 216 can slide smoothly without being caught, and the sliding resistance can be reduced. As a result, the force (insertion force) when inserting the endoscope treatment instrument 100 into the channel 216, the force (pulling force) when pulling the biological tissue with the forceps 5, and the force (pulling force) when pulling the endoscope treatment instrument 100 out of the channel 216 can be reduced, thereby improving the operability of the endoscope treatment instrument 100.

[0107] The first embodiment of the present invention has been described in detail with reference to the drawings, but the specific structure is not limited to the embodiment and includes design changes within the scope of the present invention. In addition, the structural elements shown in the above embodiment and modified examples can be combined as appropriate.

[0108] (Variant 1) The shape and structure of the wire operation part 8 are not limited to Figure 1 and Figure 2 The form shown. Fig.11 1 is a perspective view showing a wire operating unit 8A according to Modification 1. Fig.12 1 is a cross-sectional view showing the wire operating unit 8 in Modification 1. In the following description, the same reference numerals are given to the same configurations as those already described, and duplicate descriptions are omitted.

[0109] The wire operation part 8A includes an operation part body 81A, a body cover 82, and a slider 83A. Fig.12 As shown, the base end 1b of the sheath 1 is fixed to the operation portion main body 81A. In addition, the main body cover 82 is detachably provided on the operation portion main body 81A.

[0110] Similar to the wire operating section 8 of the aforementioned embodiment, the slider 83A is mounted on the operating section body 81A so as to be movable along the longitudinal direction A. The wire operating section 8A is an operating section that advances and retreats the operating wire 2 connected to the slider 83A by advancing and retreating the slider 83A relative to the operating section body 81A.

[0111] The operation portion main body 81A includes a main body portion 81 a , a first grip portion 81 b , a first grip expansion portion 81 c , and a second grip expansion portion 81 d .

[0112] The main body 81a is a substantially cylindrical member extending along the central axis O3 of the wire operating portion 8A. The central axis O3 of the wire operating portion 8A coincides with the longitudinal direction A.

[0113] The first gripping portion 81b is provided at the base end of the main body 81a and is substantially U-shaped and opens toward the front end side A1. Both ends of the base end side A2 of the first gripping portion 81b are connected to the base end of the main body 81a. A gripping space 81h that penetrates in the direction along the axis O4 is formed in the space surrounded by the base end of the main body 81a and the first gripping portion 81b. The gripping space 81h is a space where the user of the wire operating portion 8A mainly places his thumb.

[0114] In addition, a first gripping expansion portion 81c and a second gripping expansion portion 81d extending in the direction along the axis O4 are provided at the base end of the main body 81a. The first gripping expansion portion 81c and the second gripping expansion portion 81d protrude from the base end of the main body 81a in the direction away from the central axis O3 in the direction along the axis O4. The first gripping expansion portion 81c and the second gripping expansion portion 81d protrude in directions opposite to each other.

[0115] The surface on the proximal end side A2 of the first grip expansion portion 81c and the surface on the proximal end side A2 of the second grip expansion portion 81d are smoothly connected to form a first grip surface 81s. The first grip surface 81s is the surface that the user of the wire operation portion 8A mainly contacts with the thumb.

[0116] The slider 83A includes a slider body 83a, a second gripping portion 83b, and a third gripping portion 83c. The slider body 83a is a portion that the user of the wire operating unit 8A operates mainly with the index finger and the middle finger.

[0117] The slider body 83a is substantially cylindrical and extends from the central axis O3. The second gripping portion 83b is connected to the base end of the slider body 83a and protrudes in a direction away from the central axis O3 compared to the outer shape of the slider body 83a. Fig.11 As shown, the second gripping portion 83b protrudes in a direction perpendicular to the central axis O3 and the axis O4, and a second gripping surface 83s is formed on the front end side A1 of the second gripping portion 83b.

[0118] The second gripping surface 83s is a surface that the user of the wire operating unit 8A mainly contacts with the fingertips of the index and middle fingers when moving the slider 83A toward the base end side A2 relative to the operating unit body 81A. At this time, the user places the index and middle fingers so that they hold the slider body 83a.

[0119] The third gripping portion 83c is connected to the front end of the slider body 83a, and protrudes in a direction away from the central axis O3 compared to the outer shape of the slider body 83a. The third gripping portion 83c is a surface that the user of the wire operating portion 8A mainly contacts with the index finger and the middle finger when the user moves the slider 83A toward the front end side A1 relative to the operating portion body 81A. At this time, the user contacts the nail or the part near the nail on the opposite side of the finger pulp of the index finger and the middle finger with the surface of the base end side A2 of the third gripping portion 83c.

[0120] Next, the dimensions of the wire operating portion 8A will be described. Fig.11The dimension L1 shown is the dimension from the outer peripheral surface of the slider body 83a to the outer periphery of the second gripping portion 83b in the second gripping surface 83s. If the connecting portion between the slider body 83a and the second gripping surface 83s and the outer periphery of the second gripping surface 83s are chamfered or R-processed, the dimension L1 is not limited thereto, and the dimension L1 may be the dimension (width) of the portion that the user of the wire operating portion 8A can contact with the index and middle fingers.

[0121] The dimension L1 is not less than 10 mm and not more than 12 mm. The dimension L1 is preferably 11.7 mm.

[0122] Fig.12 The dimension L2 shown is the dimension of the first grip surface 81s in the direction along the axis O4. Specifically, it is the dimension obtained by adding the dimension of the first grip extension 81c in the direction along the axis O4 and the dimension of the second grip extension 81d in the direction along the axis O4. In the case where the outer edges of the first grip extension 81c and the second grip extension 81d are chamfered or R-processed, the dimension L2 is not limited to this, and the dimension L2 can be the dimension (width) of the portion that the user of the wire operation unit 8A can contact with the thumb.

[0123] The dimension L2 is not less than 22 mm and not more than 24 mm. The dimension L2 is preferably 23.9 mm.

[0124] Fig.12 The dimension L3 shown is a dimension indicating the distance from the first grip surface 81s to the second grip surface 83s in the longitudinal direction A. For example, the dimension L3 is the distance from the tip of the thumb to the tip of the index finger and the tip of the middle finger in the user's right hand when the user operates the wire operation portion 8A with the right hand to move the slider 83A toward the base end side A2 relative to the operation portion body 81A.

[0125] The dimension L3 is 40 mm or more. By setting the dimension L3 to 40 mm or more, when the user operates the wire operating portion 8A, it is easy to apply force to the wire operating portion 8A, so that the operability can be improved. By setting the dimensions L1 , L2 , and L3 as described above, it is possible to increase the gripping force of the wire operating portion 8A, and increase the amount of living tissue collected by the forceps 5 .

[0126] Fig.12 The dimension L4 shown is the amount of movement (stroke) that the slider 83A can move relative to the operating portion body 81A toward the base end side A2 when the open forceps 5 are operated to the closed state. When the open forceps 5 are operated to the closed state, the slider 83A is moved relative to the operating portion body 81A toward the base end side A2. At this time, the base end of the slider 83A is in contact with the base end side A2 of the operating portion body 81A. Fig.11 and Fig.12The limiting surface 81w shown abuts.

[0127] The dimension L4 affects the pulling force (wire connection part pulling force) when the wire connection part 39 connected to the front end of the operation wire 2 is retracted. Here, the dimension L4 is set to a dimension that makes the wire connection part pulling force less than 40N. By making the wire connection part pulling force less than 40N, when the operation wire 2 is retracted, the force-bearing components (for example, the wire connection part 39 and the operation wire 2) are prevented from being damaged or broken, and the components are prevented from falling off due to breaking.

[0128] (Variant 2) The shape and structure of the operation part main body 81, 81A are not limited to Figure 1 , Figure 2 , Fig.11 and Fig.12 The form shown. Fig.13 It is a perspective view showing an operation portion main body 81B according to Modification 2.

[0131] like Fig.13 As shown in FIG. 8 , the through hole 811B is penetrated by the first gripping portion 81Bb in an annular shape. In addition, a gripping space 81Bh is formed in a space surrounded by the first gripping portion 81Bb and the first gripping expansion portion 81Bc.

[0132] The first gripping surface 81Bs is formed on the surface of the base end side A2 of the first gripping expansion portion 81Bc. The first gripping surface 81Bs has a flat portion 812B and an inclined portion 813B. The flat portion 812B and the inclined portion 813B are not on the same plane. Fig.13 As shown, the first holding surface 81Bs is arranged in a curved shape.

[0133] The first grip surface 81Bs is a surface that the user contacts with the tip of the thumb. For example, the user contacts the inclined portion 813B with the tip portion before the first thumb joint and contacts the flat portion 812B with the base end (palm side) after the first thumb joint.

[0134] Fig.14 It is a perspective view showing an operation portion main body 81C according to Modification 2.

The front end of the first gripping portion Db is connected to the first gripping surface 81Ds. Preferably, the first gripping portion 81Db is connected to the flat portion 812D of the first gripping surface 81Ds.

[0144] A gripping space 81Dh is formed in a space between the first gripping portion 81Db and the first gripping surface 81Ds.

[0145] The first gripping surface 81Ds is a surface that the user contacts with the thumb pulp. For example, the user contacts the front end portion before the first thumb joint with the inclined portion 813D, and contacts the base end (palm side) portion after the first thumb joint with the flat portion 812D.

[0146] Fig.16 It is a cross-sectional view showing operation portion main bodies 81C and 81D according to Modification 2. Fig.16 The dimension L5 shown is the dimension of the distance between the boundary P1 between the flat portions 812C, 812D and the inclined portions 813C, 813D and the end P2 of the inclined portions 813C, 813D. That is, the dimension L5 is the dimension indicating the length of the inclined portions 813C, 813D.

[0147] Fig.16 The dimension L6 shown is the dimension indicating the distance between the boundary P1 and the end P3 of the flat portions 812C and 812D. That is, the dimension L6 is the dimension indicating the length of the flat portions 812C and 812D. Fig.16 In the cross section shown, the dimension is a dimension along a direction perpendicular to the central axis O3. In addition, the dimension L6 is a dimension along a direction of the axis O4 of the holding spaces 81Ch and 81Dh.

[0148] Here, for example, a dimension L6 indicating the length of the flat portions 812C and 812D is not less than 2.5 times and not more than 5.0 times a dimension L5 indicating the length of the inclined portions 813C and 813D.

[0149] Fig.16 The dimension L7 shown is the dimension indicating the distance between the end P2 of the inclined portion 813C, 813D and the end P3 of the flat portion 812C, 812D. Fig.16 In the cross section shown, the dimension is a dimension along a direction perpendicular to the central axis O3. In addition, the dimension L7 is a dimension along a direction of the axis O4 of the holding spaces 81Ch and 81Dh.

[0150] Fig.16The central axis O5 shown is the central axis of the dimension L6. The central axis O5 extends parallel to the central axis O3.

[0151] Fig.16 The central axis O6 shown is the central axis of the dimension L7. The central axis O6 extends parallel to the central axis O3 and the central axis O5.

[0152] Here, the central axis O3 of the main body portions 81Ca and 81Da is located between the central axis O5 of the dimension L6 and the central axis O6 of the dimension L7.

[0153] in addition, Fig.16 The angle ɑ shown in the figure represents the angle formed by the flat portions 812C and 812D and the inclined portions 813C and 813D. Here, the angle ɑ is not less than 130° and not more than 160°.

[0154] (Variation Example) In the above embodiment, when the forceps 5 is in the closed state, the base end 39c of the wire connection portion 39 is located closer to the base end side A2 than the base end 32b of the coil sheath connection portion 32 in the length direction A. However, the base end 39c of the wire connection portion 39 is not limited to the closed state, and the base end 39c of the wire connection portion 39 may be located closer to the base end side A2 than the base end 32b of the coil sheath connection portion 32 when the forceps 5 is in the open state.

[0155] Fig.17 It is a cross-sectional view showing a modified example of the distal end portion (treatment portion 110 ) of the treatment instrument 100 in the open state. Fig.17 The base end 39Ac of the wire connecting portion 39 of the illustrated modification example is located closer to the base end side A2 than the base end 32b of the coil sheath connecting portion 32 when the forceps 5 is in the open state.

[0156] Even if the base end 39Ac of the wire connection part 39 is located closer to the base end side A2 than the base end 32b of the coil sheath connection part 32 when the forceps 5 is in the open state, the length of the hard part of the treatment instrument 100 can be suppressed from increasing, thereby improving the operability of the treatment instrument 100.
