[0056] In the description of the present disclosure, it should be understood that the terms “center” , “longitudinal” , “transverse” , “length” , “width” , “thickness” , “upper” , “lower” , “front” , “rear” , “left” , “right” ,  “vertical” , “horizontal” , “top” , “bottom” , “inner” , "outer” , “clockwise” , “counterclockwise” , “axial” , “radial” , “circumferential direction” are based on the azimuth or position relationship shown in the attached drawings, which are only for the convenience of describing the present disclosure and simplifying the description, rather than indicating or implying that the device or element must have a specific azimuth, be constructed and operated in a specific azimuth, so such terms cannot be understood as a limitation of the present disclosure.

[0057] In addition, the terms “first” and “second” are only used for descriptive purposes and cannot be understood as indicating or implying relative importance or implicitly indicating the number of indicated technical features. Thus, the features defined with “first” and “second” may explicitly or implicitly include at least one of the features. In the description of the present disclosure, “aplurality of” means at least two, such as two, three, etc., unless otherwise expressly and specifically defined.

[0058] In the present invention, unless otherwise expressly specified and limited, the terms “mount” , “connect” , “contact” , “fix” and other terms should be understood in a broad sense, for example, they can be fixed connections, detachable connections, or integrated. They can be mechanical connection or electrical connection. They can be directly connected or indirectly connected through an intermediate medium. They can be the connection within two elements or the interaction relationship between two elements, unless otherwise expressly limited. For those skilled in the art, the specific meaning of the above terms in the present disclosure can be understood according to the specific situation.

[0059] In the present invention, unless otherwise expressly specified and limited, the first feature “above” or “below” the second feature may be in direct contact with the first and second features, or the first and second features may be in indirect contact through an intermediate medium. Moreover, the first feature is “above” the second feature, but the first feature is directly above or diagonally above the second feature, or it only means that the horizontal height of the first feature is higher than the second feature. The first feature is “below” of the second feature, which can mean that the first feature is directly below or obliquely below the second feature, or simply that the horizontal height of the first feature is less than that of the second feature.

[0060] It should be noted that when an element is called “fixed to” or “provided on” another element, it can be directly on another element or there can be a centered element. When an element is considered to be “connected” to another element, it can be directly connected to another element or there may be intermediate elements at the same time. The terms “vertical” , “horizontal” , “up” , “down” , “left” , “right” and similar expressions used herein are for the purpose of illustration only and do not represent the only embodiment.

[0061] The implantation of many medical devices requires the formation of tunnels through body tissue to place elongated members such as catheters and percutaneous drivelines. For example, the percutaneous driveline must pass through the body tissue to connect an electronic device that have been implanted in the human body for a long time to an external device such as a controller or a power supply outside the body. The external device can provide power and transmit data to the implanted electronic device through the percutaneous driveline. As a common application, an implantable left ventricular assist system (LVAD, or artificial heart) usually includes a percutaneous driveline, one end of the percutaneous driveline is fixed on the LVAD, and the other end of the percutaneous driveline is connected to an external controller and a battery through the patient's body tissue.

[0062] A surgical procedure of creating a tunnel to pass the elongated member through body tissue is generally referred to as tunneling. The tunneling is an invasive procedure that can lead to infection. A surgical  tool used to perform tunneling is called a tunneling tool. The tunneling tool generally includes a driveline cap and a percutaneous tunneling needle, which in turn includes a needle shaft and a handle connected to each other. When performing tunneling, an operator firstly uses the percutaneous tunneling needle to puncture the body tissue to form a tunneling channel, then mounts the driveline cap connected to the percutaneous driveline to a lance in the body, and holds the handle outside the body to continuously adjust the lance, and pulls the driveline cap and percutaneous driveline outward outside the body.

[0063] In the related art, the driveline cap is generally connected to the lance through threads. When connecting, the operator must hold the driveline cap and rotate it relative to the lance to achieve the connection. This connection process is relatively complicated, which not only makes the surgery time longer, but also the metal debris generated by the friction of the thread connection may remain in the body, which increases the risk of the surgery.

[0064] A tunneling tool is provided according to an embodiment of the present disclosure. According to the tunneling tool, a driveline cap and a handle can be directly inserted into a lance. The connection and extraction operations are relatively simple, not prone to errors, and will not produce additional metal debris. The connection is safe and reliable, thus saving surgical time and reducing surgical risks. On the other hand, the structure of the driveline cap is relatively simple and the outer outline size of the driveline cap is small, which can reduce the resistance when being pulled out, thereby reducing trauma of a patient and infection rate.

[0065] FIG. 1 is a schematic view showing a connection between a percutaneous driveline extending through body tissue and an external electrical unit outside a body.

[0066] In order to better understand the present disclosure, it is first necessary to explain a connection relationship between the percutaneous driveline passing through the body tissue and an external electrical unit outside the body. Referring to FIG. 1, the electrical device includes an internal electrical unit 71 (i.e., a ventricular assist device) implanted in a body of the human body or animal and an external electrical unit 72 (i.e., a controller and a power supply) placed outside the body. The internal electrical unit 71 is electrically connected to the external electrical unit 72 by a percutaneous driveline 74 passing through body tissue 73. One end of the percutaneous driveline 74 is fixed to the internal electrical unit 71, and the other end of the percutaneous driveline 74 is electrically connected to a driveline connector 75 connected to the external electrical unit 72. The external electrical unit 72 provides power and control signals to internal electrical unit 71 through the percutaneous driveline 74. The internal electrical unit 71 is attached to an organ 76 in the body.

[0067] A tunneling tool 100 according to the embodiment of the present disclosure is configured to form a tunneling channel in body tissue. When the tunneling tool 100 is connected to the driveline connector 75, the driveline connector 75 and the percutaneous driveline 74 can be pulled out of the body through the tunneling channel.

[0097] When an external force is applied to the second locking member 62, for example, when a human hand presses the second locking member 62, the second locking member 62 overcomes the elastic force of the second spring 67 and is located in the fourth position (not shown) , at this time the second through hole 621 of the second locking member 62 coincides with the third connecting hole 63. The entire second locking member 62 enters the first channel 53 and is disengaged from the engaging portion 101 of the lance 10. At this time, the lance 10 can be taken out from the third connecting hole 63. When the external force is removed, the second locking member 62 will return to the third position under the elastic force of the second spring 67. It should be understood that the third position and the fourth position refer to positions of the second locking member 62 relative to the third connecting hole 63, no matter whether the lance 10 is located in the third connecting hole 63, the second locking member 62 has the third position and the fourth position.

[0098] In the embodiment, the handle 60 further includes the stopping member 68 that extends through the handle body 61 and the second locking member 62 and extends into the first channel 53. The second locking member 62 can move in the second direction S relative to the stopping member 68.

[0099] In the embodiment, referring to FIG. 10, the second locking member 62 is provided with a third through hole 622, and the third through hole 622 is a strip hole extending along the second direction S. In this way, when the stopping member 68 is fixedly connected to the handle body 61, the movement of the second locking member 62 in the second direction S will not be affected. The second locking member 62 may be a button, which is convenient for an operator to press to switch positions.

[0100] In addition, the arrangement of the stopping member 68 can also prevent the second locking member 62 from being disengaged from the first channel 53. The second locking member 62 can be coplanar with or slightly protruded from an outer surface of the handle body 61, so as to facilitate the operator's operation. Furthermore, due to the arrangement of the stopping member 68, the lance 10 cannot rotate relative to the handle body 61.

[0101] In the embodiment, referring to FIG. 9, at an end of the handle body 61 away from the third connecting hole 63 is provided with a fourth connecting hole 64. The unlocking member 80 is detachably provided in the fourth connecting hole 64, so that the operator can easily reach the unlocking member 80 when unlocking the first locking member 50.

[0102] Further, referring to FIG. 7, as mentioned above, the end of the connecting body 30 along the first direction F is provided with the operating hole 36 in communication with the first accommodating cavity A. The unlocking member 80 includes an operating end 81. Adiameter of the operating end 81 is less than an inner diameter of the operating hole 36, so that the operating end 81 can extend into the first accommodating cavity Afrom the operating hole 36.

[0103] In the embodiment, the unlocking member 80 is magnetically attracted and connected to the handle 60. For example, referring to FIG. 9, the handle 60 further includes a first magnetic member 691 adjacent to a bottom wall of the fourth connecting hole 64, and the unlocking member 80 includes a second magnetic member 692 configured to magnetically attract the first magnetic member 691.

[0104] An avoidance groove 693 is further provided on the bottom wall of the fourth connecting hole 64. When the unlocking member 80 is located in the fourth connecting hole 64, the operating end 81 of the unlocking member 80 is accommodated in the avoidance groove 693.

[0105] FIG. 12 is a partial view of the tunneling tool 100 according to an embodiment showing the connection of the driveline cap 20, the driveline connector 75, and the lance 10.

[0106] In the embodiment, referring to FIGS. 2, 3, 4, 7 and 12, the driveline cap 20 further includes a protective housing 40 having a cylindrical shape with openings at both ends thereof. The protective housing 40 is sleeved on the connecting body 30. For example, as shown in FIG. 4, the protective housing 40 is configured to slide in the third direction T relative to the connecting body 30, so as to disengage a part of the protective housing 40 from the main connecting portion 34. The third direction T is a direction from the first connecting hole 31 towards the second connecting hole 32. In this way, when the lance 10 drives the driveline cap 20 to be pulled out of the body through the tunneling channel, the protective housing 40 slides along the third direction T, for example to the position shown in FIG. 12, and covers a part of the driveline connector 75, the driveline connector 75 can be protected to prevent the driveline connector 75 from accidentally falling off.

[0107] In an embodiment, referring to FIGS. 7 and 12, an end of the connecting body 30 in the third direction T is provided with a first protrusion 33, and an end of the protective housing 40 facing the lance 10 is provided with a second protrusion 41 configured to cooperate with the first protrusion 33 to prevent the protective housing 40 from falling off. In this way, the protective housing 40 moves along the third direction T to a position where the second protrusion 41 is blocked by the first protrusion 33, the protective housing 40 cannot move further, thereby preventing the protective housing 40 from falling off.

[0108] In the embodiment, a plug end of the driveline connector 75 is provided with a locking elastic sheet 750, and an end of the locking elastic sheet 750 is provided with a third protrusion 751. When the locking elastic sheet 750 is inserted into the second connecting hole 32, the locking elastic sheet 750 is locked in the connecting groove 752 formed in the third connecting hole 63 to achieve axial locking.

[0109] In addition, a sealing ring 753 is provided between the plug end of the driveline connector 75 and the second connecting hole 32, so as to prevent liquid from entering the second connecting hole 32 and touching a conductive pin core of the driveline connector 75 during the operation. When the driveline connector 75 needs to be pulled out, the locking elastic sheet 750 on the driveline connector 75 is pinched by fingers and pulled outwards. The locking spring sheet 750 retracts inward and is disengaged from the connecting groove 752 to achieve unlocking, so that the driveline connector 75 can be pulled out from the driveline cap 20.

[0110] In the embodiment, referring to FIGS. 2, 8 and 11, the lance 10 includes a middle section 14 and two first sections 15 symmetrically connected to both ends of the middle section 14.

[0111] The first section 15 includes a puncture section 151, a transition section 152, and an auxiliary section 153 connected sequentially along a fourth direction. Adiameter of the auxiliary section 153 is greater than a diameter of the middle section 14 and a diameter of the transition section 152. The fourth direction is a direction from the first section 15 to the middle section 14.

[0112] A cross-section of a portion of axial section of the transition section 152 is polygonal. Polygonal in cross-section means that a portion of the axial section surface of the transition section 152 has continuous adjacent polygonal surfaces. The polygon here may be, for example, a regular hexagon or other regular polygons.

[0113] An end of the puncture section 151 away from the transition section 152 has a tapered tip, which can easily penetrate the skin and promote the lance 10 to penetrate the soft tissue of the body. The annular groove 13 is formed on the transition section 152, so as to form the engaging portion 101 at a junction between the puncture section 151 and the transition section 152.

[0114] In addition, a surface of the auxiliary section 153 is provided with a plurality of blood guiding grooves 1531 that are circumferentially spaced, and each of the blood guiding grooves 1531 extends along a length direction of the lance 10. In this way, when the lance 10 extends through the body tissue, the fluid such as the body fluid can be easily guided.

[0115] In the embodiment, both ends of the auxiliary section 153 have a conical shape. A taper of an end of the auxiliary section 153 connected to the transition section 152 is less than a taper of an end of the auxiliary section 153 connected to the middle section 14. Since the taper of the end of the auxiliary section 153 connected to the transition section 152 is relatively small, when the lance 10 penetrates the body tissue, a diameter of the through hole can be expanded with a smaller push force, so that when the percutaneous driveline 74 is pulled out of the body, the patient will not be greatly traumatized due to excessive pulling resistance. The taper of the end of the auxiliary section 153 connected to the middle section 14 is relatively large, so that in the initial stage of tunneling, the operator can hold the middle section 14 and apply force against the end of the auxiliary section 153 connected to the middle section 14.

[0116] In an embodiment, the lance 10 may be made of metal, synthetic plastic, composite materials, or other materials known in the art that are suitable for surgical instruments. Alength of the lance 10 may be greater than 300mm, 400mm or other lengths, which depends on an area of the patient's body that is being tunneled and the method of tunneling.

[0117] In the lance 10, the middle section 14 has a certain degree of flexibility and can be manually bent into an arc by the user to penetrate the arc tunnel in the body tissue. On the one hand, the length of the tunneling channel can be increased and a contact area between the percutaneous driveline and the muscle tissue can be increased. The larger contact area facilitates the application of anti-infective drugs to a surface of the percutaneous driveline through the blood in the muscle tissue, thereby reducing the infection rate. On the other hand, the lance 10, which can be bent into an arc shape, can specify the positions of an inlet and an outlet of the tunneling channel according to the needs of the operation.

[0118] Referring to FIG. 2 and FIG. 11, it should be noted that the structures of the two first sections 15 are substantially the same and are symmetrically arranged at both ends of the middle section 14. In this way, the handle 60 can be connected to one of the first sections 15, and the driveline cap 20 can be connected to the other one of the first sections 15. It should be understood that, the positions of the handle 60 and the driveline cap 20 can also be exchanged according to actual needs, which facilitates the operator to perform two-way operations, from the inside to the outside, or from the outside to the inside.

[0119] A method of using the tunneling tool 100 according to an embodiment of the present disclosure  will be described below. A tunneling method using the tunneling tool 100 generally includes outside-in operation and inside-out operation.

[0120] The outside-in operation includes the following steps:

[0121] The driveline connector 75 is inserted into the second connecting hole 32 of the driveline cap 20 to connect the driveline connector 75 to the driveline cap 20.

[0122] One of the first sections 15 of the lance 10 is inserted into the third connecting hole 63 of the handle 60 to connect the lance 10 to the handle 60.

[0123] The handle 60 is held for operation, and the lance 10 (the puncture section 151 of the other one of the first sections 15 toward the human body) punctures from the abdomen outside the patient's body into the body, such as into the chest cavity, to form the tunneling channel within the patient's body tissue.

[0124] The first section 15 of the lance 10 in the chest cavity is inserted into the first connecting hole 31 of the driveline cap 20, and the handle 60 is operated to pull the lance 10 outside the body, so that the driveline cap 20, the driveline connector 75, and the percutaneous driveline are pulled out of the human body through the tunneling channel.

[0125] The driveline connector 75 is pulled out from the second connecting hole 32 of the driveline cap 20 and connected to the external electrical unit 72.

[0126] The inside-out operation includes the following steps:

[0127] The driveline connector 75 is inserted into the second connecting hole 32 of the driveline cap 20 to connect the driveline connector 75 to the driveline cap 20.

[0128] One of the first sections 15 of the lance 10 is inserted into the third connecting hole 63 of the handle 60 to connect the lance 10 to the handle 60.

[0129] The handle 60 is held for operation, and the lance 10 (the puncture section 151 of the other first section 15 toward the human body) punctures from inside the patient's body, such as the chest cavity, until it penetrates the patient's abdominal skin and reaches the outside of the body, so as to form the tunneling channel in the patient's body tissue.

[0130] The first section 15 of the lance 10 located in the chest cavity is pull out from the third connecting hole 63 of the handle 60, the first section 15 located in the chest cavity is inserted into the first connecting hole 31 of the driveline cap 20, and the driveline cap 20 is connected to lance 10;

[0131] The first section 15 of the lance 10 located outside the body is insert into the handle 60 to connect the lance 10 to the handle 60.

[0132] The handle 60 is operated to pull the lance 10 toward the outside of the body, and the lance 10 drives the driveline cap 20, the driveline connector 75, and the percutaneous driveline to be pulled out of the human body through the tunneling channel.

[0133] The driveline connector 75 is pulled out from the second connecting hole 32 of the driveline cap 20 and connected to the external electrical unit 72.

[0134] In the description of this specification, reference to the terms "some embodiments, " "other embodiments, " "desirable embodiments, " etc., means that a particular feature, structure, material, or characteristic described in connection with the embodiment or example is included in at least one embodiment or example of the present disclosure. In this specification, schematic descriptions of the above terms do not necessarily refer to the same embodiment or example.
