Description

Technical Field

[0001] The present invention relates to a catheter used as a guide when a stomach is resected using an automatic suturing device or the like, and a catheter set including the catheter and a manual pump.

[0002] This application claims priority based on Japanese Patent Application No. 2019-209221 filed in Japan on November 20, 2019, the contents of which are incorporated herein by reference. Background Art

[0003] Regarding weight loss as a countermeasure for obesity, diabetes, etc., it is known that dietary restriction by surgical operation (laparoscopic sleeve gastrectomy) rather than medical treatment is effective in the long term.

[0004] Specifically, in laparoscopic sleeve gastrectomy, the operator inserts a small-diameter endoscope into the abdominal cavity from around the umbilicus, and removes about 80% of the greater curvature of the stomach while checking the monitor projected by the endoscope, thereby forming a sleeve gastric tube with a volume of about 100 mL.

[0005] In particular, when performing gastric resection, if a catheter having a holder for maintaining the position in the stomach attached to the front end and connected to a suction source is used, the operation can be performed efficiently and accurately.

[0006] Specifically, the operator inserts a catheter into the stomach through the mouth, connects a suction source to the catheter, and discharges the fluid in the stomach to the outside through the catheter to reduce pressure. In this way, the stomach is tightly attached to the catheter. The operator can easily cut the stomach along the catheter to remove a part of the stomach.

[0007] For example, Patent Document 1 discloses a catheter (described as a catheter in the document) also used in laparoscopic surgery. The catheter includes a retaining body (described as a balloon in the document) for retaining a position in the stomach.

[0008] The catheter is provided with an opening (described as a hole in the document) that allows gaseous fluid or liquid fluid to be drawn out and injected into the stomach and connects the lumen of the tube to the outside.

[0009] Furthermore, for example, Patent Document 2 discloses a catheter (described as a calibration tube in the document) which is also used in laparoscopic surgery.

[0010] This document describes that fluid is drawn out from the stomach to the outside by connecting a catheter to a suction source (described as a vacuum source in this document).

[0014] Patent Document 2: International Publication No. 2015 / 116290 Summary of the invention

[0015] Problem that the invention aims to solve

[0016] However, in the catheter of Patent Document 1, the opening is formed on the side of the side where the holding body is provided.

[0017] Therefore, when the stomach is decompressed by suctioning the fluid in the stomach through the catheter using a suction source, the opening may be blocked by the lateral stomach wall. In this case, the amount of decompression on the side opposite to the side where the holder is provided may be reduced.

[0018] Improvements are required to improve the adhesion of the cutting target portion located on the opposite side to the side where the holder is provided to the catheter or the adhesion of the cutting target portions near the catheter to each other, thereby improving the visibility of the "stomach cutting line".

[0019] Furthermore, for example, in order to check the airtightness of the cut or sutured portion of the stomach (leak test), a fluid supply source is sometimes connected to the catheter so that the outside of the stomach can be filled with saline and air can be sent into the stomach.

[0020] In this case, it is necessary to prepare a fluid supply source (for example, a syringe) different from the suction source of the catheter of Patent Document 2, and an instrument that can perform the operation more smoothly is required.

[0021] The present invention has been made in view of the above-mentioned problems, and an object of the present invention is to provide a catheter capable of improving the visibility of the gastric cutting line and a catheter set capable of smoothly performing surgery.

[0022] Means for solving technical problems

[0023] The catheter of the present invention is a catheter inserted into the stomach through the mouth to guide the cutting line when a part of the stomach is removed. The catheter is characterized in that it comprises: a tube body having a lumen that can be connected to a suction source and communicated from the proximal side to the distal side and an opening that can communicate the stomach with the lumen; and a retainer that engages with the stomach on one radial side at the distal part of the tube body to maintain the position of the tube body relative to the stomach. The fluid in the stomach is sucked from the opening into the lumen by the suction source, so that the tube body is closely attached to a part of the stomach, and the opening is at least provided on the side opposite to the one side, that is, the other radial side of the tube body.

[0024] Furthermore, the catheter set of the present invention is characterized in that it comprises: the catheter; and a manual pump, which serves as the suction source and serves as a delivery source for delivering gas from the outside to the stomach. The manual pump comprises: a flow path component having a flow path connected to one end and the other end of the manual pump; an expansion and contraction portion, which forms a part of the flow path and can expand and contract to change the pressure in the flow path; and a check valve, which is arranged in the flow path. The manual pump serves as the suction source by connecting the one end of the flow path component to the proximal end of the catheter, and serves as the delivery source by connecting the other end.

[0025] Effects of the Invention

[0026] According to the catheter of the present invention, by providing the opening portion capable of communicating with the stomach and the lumen on the side opposite to the holding body, the visibility of the stomach cutting line can be improved.

[0027] Furthermore, according to the catheter set of the present invention, since the manual pump having the check valve is provided, surgery can be smoothly performed. BRIEF DESCRIPTION OF THE DRAWINGS

[0043] <Outline>

[0044] First, the main reference Figures 1 to 5 The outline of the catheter 1 and the catheter group S according to the present embodiment will be described.

[0045] Figure 1 FIG. 2 is a schematic diagram showing a catheter set S according to the present embodiment, and shows a state where the catheter 1 is inserted into the stomach 50 and the distal end thereof is fixed in the stomach 50. Figure 2 1 is a schematic diagram showing the catheter 1 . Figure 3A and Figure 3B The opening of the balloon 3 and the tube body 2 (the back opening 2b) is schematically shown. Figure 3A is a side view, Figure 3B It is the rear view. Figure 4A The cross section of the capsule 3 and the tube body 2 is Figure 3B IVA-IVA cross-sectional view, Figure 4B yes Figure 4A Magnified view of part IVB. Figure 5 Schematic diagram showing the manual pump 7.

[0046] like Figure 1 As shown, the catheter 1 according to this embodiment is inserted into the stomach 50 through the mouth to guide the cutting line L when a part of the stomach 50 is cut using an automatic suturing device or the like. The cutting line L is Figure 1 The center is a line represented by a double-dashed line.

[0047] More specifically, the catheter 1 is used when a portion of the stomach 50 is removed by cutting the stomach 50 linearly along the catheter 1, and is inserted into the stomach 50 to aspirate the fluid in the stomach 50, thereby making the outline of the catheter 1 protrude, thereby guiding the linear cutting. That is, "guiding the cutting line L" means "making the operator visually recognize the cutting line L".

[0048] The catheter 1 comprises: a tube body 2, having a lumen 2a which can be connected to a suction source (manual pump 7) at a proximal end and communicated from the proximal side to the distal side, and an opening (back opening 2b) which can communicate the inside of the stomach 50 and the lumen 2a at the distal part; and a retaining body (balloon 3) which is engaged with the stomach 50 on one radial side at the distal part of the tube body 2 to maintain the position of the tube body 2 relative to the stomach 50.

[0049] The fluid in the stomach 50 is sucked into the lumen 2a from the back opening 2b by the manual pump 7, so that the tube body 2 is closely attached to a part of the stomach 50. The back opening 2b is characterized in that it is provided at least on the side opposite to the side engaged with the stomach 50, that is, on the other side of the tube body 2 in the radial direction.

[0050] The suction source may be a syringe, an electric pump, etc. (not shown) in addition to the manual pump 7 described later. Furthermore, the retaining body is not limited to such a structure as long as it is non-invasive and can fix or position the distal end of the catheter 1 in the stomach 50, and may be a buffer having an expandable and contractible structure.

[0051] According to the above structure, the holding body (balloon 3) is engaged with the pyloric side of the stomach 50 on one side, and fluid is sucked from the opening (back opening 2b) located on the side opposite to the balloon 3 (the side opposite to the inflated side of the balloon 3), thereby making a part of the stomach 50 closely attached to the tube body. Therefore, the stomach 50 can be easily cut along the tube body 2 on the side opposite to the balloon 3 (the side opposite to the pyloric side of the stomach 50 engaged with the balloon 3).

[0052] like Figure 1 As shown in FIG. 1 , the catheter set S according to the present embodiment includes a catheter 1 and a manual pump 7 (described later) which serves as a suction source and a delivery source for delivering gas from the outside into the stomach 50 .

[0053] like Figure 5As shown, the manual pump 7 comprises: a flow path component 7a having a flow path connected to one end 7aa and the other end 7ab of the manual pump 7; an expansion and contraction portion 7b forming a part of the flow path and capable of expanding and contracting to change the pressure in the flow path; and check valves 7c and 7d arranged in the flow path.

[0054] The manual pump 7 becomes a suction source by connecting one end 7aa of the flow path member 7a to the proximal end of the catheter 1, and becomes a delivery source by connecting the other end 7ab.

[0055] The “connection” between the manual pump 7 and the proximal end portion of the catheter 1 includes not only a direct connection but also an indirect connection.

[0056] For example, the catheter set S may further include a three-way cock (not shown). Furthermore, the manual pump 7 and the proximal end of the catheter 1 may be connected via the three-way cock.

[0057] According to the above configuration, by switching the end of the manual pump 7 that is connected to the catheter 1 (switching between one end 7aa and the other end 7ab), the direction of the fluid is switched, and it is possible to smoothly transition from the process of draining the fluid in the stomach 50 to the process of supplying the fluid into the stomach 50. That is, it is possible to easily transition from the resection preparation process and the resection process of making a part of the stomach 50 close by sucking the fluid in the stomach 50 to the leakage test process by sending gas into the stomach 50.

[0058] <Overall structure>

[0059] The catheter set S according to this embodiment is used when performing laparoscopic sleeve gastrectomy. As described above, the catheter set S includes the catheter 1 and the manual pump 7 described later, which serves as a suction source and a delivery source for delivering gas from the outside to the stomach 50 .

[0060] As a specific operation, the surgeon Figure 1 After the catheter 1 is arranged as shown, the fluid is discharged from the stomach 50 by the manual pump 7 described later, so that the stomach 50 is closely attached to the periphery of the tube body 2. Then, the operator uses an automatic suturing device or the like to suture the tube body 2 ( Figure 1 The stomach 50 is removed by cutting line L in the figure.

[0061] <Structure of each department>

[0062] Next, the configuration of each part of the duct set S will be described with reference to the drawings.

[0063] [catheter]

[0064] The catheter 1 is inserted into the stomach 50 through the mouth, and is used as a guide for the resection position when the stomach 50 is resected using an automatic suturing device or the like.

[0065] The catheter 1 includes a tube body 2 having a lumen 2a, a balloon 3 (described later) provided at a distal end of the tube body 2, and a syringe connector 4 to which a syringe (not shown) for delivering or discharging a fluid to or from the balloon 3 is connected.

[0066] The outer diameter of the tube body 2 involved in this embodiment is about 5 to 20 mm, preferably about 10 to 15 mm, more preferably about 12 mm, and the length is about 50 to 150 cm, preferably about 75 to 100 cm, more preferably about 90 cm. In addition, scale lines (not shown) are printed on the tube body 2 so that the insertion length of the catheter 1 can be known from the outside. The lumen 2a extends in the axial direction through the central part.

[0067] The tube body 2 is formed with: a circular back opening 2b, which is located on the opposite side of the tube body 2 to the later-described bladder 3 (the opposite side to the expansion side of the bladder 3); and a circular side opening 2c, which is located in a direction intersecting the direction connecting the bladder 3 and the back opening 2b.

[0068] like Figure 3B As shown, at least a portion of the opening (back opening 2b) is located at a position overlapping with the retaining body (bladder 3) in the far-near direction (length direction). In particular, in this embodiment, the back opening 2b is provided at a plurality of positions overlapping with the bladder 3 in the far-near direction, preferably about 2 to 5 positions, and more preferably 2 to 3 positions.

[0069] Although the back opening 2b and the side opening 2c are described as circular, they may be elliptical, oblong, rectangular, or polygonal as long as they can communicate with the lumen 2a and the stomach 50.

[0070] By engaging the retainer (the balloon 3), a portion of the stomach 50 is inevitably arranged to cover the balloon 3. According to the above structure, by sucking the fluid in the stomach 50 from the opening (the back opening 2b) of the above arrangement, a portion of the stomach 50 expanded to cover the balloon 3 can be effectively fitted to the tube body 2, and the stomach 50 can be easily cut off.

[0071] Furthermore, the back opening 2b is preferably provided in a plurality (preferably about 2 to 5, preferably about 2 to 5) on the distal side and the proximal side of the capsule 3 at a position overlapping with the capsule 3 in the distal direction. Figure 3B 2 in the middle).

[0072] According to such a configuration, the balloon 3 can be wrapped by the stomach 50 sucked by the distal and proximal openings, so that the engaged state of the balloon 3 can be stabilized.

[0073] Furthermore, according to such a configuration, the stomach 50 can be easily brought into close contact with the tube body 2 at the resection start position by an automatic suturing device or the like.

[0074] And, if Figure 3A As shown, the back opening 2b and the side openings 2c are formed differently from each other at positions offset in the length direction.

[0075] According to the above configuration, the back opening 2b and the side opening 2c are provided on the outer surface of the curved surface of the tube body 2. Therefore, even if there is a protrusion at a position where the opening contacts the inner wall of the stomach 50 or there is a foreign object between the inner wall and the opening, the stomach 50 can be appropriately sucked by locating the opening at a staggered position, compared with the case where the opening is located on a straight line.

[0076] like Figure 4A As shown, an air supply tube 4a is connected to a syringe connector 4 to which a syringe (not shown) for delivering or discharging a fluid to or from a balloon 3 described later is connected.

[0077] The air supply tube 4 a is independently provided so as not to communicate with the lumen 2 a , and is disposed along the inner surface of the tube body 2 located on the balloon 3 side and is bonded with a bonding agent or the like.

[0078] Specifically, if Figure 4B As shown, the tube body 2 is formed with a through hole 2d penetrating toward the balloon 3, and the distal end of the air supply tube 4a is fitted in a manner of closely fitting to the wall surface of the through hole 2d. With such a structure, the interior of the air supply tube 4a is not connected to the lumen 2a.

[0079] The air supply tube 4a of this embodiment is arranged along the inner surface of the tube body 2, is not exposed to the outer surface of the tube body 2, and has no irregularities on the outer surface of the tube body 2. Therefore, the air supply tube 4a is preferred in that it can reduce invasiveness when inserted into the body.

[0080] However, the present invention is not limited to such a structure, and the air supply pipe 4a may be arranged and joined along the outer surface of the pipe body 2. According to such a structure, the air supply pipe 4a can be easily arranged and joined to the long pipe body 2.

[0081] Furthermore, as long as gas can be fed into the bladder 3, it may not be a tubular structure (air supply pipe 4a), but may be an air supply hole (not shown) provided inside the tube body 2. Furthermore, a part of the part may be tubular and other parts may be air supply holes connected to the tubular part.

[0082] [Capsule]

[0083] The holding body involved in this embodiment is a balloon 3 that can expand and contract. Figure 1 As shown, the balloon 3 is expandably disposed on one side (the pyloric side) in the radial direction at the distal portion of the tube body 2. The maximum capacity of the balloon 3 involved in this embodiment is about 50 to 100 mL, preferably about 75 mL. Figure 3A As shown, the opening portion (back opening 2 b ) is provided on the side opposite to the inflated side of the bladder 3 .

[0084] In addition, the opening may be provided at a position other than the side opposite to the inflation side of the bladder 3 , as in the side opening 2 c , in addition to the back opening 2 b .

[0085] According to the above structure, the inflated balloon 3 is engaged with the pyloric side in the stomach 50, and fluid is sucked (discharged to the outside of the body) from the opening (back opening 2b) located on the side opposite to the inflated side of the balloon 3, thereby making a part of the stomach 50 located on the opposite side to the pyloric side closely fit to the tube body. Therefore, the operator can easily cut the stomach 50 along the part closely fitting to the tube body.

[0086] As described above, if the holding body is a balloon 3, it can be expanded by introducing fluid (gas or saline) into the balloon 3, and the soft resin of the balloon 3 can suppress the application of a large concentrated load to the inner wall of the stomach 50, which is preferred.

[0087] A portion of the balloon 3 is joined to the periphery of the back opening 2b of the tube body 2 (in a manner that covers the periphery). The joining portion 3a of the balloon 3 and the tube body 2 is Figure 3B The area in the middle is indicated by slashes.

[0088] The balloon 3 is originally formed in a sheet shape, and the tip of the entire periphery except for the periphery of the back opening 2 b in the portion other than the expanded portion is joined to the tube body 2 .

[0089] In this embodiment, when the retainer (balloon 3) is mounted on the tube body 2, a portion of the balloon 3 is joined to the tube body 2, and an opening (back opening 2b) that reaches the lumen 2a is formed in the joined region. According to such a forming method, the balloon 3 can be easily mounted and the back opening 2b can be formed, and the inside of the balloon 3 can be prevented from communicating with the back opening 2b (the inside of the stomach 50).

[0090] [Hand Pump]

[0091] Next, except Figure 5 In addition, the main reference Figure 6 , Fig. 7A and Figure 7B , the manual pump 7 will be described. Figure 6 It is a perspective view showing the negative pressure releasing unit 8 . Fig. 7A and Figure 7B yes Figure 6 View from direction VII, Fig. 7A 2 is a diagram showing a state of the negative pressure release unit 8 before use. Figure 7B This is a diagram showing a state where the opening 8d is opened by pinching the arm portion 8b.

[0092] The manual pump 7 is used to reduce the pressure in the stomach 50 when a part of the stomach 50 is brought into close contact with the tube body 2 , and is connected to the proximal end of the catheter 1 with one end 7aa of a flow path member 7a described later to serve as a suction source.

[0093] Furthermore, the manual pump 7 is also used to send air into the stomach 50 via the catheter 1 when checking air leakage at the suture portion after the stomach 50 is removed, and the other end 7ab of the flow path member 7a described later is connected to the proximal end of the catheter 1 to become a sending source. The air leakage check (leak test) of the suture portion is a test in which the periphery of the stomach 50 (especially, in a manner connected to the suture portion) is filled with physiological saline, external air is sent into the stomach 50 by the manual pump 7 to increase the internal pressure, and then it is checked whether there is air leakage from the stomach 50 to the outside (generation of bubbles).

[0094] The manual pump 7 comprises: a flow path component 7a having a flow path connected to one end 7aa and the other end 7ab of the manual pump 7; an expansion and contraction portion 7b forming a part of the flow path and capable of expanding and contracting to change the pressure in the flow path; and check valves 7c and 7d arranged in the flow path.

[0095] Both ends of the flow path member 7a are formed into a shape (bamboo joint shape) in which a plurality of portions (tapered portions) whose outer diameter narrows toward the ends (one end 7aa or the other end 7ab) are continuous with steps.

[0096] Two check valves 7c and 7d are provided in the flow path on the one end 7aa side and the other end 7ab side across the expansion and contraction portion 7b so as to restrict the flow in the same direction.

[0097] And, in this case, in Figure 5 On the paper, the check valves 7c and 7d are arranged inside the flow path component 7a to allow flow from the left side (the side where one end 7aa is located) to the right side (the side where the other end 7ab is located) and restrict the direction of flow from the right side to the left side.

[0098] According to the above structure, when delivering gas into the stomach 50, it is possible to suppress leakage of gas to the outside of the manual pump 7, thereby enabling efficient delivery of gas into the stomach 50. In addition, even if there is only one check valve, the manual pump 7 can be operated by adopting a structure in which the elastic force of the expansion and contraction portion 7b is high, but the above structure is preferred in terms of suction efficiency and delivery efficiency.

[0099] And, regarding the check valve 7c and the check valve 7d, Figure 5 In FIG. 1 , the check valve 7c is shown to be larger than the check valve 7d, but the sizes of the check valve 7c and the check valve 7d may be the same and may be set to any size.

[0100] Furthermore, in this example, if the check valves 7c and 7d are Figure 5 The duckbill valve schematically shown in the figure can be arranged in a small space, so it is preferred, but it is not limited to such a structure as long as the backflow can be restricted. For example, it can be an umbrella valve or a swing check valve, a lift check valve, a wafer check valve, etc.

[0101] A negative pressure release unit 8 is provided in a region between the check valve 7c for limiting the flow of fluid from the expansion / contraction unit 7b of the manual pump 7 to the one end 7aa and the one end 7aa. The negative pressure state in the region can be released by the operator operating the negative pressure release unit.

[0102] like Figure 6 , Fig. 7A and Figure 7B As shown, the negative pressure release portion 8 mainly includes a tube 8a led out from the space between the check valve 7c and one end 7aa, U-shaped arm portions 8b arranged on two radially outer sides of the tube 8a, and claw portions 8c respectively arranged on the opposing inner surfaces of the arm portions 8b.

[0103] The tube 8a has an open port 8d formed therein. The open port is closed in a natural state and is opened when the tube 8a is compressed radially inward so as to allow the space between the check valve 7c and the one end 7aa to communicate with the outside.

[0104] The operator operates the negative pressure release unit 8 to release the negative pressure state in the above region, thereby making it easy to remove the manual pump 7 from the catheter 1. Specifically, the operator pinches the arm 8b in a direction to bring the claw 8c close to the tube 8a, and collapses the tube 8a to open the opening 8d, thereby releasing the negative pressure state in the above region.

[0105] Furthermore, as described above, by providing a three-way stopcock (not shown) between the manual pump 7 and the catheter 1 , the flow path from the manual pump 7 to the catheter 1 is cut off, thereby easily switching to a state in which the inside of the catheter 1 is maintained at a negative pressure.

[0106] That is, according to the three-way cock, the manual pump 7 can be removed from the three-way cock while maintaining the inside of the catheter 1 in a negative pressure state.

[0107] On the other hand, by opening the flow path from the manual pump 7 to the catheter 1 , the manual pump 7 can be operated to suck fluid from the stomach 50 or to supply gas into the stomach 50 .

[0108] <First Modification>

[0109] Next, refer to Figure 8 The terminal 5 according to the first modification will be described. Figure 8 It is a schematic diagram showing a structure in which the terminal 5 according to the first modification is attached to the distal end portion of the tube body 2 .

[0110] In this example, the catheter 1 further includes a terminal 5 provided at the distal end of the tube body 2. The terminal 5 has no opening that can communicate the stomach 50 with the lumen 2a. That is, the terminal 5 closes the distal end of the tube body 2.

[0111] For example, if an opening is provided at the terminal end 5 located at the distal end of the tube body 2, the opening may get caught on the body wall when the catheter 1 is inserted into the body.

[0112] According to the above-mentioned structure, since the terminal end 5 located at the distal end of the pipe body 2 is not provided with an opening, such a situation does not exist.

[0113] Furthermore, when the fluid in the stomach 50 is sucked from the opening of the tube body 2 , since the terminal 5 is not provided with an opening, it is possible to prevent the balloon 3 from being sucked into the opening and disturbing the retention of the balloon 3 in the stomach 50 .

[0114] 4 , the distal end of the lumen 2a formed in the tube body 2 may be closed by the tube body 2. That is, whether the distal end of the tube body 2 is open or closed, the tip 5 may be attached to either one.

[0115] The terminal end 5 is preferably formed of a material softer than the tube body 2. In particular, the distal end surface of the terminal end 5 is formed in a hemispherical shape (curved surface shape).

[0116] According to the above configuration, since the tip 5 located at the distal end of the tube body 2 is formed to be softer than the tube body 2 , the invasiveness of the distal end of the catheter 1 can be reduced.

[0117] <Second Modification>

[0118] Finally, reference Fig. 9 The terminal 5X according to the second modification will be described. Fig. 9 It is a schematic diagram showing a structure in which a terminal 5X according to a second modification is attached to a distal end portion of a tube body 2 .

[0119] The catheter 1 further includes a terminal end 5X provided at a distal end portion of the tube body 2. The terminal end 5X has other openings (terminal openings 5a, 5b) that can communicate the inside of the stomach 50 with the inside of the lumen 2a.

[0120] The distal end of the lumen 2 a formed in the tube body 2 according to this example is not closed but is open in the tube body 2 . Fig. 9 The terminal openings 5a and 5b shown are opened in a direction intersecting the extending direction of the lumen 2a and communicate with the lumen 2a.

[0121] Furthermore, the terminal end 5X according to this example is also preferably formed of a material softer than the tube body 2 .

[0122] Fluid in the stomach 50 is sucked from the opening (terminal openings 5a, 5b) of the terminal 5X located at the distal end of the tube body 2 to create negative pressure in the stomach 50, thereby making the stomach 50 closely attached to the terminal 5X, thereby making it easy to start cutting the stomach 50.

[0123] In particular, the tip 5 is located more distally than the portion joined to the balloon 3. Therefore, when the fluid in the stomach 50 is sucked from the terminal openings 5a and 5b of the tip 5, the stomach 50 on the distal side of the balloon 3 is easily brought into close contact with the distal end of the catheter 1, and the balloon 3 is easily engaged with the stomach 50.

[0124] Specifically, the other opening is a first other opening (the terminal opening 5 a ), and is provided on one side in the radial direction of the tube body 2 (the side on which the balloon 3 is provided).

[0125] According to the above structure, the terminal opening 5a located at the distal end of the tube body 2 is set on one side of the radial direction in the tube body 2, thereby enabling the volume of the stomach 50 on the pyloric side to be narrowed from the distal side, thereby making it easier for the retaining body (balloon 3) to engage with the stomach 50.

[0126] Furthermore, the other opening is a second other opening (terminal opening 5 b ), and is provided on the other side in the radial direction of the tube body 2 (the side opposite to the side where the balloon 3 is provided).

[0127] According to the above structure, the terminal opening 5b located at the distal end of the tube body 2 is set on the other side of the tube body 2 in the radial direction, thereby making it possible to make a part of the stomach 50, which is the part where the stomach 50 is started to be cut, closely fit to the tube body 2, making it easy to start cutting the stomach 50.
