The invention provides an anti-scale agent for a water-in-oil system in an oilfield high-temperature environment as well as a preparation method and a use of the anti-scale agent. The anti-scale agent is characterized by being prepared from bis(1,6-hexamethylene triamine penta(methylene phosphonic acid)), a phosphinoacrylic acid multipolymer, poly(epoxy sulfocarboxylic acid), 2-phosphonobutane-1,2,4-tricarboxylic acid and alkylphenol polyoxyethylene ether, wherein the total content of the active constituents of the anti-scale agent is not lower than 40%. The preparation method comprises the following steps: orderly adding the raw materials, mixing evenly, adjusting the pH value to 7, stirring for 20 minutes, cooling until the temperature reaches a room temperature, next, adding water, and finally, adding the emulsifier alkylphenol polyoxyethylene ether and stirring for 15 minutes to obtain the anti-scale agent. The anti-scale agent is applied to the scale prevention use of the water-in-oil system in the high-temperature environment of 200 DEG C and below, and in terms of the water quantity in the oil-water mixture, the use amount of the anti-scale agent is 60-100mg / L.
