Abstract
Description
Technical Field
[0001] The present invention relates to the field of biomedicine, and in particular to a gene combination and application thereof for pre-donor screening of alloHCT in different modes and post-transplant aGVHD risk prediction. Background Art
[0002] Allogeneic hematopoietic cell transplantation (alloHCT) can effectively treat more than 70 diseases, including leukemia, genetic diseases, and autoimmune diseases. Acute graft-versus-host disease (aGVHD) is a common fatal complication after alloHCT, which seriously affects the prognosis and survival of patients. For a long time, whether the HLA genotype between the donor and the recipient is matched has been considered to be the decisive factor affecting the occurrence of aGVHD. Selecting a donor who is completely HLA-matched with the recipient was once the "gold standard" in the field of alloHCT. After decades of development and improvement, the HLA haplotype-matched alloHCT regimen based on antithymocyte globulin (ATG) and granulocyte colony-stimulating factor (G-CSF)-induced immune tolerance originally developed by the Institute of Hematology of Peking University has achieved clinical efficacy comparable to that of HLA-matched transplantation, becoming the most important alloHCT model in my country and widely used in countries around the world.
[0003] The great success of the HLA haplotype-matched transplantation program means that the classic immunological barrier of HLA incompatibility has been overcome in clinical practice. However, aGVHD will occur after both HLA haplotype-matched and HLA fully matched transplantation, which highly suggests that there are other genetic factors that affect the occurrence of aGVHD. On the other hand, the successful application of HLA haplotype-matched transplantation has greatly solved the problem of lack of donor sources, making the patient's parents, children, siblings and collateral relatives potential candidate donors. When a patient has multiple potential donors, how to choose the most suitable donor is of great significance for reducing the risk of aGVHD and improving the patient's overall survival rate. Summary of the invention
[0004] The object of the present invention is to provide a gene combination and its application for donor screening before HLA haplotype matching and HLA fully matching allogeneic hematopoietic cell transplantation (alloHCT) and risk prediction of acute graft-versus-host disease (aGVHD) after alloHCT.

[0105] In practical applications, any two or three or more gene pairs (including 35 gene pairs) among the above 35 gene pairs can also be used to predict the risk of acute graft-versus-host disease in patients after alloHCT of different modes or to diagnose whether acute graft-versus-host disease occurs in patients after alloHCT. When multiple gene pairs are used for joint prediction or diagnosis, each gene pair needs to meet its own judgment criteria for prediction or diagnosis, otherwise the prediction or diagnosis method is invalid.
[0106] When the 35 gene pairs are used for prediction or diagnosis, in addition to referring to the judgment criteria of each gene pair mentioned above for prediction or diagnosis, prediction or diagnosis can also be performed by comparing the cumulative score of the mutant gene pairs with the threshold.
[0107] Specifically,
[0108] The method for predicting the risk of acute graft-versus-host disease in patients after alloHCT based on the cumulative score and threshold of mutation gene pairs is as follows: the risk of acute graft-versus-host disease in patients after alloHCT whose cumulative score of gene mutation pairs is greater than or equal to the threshold is higher than that in patients after alloHCT whose cumulative score of gene mutation pairs is less than the threshold.
[0109] Whether acute graft-versus-host disease occurs in patients after alloHCT is diagnosed based on the cumulative score and threshold of the mutation gene pair: if the cumulative score of the gene mutation is greater than or equal to the threshold, the patient has developed or is a candidate for developing acute graft-versus-host disease after alloHCT; if the cumulative score of the gene mutation is less than the threshold, the patient has not developed or is a candidate for developing acute graft-versus-host disease after alloHCT.
[0110] The cumulative score of the mutant gene pair is obtained according to the following method: detecting the mutation status of the protective mutant gene pair in the peripheral blood of the patient and the donor, and obtaining the cumulative score of the protective mutant gene pair; detecting the mutation status of the promoting mutant gene pair in the peripheral blood of the patient and the donor, and obtaining the cumulative score of the promoting mutant gene pair; the sum of the cumulative score of the protective mutant gene pair and the cumulative score of the promoting mutant gene pair is the cumulative score of the mutant gene pair of the patient;
[0111] The protective mutant gene pairs are ADAMTS7-APOL4, AGAP3-ACR, ANAPC1-ACR, CARD8-SIRPB1, DCAF15-ACR, DENND3-ACR, FRMD5-MYCT1, HAPLN3-ACR, HRNR-ACR, MAGEE2-ACR, MYCT1-KRTAP10-1, MYCT1-OAS1, NUDT1-ACR, OAS1-ACR, PABPC3-ACR, SIRPA-ACR, SLC35G6-ACR, TLR8-ACR, and ZFPM1-ACR;
[0112] The promoting mutation gene pairs are ACTN2-OBSCN, CDH15-FRMD5, CDH15-GPR161, CDH15-OBSCN, GNRH2-FRMD5, GNRH2-KRT24, GNRH2-MAGEE2, GNRH2-OBSCN, GPSM1-OBSCN, HAPLN3-OBSCN, LCN10-CD33, MS4A12-ACTN2, MS4A12-ZNF211, RBMX-CD33, SIRPB1-ACTN2, SIRPB1-OBSCN;
[0113] The cumulative score of the protective mutation gene pairs is calculated as follows: if none of the 19 protective mutation gene pairs in the peripheral blood of the patient and the donor is mutated, it is recorded as 0; if one protective mutation gene pair (any one of the 19 protective mutation gene pairs) is mutated in the peripheral blood of the patient and the donor, it is recorded as -1; if two protective mutation gene pairs (any two of the 19 protective mutation gene pairs) are mutated in the peripheral blood of the patient and the donor, it is recorded as -2; and so on, if 19 protective mutation gene pairs are mutated in the peripheral blood of the patient and the donor, it is recorded as -19;
[0114] The calculation method for the cumulative score of the promoting mutation gene pairs is as follows: if none of the 16 promoting mutation gene pairs in the peripheral blood of the patient and its donor are mutated, it is recorded as 0; if one promoting mutation gene pair (any one of the 16 promoting mutation gene pairs) in the peripheral blood of the patient and its donor is mutated, it is recorded as 1; if two promoting mutation gene pairs (any two of the 16 promoting mutation gene pairs) in the peripheral blood of the patient and its donor are mutated, it is recorded as 2; and so on, if 16 promoting mutation gene pairs in the peripheral blood of the patient and its donor are mutated, it is recorded as 16.
[0115] In practical applications, the risk of acute graft-versus-host disease after alloHCT of different modes can be predicted according to the above method, and then the pre-alloHCT donors can be screened according to the risk of acute graft-versus-host disease, or other clinical indicators can be combined to assist in the screening of pre-alloHCT donors, so as to optimize the selection of pre-alloHCT donors and improve the survival rate and quality of life of patients before alloHCT.
[0116] Mutation in any of the above gene pairs means that both the gene corresponding to the donor (i.e. the first gene before the “-” in the above text) and the gene corresponding to the recipient (i.e. the second gene after the “-” in the above text) in the gene pair are mutated.
[0117] Mutations in any of the above genes include any base mutations that can change the amino acid sequence of the protein encoded by the gene. The base mutations include base insertions, base deletions, base substitutions, etc. The number of base mutations includes 1 or 2 or more base mutations.
[0118] The fact that any of the above genes have not undergone mutations includes the fact that there are no base mutations (completely identical to the wild-type sequence of the gene) or any base mutations that do not change the amino acid sequence of the protein encoded by the gene.
[0119] Any of the above thresholds may specifically be 1.5 or 3.5.
[0120] In any of the above applications or kits, the donor-recipient refers to a donor and a recipient. The donor is a healthy individual who provides hematopoietic cells before alloHCT, and the recipient is a patient before alloHCT.
[0121] In any of the above applications or kits, the alloHCT is allogeneic hematopoietic cell transplantation.
[0122] The allogeneic hematopoietic cell transplantation includes HLA haplotype-matched allogeneic hematopoietic cell transplantation and HLA fully-matched allogeneic hematopoietic cell transplantation.
[0123] The patient before alloHCT may be a patient with acute myeloid leukemia, a patient with acute lymphoid leukemia, a patient with chronic myeloid leukemia, a patient with chronic lymphoid leukemia, a patient with multiple myeloma, a patient with myelodysplastic syndrome, or a patient with primary systemic amyloidosis.
[0124] The use of ADAMTS7, APOL4, AGAP3, ACR, ANAPC1, CARD8, SIRPB1, DCAF15, DENND3, FRMD5, MYCT1, HAPLN3, HRNR, MAGEE2, KRTAP10-1, OAS1, NUDT1, PABPC3, SIRPA, SLC35G6, TLR8, ZFPM1, ACTN2, OBSCN, CDH15, GPR161, GNRH2, KRT24, GPSM1, LCN10, CD33, MS4A12, ZNF211 and / or RBMX as targets in the preparation of products for preventing and / or treating acute graft-versus-host disease also falls within the scope of protection of the present invention.
[0125] The IDs of any of the above-mentioned ADAMTS7, APOL4, AGAP3, ACR, ANAPC1, CARD8, SIRPB1, DCAF15, DENND3, FRMD5, MYCT1, HAPLN3, HRNR, MAGEE2, KRTAP10-1, OAS1, NUDT1, PABPC3, SIRPA, SLC35G6, TLR8, ZFPM1, ACTN2, OBSCN, CDH15, GPR161, GNRH2, KRT24, GPSM1, LCN10, CD33, MS4A12, ZNF211, and RBMX in the NCBI gene library are shown in Table 1 below.
[0126] The present invention has discovered gene combinations with predictive and diagnostic value for the occurrence of aGVHD after alloHCT in different modes (HLA haplotype matching and HLA full matching) through whole genome sequencing and target gene capture sequencing. On the one hand, by detecting the gene combination mutations of donors and recipients for comprehensive judgment and evaluation, the selection of donors before alloHCT is optimized, and the survival rate and quality of life of patients are improved; on the other hand, the risk of aGVHD after alloHCT in recipients can be predicted by detecting the gene combination mutations of donors and recipients, providing a basis for potential diagnosis of aGVHD; thirdly, the present invention provides a new genetic basis for revealing the pathogenesis of aGVHD and provides a new drug target for the treatment of aGVHD. [0133] The present invention is further described in detail below in conjunction with specific embodiments, and the examples provided are only for illustrating the present invention, rather than for limiting the scope of the present invention. The examples provided below can be used as a guide for further improvements by those of ordinary skill in the art, and do not constitute a limitation of the present invention in any way.
[0134] The experimental methods in the following examples, unless otherwise specified, are all conventional methods, and are performed according to the techniques or conditions described in the literature in the field or according to the product instructions. The materials, reagents, etc. used in the following examples, unless otherwise specified, can all be obtained from commercial channels.
[0135] The HLA haplotype matching involved in the present invention means that the HLA-A, HLA-B, and HLA-DRB1 alleles between the related donor and recipient are the same on the same chromosome, but at least one of the above alleles on another chromosome is different (i.e. 3 / 6, 4 / 6, or 5 / 6 matching).
[0136] The HLA full match involved in the present invention means that the HLA-A, HLA-B, and HLA-DRB1 alleles between the sibling donor and recipient are the same on both chromosomes (i.e., 6 / 6 match).
[0137] Example 1. Acquisition and verification of LoF mutation gene pairs associated with aGVHD
[0138] 1. Based on the HLA haplotype-matched alloHCT clinical cohort, LoF mutation gene pairs associated with aGVHD were found
[0139] 1. Research subjects
[0140] The research subjects of this example are as follows: peripheral blood samples of 106 patients with aGVHD before hematopoietic cell transplantation and their donor peripheral blood samples (defined as the aGVHD positive group), and peripheral blood samples of 106 patients without aGVHD before hematopoietic cell transplantation and their donor peripheral blood samples (defined as the aGVHD negative group).
[0141] Inclusion criteria: 1. Patients underwent HLA haplotype-matched alloHCT at Peking University People's Hospital in 2018 and 2019; 2. Patients were diagnosed with hematological malignancies before hematopoietic cell transplantation, including acute myeloid leukemia, acute lymphoid leukemia, chronic myeloid leukemia, chronic lymphoid leukemia, multiple myeloma, myelodysplastic syndrome, and primary systemic amyloidosis; 3. The ages of patients and donors were between 14 and 65 years old; 4. The aGVHD-positive group included patients diagnosed with aGVHD 2-4 within 100 days after hematopoietic cell transplantation; the aGVHD-negative group included patients who did not develop aGVHD symptoms within 100 days after hematopoietic cell transplantation, and were paired 1:1 with the aGVHD-positive group. There was no statistical difference in age and gender between the two groups.
[0142] Exclusion criteria: 1. Patients diagnosed with non-acute promyelocytic leukemia or aplastic anemia before hematopoietic cell transplantation; 2. Patients who were not undergoing alloHCT for the first time; 3. Patients who died within three months after hematopoietic cell transplantation and the cause of death was not diagnosed as aGVHD.
[0143] 2. Sample preparation
[0144] The genomic DNA was extracted from the collected peripheral blood samples using the phenol-chloroform method to test the purity, concentration, integrity and contamination of the DNA. The qualified samples were sequenced for genome sequencing, and the experimental process was carried out according to the library preparation steps and sequencing steps provided by Illumina.
[0145] 3. Whole genome sequencing
[0146] The library was constructed using the human whole exome sequence capture kit Truseq Nano, and hybridized with probes for capture and enrichment. The samples were then sequenced using the Illumina Novaseq S4 PE150 high-throughput sequencing platform, with an average sequencing depth of 35x for each sample.
[0147] 4. Analysis of sequencing results
[0148] FastQC software was used to evaluate the quality of the output raw data, and BWA (parameter-M) software was used to align the measured data with the reference genome. All analyses were performed based on the human reference genome hg38. The mutations that affect protein structure annotated by ANNOVAR were defined as loss-of-function mutations (LOF mutations) in gene expression proteins in this study. The ANNOVAR annotations included "stopgain", "startloss", "frameshift insertion", "frameshift deletion" and "frameshift block substitution". When LoF mutations occur in two genes in a pair of donors and recipients (such as mutations in gene A in the donor and mutations in gene B in the recipient), the donor A gene and the recipient B gene are defined as a LoF mutation gene pair.
[0149] A total of 35 LoF mutation gene pairs associated with aGVHD were obtained by screening in the HLA haplotype alloHCT clinical cohort, as shown in Table 1.
[0150] 5. Correlation analysis between LoF mutation gene pairs and aGVHD
[0151] In the HLA haplotype-matched alloHCT cohort, each LoF mutation gene pair was analyzed for correlation with the occurrence of aGVHD.
[0152] The results are shown in Table 1. It can be seen from Table 1 that there are significant statistical differences between the aGVHD-positive group and the aGVHD-negative group in the 35 LOF mutation gene pairs, indicating that the 35 LOF mutation gene pairs are significantly correlated with the occurrence of aGVHD after HLA haplotype-matched alloHCT. Among them, 19 LOF mutation gene pairs are significantly negatively correlated with the occurrence of aGVHD (marked as aGVHD- in the table), and 16 LOF mutation gene pairs are significantly positively correlated with the occurrence of aGVHD (marked as aGVHD+ in the table).

[0163] 200 ng of genomic DNA was randomly fragmented into 150-200 bp average length using enzyme digestion, and then end-repaired and flattened with an A base added to the 3' end to form a sticky end. Then it was connected to the adapter containing the target gene-specific barcode sequence. Magnetic bead screening was used to remove incomplete connection products and adapter self-connection products. PCR amplification was performed using universal primers complementary to the adapter sequence to form a sequencing library. Then, the library was used to generate the sequencing library. The pool probes were hybridized in the hybridization buffer, and the hybridized target fragments were captured using magnetic beads and separated and purified. The captured DNA fragments were amplified by PCR and the PCR products were purified. The quality control of the captured library was then performed, including Qubit concentration determination and 2100 Bioanalyzer fragment length determination. Finally, PE150 high-throughput sequencing was performed on the NovaSeq6000 sequencer according to Illumina's standard process.
[0164] 4. Analysis of test results
[0165] FastQC software was used to evaluate the quality of the raw data, and BWA (parameter-M) software was used for genome alignment. All analyses were performed based on the human reference genome hg38.
[0166] 5. Correlation analysis between LoF mutation gene pairs and aGVHD
[0167] In the HLA-matched alloHCT cohort, each LoF mutation gene pair was analyzed for correlation with the occurrence of aGVHD.
[0168] The results are shown in Table 1. It can be seen from Table 1 that the 35 LOF mutation gene pairs screened in step 1 showed significant statistical differences between the aGVHD positive group and the aGVHD negative group, indicating that the 35 LOF mutation gene pairs were still significantly correlated with the occurrence of aGVHD after HLA fully matched alloHCT. Among them, 19 LOF mutation gene pairs were significantly negatively correlated with the occurrence of aGVHD (labeled as aGVHD- in Table 1), and 16 LOF mutation gene pairs were significantly positively correlated with the occurrence of aGVHD (labeled as aGVHD+ in Table 1). This result is consistent with the analysis results in the HLA haplotype-matched alloHCT cohort.

[0495] Example 2: Application of 35 LOF mutation gene pairs in predicting the risk of aGVHD after HLA haplotype-matched and / or HLA-matched alloHCT
[0496] 1. The 212 patients and their donors (212 pairs of donors and recipients) in Example 1 were used as research subjects. The cumulative scores of the LOF mutation gene pairs of the donors and recipients and the occurrence of aGVHD were tested by Mann-Whitney U. The results showed that the cumulative scores of the LOF mutation gene pairs in the aGVHD-positive group were significantly higher than those in the aGVHD-negative group ( Figure 1 , P<0.0001).
[0497] The receiver operating characteristic (ROC) curve was used to analyze the specificity and sensitivity of predicting aGVHD based on the cumulative score of LOF mutation gene pairs. The results showed that the area under the curve of the cumulative score of LOF mutation gene pairs for predicting aGVHD was 0.776 ( Figure 2, 95% confidence interval was 0.714-0.838), the cumulative score of the LOF mutation gene pair corresponding to the maximum Yoden Index (used to calculate the diagnostic cut-off value) was 1.5. Taking 1.5 as the diagnostic threshold for aGVHD, the detection sensitivity was 73.6% and the specificity was 71.7%.
[0498] 2. The 353 patients and their donors (353 pairs of donors and recipients) in Example 1 were used as research subjects. The cumulative scores of the LOF mutation gene pairs of the donors and recipients and the occurrence of aGVHD were tested by Mann-Whitney U. The results showed that the cumulative scores of the LOF mutation gene pairs in the aGVHD-positive group were significantly higher than those in the aGVHD-negative group ( Figure 3 , P<0.0001).
[0499] The receiver operating characteristic (ROC) curve was used to analyze the specificity and sensitivity of LOF mutation gene for predicting aGVHD. The results showed that the area under the curve of LOF mutation gene for predicting aGVHD was 0.729 ( Figure 4 , 95% confidence interval was 0.663-0.795), and the cumulative score of the LOF mutation gene pair corresponding to the maximum Yoden Index (used to calculate the diagnostic cut-off value) was 3.5. Taking 3.5 as the diagnostic threshold for aGVHD, the detection sensitivity was 53.5% and the specificity was 77.3%.
[0500] 3. All 565 patients and their donors (565 donor-recipient pairs) in Example 1 were used as research subjects. The cumulative scores of LOF mutation gene pairs of donors and recipients and the occurrence of aGVHD were tested by Mann-Whitney U. The results showed that the cumulative scores of LOF mutation gene pairs in the aGVHD-positive group were significantly higher than those in the aGVHD-negative group ( Figure 5 , P<0.0001).
[0501] The receiver operating characteristic (ROC) curve was used to analyze the specificity and sensitivity of LOF mutation gene for predicting aGVHD. The results showed that the area under the curve of LOF mutation gene for predicting aGVHD was 0.726 ( Figure 6 , 95% confidence interval was 0.682-0.770), the maximum Yoden Index (used to calculate the diagnostic cut-off value) corresponding to the gene pair mutation cumulative score was 1.5, and 1.5 was used as the diagnostic threshold for aGVHD. At this time, the sensitivity of the test was 74.6% and the specificity was 59.3%.
[0502] Therefore, in practical applications, the risk of aGVHD in patients with different modes of allogeneic hematopoietic transplantation can be predicted based on the cumulative scores of 35 LOF mutation gene pairs in donors and recipients. Then, pre-alloHCT donors can be screened or pre-alloHCT donors and recipients can be matched based on the risk of aGVHD, so as to optimize the selection of pre-alloHCT donors and improve the survival rate and quality of life of pre-alloHCT patients.
[0503] The cumulative score of the LOF mutation gene pair of each patient and their donor was obtained according to the following method (one patient was used as an example for explanation): the mutation of the protective mutation gene pair in the peripheral blood of the patient and their donor was detected to obtain the cumulative score of the protective mutation gene pair; the mutation of the promoting mutation gene pair in the peripheral blood of the patient and their donor was detected to obtain the cumulative score of the promoting mutation gene pair; the sum of the cumulative score of the protective mutation gene pair and the cumulative score of the promoting mutation gene pair was the cumulative score of the LOF mutation gene pair of the patient;
[0504] The protective mutant gene pairs are 19 LOF mutant gene pairs shown in Table 1 in Example 1 that are significantly negatively correlated with the occurrence of aGVHD (labeled as "aGVHD-" in Table 1), specifically as follows: ADAMTS7-APOL4, AGAP3-ACR, ANAPC1-ACR, CARD8-SIRPB1, DCAF15-ACR, DENND3-ACR, FRMD5-MYCT1, HAPLN3-ACR, HRNR-ACR, MAGEE2-ACR, MYCT1-KRTAP10-1, MYCT1-OAS1, NUDT1-ACR, OAS1-ACR, PABPC3-ACR, SIRPA-ACR, SLC35G6-ACR, TLR8-ACR, ZFPM1-ACR;
[0505] The promoting mutation gene pairs are 16 LOF mutation gene pairs shown in Table 1 in Example 1 that are significantly positively correlated with the occurrence of aGVHD (labeled as "aGVHD+" in Table 1), specifically as follows: ACTN2-OBSCN, CDH15-FRMD5, CDH15-GPR161, CDH15-OBSCN, GNRH2-FRMD5, GNRH2-KRT24, GNRH2-MAGEE2, GNRH2-OBSCN, GPSM1-OBSCN, HAPLN3-OBSCN, LCN10-CD33, MS4A12-ACTN2, MS4A12-ZNF211, RBMX-CD33, SIRPB1-ACTN2, SIRPB1-OBSCN;
[0506] The cumulative score of the protective mutation gene pairs is calculated as follows: if none of the 19 protective mutation gene pairs in the peripheral blood of the patient and the donor is mutated, it is recorded as 0; if one protective mutation gene pair (any one of the 19 protective mutation gene pairs) is mutated in the peripheral blood of the patient and the donor, it is recorded as -1; if two protective mutation gene pairs (any two of the 19 protective mutation gene pairs) are mutated in the peripheral blood of the patient and the donor, it is recorded as -2; and so on, if 19 protective mutation gene pairs are mutated in the peripheral blood of the patient and the donor, it is recorded as -19;
[0507] The calculation method for the cumulative score of the promoting mutation gene pairs is as follows: if none of the 16 promoting mutation gene pairs in the peripheral blood of the patient and its donor are mutated, it is recorded as 0; if one promoting mutation gene pair (any one of the 16 promoting mutation gene pairs) in the peripheral blood of the patient and its donor is mutated, it is recorded as 1; if two promoting mutation gene pairs (any two of the 16 promoting mutation gene pairs) in the peripheral blood of the patient and its donor are mutated, it is recorded as 2; and so on, if 16 promoting mutation gene pairs in the peripheral blood of the patient and its donor are mutated, it is recorded as 16.
