The puncture positioning guider comprises a first guide tube and a second guide tube which are coaxially and separately arranged, wherein a first connecting tube is connected to one end, away from the second guide tube, of the first guide tube, and the other end of the first connecting tube is connected to the second guide tube via a second connecting rod; a guide passage used for allowing passing of a guide wire runs through inner cavities of the first guide tube and the first connecting tube; a guide outlet hole allowing penetrating-out of the guide wire is formed in the first connecting tube; the second guide tube is a hollow semicircular tube with an upward opening, and an upper opening space thereof is a guide passage allowing passing of a core of a puncture device. The problem that there is no proper instrument for performing subcostal clearance separation and guidance in a subcostal anterior mediastinum surgery via a novel endoscope is solved.
