The invention discloses a method for transmitting audio / video media in an SIP (Session Initiation Protocol) audio / video conference. The method comprises the steps that: when a private network terminal and a public network conference server carry out media negotiation by using an SDP (Service Discovery Protocol), using a media address carried by the SDP as a private network address (IP+Port), adding a stream media ID in the SDP media attribute, wherein the stream media ID is used as an identification of a subsequence media stream; after the terminal obtains a media address of the server, directly sending an audio / video media stream to the terminal; after the server receives the media stream, judging whether the stream ID of the server is same as that carried in the SDP of the terminal, if yes, representing that the media stream is legal; and after the server obtains a source address of the media stream from the media stream, using the source address as a destination address of the server media stream, therefore, establishing a connection between the private network terminal and the public network server media. According to the method, the process of obtaining a public network address (IP+Port) is saved, and an STUN (Simple Traversal of User Datagram Protocol (UDP) Through Network Address Translators (NATs)) server used for obtaining the public network address is also not needed, thus the process of establishing the media stream is simplified.
