Embodiment Construction

[0055] In a first embodiment of the surgical system, the stapling instrument and the retractor are designed as separate components. This embodiment is illustrated by Figures 1-17.

[0056] Figure 1 shows an isometric view of the stapling instrument, while Figure 2 is a side view. The stapling instrument 1 comprises a handle 2 in the proximal region of the instrument, a flexible shaft 4 and a staple fastening assembly 6 mounted at the distal end of the shaft 4 . To operate the staple fastening assembly 6, a firing device 8 and a rotatable knob 9 arranged on the handle 2 can be used, as will be described in more detail below.

[0057] In this embodiment, the stapling instrument 1 is designed for use in surgical procedures for the treatment of gastroesophageal reflux disease (GERD). Accordingly, the staple fastening assembly 6 is dimensioned such that it can be introduced into the stomach through the patient's mouth and esophagus, and the flexible shaft 4 is slightly longer tha...

Abstract

In a surgical system, a stapling instrument comprises a flexible shaft device, a handle, and a staple fastening assembly. The staple fastening assembly includes a curved cartridge, which comprises at least one curved open row of staples, and, opposite to the cartridge, a curved anvil, which has a staple forming face and is adapted to cooperate with the cartridge for forming the ends of the staples exiting from the cartridge. Preferably, the staple fastening assembly is adapted to allow unobstructed access towards concave inner faces of the cartridge and of the anvil. The anvil can be moved relatively with respect to the cartridge, from a spaced position for positioning tissue there between to a closed position for clamping the tissue. The surgical system further includes a retractor adapted to draw tissue into the space between the cartridge and the anvil, when the cartridge and the anvil are in the spaced position. In a preferred application, the surgical system is used in the treatment of gastroesophageal reflux disease (GERD).

Description

technical field [0001] The present invention relates to a surgical system with a stapling instrument and a retractor that can be used in treatments such as gastric reflux. Background technique [0002] Gastroesophageal reflux disease (GERD) is widespread. For example, in the United States, 7% of the adult population suffers from symptoms related to it, such as heartburn and nausea. Complications include Barrett's esophagus, adenocarcinoma, and often include decreased quality of life. While medical treatment with secretory inhibitors is used in most cases, surgical treatment is required in about 15% of cases. [0003] In the Nissen technique (see, e.g., "20 years in the management of reflux disease using fundoplication" by R. Nissen et al., Chirurg 48:10, 634-9, October 1977), all or part of the gastric wall at the greater curvature surrounding the esophagus to form a more efficient lower esophageal sphincter (LES) flap. The surgery is performed endoscopically or through ...
