The invention discloses a real-time media stream access method and device, communication equipment and a terminal. The method comprises the steps that the communication equipment determines a media stream identification corresponding to a real-time media stream in a process of establishing a real-time media stream session; the communication device allocates a home terminal port to the real-time media stream, wherein each home terminal port corresponds to one or more real-time media streams; and after a real-time media stream session is established, the communication equipment forwards a mediamessage carrying the real-time media stream, and the media message carries the media stream identifier. According to the method disclosed in the embodiment of the invention, the media message carriesthe terminal media stream identifier, and a plurality of real-time media streams can share one home terminal port, so that the utilization rate of the media port is improved, the system capacity is improved, and the requirement of super-large-scale terminal access is met.
