[0090] Example 1: Surgical Procedure

[0091] After the anesthesia and sterile preparations have taken effect, a temporary transvenous pacing line is inserted through the femoral vein into the right ventricle. Percutaneous femoral vein access was performed, including the technique of performing a pre-closure using two orthogonal Proglides and inserting the wire through the same puncture site in a standard manner. A sheath for the TAVR procedure, having an internal dimension of 14-24 Fr, was inserted over the wire. Subjects were heparinized with an activated clotting time (ACT) in excess of 250 seconds. The aortic valve was retrogradely crossed and a stiff guidewire was inserted into the left ventricle in a standard manner. The transcatheter valve is advanced over the guide wire and attempts are made for proper placement and implantation with rapid right ventricular pacing but results in embolization of the heart valve in the left ventricle.

Abstract

The present invention provides devices and methods for percutaneously retrieving an embolized or malpositioned heart valve or foreign bodies from a patient's body. Several of these devices may include a catheter system with expandable and retractable baskets attached to a catheter and guidewire system. The guidewire may be inserted through the lumen of the embolized or malpositioned heart valve, capturing the valve in the baskets, and deploying a mechanism to crush the valve within the baskets. Then the crushed baskets and valve may be retrieved either from the body or to a safer location such as but not limited to the descending aorta. This system is advantageous because it does not require open surgery to retrieve the valve or embolized material.
